The API or bulk drug (raw material) of Ibuprofen has seen supply shortages in the 3 quarters in 2020, due to production disruption in India in January and subsequently in China which are the major supplier.

Govt fillip for API, medical device production

NEW DELHI: Giving a major boost to local production of drug API and high end medical devices, the government on Monday released guidelines of four incentive schemes for the development of bulk drug and medical device parks across the country. The production linked incentive schemes were approved by the Cabinet in March and are worth over Rs 10,000 crore. The schemes are aimed at reducing India’s dependence on China for raw materials to produce crucial antibiotics, anti-HIV drugs, vitamins, and cardio medicines, and in production of medical devices, for which India is crucially dependent upon imports. “In line with the vision of Prime Minister Narendra Modi, the schemes have been conceptualised for making India Atma Nirbhar in pharma sector,” Union minister of chemicals and fertilizers D V Sadananda Gowda said. The scheme primarily deals with production of 53 critical active pharmaceutical ingredients (APIs) or key starting materials (KSMs). The coronavirus crisis has exposed the weakness in global supply chains and posed threat to the health security of the country, Gowda said. While proactive steps by the Department of Pharmaceuticals and the National Pharmaceutical Pricing Authority ensured steady supply of medicines, and prevented any shortage even during the lockdown, it was felt that domestic capabilities in production of APIs and medical devices have to be ramped up in order to eliminate any risks to the health security, he added. “The details of the schemes have been drawn up meticulously after having in depth consultations with stakeholders including industries and state governments. The selection of their location will be based on objective criteria, and in spirit of competitive federalism,” Gowda said. Similarly, eligible manufacturers will be selected for the PLI scheme on the basis of marks obtained in the evaluation criteria as per the guidelines, he added. “Supported by both central and state governments, these parks will be based on plug and play model with prior regulatory approvals, state of art infrastructure, excellent connectivity, affordable land, competitive utility charges, and strong R & D ecosystem and so on,” Gowda said. “I am sure that these schemes will elicit good response from interested companies. These parks will be able to attract significant investment as well as latest technology. Once operational, in about two to three years, these parks will generate thousands of direct and indirect jobs, reduce import dependency and, make India a global pharmaceutical hub,” Gowda said. The idea is also to cement India’s position as global drug supplier. As already known, in spite of various odds, India supplied critical medicines such as hydroxychloroquine and paracetamol to number of countries, he added.

The government has notified the production linked incentive (PLI) scheme for promotion of domestic manufacturing of critical key starting materials (drug intermediates) and active pharmaceutical ingredients (bulk drugs) in India. The scheme, open for four months from June 2, will allow investors to propose establishment of greenfield facilities for any of the 53 key drug intermediates and bulk drugs that are hardly manufactured in India today. The list includes key ingredients that go into the manufacturing of commonly prescribed medicines like paracetamol, aspirin, metformin, atorvastatin etc.

Bulk of the imports of these drug intermediates and bulk drugs are from China. The government strategy is expected to reduce dependence on China and be self reliant on the production of live saving and critical medicines throughout its production chain.

For fermentation-based eligible products, the scheme provides 20 percent of the annual incremental sales of these products for the first four years. For the fifth and sixth year, the incentive will be 15 percent and 5 percent respectively. For products that are chemically synthesised, it will be 10 percent incentive on the incremental sales for the first five years. The government proposes to spend a total of Rs 6,940 crore as incentive during the tenure of the scheme.  

The notification on June 2 by the department of pharmaceuticals also specifies that the eligibility for the incentive scheme will depend on companies’ threshold investment to set up the greenfield manufacturing unit. Functional guidelines for the operationalisation of the scheme is to be issued by the department soon.

DUMAGUETE CITY, Nov. 20 (PIA) -- Health officials in Negros Oriental cautioned the public from buying medicines online, stressing that people should only purchase their medicines in legitimate pharmacies and drug stores.

This reminder came as the country observes National Consciousness Week against Counterfeit Medicines (NCWM) this third week of November.

Food and Drug Administration (FDA) Visayas Cluster Regulation Officer Vicky Sibala noted a possibility of consumers receiving counterfeit medicine when ordering online.

Sibala explained that although counterfeit medicines may not have an effect on the body, it could lead to the worsening of a person’s illness.

“Some products ang ilang content is starch (which is) wala siya effect. Instead, (if) hypertensive siya, dili mo-lower imo blood pressure. That’s dangerous (In some products, most of their content is starch and it does not have any effect. Instead of treating hypertension, your blood pressure will not be treated. That’s dangerous),” she said.

Department of Health (DOH) Provincial Officer Dr. Socrates Villamor also added that taking counterfeit medicines may lead to other complications since it does not contribute to the curing of the disease.

“Kung paracetamol, it is found out na iya active ingredient is less than 80 percent gi-consider man siya as counterfeit, ang iya effect dili siya makapa-usos sa hilanat. Practically wala siya effect. Kaning sa mga antibiotic, ang mahitabo ana mo-progress imong sakit. Mo-progress imo infection. It could be (na) ma-worsen or (lead to) other complication (In the case of paracetamol, if the active ingredient is already less than 80 percent we consider that as counterfeit already. This will not help you recover from flu. Practically it has no effect. In the case of antibiotics, it does not treat the infection, it could lead to the worsening of your condition or other complications),” Villamor explained.

Villamor and Sibala said that paracetamol, antibiotics, anti-diabetic, and anti-hypertensive medicines are commonly counterfeited.

Based on Republic Act 8203, a drug is considered counterfeit if it has the wrong ingredients, does not contain active ingredients, the amount of ingredients is below 80 percent, and has fake packaging.

Villamor also noted that if a medicine is imported from other country, it is also considered counterfeit if it is not approved by FDA.
