2. the reason it matters that psychiatric diagnoses do not have a 'biology' is not because every disease must have a single specific biomarker; it is correct that some do not. however, the way patient complaints are sifted into categories labelled 'psychiatric' versus '(otherwise) medical' begins essentially with determining whether the distress is 'physical' or 'mental'. in other words, in the case of, say, the chronic fatigue syndrome (famously, lacking a known specific biomarker), the symptoms being investigated by the non-psychiatrist physician are still physical (PEM; mast cell dysregulation; pain; etc) whereas a diagnosis of depression may be accompanied by, but requires no, physical symptoms or presentation. the psychiatric claim that its diagnoses have biological causes and correlates is specifically a claim about the role of neurobiology in the causation of affective states; thus, the comparison to physical complaints is meaningless here
3. this person goes on to claim that depressives do in fact share, though not universally, certain biomarkers such as mitochondrial dysregulations. such claims typically come from various imaging studies plagued with systemic problems in the selection and definition of patient populations as well as the subjectivity of result interpretation and analysis. these claims are not well supported and typically rely on circular selection and definition of patient populations
4. speaking philosophically, it is in fact often correct to challenge the notion that a physical 'disease' chronically lacking a specific biomarker is indeed a disease, in any sense besides the colloquial one. that is, diseases that cannot be correlated with one cause or presentation are often better understood as 'syndromes', which is to say, as a taxonomical heuristic that is likely grouping together multiple disparate physical (anatomical, physiological, functional, &c) problems with multiple disparate causes. this is almost certainly the case for chronic fatigue syndrome, for example. this is a philosophical distinction that matters for research and understanding, and does not mean or imply anything to minimise or contradict the patient experience of the syndrome or symptoms. it matters because, for instance, CFS triggered by the epstein-barr virus may indeed turn out to have different disease mechanisms to CFS triggered by, say, covid-19, or may have different specific mechanisms when running in certain families, and so on. distinguishing these much more specific presentations, and possibly distinct diseases, from the current discursive schema of the overlying syndrome is potentially very good for patients, who likely have different needs and treatments to one another despite currently all sharing the same label in their charts

Progressive Hereditary Blindness
We're gonna talk about Leber Hereditary Optic Neuropathy (LHON), which is a disease of the eye (obviously) which causes progressive centralized loss of vision.
Symptoms are usually gonna appear between the age of 15 and 35, and it usually happens to males (like 80-90% of cases). No one has really any idea why. Anyway, symptoms start in the central visual field of one eye. The area might appear "misty" or can start to lose color. Symptoms are painless. Within a year, the condition will affect both eyes.
Patients will develop what is called a scotoma, which is basically a blind spot in the middle of their vision. The area around that will also usually have poor color and poor acuity (like 20/200 vision) Physically, there isn't always something wrong with the eye, especially before their vision starts to go to shit. There's also not really a treatment either, so most people are going to go mostly blind.
LHON is maternally inherited because it is due to mutations in mitochondrial DNA (mtDNA). While the rest of your DNA is a combination of both your parents' DNA, your mtDNA only comes from the egg, and is therefore 100% from your mother. The mutations in the mtDNA affect the function of Complex I of the electron transport chain (important for making energy), which ultimately ends in the death of retinal ganglion cells and atrophy of the optic nerve. That is why someone will go blind.
This disease always kinda interested me because it affects people so young, and it's progressive. So someone might notice a problem with their vision, and then their doctor tells them they're going to go blind and there's nothing they can really do about it. That would be pretty hard to go through, so I someone would hopefully have a pretty good support system.

New study offers clues as to how exhaustion could arise in people with ME/CFS—and potentially related conditions such as Long Covid
People living with myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) struggle not only with extreme exhaustion and the cognitive problems known as brain fog, but with a profound lack of information about what causes their symptoms and how to treat them. Scientists have yet to pin down the biology underlying the condition, which affects up to 2.5 million people in the United States alone, according to some estimates.
Now, researchers have identified a protein that’s present at unusually high levels in the muscles of people with ME/CFS and that disrupts cells’ ability to generate energy. The findings, reported today in the Proceedings of the National Academy of Sciences, could point to new therapeutics for this condition and for illnesses that share similar characteristics, such as Long Covid.
Akiko Iwasaki, an immunobiologist at Yale School of Medicine who was not involved in the work, praises the research as “very well done” but cautions that the suspect protein is likely “a piece of the puzzle, as opposed to explaining the whole disease.” The findings suggest it could act as one of several “middlemen” between whatever sparks the illness and symptoms such as fatigue, she says.

The NHLBI researchers wondered whether WASF3 was interacting with mitochondria, cellular compartments responsible for energy generation that have been suggested to malfunction in people with ME/CFS and Long Covid. Sure enough, by changing levels of WASF3 inside cultured cells from S1 as well as in other human and mouse cells, the team found the protein could disrupt mitochondrial function. Specifically, high levels of WASF3 interfered with the assembly of mitochondrial proteins into molecular complexes that support normal energy production.
