Yes, I know it’s early...
So recently I’ve been working night shift, and since I have the night off, naturally I can’t get to sleep and I decided to write a post. Let me tell you working nights is not for everyone, but strangely enough, I am handling it very well.
As I mentioned before, the hospital I work at is small. They have 16 beds for inpatients and an ER with 8 rooms. It gets pretty slow at night and it can get boring at times.
But really, it’s either feast or famine. There is no happy medium; it’s either dead or chaotic.
When I’m working night shift, I do all the maintenance and quality control in all the departments, which really doesn’t take long, unless my QC results are out. It doesn’t happen too often, thankfully. I clean the place up, stock supplies, and pace around for a while, trying to think of other things to do...
Depending on how many morning draws I have, I go to the floor around 0530 to 0545.
Phlebotomy so far has been the most challenging aspect of the job. Besides having the technique down, having the confidence is really a huge part in being successful. Finding your rhythm and getting a routine down for drawing will really help your confidence. I have had several really great coworkers who have helped me with my technique and it’s becoming a little easier every day.
A couple of tips I have so far:
1. Anchor the vein really well. Keeping those suckers pinned down can be tough, but getting a good hold on them and keeping the skin where you are drawing pulled taunt is the key to a fairly painless draw. Hand veins are notoriously hard to keep still in my opinion, and they are still something I struggle with frequently.
2. Just because you didn’t get a flash right away doesn’t mean that you missed and need to take the needle out. If I don’t get a flash, I check with my finger to see if my vein is nearby. Sometimes a little rearranging is just what you need. Figure out which direction your vein is running and try to intersect it enough to penetrate the vein, but not go through it completely. I try not to “dig” for the vein. If it’s close by, rearranging should be easy enough. If you can’t find your vein again after you’ve poked, it’s probably best to just try again.
3. Tourniquets are only useful if they are nice and snug. Obviously if you are pinching your patient’s arm and causing them pain, it’s too tight. I’ve found that I’ve missed several easy veins because my tourniquet wasn’t tight enough to get them to pop. Don’t be shy, the patient will let you know if it’s too tight.
4. There are more veins to draw from than just the antecubital or the back of the hand. The AC is definitely a good place to look first don’t get me wrong. But sometimes it’s unavailable. They may have an I.V. in there or they may not have a good obvious AC. Hands can be tricky too. Often times, they are too weak to handle the vacuum from the tubes or syringe. Good veins can often be found on the forearm or along the arm on the thumb side. Once in a while it might be necessary to get creative and draw on the “back” of the arm (pinky side). Try to stay away from the wrist, because there’s a lot of tendons that are close to the surface.
5. Draw extra tubes if you can. A lot of times, the Doc will add on tests after you’ve already drawn them. So while you’re in their vein anyway, just draw a couple extra. If my patients are in the ER, I always draw a rainbow. Your hospital may have a different policy, but I really don’t see the problem. Multiple draws can be really irritating to a patient. If they are an inpatient who hasn’t had any other labs done for the day, I usually draw a blue (coag), light green (chemistry), and a lavender (heme) and any other tube the Doc has order. If you know it’s going to be a hard stick, just go with the tubes that have been ordered.
6. If you move sometime on the patient’s bed, put it back. It’s totally fine to put the rail down or raise the bed to save your back, but putting it back after can be hard to remember and can pose dangers to certain patients. If they are on pain meds or are at risk for falls, they could easily fall out of the bed and get injured. I’ve found that it’s hard to remember to do this, so I’ve started to accustom myself to drawing with the rail up. Since I’m tall, raising the bed is sometime that I really should do to save my back, but I feel too forgetful and would rather not get sued for endangering a patient, so I leave it at whatever level it is.
7. They probably won’t be happy to see you. With patients, it can be really hit or miss on their attitude about being woken up a 6 in the morning. I’ve had people who go, “Really? It’s not even daylight! Go away.” and some who are the most chipper and fun people I’ve met. If you have a patient who’s just a huge crabby pants, don’t take it personally. A lot of patients don’t sleep well in the hospital either because of pain or because they have noisy neighbors.
Remember, everyone misses sometimes, so don’t let it get you down. Take your time picking your vein and don’t second guess yourself. It takes time to get the hang of drawing blood, so be patient with yourself.
