Physical Activity, Exercise, Weight Reduction and Oxidative DNA Damage in Colorectal Cancer Risk Groups. Is there a Hormesis Relationship Between Risk Biomarkers and Lifestyle Factors?
Authored by H Allgayer
Abstract
Patients with obesity and/or type 2 diabetes have an increased tumor risk including colorectal cancer. Physical acrtivity (PA), exercise (EX) and weight reduction (WR) are effective strategies to decrease this risk. Highly sensitive tumor risk biomarkers such as oxidative DNA damage products (8-oxo-2’deoxyguanosine [8-oxo-dG]) increasingly are used in epidemiological and clinical (intervention) studies to better quantify this risk and to investigate potential effects of PA/EX/WR on this biomarker with the aim to further improve prevention and patient management programs. Based on the available data a U-shaped relationship between urinary 8-oxo-dG excretion and the level of PA/EX intensity is assumed, on the other hand, the relationship with BMI seems to be less clear. In this minireview this data will be discussed with regard to clinical and epidemiologic aspects.

Introduction
Epidemiologic and clinical studies in the last three decades have convincingly demonstrated that patients with type 2 diabetes (T2D) have a higher tumor risk including colorectal cancer (CRC) and that CRC-patients with T2D have an elevated risk of a more aggressive disease course compared to non-diabetic individuals [1,2]. In addition, there is convincing evidence that elevated body mass index (BMI [kg/m2]) particularly visceral fat mass may also contribute to an increased tumor risk including CRC, whereas on the other side weight reduction was found to decrease this risk [3-5]. Physical activity/exercise (PA/EX) was shown to be associated in a dose-dependent and inverse relationship with a 30 to 50 percent lower decrease of tumor risk in the most active individuals [6]. To further improve prevention strategies and therapeutic management in risk groups more clinical and epidemiological studies with quantitative data using highly sensitive risk biomarkers and their modification by lifestyle factors are urgently needed.
Go to
Disease, Oxidative DNA Damage Products and Risk Prediction Models
To quantify tumor risks (including CRC) a series of compounds modifiable by lifestyle factors (diet, PA/EX) have been proposed, among them 8-oxo-2’-deoxy-guanosine (8-oxo-dG) which was shown to reflect (mutagenic) oxidative DNA damage deriving from interactions with reactive oxygen species (ROS) produced in excess as a consequence of increased endogenous and exogenous oxidative stress [7,8]. Elevated levels of 8-oxo-dG have been found in many conditions and diseases with an increased cancer risk including T2D [9-14]. In a recently published clinical crosssectional study comparing urinary 8-oxo-dG in individuals with different CRC-risks we found that urinary excretion in patients with T2D (having a moderately elevated CRC risk) was in between those with curatively treated CRC (high risk for recurrence) and non-diabetic controls (average risk) [15]. Urinary excretion of 8-oxo-dG was significantly correlated with BMI (p=0.04) in all study participants, but not with the smoking status, age, alcohol consumption or gender. One clinically important application would be the implementation of stress biomarkers into predefined CRCprediction models to further improve accuracy with inclusion into appropriate prevention programs. Thus, in a recently published meta-analysis compiling the data of two large scale populationbased studies with >4000 individuals (ESTHER and Tromsö study) it was shown that the inclusion of reactive oxygen metabolites (ROM) assessed as total thiol (TTL) and hydrogen peroxide levels (d-ROM) into already existing models significantly improved their accuracy (C-statistics) [16]. Another study from the same group could show that CRC patients in the presence of elevated blood TTL and d-ROM levels had a poorer prognosis compared to those without these parameters [17]. Similar population-based trials with implementation of additional risk biomarkers such as urinary excretion levels of 8-oxo-dG into those prediction models presently are not available but are likely to further improve our prevention programs particularly in patients at an increased CRCrisk.
Go to
Physical Activity/Exercise and Oxidative DNA Damage (8-oxo-dG)
Quantitative data concerning beneficial effects of PA/EX on the individual tumor risk in terms of risk biomarkers are clinically important, but not widely available and not entirely consistent. The great majority of clinical and experimental intervention studies in healthy volunteers as well as in patients belonging to risk groups such as T2D or curatively treated CRC have shown decreased systemic and urinary 8-oxo-dG levels (and other risk biomarkers) in the range of 20–30 percent following moderate intensity of EX, whereas at higher intensity levels oxidative DNA damage increased suggesting a U-shaped (hormesis) relationship. [18-25]. One possible explanation for this effect may be that cellular and humoral compensatory mechanisms such as exerciseinduced activation of antioxidant defenses (e.g., superoxide dismutase, DNA repair systems) would be exhausted at higher EX intensities with a shift to more prooxidant compounds [26-28]. In our recently published study [15] we found that urinary 8-oxo-dG excretion was independent of the individual level of PA in all study participants, an observation being somewhat surprising and in contrast to data from interventional studies mostly showing clear beneficial effects of regular exercise on genome stability and DNA damage in risk groups such as patients with T2D [19,20]. A possible explanation of this discrepancy may be that exercise-induced changes of 8-oxo-dG levels are closely linked to rapid changes of humoral and cellular redox systems [26,27] and that these rather short lived effects may have been missed in non-interventional settings such as our study, a view based on a closer look into the time protocols of these studies e.g times at which biomarkers had been assessed in relation to previous PA/ EX [15]. Despite these limitations further biomarker-guided longterm prospective interventional trials with individually tailored exercise programs are urgently needed to improve patient management particularly in those at increased CRC-risk such as obesity and T2D with particular emphasis on moderate intensity protocols to obtain optimal individual benefit.
Go to
Body Mass Index and 8-oxo-dG
In contrast to PA/EX the relationship of BMI oxidative DNA damage seems to be less clear as there are studies reporting inverse (negative) as well as positive correlations [29-32]. In a longitudinal study investigating a cohort of 179 healthy office workers Mizoue et al. found a negative correlation up to BMI 27 kg/m2 [31], in morbidly obese patients undergoing bariatric surgery, however, a positive correlation was observed with a highly significant decrease 6 months after surgery [32]. Based on these data the authors of both groups suggested that there may possibly be a similar relationship of BMI with urinary 8-oxodG excretion as seen for PA/EX.; a negative correlation being observed at low/normal BMI (<27 kg/m2) turning positive above this value [31,32]. When the participants in our recently published study (T2D, CRC, non-diabetic controls) were analyzed separately according to their diagnosis the observed positive correlation (p=0.04) was no longer significant. Additional BMI-models with multiple adjustments for the cofactors gender, age, BMI, alcohol consumption and smoking status yielded similar results (no correlation) [15]. In line with this data are results of our earlier study showing no significant correlation of BMI with urinary 8-oxodG excretion in CRC-patients [21]. It is, therefore, important to adjust for multiple confounding factors, when correlations of BMI with 8-oxo-dG in clinical and/or epidemilogical studies are investigated. Despite these limitations oxidative DNA damage biomarkers should be included into weight reduction programs particularly for the obese/morbidly obese individuals and those in whom bariatric surgery is planned [32]. Our finding of a positive correlation of BMI with urinary 8-oxo-dG excretion (in the nondiabetic controls) may further support the view that long-term (beneficial) effects of PA (or energy expenditure during EX) on risk biomarkers may be mediated at least partially by the weight loss (BMI reduction) as observed during appropriate exercise programs.

COVID-19 Pandemic; Impact on the Colposcopy Service
Authored by Fani Gkrozou
Summary
Objectives: The COVID-19 pandemic caused by SARS-CoV-2 has radically changed global healthcare and colposcopy services are no exception. The aim of this study was to review the effect of COVID-19 pandemic on colposcopy services in a tertiary center.
Study design: A retrospective observational study was conducted in a tertiary center between March to August 2020. We included all women referred to our colposcopy services. The practice has been reviewed according to guidance from BSCCP.
Results: For high grade dyskaryosis referral, 64% of patients were offered an appointment within the target 2 week-wait time frame. For low grade dyskaryosis referral, 47% of patients were seen within the target 12-week timeframe.
Conclusions: The COVID-19 pandemic has resulted in longer waiting times for both high- and low-grade colposcopy referrals. This highlights the need for better preparation for potential future disruption of services and improvement of the existing service provided in order to support a faster and more efficient recovery from the pandemic.
Keywords: Colposcopy service; Covid 19 pandemic; Service provision
Introduction
In March 2020, the World Health Organization (WHO) declared the novel coronavirus disease 2019 (COVID-19) as a pandemic [1]. In the space of a few months after the start of the pandemic crisis, profound organizational changes have been adopted worldwide to optimize the healthcare facilities resources utilization.
Cervical cancer is the only one of the five gynaecological cancers to have an effective screening programme, however, during the COVID-19 pandemic, some of the 2 weeks wait cancer referrals were delayed or avoided when hospital’s capacity was particularly constrained, which affected cervical cancer prevention and management, including Human Papilloma Virus (HPV) vaccination, screening programme, colposcopy, and outpatient surgery.
A guidance by British Society for Colposcopy and Cervical Pathology [2] was put in place to triage patients who needed prompt evaluation in a colposcopy clinic including patients who had a high-grade smear, borderline nuclear change in endocervical cells, glandular neoplasia or suspicion of invasive disease [2]. Virtual consultations were advised for low grade referrals and for these patients to only be seen if resources permitted and to be deferred if colposcopy clinic capacity were reduced [2].
The aim of this study is to review the effect of the COVID-19 pandemic on colposcopy services in a tertiary center. We evaluated the process of patient selection in this tertiary unit’s colposcopy services during the pandemic benchmarked with Trust’s policy during the first wave of the pandemic and BSCCP guidelines. Our study will help understand the effect of such pandemic on our cervical cancer referral pathways to help plan our services in the future, especially should this pandemic result in a third wave of cases [3].
Materials and Methods
The colposcopy cases were undertaken at a United Kingdom (UK) teaching hospital trust, University Hospitals Birmingham NHS Foundation Trust, where colposcopy service runs at three hospital sites (Heartlands, Good Hope and Solihull Hospitals) from March 2020 to August 2020, during the first wave of the pandemic. That period, it was decided by Trust to offer appointments only to women with high grade cytology after smear test and with urgent referral from General Practitioner (GP). Results are presented for 151 patients referred with high grade dyskaryosis (group A) and 538 women with low grade cytology results (group B), from March 2020 to August 2020. Mean patient age at referral was 35.5±8.46 years, ranged (25-71) for group A and 35.4±9.31 years, ranged 24 to 66 years for group B.
Retrospective observational study on data from a populationbased cervical cancer screening database. This is the national database which is used to each colposcopy department in the UK and is provided by NHS England. For each patient information gathered regarding age, reason of referral/ smear result, histology result and treatment, when necessary, based on the information from colposcopy database. Steps in the screening process being disrupted (primary screening, surveillance, colposcopy, excisional treatment) were documented.
Baseline histology testing and delay in diagnosis were reported in patients referred for colposcopy. Categorical variables were presented as frequencies and percentages. It has to be mentioned that before the pandemic, our service was able to follow the national guidelines and keep the targets recommended by NHS England without any delays.
Precautions undertaken during COVID-19
Precautions undertaken for diagnostic and operative colposcopy were reviewed in line with Trust’s guidance and BSCCP guidelines including appropriate personal protective equipment (gloves, apron and appropriate mask) and minimum staff presence during procedures (BSCCP, 2020). BSCCP advised the avoidance of laser ablation and excision as well as use of coagulation procedures with diathermy due to vaporization [2]. For any large loop excision of transformation zone (LLETZ) procedure, the use of a serviced smoke extractor was advised [2]. For patients with symptoms suggestive of COVID-19 infection, colposcopy assessment was delayed until symptoms resolved, or patient tested negative. In accordance with our Trust’s policy, the number of women seen in colposcopy clinic had to be reduced by 5 for each clinic, to avoid unnecessary contact in waiting area. In addition, waiting area had to be reformed, to keep the rules for distance of two meters.
Results
High grade dyskaryosis colposcopy referrals
In total we had 151 patients referred with high grade dyskaryosis during this time period, out of which 149/151 (99%) attended. The commonest index cytology for referral was severe dyskaryosis 82/149 patients (55%) followed by moderate dyskaryosis 59/149 patients (40%). Three patients were referred directly due to suspected malignancy from cytology. Eight more patients were referred urgently with glandular cytology or post coital bleeding with known previous borderline cytology (Table 1).
Furthermore, we had 128/149 (86%) patients attending their first appointment compared to 21/149 (14%) of patients failing to attend. All patients that failed to attend their initial appointment were re-offered a second appointment date. Out of the 21 patients who did not attend their initial appointment date, 9/21 patients (43%) attended the second appointment date offered. 8/21 patients (38%) attended on their third appointment date offer. 4/21 patients (19%) were discharged back to their GP after failing to attend their second appointment date that was offered. On colposcopy database the reason of patient’s nonattendance was not documented.
Overall, 4/149 patients (3%) did not attend at all and were discharged back to their GP. All 149 patients who attended their appointments had a biopsy taken. The commonest histological finding was CIN 3 which was 67/149 patients (45%) followed by CIN 2, 29/149 patients (20%). Furthermore, we had 12/149 (8%) reported as CIN 1, and 41/149 (28%) were reported as cervicitis due to HPV changes or normal.
Out of the 149 patients with high grade cytology referrals, 9/149 patients (6%) were diagnosed with cancer. Five of these were offered an initial appointment within 2 weeks of referral and 4 patients had their first appointment date after 2 weeks of referral. Two patients failed to attend the first initial appointment that was offered. The average time from referral to patient being seen for these 9 patients was 35.8 days (Table 3).

Trump pushes for COVID-19 treatments, claims slowdown at FDA
Pushing for breakthroughs in treatments for the coronavirus, White House officials suggested Sunday there were politically motivated delays by the Food and Drug Administration (FDA) in approving a vaccine and therapeutics for the disease. The accusations, the latest assault from President Donald Trump’s team on the so-called “deep state” bureaucracy, were presented without evidence and just hours before Trump was set to hold a news conference to announce an apparent advancement in therapeutics. “This president is about cutting red tape,” said White House chief of staff Mark Meadows. “He had to make sure that they felt the heat. If they don’t see the light, they need to feel the heat because the American people are suffering.”
The push on Sunday came a day after Trump tweeted sharp criticism on the process to treat the virus, which has killed more than 175,000 Americans and imperiled his re-election chances.

Experts flag risks in India's use of rapid tests for virus
In June, India began using cheaper, faster but less accurate tests to scale up testing for the coronavirus — a strategy that the United States is now considering.
But India’s experience also highlights the inherent pitfalls of relying too heavily on antigen tests, at the expense of more accurate tests. The danger is that the tests may falsely clear many who are infected with COVID-19, contributing to new spread of the virus in hard-hit areas.
Rapid test results can be backstopped with more accurate laboratory tests, but these are slower and expensive. Experts also warn that since the two types of tests vary in accuracy, they need to be interpreted separately to properly assess the spread of infection -- something India isn’t doing.

Of the overall tally of 30,44,940, India now has 7,07,668 active cases while more than 22 lakh people have recovered from the infection
India's coronavirus case count raced past 30 lakh on Saturday, showed an unofficial tally based on information provided by staes and Union territories, while the toll rose to 56,762 and recoveries climbed to 22,71,054.

The COVID-19 case count in Telangana crossed the one-lakh mark with 2,474 new cases while the toll touched 744. Recoveries exceed active cases by 15 lakh, says health ministry
The home ministry, in its morning update, said that there are 6,97,330 active cases of coronavirus infection in the country, comprising 23.43 percent of the total caseload in the country.
While the country's COVID-19 case count has surged to 29.75 lakh, the total number of recoveries has surged to 22,22,577 and exceed active cases by over 15 lakh as on date, it said.
Union Health Minister Harsh Vardhan, at the inauguration of a make-shift NDRF hospital in Ghaziabad, said that India has the "best" COVID-19 recovery rate, which is improving every day, and the "lowest" mortality rate in the world.
Taking a potshot at government critics, Vardhan said "many intelligent people, scientists and naysayers" had estimated that India, with a population of about 135 crore, will see 30 crore COVID-19 cases and about 50-60 lakh people will die by July-August, and the country's healthcare system was "incapable" to combat the disease.
"However, I am happy to say that in the eighth month of the battle, India has the best recovery rate of 75 percent and against an estimate of 30 crore affected we have not even reached 30 lakh cases."
"In fact, 22 lakh patients have recovered and gone home and another seven lakh are going to be cured very soon," he said.
The minister said these successes were achieved due to the "coordinated" efforts with the participation of everyone — the government and the people.
Centre asks states to ensure unrestricted movement of goods, people
Meanwhile, the Centre asked all states to ensure that there are no restrictions on inter-state and intra-state movement of persons and goods during the ongoing unlocking process. In a communication to chief secretaries of all states and Union Territories, Union Home Secretary Ajay Bhalla said there were reports that local level restrictions on movement were being imposed by various districts and states.
Drawing attention to the Unlock-3 guidelines, Bhalla said such restrictions are creating problems in inter-state movement of goods and services and are impacting supply chains, resulting in disruption of economic activity and employment.
The unlock guidelines clearly state that there shall be no restrictions on inter-state and intra-state movement of persons and goods, he said in the letter. The guidelines also stated that no separate permission, approval or e-permit will be required for movement of persons and goods for cross land border trade under treaties with neighbouring countries.
The home secretary said the restrictions amount to violation of the guidelines issued by Ministry of Home Affairs under provisions of Disaster Management Act, 2005.

Programmed Death Ligand-1 expression in stage II colon cancer - experiences from a nationwide populationbased cohort
Abstract
Background
Patients suffering from high risk stage II colon cancer (CC) may benefit from adjuvant onco-therapy, but additional prognostic markers are needed for better treatment stratification. We investigated the prognostic value of Programmed Death Ligand-1 (PD-L1) in a true population-based cohort of patients with stage II CC.
Methods
PD-L1 expression on tumour cells was evaluated by immunohistochemistry in 572 colon cancers. Whole sections from tumour blocks representing the deepest invasive front of the primary tumour were used for analysis. A cut-off of 5% positivity was used for dichotomizing the data. The prognostic value was investigated in Cox proportional hazard models for recurrence-free survival (RFS) and overall survival (OS).
Results
Overall, 6% of the tumours were classified as high PD-L1. High PD-L1 was related to female gender (p = 0.028), high malignancy grade (< 0.001), right side localization (p < 0.001) and microsatellite instability (MSI) (p < 0.001). Thirty-one (18%) of the MSI and 4 (1%) of the microsatellite stable tumours were classified as high PD-L1, respectively. PD-L1 expression provided no prognostic value as a single marker. In patients with MSI tumours, high PD-L1 expression had no significant impact regarding OS or RFS.
Conclusions
PD-L1 expression in tumour cells of stage II CC did not provide any prognostic impact, neither in the entire population-based cohort nor in the group of MSI patients. Additional investigations of the immunogenic microenvironment are needed for evaluating the prognostic information in CC.

12:22 (IST) Coronavirus in Telangana Latest Updates 2,384 new cases in Telangana The total number of coronavirus cases in Telangana rose to 1,04,249 after 2,384 new infections were reported on Saturday. "However, I am happy to say that in the eighth month of the battle, India has the best recovery rate of 75 percent and against an estimate of 30 crore affected we have not even reached 30 lakh cases." "In fact, 22 lakh patients have recovered and gone home and another seven lakh are going to be cured very soon," he said. The minister said these successes were achieved due to the "coordinated" efforts with the participation of everyone — the government and the people. Trends in the risk and burden of second primary malignancy among survivors of smoking‐related cancers in the United States
While there are a growing number of cancer survivors, this population is at increased risk of developing second primary malignancies (SPMs). We described the incidence, most common tumor sites, and trends in burden of SPM among survivors of the most commonly diagnosed smoking‐related cancers. The current study was a population‐based study of patients diagnosed with a primary malignancy from the top 10 smoking‐related cancer sites between 2000 and 2014 from Surveillance, Epidemiology, and End Results data. SPM risks were quantified using standardized incidence ratios (SIRs) and excess absolute risks (EARs) per 10,000 person‐years at risk (PYR). Trends in the burden of SPM were assessed using Joinpoint regression models. A cohort of 1,608,607 patients was identified, 119,980 (7.5%) of whom developed SPM (76% of the SPMs were smoking‐related). The overall SIR of developing second primary malignancies was 1.51 (95% CI, 1.50–1.52) and the EAR was 73.3 cases per 10,000 PYR compared with the general population. Survivors of head and neck cancer had the highest risk of developing a SPM (SIR=2.06) and urinary bladder cancer had the highest excess burden (EAR=151.4 per 10,000 PYR). The excess burden of SPM for all smoking‐related cancers decreased between 2000 and 2003 (annual percentage change [APC] = ‐13.7%; p=0.007) but increased slightly between 2003 and 2014 (APC=1.6%, p=0.032). We show that 1‐in‐12 survivors of smoking‐related cancers developed an SPM. With the significant increase in the burden of SPM from smoking‐related cancers in the last decade, clinicians should be cognizant of long‐term smoking‐related cancer risks among these patients as part of their survivorship care plans.

Suicidal death within a year of a cancer diagnosis: A population‐based study
Background
The suicide risk after a new cancer diagnosis remains a controversial issue. This study examines the suicide risk within the year after a cancer diagnosis. This is the largest study to assess recent trends in suicide risk after a cancer diagnosis.
Methods
Data were obtained from the Surveillance, Epidemiology, and End Results Program. All patients diagnosed with cancer between 2000 and 2014 were selected. The event was defined as death due to suicide within the first year after a cancer diagnosis, and patients who experienced the event after their diagnosis were observed. The observed/expected (O/E) ratio was assessed as well as the excess risk per 10,000 person‐years to determine the suicide risk change after the diagnosis in comparison with the general population.
Results
A total of 4,671,989 patients with cancer were included; 1585 committed suicide within 1 year of their diagnosis. The risk of suicide increased significantly with an O/E ratio of 2.52 and with an excess risk of 2.51 per 10,000 person‐years. When the risk of suicide was studied according to the cancer site, the highest increases in the O/E ratio came after diagnoses of pancreatic cancer (8.01) and lung cancer (6.05). The risk of suicide also increased significantly after a diagnosis of colorectal cancer with an O/E ratio of 2.08. However, the risk of suicidal death did not increase significantly after breast and prostate cancer diagnoses.
Conclusions
The risk of suicide increases significantly in the first year after a diagnosis of cancer in comparison with the general population, and this increase varies with the type and prognosis of cancer. Close observation and referral to mental health services, when indicated, are important for mitigating such risk.
