he likes to bring a package of cookies and the two of you eat them while you have your session
bokuto still uses the term ‘ouchie’ and it’s adorable
he takes really good of himself and his body so he isn’t prone to serious injuries. as soon as he knows something feels off, he’ll stop himself from working and come to your clinic

in interviews, bokuto loves to mention all the people who helped him on his journey and ngl it’s a REALLY LONG LIST but he always, always mentions you as ‘the best physical therapist ever!!’
and it makes you so soft you love your job so much
he actually ends up recommending you to his friend, akaashi keiji, because bokuto knows his shoulders and back hurt

he got LOST TRYING TO FIND YOUR CLINIC
like, you were checking your watch wondering where your client was before he burst into your clinic thirty minutes late
also for some reason he was really nervous to see you poor baby he had the misconception that you were there to find what was wrong with him and then after he wouldn’t be able to play again
so he was genuinely surprised after you just made him do a few stretches for him, recommended some ways to move his body to utilize it better, and just sent him off
hinata: just... just like that?
you: yeah, pretty much. unless you feel that something else is wrong
after that he’s not so afraid to visit your clinic. in fact, he enjoys it quite a lot!
hinata really takes note of the advice you gave him (he brings a notepad and everything) and he likes to call you up to let you know that they worked
he actually calls you A LOT, like you’re probably the person he texts and calls the most other than natsu
at one point he was calling you every single day asking about what to eat until you decided to make a whole-ass weekly meal plan for him to just cycle through
 he’s actually quite flexible (not as much as sakusa though) and he’s so proud of being able to reach farther than his toes
when you’re not looking though he measures himself against the height chart on your wall and you think it’s absolutely adorable
hinata knows how to take care of his body and TRIES not to overwork himself but sometimes he gets really impatient about wanting to improve so he’ll stay late in the gym, practicing his spikes and serves
BUT since his teammates know hinata listens to you they’ll threaten to tell you when he’s overworking
sakusa: hinata, if you don’t leave the gym right this instant i am calling y/n
hinata: just five more minutes!!
sakusa: *pulls out his phones and presses some random numbers in*

it’s kind of like the adult version of your parents calling the doctor when you don’t want to eat
also you have no idea that this happens
even though he’s moved on from it, hinata still gets a bit paranoid about playing a major game while being sick
you’re always at their games as part of their medic crew and before it starts, hinata always comes up to you and asks if you can take his temperature
you didn’t know why until atsumu explained everything so now you make sure to always have a temperature on you for hinata’s peace of mind

ngl you’re kinda shookt at how large he is in real life when he walks into your clinic 
he also assumes right away that you want an autograph so he gifts you a signed picture of himself which you always have on your desk from now on because come on you are a fan
atsumu’s really nice to talk to, he does flirt quite a bit but he genuinely asks about how you are too or if your family is doing alright

atsumu’s the one who visits your office often but for reasons that are actually relevant to your job because this man just overexerts himself too much
and he keeps brushing it off and saying that he’s alright but you know better you’re a physical therapist for god’s sake

he’s the one that you have to constantly keep tabs on and thanks to the help of his teammates and a spare key, you’re able to drop by his apartment once in a while
but no matter how much of a pain in the ass he is you still can’t help but want to take care of him. whenever you make yourself dinner you find yourself making seconds for atsumu
before you know it, you’re making dinner for your favorite MSBY boys because you don’t want them to feel left out
you also have to stop atsumu from overdoing on the protein shakes and drinking vitamins
because he’s addicted protein shakes especially the strawberry-flavored one but you’re the only one who knows that so shhhhhh
atsumu: if one vitamin capsule makes me healthy, does that mean drinking more will make me even more healthy ??
you: NO STOP. STOP WHATEVER YOU’RE DOING. RIGHT NOW.
one time you got a call in the middle of the night from atsumu saying that he ate an entire container of gummy vitamins and you had to run to his place in your pajamas
idk you just see him so much because he keeps getting injuries and it’s hilarious

probably your most normal client among the four of them but that doesn’t mean you don’t enjoy his visits to the clinic 
well, actually when he first visited your clinic, everyone else stalked him because they wanted to see his Flexible Wrists in action
you had to chase them out
sakusa is SO HAPPY that you just took one look at his Flexi-Wrists and noted it down without going batshit over it
you always spray the cots and your instruments with alcohol before he comes in and sakusa knows cause he smells it and he appreciates that so much
one thing that you do notice is that sakusa,,, frequently glances at the jar of lollipops you keep on your desk and give out to your kid patients
and after a session he just lingers for a bit before saying good bye and leaving so one day you just take a chance
you: would you,,, like a lollipop?
sakusa: .... yes please
he chooses a cherry flavored one and is about to leave when he sits back down on the chair
sakusa: they’ll never let me hear the end of it if they see me
so you let him stay in your clinic for a bit longer before deciding to also get a lollipop and that’s how the two of you end your sessions
no talking. just lollipop eating
(i know how it looks but pls don’t add sexual connotations i jus want sakusa to enjoy a fricking lollipop)
he actually likes to remember the scientific muscle names and everything just to impress you 
also, he doesn’t want to give you a hard time but he also likes visiting your office and talking to you (and eating the lollipops) so he makes up all these excuses to come by your clinic

you: okaay, do you need a bandaid?
sakusa: it was the small toe
sakusa’s quite selective about the people he’s with so he’s kind of attached to you now. like it or not, you’re his personal doctor

you: also, ginger tea is supposed to be good for you
he’s actually your eyes and ears when it comes to monitoring the team. kind of like the kid who tattles to the teacher in pre-school

THE NATIONAL HEALTH SERVICE is free, so it is also rationed. But in some parts of the country, including St Austell on the Cornish coast, access to the rationers is itself now rationed. “You can’t book an appointment to see me here,” explains Stewart Smith, a 39-year-old GP, one of a team in charge of an innovative new medical centre. “You go on a list and then we triage you.”
It is an approach that will soon be familiar to more patients. Simon Stevens, chief executive of NHS England, has said that being a GP is arguably the most important job in the country. There is, however, a severe shortage of them. According to the Nuffield Trust, a think-tank, there are 58 GPs per 100,000 people, down from 66 in 2009—the first sustained fall since the 1960s. Only half of patients say they almost always see their preferred doctor, down from 65% six years ago. The average consultation lasts just nine minutes, among the quickest in the rich world.

Although the NHS hopes to train and recruit new family doctors, the gap won’t be plugged any time soon. A new five-year contract to fund GP practices will eventually include £891m ($1.1bn) a year for 20,000 extra clinical staff, such as pharmacists and physiotherapists, with the first cash for such roles arriving on July 1st. To access the money, practices will have to form networks which, it is hoped, will help them take advantage of economies of scale and do more to prevent illnesses rather than merely treating them.
When the four practices serving St Austell merged in 2015, it was an opportunity to reconsider how they did things. The GPs kept a diary, noting precisely what they got up to during the day. It turned out that lots could be done by others: administrators could take care of some communication with hospitals, physios could see people with bad backs and psychiatric nurses those with anxiety. So now they do. Only patients with the most complicated or urgent problems make it to a doctor. As a result, each GP is responsible for 3,800 locals, compared with an average of 2,000 in the rest of Cornwall.
Although few practices have made changes on the scale of St Austell Healthcare, across England the number of clinical staff other than GPs has grown by more than a third since 2015. The logic behind the introduction of these new roles is compelling, says Ben Gershlick of the Health Foundation, another think-tank. The NHS estimates that 30% of GPs’ time is spent on musculoskeletal problems, for instance, which could often be handled by a physiotherapist. Another estimate suggests 11% of their day is taken up by paperwork. Doctors complain that they are overworked, and growing numbers retire early. They are also expensive: the starting salary for a GP is £57,655, whereas a physio costs around half as much.
NHS leaders hope the new workers will help practices play a more active role in their community, linking up with services provided by local authorities and charities. Each network will be responsible for a population of 30,000-50,000. The plan is that they will use data analysis to intervene early to prevent illness, and that practices will often share the new staff with others in their network.
Those that are further down the road sing the benefits of the new approach. Caroline Taylor of the Beechwood Medical Centre in Halifax says that new roles quickly show their worth. Her practice took in a “work wellness adviser” employed by the council. The adviser’s goal was to help ten people over the age of 50 with poor mental health back to work in a year—a task which she completed in just six weeks. In St Austell two pharmacists last year helped to cut more than £140,000 from prescribing costs. Far fewer staff now report that they are burnt out.
Working in a team will nevertheless require a big shift in mindset for many doctors, particularly those in surgeries that have never before employed anyone else aside from the odd nurse. One worry is that practices will end up doing what they must to get the extra funding, but little more. There are also more practical problems. Seven in ten GPs say their practices are too cramped to provide new services, and it is not clear where some of the extra staff will be hired from.
Perhaps the biggest problem is that patients have grown used to having a doctor on demand. Although those who no longer have to queue for an appointment may be happy, others might feel fobbed off if diverted to another clinician. A study published last year by Charlotte Paddison of the Nuffield Trust, and colleagues, in the British Medical Journal found that patients had less trust in the care provided by a nurse if they initially expected to see a doctor. Patients who have a close relationship with their GP tend to be more satisfied and enjoy better health outcomes than others.
But other evidence suggests that, for some conditions, nurses provide care that is as good as or better than that provided by GPs. The aim, says Nav Chana of the National Association of Primary Care, which helped develop the new approach, is therefore to use small teams of doctors and other clinical staff to replicate the sort of relationship with patients that used to be more common. Just parachuting in “a lot of people who look like doctors” will not raise standards, he warns.
The shortage of GPs leaves the NHS with little choice but to try something new. “A lot of the world has either copied or is trying to copy English primary care,” in particular its openness to all and the continuity of care that it provides, says Dr Chana. Keeping these strengths, while changing how primary care works, is the task NHS officials are now facing up to. Even if they succeed, it will take time for the public to adjust. Having explained the benefits of the new way of doing things, one GP pauses, before adding: “I should say, though, patients don’t love it.” ◼

Based in Staten Island with a satellite office in midtown Manhattan, the lab works with athletes from just about every sport imaginable, from college football players to the Columbia University archery team. Most of the golfers they work with are collegians as well, but as they expand — plans call for a West Coast site in the next few years — they expect to get a broader base of clients, including top pros and serious amateurs.
FOCUS ON STRENGTH AND FLEXIBILITY
For Rushi Shahiwala, a clinical Instructor at Columbia and sports physiotherapist, helping golfers maximize their potential holds a special place in his heart … and in his once-overworked muscles. “When I was young I started hitting the golf ball without knowing much,” he said from the lab’s headquarters. “A friend took me to the range and I started having some back issues and inflammation. Then I got into the physical therapy field and learned biomechanics. The technology and knowledge changed my life. I was just swinging with-out training from a golf pro; I was using my back instead of my right hip and other muscles. It screwed me up and caused me to get into more detail.”

That process is one hour to and hour and a half long. Once we identify objectively where the athlete is strong, weak and at risk for injury, we develop a training program specific to the athlete that fit with their goals. We look at their body composition — their muscles compared to their fat and their water content … what segments of their bodies are leaner than others. It’s a baseline. Via training or activity, we monitor the changes. After that we go into strength and flexibility.”
Those two words are familiar to any modern-day golfer who is at all clued in to the basics of round-ready fitness. Shahiwala explains why in physio-geek detail, and how he and his colleagues turn the words into action. “We know that to be a good golfer and hit the ball more yards, you have to have a certain degree of motion in the thoracic spine, lumbar spine, hips, ankles. First thing we do is strength and flexibility motion analysis, starting from the big toe, the ankle, proper movement in the hips and lower back, plus all the major muscles required for the swing — latissimus dorsi, quadriceps, hamstrings, calves. My background is as a physical therapist, so I’m familiar with what muscles to look for and what we need for golfers — rotator cuff activation, hip muscles, knee muscles. We check them the old-fashioned way, physically, just to get a basic idea of where they’re lacking. After the body composition, I briefly look them over to see if there’s a missing component — tightness somewhere that we should avoid. The third thing we do is 3D motion analysis and surface EMG (muscle response analysis/electromyography).”
THE PICTURE OF POSTURE
Added Greene, “The 3D analysis gives the sports scientist some X-ray vision to track joint angles and functional movements. This includes the Y balance (mobility, motor control and function asymmetries that could be a factor in increased risk to injuries and efficiency of movement). We look at posture — rounded shoulders, hip rotation, flat feet that cause the knees to turn in and down. We get an idea of capabilities and propensity to injuries. In some cases we’ll look at the golf swing in 3D analysis. This is where Surface EMG comes into play; we look at muscle activity during the motion. We can see if certain muscles are firing in the proper sequence, what’s overactive and underactive. We correspond that information with the joint angle, head movement, spinal rotation — making sure everything’s firing in the proper sequence according to our research.
Shahiwala says that research on just what that “proper sequencing” is reveals, at least so far, three protocols. “ For instance, we know that from the backswing through the follow-through, a lot of action needs to come from the hips, the glutes and the spine, so through the surface EMD we can see if the muscles we need to work more are working in the proper sequence. Some golfers with injuries use the hips the wrong way — swing them forward or sideways while other muscles fire more. We can easily capture these things and use them for their benefit — let them know they’re firing the wrong the muscle group, train them correctly and prevent further injury.
“Posture is the number one thing golfers don’t keep in mind. They think that once they work on their hips and back, everything else falls into place. But they have rounded shoulders, which puts their scapula, or shoulder blades, in the wrong position. That eventually affects the firing pattern of shoulder stabilizers — they’re interconnected with the rotator cuffs, which are very important when you’re doing any kind of hitting, impact or follow-through. So if your posture is bad, there’s a very high chance of injury due to poor rotation in the thoracic spine [chest area], and poor control of the rotator cuffs.”
Once the Science Lab team develops a clear picture of one’s muscular deficits and needs, they move on to the electric impulses behind every human movement — the nerves.
FINDING THE RIGHT RHYTHM
“The last aspect of our testing is neural efficiency — timing, rhythm, coordination,” Greene says. “It combines principles of music, sports and neural elasticity to identify inefficiencies. We test them with and without distractions. When we identify where they are in terms of timing, and incorporate that into training. We find that certain sports have trained inherent characteristics within the athlete — they have good timing — then you have the outliers who have poor timing. From our research and experiences, we notice that after they’ve been on our neuro-cognitive trainer, their ability to control emotions, maintain consistency of mechanics, coordination, focus and rhythm improves.”
But, just as with muscles, the nerves need a warm-up, and we’re not talking three cups of coffee. At the Science Lab it means some quality time on what’s called the Huber Motion Lab, which utilizes a technology called neuro-priming. The subject wears a device that improves the connections between the brain and the muscles. The Huber is the only device in the world that trains 180 deep core and spinal muscles; the majority of professional sports teams own one now. “Athletes are experiencing 13 percent more gains by using the neuro-priming device,” Greene reported.
The next step in the protocol involves what the Greene called “isoinertial training.” “Instead of lifting free weights which fight gravity, with this training you’re fighting the resistance provided by a flywheel. LONG-HAUL SUCCESS
So how long does it take the average athlete to travel through the entire, intensive Sports Science Lab regimen? “We have programs of 12, 24 or 36 sessions,” Green said. “It depends on the athlete and what they’re preparing for. We see results very quickly and it’s a testament not only to our staff but to the equipment giving us the information on balance and weakness, so it takes the guesswork out of training for us.”
Once the program is completed, Shahiwala says that his team gives clients “the full flexibility and strength training program to do at home. We also have a rehab center; if a golfer comes in with carpal tunnel issues, wrist injuries or back injuries, we have a chiropractor and physical therapy. We have a big recovery component here as well, plus cryotherapy to help with pain reduction and healing.”
So does all this high-tech training at Sports Science Lab replace your favorite teaching pro back home? Heck no, Shahiwala said; their job is to complement what happens on the practice tee. “We don’t interfere with golfers’ swings as far as technical detail,” he said. “Though through our analysis we know how the golf swing should work, we don’t work with their stance, tech-niques, clubs — we aren’t trying to take that away from pros, we make it clear we’re there to help them and their students.”

Brain Training With Sound
Training one’s timing is an important but often neglected aspect of performance. This protocol utilizes music elements such as the metronome to prime the brain and improve movements and performance by improving the signals going from the senses (hearing, vision, touch) to the brain and the subsequent responses (sensory-motor skills) such as driving the golf ball and timing the swing.

How dysfunctional breathing can affect your performance and pain
Many clients wonder why physiotherapists often work on breathing as part of your recovery or rehabilitation. Improvement in the mechanics and pattern of your breath can have a major impact on your performance during activity, minimize tension around the neck and shoulders, and help with muscular stability during exercise. In fact, several studies have demonstrated that having better control and efficiency with breathing can minimize nervous system and stress responses that alter pain.
Notice what happens when you take a breath in. Is one area of your rib cage expanding more than others? Many dysfunctional breathers will have their chest rise first, shoulders elevate or muscles around the neck tense up. These people are using their accessory muscles as opposed to their diaphragm to help drive breathing. As a consequence, more energy is expended, a shallower breath is taken and over time chronic tension can build up around the neck, shoulders and chest.
Before starting any exercise, I make sure that my clients can breathe three-dimensionally. Taking from principles in clinical Pilates- this 3D breathing means more than just a belly breath. Few people have issues breathing into the belly, but expanding through the side and lower portion of the ribcage is where many struggle. One needs to be able to expand the ribcage front to back as well as side to side. The abdomen should expand slightly, lower ribcage moving out to the sides and back filling up from the bottom of the lungs first where gas exchange is more efficient. There should be minimal engagement of the muscles around the neck, and shoulders should stay relaxed.
Depending on the type of exercise you are doing, timing is also important. For example with running, I typically recommend a 2:2 pattern-inhaling for 2 steps and exhaling for 2 steps. Timing your breath with your stride can help to maximize oxygen intake, prevent getting side stitches (thought to be caused by diaphragm spasms) and improve endurance. With resistance activity or weight training, inhaling during the less strenuous part of the exercise, and exhaling during the more strenuous part of the movement can help to engage the abdominals and keep proper form during your workout. During stretching or slower moving hatha yoga classes for example, slower, deeper breaths can aid in relaxing musculature and move deeper into a pose or stretch.
Breathing takes practice! Most of us are unaware of our breathing at any one time and proper breath patterning requires conscious effort and practice before it becomes automatic. Start by lying on your back and bringing your hands around your lower ribcage, think about breathing into your hands first before your chest rises. If you have a hard time knowing whether you are overworking in your neck and chest, try holding a light resistance band around your lower ribcage with one hand (using your breath to resist against the band) and place your opposite hand on your neck and chest to monitor for unwanted activity.
