thank you!

being intersex is quite possibly the most isolating thing i’ve ever experienced. all my life, i felt alone. that no one in the world knew what was wrong with me. that i was a weird, abnormal freak.
i felt horrible. and so, i made sure that i would make people like me feel seen
every day, i see people sharing the terms i created, talking about how relatable they are. when i first saw someone talking about how seen they felt (i love you smilepilled!!!!) my heart was a damn opera, it was singing so much!
while things are pretty bad right now, i’m grateful to have provided a little bit of comfort to those who need it

Sex is Binary
Despite efforts to confuse matters, there's one simple fact that underlies the sex binary in animals: anisogamy.
Anisogamy refers to the difference in size of the two types of gametes [1, 2]. Gametes are the sex cells (eggs and sperm), which are combined during sexual reproduction [2]. By definition [3, 4], the females produce the larger gamete (the egg or ova) and males produce the smaller gamete (the sperm).
There are no intermediate gametes; therefore, there are no additional sexes. No human has ever produced both ova and sperm; therefore, every human is either female or male.
Importantly, the sex binary is not equivalent to sexual differentiation or sexual dimorphism. (Although they are strongly correlated.)
Sexual Differentiation
Sexual differentiation refers to the process in which "male and female sexual organs develop from neutral embryonic structures" [5].
Every embryo has both precursor structures: the Müllerian duct, which can develop into the female organs, and the Wolffian duct, which can develop into the male organs. Once sexual differentiation begins, one of these precursor structures will develop into maturity, and the other will atrophy. [5]
In humans, the process of sexual differentiation is controlled by various hormonal secretions by the embryo at specific times during development. These hormonal secretions are determined by the genetic makeup of the fetus. For example, the SRY gene, found on the Y chromosome, regulates the expression of multiple genes which together result in the differentiation of Sertoli cells. In the third month of development, the Sertoli cells secrete Müllerian inhibiting hormone, which initiates the atrophy of the Müllerian duct. [5, 6]
Importantly, therefore, the process of sexual differentiation depends on biological sex. Or, as this article [1] puts it, sexual differentiation is a consequence of biological sex. In a healthy organism, the embryo has the genetic instructions necessary to initiate the process of sexual differentiation that will ultimately result in the production of either ova or sperm (which defines biological sex). This expected capacity or "developmental trajectory" allows us to categorize an organism's sex even in the absence of successful gamete production. Put simply, the organism's biological sex is defined by the gamete they would have produced, had they been capable of producing a gamete.
Disorders/differences of Sex Development (DSD, Intersexism)
The complex process of sexual differentiation can be disturbed at various points, which can result in disorders/differences of sex development (i.e., intersexism). Importantly, however, every embryo starts out with the capacity to produce only one type of gamete. [5]
Further, the development of the ovary is not a "default pathway" [6, 7]. Instead, ovarian development is an active process that necessitates the activity of many factors. The mere absence of complete instructions for male development does not mean a fetus will undergo normative female development.
In other words, each embryo has the genetic instructions necessary to develop the capacity to produce either sperm or ova. However, genetic mutations in many different genes can disrupt this normative development, typically resulting in the absence of any gamete production. These individuals still have a biological sex of either male or female; it is simply determined by which gamete they would have produced had they not had their genetic mutation(s) and instead undergone normal sexual differentiation.
Astonishingly, despite the many possible points of disruption in sexual differentiation, the frequency of genuine intersex conditions is extremely low. Based on the definition of biological sex and our understanding of sexual differentiation, we can describe intersex conditions as conditions in which an individual undergoes some degree of sexual differentiation that is inconsistent with the differentiation for their expected gametic production.
In practical terms, this means that intersex conditions include cases where an individual's chromosomal sex is inconsistent with their phenotype (primary sex characteristics) or they have an ambiguous phenotype. Using this definition, we can estimate that approximately 0.018% of the population has an intersex condition. [8]
And allow me to emphasize again, that these individuals are still either male or female. Their sex can be identified by what type of gamete they would have been capable of producing, had they not had a genetic condition that disrupted their sexual development.
Another way to think of this is that, had these individuals actually been the opposite sex, their genetic mutation would not have impacted their sexual development. For example, people with Complete Androgen Insensitivity Syndrome (CAIS) have a genetic mutation that means their "cells do not respond to testosterone or other androgens", leading to female-typical external genitalia.
This genetic mutation only disrupts the sexual differentiation of biological males. If this person had been biologically female, their sexual differentiation would have proceeded normally. It is possible they may have some other health condition, but they would not have an intersex condition.
Further, the occasional disruption of a biological process does not negate the validity of that process for the species. For example, normative human development results in the development of two (complete) arms and legs. However, some humans are born missing part or all of one or more limbs, which is called limb reduction. In fact, the frequency of this is estimated at 0.05%, which is almost three times greater than intersex conditions. Despite this, we understand that the statement "humans have two arms and two legs" is accurate and does not imply the absence of non-normative phenotypes. [9]
And, in addition to all of this, people with access to modern medicine do not go their "whole life" without knowing they are intersex. Intersex conditions that are not associated with ambiguous genitalia will generally be identified at the onset of puberty or when investigating the cause of infertility. If you were not identified as having an intersex condition at birth, you underwent typical puberty, and you are not having fertility issues, you are not intersex. If you did not undergo typical puberty and/or you are infertile, you may have an intersex condition, but there are also many other more common conditions that cause these issues.
People (now or in the past) who did/do not have access to modern medicine may not have been identified as having an intersex condition if it was not physically obvious at birth/puberty. However, these individuals would present with later health issues, including, but by no means limited to, infertility.
Sexual Dimorphism
Most of the arguments suggesting sex is not binary are based on the assertion that either (1) humans are not sexually dimorphic or that (2) the partial overlap in dimorphic traits suggests a continuum in human sex. Neither of these is true.
First, I've recently made a post on sexual dimorphism, describing a sample of the many ways humans are sexually dimorphic. Hopefully, that is sufficient to refute the first assertion.
Second, I state in that same post that:
It's important to note that it is not necessary for there to be absolutely no overlap between the sexes for any particular characteristic to be considered sexually dimorphic. It is merely necessary for there to be an average difference on the population level. We should also note that, due to the many, many traits in which humans are sexually dimorphic, an individual with an opposite-sex-average trait will typically still be same-sex-typical for the many other traits.
In other words, sexual dimorphism is not equivalent to biological sex, but a consequence of it. Biological sex normally determines sexual differentiation which in turn determines sexually dimorphic traits. However, while there is some degree of overlap in sexual dimorphism (e.g., a man may be as tall as the female-average), there is no overlap in biological sex (i.e., a human cannot produce both eggs and sperm).

listen I am no expert on DSD conditions so correct me if I’m wrong but it seems like the situation with Imane Khalif is more complex than just a male who was raised male who enjoys beating women for sport. I think conversations about the fairness of Imane competing with women who do not have DSD conditions are completely reasonable, however I do not see this as a black and white situation.
Idk. It just seems strange to me to see someone who was raised and socialized female in a deeply misogynistic society, was almost not allowed to do sports bc girls in Algeria aren’t encouraged to have hobbies, as no different than the ppl who are unambiguously male, raised male, went through male puberty, and then in their 20’s started identifying differently and are allowed to compete in female sports despite knowing damn well how the blatantly unfair it is. am I missing something here?

Does anybody know why vas deferens aplasia (or CAVD) is listed as a DSD but not an intersex variation?
Per the title. Anybody in the intersex community here have any insight? 😵💫
I've recently been putting together a resource on intersex variations. Which led me to learning that vas deferens aplasia, also known as congenital absence of the vas deferens (CAVD), is actually... rather common? Like the medical articles I'm reading are saying 1% of men have this variation (albeit the unilateral variety, which is more common than both vasa deferentia being absent).
The same medical articles say 90% of cases co-occur with absence of the seminal vesicle(s), and it seems that most (60-70%?) cases are a result of the mesonephric duct system not fully developing (i.e. getting some signals to go the female-typed development route -- basically, the typical path is every foetus starts with these ducts, and then if the "female" switch is on during the relevant week of gestation, these ducts go away, otherwise they become the vas deferens & epididymis & seminal vesicles).
I've been uncertain as to whether to categorize this as an intersex variation. The medical community seems to understand it straightforwardly as a mesonephric (Wolffian) duct anomaly, and hence a "DSD". 🤔
But like... every time I search for "CAVD" or "vas deferens aplasia" plus "intersex", the only results I get are medical articles. 😵💫
It's not in @interactyouth's Intersex Variations Glossary, even though they explicitly include the homologous paramesonephric (Müllerian) duct anomalies:
I highlighted the part that notes that paramesonephric duct anomalies don't cause gonadal or hormonal differences, as gives me the impression that this is not a requisite for duct-based variations to be seen as intersex.
And yes, I know the interACT glossary explicitly says it's not comprehensive. But it has some extremely rare variations in it and this is apparently a pretty common thing?
CAVD is not on other lists of intersex variations, such as:
The book "A Comprehensive Guide to Intersex" by Petersen (who is intersex, but also kinda exclusionist)
@dhddmods's intersex guide (which also includes homologous variations)
@intersex-support's list of variations (includes MRKH which is roughly homologous)
Intersex Canada's list of intersex variations (also includes MRKH)
Intersex Aotearoa's list of intersex variations (includes MRKH)
IC4E's list of intersex variations (includes MRKH)
I don't have this variation and I don't know anybody with this variation, and the only posts I've found on social media from people with this variation talk only about infertility and nothing on identity. (And kind of noticeably most of the social media posts come from wives of people with this variation posting about their infertile husbands rather than the guys talking about it themselves... 👀)
So... yeah. I'm kinda stumped. Anybody have any insights why this is (or isn't) considered intersex? It doesn't seem to be like PCOS or gynecomastia where there's Discourse about its categorization... it just seems to not be talked about by the intersex community? 🫤

1. Egészen elképeszt, hogy nagyszerű kollégáim sorra úgy kezelnek egy egyetlen, ellenőrizhetetlen orosz forrásból származó "információt", mintha az tény lenne.
2. Valahogy sehogy se vevődik észre, nem tudatosul, hogy az orosz hibrid haderő telivér, globális, elképesztően ocsmány és megdöbbentően eredményes hadműveletét van szerencsénk átélni.
3. Attól, hogy pl. Iman Helifnek XY kromoszómapárja van, vagy van Y kromoszómája, vagy interszex, vagy DSD-s (hangsúlyozom, nem tudhatjuk biztosan, fenáll-e valamelyik), akkor is hiba ezen az alapon kapásból férfinak nevezni (pláne lánybőrbe bújt szemét macsónak). És nem, nem a társadalmi neme miatt. Ezek egyikéből sem következik feltétel nélkül, hogy a biológiai neme férfi.

My long answer:
My understanding of the situation is that Khelif is a biologically male individual (i.e., "of the sex" that produces the small gamete/sperm) with a difference/disorder of sexual development (DSD, commonly referred to as "intersex") and was, as a result of this DSD, assigned the female sex at birth.
I want to take a moment here to point out that this is the exact sort of situation the AFAB/AMAB labels were created for. The vast majority of individuals are not "assigned" a sex, they are observed to be a particular sex (OFAB/OMAB?). It is in this sort of situation, where the sex is ambiguous or incorrectly determined that the “assignment” comes into play. Further, I will be referring to all AFAB individuals as "she", given the sociocultural context in which biologically male, AFAB individuals are raised and treated as women.
That being said, the participation of people with DSDs in competitive sports is an ongoing, contentious debate that is both separate from and related to the debate about the inclusion of transwomen in women's sports.
In reference to Khelif, it appears as though the original regulatory agency for boxing (IBA) disqualified her on the basis of her DSD. However, they have lost their position due to (either claims of or actual) corruption. The IOC defaulted to determining eligibility based the sex listed on the athlete's passport, which for Khelif is female (as she is AFAB).
The issue here is we do not know what her DSD is. The IBA claims she has XY chromosomes, but there are multiple conditions this can occur with. For example, as described in [1]:
Individuals with 5ARD2 are "genetic males and exhibit phenotypic male features at puberty and during adulthood". They are "raised as girls during childhood" but "usually develop a near-normal male phenotype" after puberty.
Individuals with complete androgen insensitivity syndrome (CAIS) have "no tissue response to testosterone and no masculinization" even at puberty where they can develop a "near-normal female phenotype". This is despite them having testosterone in the "normal adult male range".
Individuals with partial androgen insensitivity syndrome (PAIS) "show a range of phenotypes with progressive masculinization depending on the degree of androgen insensitivity".
Just based on this, the best solution for each of these may be different. For example, it likely would be unfair for someone with 5ARD2 to compete in the female category, although it would be reasonable for them to compete in the male category. On the other hand, it would likely be unfair to prevent someone with CAIS from competing with other women, even with their male-typical testosterone levels.
And speaking of testosterone:
The same article [1] describes how men's testosterone level is substantially greater than women's levels, even in women with PCOS. (If you look at the article, make sure to take note of the log-scale. It highlights that the mean/median level in any male subgroup is more than 5 times the mean/median level in any female subgroup.) But again ... does the amount of testosterone really matter if the tissues don't respond to it (as in CAIS)?
In contrast, this study [2] also takes note that "testosterone exposure at puberty had unique effects such as changing skeletal structure and limb length which estrogen exposure to later in life cannot suppress" in males without a DSD. So, it's unlikely that artificially lowering the testosterone in individuals with 5ARD2 or PAIS (or males without a DSD, as in transwomen) would resolve the advantage.
So ... there are clear and significant differences in testosterone between men and women, even when they have a DSD. But in some cases (e.g., CAIS) the difference may not be relevant, and in other cases (e.g., artificial hormone suppression) a lack of difference may not be relevant.
I mention all of this to highlight how the situation is nuanced, and why I don't think we can make any judgements about Khelif. But I also want to explain how this situation is, in fact, connected to the "trans athlete" debate. It's a matter of public trust —specifically public trust in the athletic regulatory agencies.
Currently, there are regulatory boards that are making decisions that are neither consistent with biological realities [1, 2] or public opinion [3-5]. These decisions allow unambiguously biologically male individuals to compete with women.
Now to be clear, this particular case (Khelif) does not fall into this category. The problem here is one of trust: how can the public (or the other athletes) trust these regulatory agencies to make sound and fair decisions on complex cases involving DSDs if they can't adhere to scientific consensus on far clearer situations?
This is important, because athletes also deserve medical privacy. I am aware that public figures are often expected to give up a degree of their personal privacy rights (although I disagree with the extent of this). However, I expect most people will agree it's unreasonable to expect an athlete with a DSD (or any other medical condition) to release the extensive amounts of personal medical information needed to prove it is fair for them to compete with women. This is why we need trustworthy regulatory agencies, so that the public and other athletes can know that this information was provided and appropriately assessed without it having to be made public.
(And none of this touches on how the current disregard for clarity of language (e.g., claiming transwomen are "biologically female") has created so much confusion that many people seem to believe Khelif was AMAB.)

In addition to all of that, the harassment and vitriol being directed at both women in this situation is excessive, unhelpful, and harmful. I've seen racist and misogynistic comments that black women are "more masculine". I've also seen misogynistic comments that Carini is "weak" for exiting the fight. Slurs are being directed at both women, and in neither case is that acceptable.
I understand why the tone of this debate is so hostile, but I do not support the behavior.
For the comic: the claim that "science and experience shows trans athletes on H.R.T are at a disadvantage" is false (see [2]). The rest of the comic neglects to consider the nuance of the situations and the current cultural context. That being said, most people arguing that Khelif shouldn't compete in women's sports are also ignoring the nuance of the situation.

You are spreading misinformation too I think, Imane Khelif was NEVER banned from the Olympics, for the IOC her testosterone levels have always been in the norm, at Tokyo she lost in the quarterfinals against a cis woman that ALSO had normal levels of testosterone (and lost very badly too, 5-0). She WAS banned from the world championship, and even then, she was not banned for her testosterone levels, but for "arbitrary reasons". Now, if you followed boxe, you would know how much of a bad reputation the IBA (a racist and bigoted association) has. Andrey (and I was a big fan of his) spread transphobic rethoric AND misinformation, using transphobic talking points (men enter women spaces saying they're trans and make them unsafe, trans-inclusivity could lead to pedophilia) contributing to the hate campaign towards a WOMAN who did NOTHING WRONG. A hate campaign that could become VERY dangerous for her real fast, considering the extremely homophobic and transphobic country she lives and practices in. There is no excuse and I hope he, at the least, acknowledges the misinformation he spread.

I do unfortunately know about the immense racism going around this field, and reading the link you provided me it’s even worst for the most part, however the fact that her testosterone levels are higher doesn’t mean she can’t lose against a woman with hormonal balance if this is what you are trying to say about the (5-0) loss, both Yang and Khelif took the test again for this year olympics and the testosterone levels where the same always higher than normal

(also I tend to use newspaper such as hindustan times, Ansa, Gazette ecc for both university and writing works because they are a-political and the most of them are independent so I am pretty sure about the info given by them ofc nobody is 100 but the universal truth is yet to be found)

i'm intersex. fuck that stupid anon.
i am not developmentally disordered! i am not a DSD-haver (or whatever term gender "criticals" came up with to use as pawns to fit their agenda)! fuck off with that dehumanizing bullshit.
i know i can't speak for all intersex people, but my variation is so much more than just a medical issue. it doesn't stop there. it doesn't just affect me physically. my intersex condition affects me psychologically and socially as well. it affects me in all aspects of my life.
that's why it's called a condition. it CONDITIONS the circumstances of my life because it makes me fundamentally different from typical people of my birth sex — genetically, biologically, phenotypically and (in my experience at least) mentally as well.
from a young age i felt different and, even before i was diagnosed, i had a gut feeling there was something different about me (not just in body but in mind too).
it's exactly this mindset that we are "disordered" that encourages forced medicalization from birth and erasure of our lived experiences. and reducing us to our reproductive capability with language like this shows you don't actually give a shit about us as people at all.
"intersex" isn't a perfect term to describe people whose biology deviates from typical notions of their assigned sex at birth, and it can be confusing, but it's the best term we have to describe our shared reality right now.
sweeping intersex conditions under the rug and pretending like we're no different than typical people of our birth sex only makes matters worse and causes us unnecessary pain & confusion.
unfortunately, anon's mindset is shared by most of the world when it comes to us and our conditions. i know from personal experience.
there's a reason i starved myself at age 12 to make my gynaecomastia disappear. there's a reason why my parents dismissed my suspicions to the point where i had to get a karyotype blood test behind their back.
it's because of intersex erasure. our conditions are ignored, overlooked and many of us remain undiagnosed during our youth. puberty can be incredibly confusing for us.
just because we're a "tiny minority of the population" or some of us are infertile, doesn't mean we don't matter or don't have anything to offer to the world.
there is so little knowledge of what being intersex is like in our society, and i doubt things will change much during my lifetime.
but, hey, at least we get to be used as talking points for gender criticals and their obsessive hatred of trans people. how very exciting! /s
