Want to live to a healthy old age? Here’s what a top doctor does based on hard science

Who among us wouldn’t want to live a good life well into our 90s, especially if we could do it disease-free while still being as witty and spry as Betty White, as eloquent as Toni Morrison or as civically engaged as Jimmy Carter?
But you’d need good genes to live that long, right?
Not necessarily, according to Dr. Eric Topol, founding director of the Scripps Research Translational Institute in La Jolla, California, and former chairman of cardiovascular medicine at the Cleveland Clinic.
For almost 20 years, Topol has been studying a group of 1,400 Super Agers, or people who reached the age of 80 and beyond without developing one of three common, chronic, age-related conditions (cancer, heart disease and neurodegenerative disease), as part of the Wellderly study.
To his surprise, when his team conducted whole-genome sequencing on all the Wellderly study participants, the researchers found nothing genetic, across the board, that conferred an age and health advantage.
Topol’s new book, “Super Agers: An Evidence-Based Approach to Longevity,” explores the other factors at play and the science that backs them up. He examines lifestyle factors such as diet, exercise and sleep. He also looks at new technologies including the following :
All these technologies show promise in helping more people, if not reach Super Ager status, then achieve a longer health span by either delaying the onset of disease or aggressively treating conditions to prevent progression and complications.
Topol, who will soon turn 71, practices what he preaches.
“I hope to be (a Super Ager),” he told CNN Chief Medical Correspondent Dr. Sanjay Gupta, recently, on his podcast Chasing Life. “If I can get another 10 or 15 years without an age-related disease, that’d be great. And I’ve really doubled down on the things that I can do to help get there.”

“I made extensive changes from the research, once we learned that it isn’t in your genes … to achieve healthy aging, free of cancer, free of cardiovascular disease and free of neurodegenerative disease,” Topol said.
Lessons taken to heart: exercise, sleep and diet
One of the biggest changes Topol said he made was to his exercise routine. “I used to only do aerobic exercise, and as a cardiologist, that was my thing,” he said. “Now I do balance training and strength training (as well). It’s an extensive difference. … And so, I’m more strong and fit than I’ve ever been.”
In fact, exercise is so important that Topol ranks it as No. 1 on the list for anyone who wants to get started on the path of a longer life.
“It’s free. And if you’re not doing anything, if you’re a sedentary, then start with walks, brisk walks if you can, and keep increasing it,” he said. “Challenge yourself (with) hills and pace … if you are working up a sweat, that’s a really good sign.” (Check with your doctor before starting any new exercise routine.)
To get your upper body in shape, he recommends resistance bands, and to improve balance, he said to practice standing on one foot, then the other. “These are all free or cost minimum,” he said, noting that with a few inexpensive items you can do this at home, no gym membership required. “So work on that and then other things will start to fall in place. … If you exercise more, you’ll sleep better, too.”
Which brings us to the next major change Topol made.
“I realized how important (sleep) is because of ‘glymphatics.’ That was a new insight in the book: about deep sleep and getting these waste products out of your brain,” he said, referring to the recently discovered glymphatic system, which helps flush out waste products in the brain and central nervous system, much like the lymphatic system does for the rest of the body.
“I was a poor sleeper,” he confessed, so he worked “extensively” on improving the different factors that could impact it. “Things like … what do I eat? And when do I eat? Also hydrating, mostly early in the day … to have less interrupted sleep.” He added that for him fasting is “detrimental” to his sleep, as are both exercising or eating a big meal too late in the evening.
And, do not give short shrift to sleep regularity, Topol advised. “I used to go to sleep all over the place (in terms of time), and now I’m very regimented, except for maybe one night a week,” he said, noting that this tweak was “phenomenally effective” in levelling up his sleep game.
As for diet, Topol (who hasn’t touched red meat in 40 years but does eat fish) said he wasn’t surprised the evidence showed that a plant-based diet with enough lean protein “prevailed as the best.”
“What was interesting, though, is how toxic (and) pro inflammatory ultraprocessed foods were,” he said, adding that he does everything he can to minimize UPF — which he jokingly calls UFOs, or alien food . “It’s hard to get it to zero, but I’ve (made a) conscious effort.”
Topol said he also increased his protein intake “some,” due to his age and because he started strength training, “but I don’t go to toxic levels that are advocated by others, that have no evidence (and) that are pro-inflammatory.”
He said he keeps protein to about 90 grams a day for his 180-pound (82-kilogram) frame, by adding nuts, legumes (including beans) and more fish, even canned tuna in water, to his diet.
“I generally eat a salad every night,” he said. “Now I’m a little more conscious about having something with it. … But (it’s) not the protein craze that’s being advocated by some of the extremists.”
Supplements, alcohol and stress
What about supplements and vitamins? “I don’t rule out that they could help (if) you have a specific deficiency — if for some reason your vitamin D is very low, or your vitamin B12,” Topol said.
But “for healthy people who are not symptomatic (and) who take good care of themselves,” he said, “there’s no data to show that these provide any benefit” much less an antiaging one.
“I think they’re a waste of money, and some of these supplements have potentially adverse effects,” he said. “It’s just infuriating because they’re not regulated, so (marketers) can say anything, and people believe it and buy it.”
When it comes to consuming alcohol, Topol said he keeps his drinking “light,” to fewer than seven drinks per week, after which the risks start adding up quickly. He said it doesn’t seem to affect his own sleep, but it might impact that of others, so he recommends watching out for that.
Another top-of-the-list item: mental health and stress management. Topol mainly turns to exercise to help navigate both of these elements, but he said being out in nature is another often-overlooked route.
“The nature prescription is part of the book. There really is something to this,” he noted, adding that he takes advantage of San Diego’s climate and location to do plenty of hiking and walking. “It’s good for mental health, it’s good for stress and anxiety, just to be out in our environment.”
It’s never too late to start any or all of these lifestyle adjustments, Topol said.
“These diseases take 20 years at least (to develop). So, you get even more (bang) if you start in your 40s and 50s or 60s … but if you start when you’re 70 (or) 80, you’re still going to derive years of healthy aging, maybe not quite as many, but still,” he said.
And because these common, chronic, age-related conditions — cancer, heart disease and neurodegenerative diseases — do take so long to fully manifest, Topol said we have time to intervene to change their trajectory.
Medical technologies gaining traction
But to fully leverage that latency period before the symptoms of a disease appear, it’s important to be aware of personal risk, so people and their doctors (and society more broadly) can focus prevention and early detection efforts on the right people at the right time.
“Knowing one’s risk — that’s the next part, which is: we’re going to prevent these three diseases in the future. We can now, and we’re going to even get better at that,” Topol said.
Topol said he learned from his polygenic risk scores that he’s at high risk for coronary artery disease, despite not having a family history. So, he has focused on getting his LDL cholesterol down. “I’ve gotten it low, and perhaps I got more aggressive about that. I’ve checked my other lipids, like Lp(a) (lipoprotein[a]), so I feel like I’ve done everything I can to go into high prevent-mode for that.”
Topol said wants to get his immunome (all the genes and proteins of his immune system) mapped, to measure how well his immune system is functioning. It’s a measurement he believes will one day soon be commonplace for everyone. He already has data on “every virus I’ve ever been exposed to and every autoantibody that I have.”
He also plans on having his organ clocks measured, because different organs (such as the heart and brain) age at different rates, putting people at higher risk for certain diseases. “I think it’s going to be very helpful, because then I’ll know (how) my heart organ clock correlates with my risk of heart disease.
“So those are some of the things I’ve done and/or I want to do,” he said.
“It’s an extraordinary time in medicine, because we have a path to prevention,” Topol said, “because of the way the science of aging (is developing), with these metrics like organ clocks and protein biomarkers and our genetics — all these layers of data — we are going to be able to prevent these diseases. And that’s exciting. We’ve never done that before.”

Herd immunity occurs when enough people have immunity so that most infected people cannot find new uninfected people to infect, leading to the end of the epidemic/pandemic. The Great Barrington Declaration (GBD) was premised on the idea that the mass infection of unvaccinated “not vulnerable” people would end the pandemic. For it to have “worked”, 330 million Americans would have had to determine whether they were “vulnerable” or “not vulnerable” and then been willing to play their assigned role. That didn’t happen. In a previous article, I explained that for that reason, we don’t have to wonder if the GBD could have worked, in the real world it failed. However, this was not the only thing what actually went wrong with the GBD.
With this in mind, let’s turn to the concept of herd immunity.
What is herd immunity?
Herd immunity occurs when enough people have immunity so that most infected people cannot find new uninfected people to infect, leading to the end of the epidemic/pandemic.
That reasonable statement comes from the GBD itself. Even though SARS-CoV-2 was less than a year old when the GBD was written, its authors were certain that the simultaneous infection of 230 million unvaccinated, “not vulnerable” people would lead to herd immunity, protecting more “vulnerable” people. They said:
When herd immunity is reached, they (vulnerable people) can live normally again with minimal risks… If focused protection is used, it will likely only take 3 to 6 months.
In 2020, the GBD used the universally accepted definition of herd immunity. They discussed it with regards to new infections and the risk the virus posed to those without immunity to it. Herd immunity occurs when enough people have immunity so that most infected people cannot find new uninfected people to infect, leading to the end of the epidemic/pandemic. This wasn’t controversial at all. Everyone agreed with it.
While the GBD’s definition of herd immunity wasn’t in dispute, many people wisely doubted whether natural immunity alone could lead to herd immunity in under 6-months. Because babies aren’t born with natural immunity, humanity never achieved herd immunity to any virus through infections alone. Only vaccines can lead to herd immunity. Prior to the vaccine, for example, measles cases would rise and fall every couple of years as a new crop of newborns were infected. Herd immunity didn’t arise until the vaccine arrived and measles cases plummeted. There have been 214 cases this year, which is 214 too many, but thanks to herd immunity from the vaccine, measles cases are not rising exponentially. We will lose herd immunity if not enough children are vaccinated.
It is also not possible to reach herd immunity for a virus/vaccine that does not create durable immunity. Unfortunately, neither SARS-CoV-2 nor the vaccine does this, and reinfections were discussed by doctors in the mainstream media as early as September 2020, specifically in response to Dr. Scott Atlas’s pleas for herd immunity through mass infection. If every word they utter is automatically believed, because they’re medical, the deal is sealed—ESPECIALLY if some members of this network are reporters and editors.
Here’s a name for you. She was a member of this network. Julie Gerberding. Former director of the CDC. Then she went to work for Merck.
There’s Dr. Anne Schuchat. Deputy director of the CDC. She was right there, responding to COVID.
Dr. Richard Besser. Former CDC director. Went on to work as a major health reporter for ABC News.
Dr. Sanjay Gupta. Not an official member of the network, but he works closely with it and then splashes reports to the public from CNN.
Dr. Tom Frieden. Former director of the CDC and commissioner of the New York City Health Department.
The network is a quiet brotherhood and sisterhood inside the CDC.
It’s long-standing.
It’s called the Epidemic Intelligence Service (EIS).
Try to find a COMPLETE list of current and former members published by the EIS.
I can’t. I’ve looked.
A federal agency keeps its comprehensive list of members and alumni secret? Members who are doctors? Doctors who receive checks based on taxpayer dollars?

Headline on the CNN website Monday: "Dr. Sanjay Gupta: Cannabis is having a senior moment."
Beneath it, celebrity sawbones Gupta reports that more and more of us old people are discovering pot's medicinal versatility.
In December 2018, when Michigan legalized the use of recreational marijuana for adults, I acquired a new hobby.
As somebody in an on-again, off-again relationship with sleep, I had, while investigating my options, read about cannabis as an alternative to sleeping pills, which I've always avoided.
A neighbor of mine, who was ahead of the curve on this, offered me some seeds. Hmmmm ... an opportunity to experiment ... and I always have enjoyed watching things grow.
I did some research, bought some equipment, started a dozen plants under a grow-light in my basement, then transferred them to a sunny patch of ground once the threat of frost had passed.
To my surprise, my crop flourished, but that was only the beginning of my education in cannabis husbandry. I still had a lot to learn - about which plants to nurture and which ones to execute; about THC and trichomes and pollination; about harvesting, drying, trimming, decarboxylation; about making cannabis-infused butter for the cookies that do, indeed, help me sleep.
Gupta, who was originally skeptical about the use of marijuana as medicine, wrote: "I would’ve never imagined a time when I would openly recommend cannabis to my own parents – or a time they would openly consider taking it."
That time has come. But don't expect an endorsement from Big Pharma; according to one estimate I read on the Internet, drug-makers lose about $10 billion every time a state legalizes pot.

What Is A Flavor House And How Do They Make Food Taste Great?
The words "flavor house" conjure images of a buzzy café dishing out international cuisine bursting with aromatic spices, sizzling seafood, and flavorful sauces and dips. Exotic music drifts through the room, and conversation flows from table to table — but in reality, a flavor house is nothing even close to that. A flavor house is a food-industry term that, at first glance, seems clinical and calculated, like a science lab but with edible specimens.
In many ways, that's exactly what it is — a place where "flavor chemists" determine how to enhance the taste of packaged food products before they go to market. But don't worry, lab-created food isn't quietly taking over grocery stores and infiltrating our kitchens. Flavor houses have been around for decades, ever since processed and preserved foods became commonplace in Western dining. In fact, they impact the foods we consume on a daily basis and there's a good chance you've already eaten the fruit of their scientific labors.
Natural and synthetic flavor creation
In the simplest terms, flavor houses are commercial food laboratories in which flavor scientists, or "flavorists," create a specific flavor to improve or define the taste of a food product. Heavily processed foods, which vastly populate the Western food supply, can lose flavor as they're baked, fried, and frozen, according to Kim Juelg, a senior flavorist and organoleptic scientist at Givaudan, the world's largest flavor house, who explained the process to Dr. Sanjay Gupta at CNN.
A primary example of how flavor scientists influence the taste of food comes in the small print on a product label, one that says something like "contains natural and artificial flavoring." Those flavors can come from natural sources or synthetic ones, and the goal is either replicating an existing flavor or creating an entirely new one. Sometimes, flavor houses also step in to meet a sourcing need, such as when a natural ingredient is in short supply or is prohibitively expensive for general consumers.
For example imitation vanilla or "maple flavored" syrup can be substituted when vanilla beans or maple tree sap is hard to obtain. Flavorings can also help improve the taste of low-fat foods. The Senopsys corporation, in describing flavor creations, defines flavor as referring to "all tastes, aromas, mouthfeels, and textures of a product." A professional formulator blends chemicals, natural extracts, and reaction products to create the desired effect.
How widespread is the practice?
Flavor houses deal in proprietary trade secrets across the food industry, even though only about 1,000 flavorists exist worldwide, dealing with thousands of aroma compounds for blending and flavoring your food. Professionals in the field typically start as chemists or food scientists, then follow a seven-year path of apprenticeship and testing to receive accreditation. If the idea of this food intervention keeps you up at night, rest assured.
Even though a single flavor typically contains 25 to 30 compounds, the flavoring in a product usually doesn't exceed 1% of its total volume. And the FDA evaluates food seasonings to determine whether to classify them as GRAS, meaning "Generally Regarded as Safe." The agency points out that every consumable item, whether fresh from Mother Nature or combined with other ingredients, contains chemical compounds influencing how they taste, look, and feel, as well as what kind of nutrition they provide to human bodies. Flavor houses just add more to the equation.

Medical myths are surfacing again. 5 tips to inoculate yourself against them

(CNN) — If you consume any kind of media, traditional or social, at some point you have encountered a health claim that emits a whiff of something being “off.” Maybe it’s the unsubstantiated reasons for a growing measles outbreak, the supposed lack of efficacy and safety of certain vaccines despite studies proving otherwise, or the urgent need to start consuming a particular supplement for hard-to-prove health benefits.
Such claims are nothing new: Self-proclaimed health prophets and doomsayers have been around for a long time, promising to cure serious conditions, espousing medical conspiracies or trying to garner attention by sprinkling doubt and panic.
Medical misinformation (if not downright disinformation) spiked during the pandemic and continues to surface with some of the Trump administration’s controversial nominations and confirmations to top public health posts.
Laurel Bristow, an infectious disease researcher at Emory University in Atlanta, started fighting misinformation and disinformation during the pandemic. Her Instagram account — formerly KingGutterBaby — focused on combating rampant myths and conspiracy theories that were circulating online at the time.
“I would be at the hospital all day, and then I would come home and sit in my kitchen and set up my little tripod and record videos,” Bristow told CNN Chief Medical Correspondent Dr. Sanjay Gupta recently on his podcast Chasing Life.
“It felt very important to me at the time because I had such a close understanding — or as much of an understanding as you could — about the virus and what was happening and infectious disease in general, that that gave me a lot of comfort,” she said. “And I wanted other people to understand it so that they could make decisions and feel empowered in the decisions that they were making rather than scared of what was happening.”
Bristow called information empowering. “(The situation is) not necessarily less scary, but it’s less anxiety-inducing, and you can feel a little bit more in control of what is happening,” she said. “And I think that is really helpful for people to stay grounded.”
Today, Bristow continues her work in infectious diseases as the associate program director for the Emory/Georgia Tuberculosis Research Advancement Center. And she is also the host of “Health Wanted,” a health podcast on Atlanta’s NPR affiliate, WABE.
And she’s still providing accurate information and debunking false claims on social media. (Her new Instagram account has a less colorful name.)
“There’s part of me that’s like, ‘Man, after five years, I thought I was going to be off the hook,’” Bristow said. “But I do feel very passionately about this. I feel very passionately about people getting accurate information and feeling empowered in the decisions that they make for themselves and their families. So, I’m going to keep doing what I do.”

How can you determine if something contains misinformation or disinformation? Bristow has these five tips to help you evaluate claims.
Don’t believe everything you read, hear or watch
“If something seems too good or too bad to be true, it probably is,” Bristow said in an email, noting that legitimate gaps in knowledge increasingly feel like they “are being filled with speculation.”
And even established facts “are being overridden with more hysterical claims and conspiracies,” she said.
“People should be encouraged to think more critically and evaluate their information sources,” she said.
Don’t fall into black-and-white thinking
“Science rarely speaks in absolutes,” Bristow said.
“If someone is presenting something as a cure-all or as ‘this is killing you,’ it is rarely that simple.”
She recommends asking yourself: Are the limits of what’s known about a situation clearly stated? Is the level of risk being put into proper context? And is the person behind the information being transparent about their qualifications?
“Someone who presents themselves as an expert without making their credentials, relevant research, or experience available to you, should give you pause,” she said.
Don’t let your buttons be pressed
“Emotional responses are a good way to get engagement,” Bristow noted.
“Untrue medical claims often highlight the danger of something and the risk it poses to you and your family,” she said. “These claims often speak in absolute or definite terms, but science is always evolving, and risks change based on nuanced circumstances.”
Instead, she said, dig deeper into information that makes you panic. “Try to find out if the things they are talking about are based on a large, well-designed study, or if it is based on anecdotal experiences,” she said.
Don’t fall for sales pitches
“If someone is telling you about an issue you didn’t know you might have, AND offering to sell you the solution, that’s a red flag,” Bristow said via email.
“You should investigatewhere the information is coming from,” she said. “Does the person or organization that is presenting this information have a conflict of interest when it comes to their funding or partners?”
If so, the bar for buying into a cure for a new condition should be raised.
Look for real experts
“Take the time to find science communicators you trust and see what they say about breaking issues,” Bristow said.
“If there is a splashy headline or news, they will likely talk about it,” she said. “A large part of science literacy is helping people understand the limitations of knowledge, which can help them spot bad science.”
Spotting bad science and addressing it before it spreads, she said, can have a huge impact.
