Asylums in the 19th Century Asylums came into existence in response to a growing social problem -- what to do with people who were mentally ill. Of course, they were not called mentally ill in those days but were referred to as victims of lunacy or madness. The perception of madness as an illness came with the rise of psychiatry, and the development of psychiatry as a profession occurred concurrently with the development of asylums. This essay will consider conditions that led to the building of asylums, how asylums were meant to function, and the shift of function that occurred late in the 19th century. Historians disagree about the efficacy of asylums for the treatment of those who were mentally ill, but all agree that conditions before asylums were for the most part degrading and inhumane to say the least. This was particularly true if the person was a pauper without funds to pay for care. In England, for example, the welfare administration -- the Poor Law -- cared for lunatic paupers. "Institutional care was provided and paid for from the public purse or was purchased by the parish at public expense from institutions run by private proprietors" (Murphy, 2001, p. 29). In London, mad houses were an industry that made lay proprietors rich and respectable. As Hughes (2002) points out, the idea of a "mixed economy" in which both state and private parties are involved in housing the mentally ill goes back to Victorian times. The two systems "public interest and private profit have operated in parallel..." (p. 328). The Whitmore House, for instance, was owned and operated by Thomas Warburton who began at Whimore as an attendant. When the owner died, he married the owner's widow and thus acquired ownership. He habitually hired brutal, thuggish keepers and failed to supervise them. A pamphlet written by a former "whistle-blowing" employee in 1816, "A Statement of the Curelties, Abuses, and Frauds which are Practised in Mad-Houses" resulted in a hearing. The pamphlet reported Warbuton took in as many people as he could get whether he had beds enough or not. Two or three people were often in the same bed. Crowding was only the beginning. "Brutal forced feeding, using objects such as a long spouted 'tea-pot' and a large key to crank open the mouth, resulted in smashed front teeth or even suffocation if the spout was pushed in too far and food passed down the windpipe. Mrs. Hodges, the wife of the vestry clerk for St. Andrew Holborn, had died of incompetent forced feeding... Pauper patients were left naked on wet straw beds in unheated rooms; soiled straw, filthy and infested with vermin, was unchanged for days. The limbs of the frail were 'mortified' by cold and neglect; one woman's foot had to be half amputated. Almost everyone was chained to the bedstead at night" (Murphy, 2001, p. 31). Inspectors, when they went to see for themselves, repeatedly gagged from the stench. According to reports (Murphy, 2001), Warburton appeared to be puzzled when he was called before the Select Committee to explain the appalling conditions. He said the patients "could readily be controlled with a leg lock and manacles" (p. 32). He said the only restraint he used was "hobbles" and explained, "...almost in the manner you would put round a cow when milking..." (p. 32). Compounding the misery, patients had nothing to do and almost no medical treatment -- definitely nothing that could benefit a mentally ill person. In England, the Madhouse Act of 1828, provided for the First Middlesex County Asylum in Hanwell, West London to be built, but condition were poor there, too, and no treatment was offered (Psychiatric Hospital web site). After the 1845 Lunatic Asylums Act, most of England's counties and boroughs were able to build asylums from public monies to care for paupers who were mentally ill (Dowbiggin, 1997). Campaigners for public asylums often described the horrors of private mad-houses as part of their arsenal of arguments for reform; however, conditions for patients with money to pay were not nearly so awful. Middle class and rich people could go to licensed houses, usually run by doctors who "opened up their own homes to patients as a species of 'paying guest'" (Murphy, 2001, p. 30). These homes tended to be cleaner, and lunatics were treated with some decency. Meanwhile, in colonial America, madness was understood as either touched by God or bedevilment. According to McGovern (1993), mentally ill people were mostly left alone. Later, as political and social changes took place in the mid-18th century, there was less tolerance in society for lunatics and the disruption they sometimes caused. Many of the mentally ill were locked in unheated attics and sheds, kept in pest houses, or confined in jails. Haller and Larsen (2003) argue that "like eighteenth-century Europe, the United States treated insanity severely and punitively" (p. 260). Clearly, something needed to be done to make treatment more humane and to deal with the problem of growing numbers of mentally ill people in the cities. In Philadelphia, Dr. Benjamin Rush, founder of psychiatry in America, called for a new view of lunacy. The solution, it seemed, was to stop seeing the victims as wayward and to build publicly-supported asylums for the humane care and treatment of them. The movement to build asylums was revolutionary and an abrupt departure from previous public policies toward mentally ill people (Dowbiggin, 1997). Webster's Ninth New Collegiate Dictionary defines the word asylum as "an inviolable place of refuge and protection giving shelter to criminals and debtors; a sanctuary, a place of retreat and security; an institution for the relief or care of the destitute or afflicted and especially the insane." The asylum had a double purpose -- custodial care in a safe place and "a therapeutic environment for those deemed curable" (McGovern, 1993, p. 688). Crowded cities and high stress levels in American society had triggered a need for better care, and asylums were seen as places where expert care could be given. No discussion of asylums would be complete without considering Dr. Thomas Kirkbride who was an influential early advocate of asylums for "moral treatment" in the United States (Kirkbride Buildings - History web site). He was strongly influenced by the ideas of Dr. Benjamin Rush in Philadelphia who preached the moral treatment of mental illness and was responsible for initiating change in the way people viewed lunatics. Instead of seeing them as moral deviants who required confinement and punishment to set them straight, enlightened people began to see them as sick people who needed to be cured. Moral treatment was a social movement in the 19th century that began in England and France in the late 1700s with the ideas of William Tuke, a Quaker who believed that madmen should be treated with tenderness and sensitivity. At the same time, Pinel in France was instituting similar reforms. Tuke founded the York Retreat in 1796 and ran it according to the principles of the Society of Friends. No shackling was allowed, no mechanical restraints, punishments, or medicine. "The Retreat was designed as an environment that would help them internalize discipline and restraint" (Haller & Larsen, 2005, p. 260). Some physical labor was provided as a way to foster wellness, as "The religious foundation of the York Retreat nurtured the belief that those who worked were the instruments of God's will" (cited in Haller & Larsen, 2005, p. 261). It also gave "guests" something to do. In the 1800s, Tuke's son described mental illness as "a distortion or destruction of intellectual and moral powers" (Haller & Larsen, 2005, p. 261). Thus, the new moral treatment was designed to help the mentally ill get control back and regain intellectual and moral power. Instead of shackles and physical restraints, fear was the weapon used to combat mental illness. "There cannot be a doubt that the principle of fear, in the human mind, when moderately and judiciously excited, as it is by the operation of just and equal laws, has a salutary effect upon society" (cited in Haller & Larsen, 2005, p. 261). Thus, fear was the internal mechanism of control. Apparently, Tuke had some success re-integrating mentally ill persons back into society because Rush and Kirkbride in America were strongly influenced by his ideas. Beginning with one in Philadelphia, Kirkbride asylums were built all over the country. Regarding the architecture of asylums, Krahn (2005) observes, "To study the processing mode of the mental asylum reveals layers of metaphor about what is human in a given culture, murmuring about who we think we are, and who we are, and how the two clash" (p. 104). One can see by examining the architecture of the Kirkbride asylums that Dr. Kirkbride believed in human beings working both the body and the mind. The asylums were laid out in a manner to encourage or "persuade" the mental patient to return to sanity. The right surroundings were imperative. He was one of thirteen founders of the asylum superintendents' association and very influential in that his asylum plans were widely copied. He believed asylums should be planned to encourage work, both physical and mental. To get away from the stress and turmoil of the city, an asylum should be erected out in the country where there was space for patients "to work, walk, and congregate. He called for plenty of large windows, one central building, separate buildings for the genders, and separate wings for wards" (Haller & Larsen, 2005, p. 262). The Kirkbride asylum had a central administration building with a dome that was flanked by two wings of tiered wards. Residents were separated according to sex and the symptoms of their illnesses, with "excited patients" on the lower floors farther away from the administrative center. Well-behaved, more rational patients were placed on uppers floors closer to the administrative section. Fresh air, natural light, and scenic views of the park-like grounds were available to all wards. Kirkbride asylums were designed to work in harmony with the principles of moral treatment. Space was allocated for a variety of wholesome patient activities (such as lectures and classes), amusements, and outdoor exercise. Kirkbride "envisioned moral treatment as a series of internal experiences" (p. 263) for a wide range of social classes. His first hospital -- Pennsylvania Hospital for the Insane -- opened in 1841 with a progressive program, board, and staff familiar with strategies such as "psychological persuasion, classification and separation by mental disorder, amusements, and physical labor" (p. 263). In a poetic essay about the architecture of asylums, Krahn (2005) reminds us, "The mental hospital is a house built by society for such people, a training house to conquer their fear or refusal or confusion or forgetfulness. Foucault has described these architectural intentions in detail, how power is demonstrated and exerted by institutional architecture which rules out certain behaviors and encourages others" (p. 104). This was certainly true of Kirkbride asylums, for as Krahn puts it, "the quality of material and shaping of space clearly spoke about who was to be where, you could see there were many different classes of people, different things you were allowed to do or told not to do" (p. 106). Kirkbride saw moral treatment as the best way to treat mental illness, but he also was highly innovative and eager to use the new, exciting technologies of the day in activities and amusements: One of the more popular efforts to enlighten and educate patients to return to sanity was the scheduling of evening magic lantern slide exhibitions and lectures. They were intended by Kirkbride to persuade, control, and amuse. He believed that they encouraged patients to fight depression and sadness, thereby regaining the happy demeanor necessary for resuming their places in the social order. He also assumed that the slides were a rhetorically persuasive tool for convincing patients that hospital officials cared for them. If the hospital provided entertainment equal or better to that in the outside world, the patients would believe that its staff members sincerely desired to make them well" (Haller & Larsen, 2005, p. 264). Magic lantern presentations were not new. What was new was the use of photography, which made the effect much more realistic; in fact, asylums were among the first to use photography in the treatment of patients. Shows could consist of slides from popular fiction, historical episodes, current themes and news, natural history, comedy, and education -- including social causes and religious movements. Magic lantern shows, in general, were very popular with American audiences during the 19th century (before movies). For mental patients they were ideal for safe and suitable entertainment and education and, according to Haller & Larsen (2005), reminded patients of their middle-class sensibilities. Moreover, Kirkbride was careful to debrief the patients afterwards so that they would not become confused and think the images they saw were reality. He helped them to understand that the magic lantern shows with their realistic 3-D images were cleverly wrought illusions (Haller & Larsen, 2005). As one of America's first psychiatrists, Kirkbride claimed he could diagnose sixteen different mental disorders in patients just by looking at them and "could determine visually from patients' conduct their degrees of insanity and sanity... assumed that their trained clinical gaze could provide them with sufficient evidence of the presence of pathology" (Haller & Larsen, 2005, p. 75). Psychiatry, needless to say, was very much in its infancy, and practitioners were eagerly trying everything they could think of to help their patients. As the superintendent of the Philadelphia Hospital for the Insane, Kirkbride, more than anyone else, influenced how other asylums were built and operated. State governments paid for the building of the best-known and largest asylums. The buildings and surroundings, according to Kirkbride principles, were considered to be active participants in the treatment. The landscaping, for instance, with its natural beauty could calm a person's mind while at the same time it improved the appearance of the asylum itself. "Farmland served to make the asylum more self-sufficient by providing readily available food and other farm products at a minimal cost to the state" (Kirkbride Buildings - History web site, p. 1). The asylum was a place meant to seclude the patient from the hectic energy and disturbing influences in the city that may have caused or contributed to his or her illness. Sunlight and air were crucial to a cheerful atmosphere, and structured activity was an important component in the treatment. Curing the patients was the primary goal. The doctors, of course, were not alone in the decision-making process. Running an asylum involved negotiations between the doctor and the family members who were paying for treatment. Suzuki (2006) argues that the family played a far more significant role than most researchers and historians have recognized. Families had considerable influence in deciding what treatments were appropriate. According to Suzuki, "the lay interpretation of madness" was a key determining factor in "diagnosis, treatment, and management practices" (cited in Smith, 2006, p. 919). Families nearly always tried at first to care for the person at home. Sometimes the police would order the family to place the patient in an open slatted cage at home to protect themselves and their neighbors from assault (Lanzoni, 2005, p. 500). When family members were no longer able to care for the person, they tended to choose institutions with more homelike environments. Suzuki speculates that moral treatment "may have originated in the family, to be subsequently adopted in the best-managed institutions..." (p. 920). Whether or not that is so, families were "authorities in their own right, who seek and coordinate asylum care, as well as make strategic decisions as to when to institutionalize their family members" (Lanzoni, 2005, p. 499). It could not have been an easy decision to make. In his famous book Asylums, sociologist Erving Goffman points out that once a person went into an asylum, it was difficult to get out, so difficult that Goffman defined asylums as impermeable institutions (Curran, 2006). No matter how homelike and cheerful the place was, the person was still incarcerated. For this reason various scholars have labeled asylums inherently abusive because once the patient was declared legally to be incompetent, he or she was deprived of civil liberties and human rights. Later in the 19th century the focus of asylum care shifted from moral treatment to custodial care. This was a direct result of social conditions (Lanzoni, 2005). The number of committed patients greatly increased and frequently overwhelmed doctors and staff, who were "overworked and lowly paid.." (McGovern, 1993, p. 669). Custodial care began to take precedence over therapeutic efforts and individualized attention. Urbanization with ever-greater numbers of people seems to have triggered the shift away from treatment. Moreover, the asylums were designed to house smaller numbers and became overcrowded. Lanzoni (2005) states, this "effectively lowered cure rates and promoted a pessimistic view of mental illness..." (p. 502). A larger number of chronic (incurable) patients were admitted than had been in earlier years. Thus, she concludes, "Overcrowding and poor care then were not an indictment of medical practice per se, but rather resulted from an array of social changes and commitment practices that resulted in high internment rates" (p. 503). An example is the South Carolina public lunatic asylum, which moved from "the ideal of moral treatment to custodial care" (Lanzoni, 2005, p. 503). By 1899 the asylum in South Carolina had become known as the worst in the country. Overcrowding, marginal recovery rates, high mortality rates, and a failure to provide even basic custodial care were the norm" (p. 504). Not surprisingly, poor black patients were disregarded, ignored, and neglected. Social changes compounded the deplorable conditions. After the civil war South Carolina was one of the poorest states and continually lacked funds to support the asylum. Plus, a huge increase of patients came when blacks were emancipated and allowed to enter the state asylum. The South Carolina asylum followed a pattern nationwide of asylums becoming overcrowded, under-funded, nasty places to be. In Ontario, there was the additional problem of old people being admitted to insane asylums when they couldn't take care of themselves anymore. They were not insane but had senile dementia, which was treated by the local authorities as insanity so that they could be admitted to Provincial asylums. Asylum administrators became alarmed that mental institutions were filling up with elderly people and began to tighten their policies. If the old person had no relatives to take care of them, they were willing to accept him or her. But if the person had a family, it was the family's responsibility to care for their own. Sometimes the only way to get an old person admitted was to have him or her arrested. If the police obtained a court order, then the institution had to take the person whether they wanted to or not. The government's official position was that people were foisting their old folks on the government because they were in the way at home. However, Montigny (1995) makes a compelling argument that this was really not the case.
