All the little eye accessories (eyeccessories?) we tend to give to Elias are on the front side of his clothes. For example eye pin, eye tie, eye buttons...

But if he wanted to include any eyes on his outfit (eyefit?) wouldn't he wanted some eyes to watching his back as well?

Big eye on his back probably wouldn't be very subtle... But maybe little eyes at the back of his shoes?
