Beware of Burnout: A Danger You Face No Matter Your Job
Psychologist Herbert Freudenberger first coined the term “burnout” in the 1970s to describe the stress and emotional depletion he saw people experiencing in helping professions, such as medicine. Today, it is widely recognized as a legitimate medical disorder, and one that can affect anyone at work, in any profession. (1, 2)
In fact, a recent Gallup survey found that about two-thirds of full-time workers experience occupational burnout. Of the nearly 7,500 employees across a variety of fields who took part, 23 percent reported feeling burned out at work very often or always, while an additional 44 percent reported feeling burned out occasionally. Participants cited reasons from an unmanageable workload and unreasonable deadlines to a lack of clear communication and support from their managers. Add to these the fact that today we can, and are sometimes expected to, work away from the office almost 24/7 via phones and laptops, and it’s not hard to see why the phenomenon stretches across sectors. Feelings of cynicism, along with frustration and anger, that lead to detachment from your job
A sense of ineffectiveness or failure
To be sure, burnout has negative consequences in our professional lives; those who are burned out are much more likely to take sick days, perhaps missing pay, for example. (6) But also not surprisingly, the effects of burnout extend far beyond the office door.
Mounting scientific evidence shows that burnout can leave a profoundly devastating mark on the body and brain—to the tune of between $125 and $190 billion in healthcare spending annually in this country. (7)
Research notes that it can lead to ruined personal relationships, anxiety and depression, and substance abuse. Burnout has been found to be a significant predictor of type 2 diabetes, high cholesterol, coronary heart disease, gastrointestinal issues, respiratory problems, and even death in those under the age of 45. And recent studies suggest that it can literally be too much for our minds to handle, rewiring neural circuits that make it hard for us to cope with stressful scenarios and eventually leading to distinctive changes in brain anatomy and cognition. Are You in the Healthcare Field? Why Burnout Hits You Hardest
Although it can affect anyone in any field, it does appear that if you work in a helping profession, as Dr. Freudenberger noted, you’re more likely than others to experience burnout. And it seems physicians, in particular, are hit especially hard by the syndrome.
Research suggests that more than 50 percent of doctors now experience burnout, with some literature proposing even higher numbers, between 70 and 80 percent of the physician population. (12, 13)
Why are practitioners so burned out, when most went into the profession with a zeal to help people? Many conventional physicians today deal with common problems like high patient volumes (upwards of 25 a day), long working hours (up to 70-hour weeks)—thanks in part to time-consuming electronic medical records upkeep and navigating the insurance maze—and the general stresses of the profit-driven, increasingly litigious, and often hostile environment of our medical system, which can leave them feeling like just another cog in the machine.
Does this describe your working situation? If so, you likely know how much of an impact these factors can have on your health and your home life, and that of your colleagues. Among physicians, burnout manifests in disproportionately high rates of substance abuse, depression, and suicide or thoughts of suicide. (14, 15, 16)
You likely also know the impact practitioner burnout can have on patients. Not only is burnout causing practitioners to leave the field in droves—the country is expected to face a shortage of up to 100,000 physicians in the next decade or so—but it’s causing those that do stay to make medical errors, including errors that have fatal consequences. (17, 18)
Is It Burnout or Something More? Understanding Moral Injury
If you’re a practitioner experiencing the symptoms of burnout described here but you don’t feel like the term really resonates with you, I get it. And you’re not alone. Many physicians feel the concept of burnout suggests a failure of resourcefulness and resilience on their part, yet I can attest that my colleagues are some of the most resilient people I know—and I’m sure you are, too. As medical practitioners, you’ve survived decades of intense education, complex on-the-job training and demanding full-time work.
It’s been suggested that “moral injury” is a more apt description for physicians feeling exhausted by and ineffective in their jobs. (19, 20) This term was first used to describe soldiers’ responses to actions during war that challenged their ethics. For practitioners, it conveys the feelings that arise from not being able to provide the high level of care doctors aspire to. If you’re like most physicians, you entered medicine believing it was your calling. Not being able to fulfill that calling and care for patients the way you envisioned in this modern, high-pressure medical environment is quite damaging.
Here’s How to Avoid Burnout at Work
So, you’ve read this far and you’re closely identifying with the characterizations of either burnout or moral injury. You’re totally exhausted and dragging yourself to work, feeling disillusioned, and you’re lacking the energy to be consistently productive—which means your performance is likely suffering. You might even have gastrointestinal upset or feel as if you’re constantly sick as a result of all the stress. Now what?
For practitioners and other professionals alike, those feelings don’t have to be inevitable. Here’s how to avoid burnout at work so you can find health and happiness in your professional and personal life again.
Be Mindful
Research has shown that mindfulness, including mindfulness meditation, can reduce burnout among nurses and physicians as well as other professionals. (21, 22, 23) How? Practicing mindfulness—even for a short time—increases positive emotions while reducing negative emotions and managing stress. It helps us tune out distractions and improve our ability to focus. And it enhances our relationships, makes us feel more connected and relaxed, and boosts our compassion for ourselves and others.
Mindfulness means being aware of your thoughts, feelings, bodily sensations, and surrounding environment on a moment-to-moment basis. It means paying attention to what is, rather than getting lost in thoughts about the future or the past. Yet the ability to control where you direct your attention is fundamental to optimal performance in whatever type of work you do. I’ve done five- and even 10-day digital detoxes in the past, during which I completely unplugged from all tech, and I can attest to how much they can boost your mood and help you regain focus and clarity. You don’t have to go that long. Even just one day or a few hours a week can be helpful. Set an email autoresponder and spend the time you’ll gain meditating, engaging in physical activity, and otherwise reconnecting with yourself and your family and friends.
Practice Good Work Hygiene
Don’t spend too much time sitting (try a standing desk or even treadmill desk) and take frequent breaks during which you step fully away from the computer (I use the Time Out app for reminders).
Prioritize Sleep and Exercise
When work consumes our lives, we can forget these basics. But don’t underestimate what adequate sleep (at least seven hours) and regular physical activity can do for your productivity, energy level, and outlook.

Before you say, But Chris, that’s not possible, hear me out. There are ways to work more efficiently so that you can work less but still get the same amount done. Batching your emails, automating repetitive tasks, and delegating can all help.

Practitioners: Make Time to Get Together with Your Colleagues
Healthcare can be very isolating. It’s common for practitioners to spend most of their time working directly with patients instead of interacting with colleagues on a personal level. That can lead to a sense of loneliness and, eventually, burnout. One way to help alleviate that is to find the time to get together and share a group experience with your coworkers.
Work retreats can serve that purpose. They provide the opportunity to deeply connect with your coworkers, talk about the issues you face professionally, and solve problems together in an informal, non-work setting.
I’ve taken part in meditation retreats for around 25 years, and I’m a big believer in their transformative power. Even if you can’t find time to schedule a work retreat, just getting together with your coworkers outside of the workplace can help you feel more connected and, ultimately, more fulfilled.
Consider Making the Switch to Functional Medicine
If you’re looking for another way to practice medicine, consider a Functional approach. Functional Medicine aims to uncover the root causes of poor health. It’s about treating those causes, rather than just covering up symptoms with prescriptions, to help patients truly heal—to make a lasting positive difference in their lives. For me, that was one of the most appealing things about Functional Medicine, and I think it’s one of the least discussed but most important gifts it can offer physicians who shift their practice in that direction. Seeing such results can help you feel successful in your work and personally fulfilled by your calling.
What’s more, Functional Medicine embraces a collaborative healthcare model. In a collaborative model, allied providers, nutritionists, and health coaches work alongside physicians to provide additional layers of support, to both doctors and patients. This streamlines the patient care process, improves the quality of patient care, and creates a better work environment for physicians. It gives doctors more autonomy and increases their professional satisfaction and quality of life, all while resulting in better outcomes for patients.
I embrace this model in my own practice, the California Center for Functional Medicine. It’s also a big part of what I teach aspiring Functional Medicine practitioners in my ADAPT Practitioner Training Program. I believe that this approach to healthcare holds the key to stemming the rising tide of chronic disease—while preventing burnout at work for practitioners.

A New Doctor
Cycle 9, Day 10
So, I now have at least a half-dozen physicians on my case. If you believe the BMJ stat that “medical misadvenure” (which is a broad category that includes, but is not limited to, doctor error, nursing error, pharmacy screw-ups, misdiagnosis, accidental overdose/drug interactions, opportunistic infections - the list goes on) is the third-leading cause of death in America (according to the same study, heart disease is #1 and cancer is #2). So, for those for those of you setting odds on my life expectancy (and, frankly, I’d be disappointed if you didn’t), it’s been an odd, extended game of “Clue,” except I’m Mr. Body, to see if disease, side-effects, or my possibly-insane physicians will get to me first. I hate to say it, but I think I’ve finally figured the odds-on favorite in this one: my GP.
This isn’t a plea for help, or even a serious medical development on my part, it’s a warning for you, the readership, as insurance enrollment comes around. First of all, if you can’t pay, hospitals or physicians can throw you out on the street (this is something able-bodied people are so disbelieving of that took a poor black woman freezing to death on-camera in Baltimore). They are only required to treat you if you in an emergency situation, thanks to some federal laws called “EMTALA.”If you have a disease that drives you to the emergency room, the prognosis gets worse. People tend believe that just because it’s the healthcare industry, the health insurance industry isn’t a corrosive force that has a vested interest in denying care and killing you. Which is odd to me; you don’t get this anywhere else (or I haven’t experienced this sort of self-delusional attitude); you don’t see people defending McDonald’s or Nabisco or RJ Reynolds or Exxon as having their best interests at heart (and, to my friends who think they’re bullet-proof because of their health insurance, read the fine print, very, very carefully; you don’t want to get a nasty shock as you’re being rolled into the OR). So, thanks to my parent’s generosity/desire not to see me die, I rolled in last year with a very expensive PPO (there are a lot of acronyms to keep track of, but PPOs allow the patient to see anyone in a preferred provider network, which tend to be large and give the patient lots of choices, so you can directly get a referral to a neurologist if you hit your head). Unfortunately, because I have pre-existing conditions (and to my bullet-proof friends, read through the list of pre-existing conditions that’ll disqualify you, your jaw will drop)(also, it’s telling that Congressmen and Senators have the option to buy into a separate, federal employee health insurance option that’s not available to us serfs)(it’s also telling that the ACA required Congresscritters, for the first time ever, to tough it out and find health insurance like their constituents)(which is why I assume all the GOP higher-ups had melt-downs over the ACA - a slight removal of privilege to help sick constituents isn’t a part of Congressional ethos, let alone job description), my premiums went from “expensive” to “leasing a sports car” within a few months. I’m extraordinarily grateful to them for providing that financial backing, because it allowed me to continue getting treatment during the crucial 6-10 week GBM post-diagnosis period that might turn this from “Guaranteed doom” to “far too close for comfort.” So, this did give me some time to do my homework (in writing about this, I’m realizing I really should consider applying to law school, because I’ll know more about medical and insurance law and ethics than some lawyers before this is up)(Hell, I probably know more than some of them right now). Anyway, I found that all the specialists I see for cancer, do take medicaid (even the specialized pharmacy I use at the cancer center). Which is good for me, especially since being on disability in California is an automatic qualification for Medicaid. Now for the bad news; although all the specialists there take medicaid, the GPs don’t. AND the specialists only take medicaid if it’s done through an HMO carrier that the state sub-contracts with.

There are a number of documentaries and documents (including an “Adam Ruins Everything” segment) on why HMO’s are unnecessary and lethally incompetent (like many other aspects of a for-profit medical system), but here’s the most basic deal: They act as a gate-keeper for the entire medical-industrial system. You can get your care at any of a dozen pre-approved hospitals, and nowhere else. Now, if an HMO or their doctors can’t treat you (or refuse to treat you - which is still the case for a lot of GBM patients), they are required to send you to a specialist who can. The economic incentive is to give less care, and keep all the patients in the system for as long as possible.
I suspect that delaying tactic is why heart disease and cancer are considered so deadly - you can’t sit long on either of those.
So, based on the financial folks at the cancer center, I picked one, and promptly forgot about it; because I’m already in the system there (the receptionists and pharmacy staff recognize me on sight)(which is comforting, until you realize it’s a cancer center, and then the panic briefly cuts in until you remember you’ve gone eight months without regowth or metastastis). I only remembered it when I got a call from the medicaid HMO telling me I should schedule an appointment with one of their physicians. This isn’t a big deal, I just need them to sign-off on any further black magic-based treatments with the Warlocks or Radiation Oncologist.
Now, before I go further, let’s talk about the people who go into medicine. Like anything in healthcare, we tend to give assume that an entire industry is moral, and just; when people go in for a variety reasons (as recently as 20 years ago, the vast majority of medical students said it was for money), and it’s worth noting that cuts across a vast majority of demographics and motives. And, for better or worse, that cuts across vast swathes of competence - for far too many folks, it’s a job - a rewarding job, but just a job. My father recently inquired about board exams and recertification as a way of guaranteeing some basic level of competence from everyone. He’s right, but the key word there is “basic.” Again, “basic” is fine for first aid and most major medical issues; it’s unacceptable if you have a disease with a 90% fiver-year mortality rate.
I bring this up because I think I chronicled my first appointment with my insurance-appointed GP five or six weeks ago and seemed perfectly satisfactory to my ongoing addiction to experimental chemotherapy. I’m certain it was within that time frame, because I had schedule a six-week follow-up. Which, sadly lands on my “week off” chemo. So, yesterday, after infusion #2 for this cycle (for those of you wondering what I’m doing to stay busy during infusions these days, well, rewriting Christmas carols for cancer patients)(”On the first day of chemo, the nurses gave to me, zofran in an IV”). I also convinced dear old Dad to take me out to lunch, because, again, when the Marizomib side effects hit, you do not fee like eating. This was in the neighborhood of the latest addition to my collection of medical people, so I thought I’d reschedule then. And was told by the receptionist to wait for everyone behind me to check in lest they be late for appointments. That would be fine, but it seems a fundamental misunderstanding of how queus work. And, any time post five-ish hours on infusion day, even though zofran might keep me from puking, it does give me an odd, oily, queasy sensation. I think I deserve some sort of gold star for not puking on this woman right away (again, if you have unconventional problems, feel free to start with an unconventional approach)(my next writing project will be titled, “Life Lessons from Necromancers”). I eventually - using the traditional method of looking down the reception counter, noticed someone not otherwise occupied, and manage to get an appointment more amenable to my schedule. For a physical.
Again, I’d love to use some four-letter words here, but even Finnish fails to meet the requirement. Now, it should be noted that, even though I’m well-aware that I’m physically Adonis-like; I am in chemo and recovering from radiation treatment, Radiation Oncologist implied a few months ago that, even though my scan was clean and looked good for someone with brain cancer, anyone unfamiliar with my case would probably freak out about them. Same thing with my abnormal, uh, “lab sample” I wrote about recently - the nurses agreed, a single abnormal test is hardly unexpected toward the end of chemo, especially since I’m now on a diet consisting mostly of protein, fiber, cafeine, and dangerous, experimental substances. However, I’d prefer not to have to point all that out to a new medical person who has the power to yank the plug on me (sadly, my original GP will be on vacation that week. (I’ll also be on Temodar, so there’s a solid chance my brains will be thoroughly scrambled and incapable of comprehension).
ANYWAY… WEIGHT: 198 lb CONCENTRATION: Pretty good, APPETITE: Normal (but this is 24 hours post-infusion. ACTIVITY LEVEL: Not great; the fatigue side effect definitely caught up with me and chewed me up last night. SLEEP QUALITY: Okay. although I’ve noticed that I definitely thrash around on chemo days. COORDINATION/DEXTERITY: Lousy. Thank Gods I don’t need the walker, and I don’t even think I need my magic ankle support, but my left leg is definitely unreliable today. MEMORY: Not bad, although I did forget my sheets were in the wash earlier today (although I recall stripping the bed and tossing them into the washer). PHYSICAL: Tired and kind of wobbly, but still a lot better than this time a year ago.. EMOTIONAL: Okay. It might just be that I spent yesterday next to my zofran-and-CDB salt-lick, but I’m starting to think I might make it through all this somewhat intact. Hang on. Am I really starting to believe my own bullshit? 3 Solutions For Surviving Healthcare Collapse In 2018
My weekly letter, Connecting the Dots, has two goals: to be informative and to be thought-provoking (subscribe here for free). Last week, it accomplished both—but not the way I expected.
Many people call the Affordable Care Act a great success that only needs minor adjustments. So when I picked the title of last week’s issue, How to Survive the Obamacare Collapse, I thought some people would be upset that I questioned its viability.
Oddly, that didn’t happen. I got more than 60 responses, and no one even tried to defend Obamacare.
Apparently, we all agree it's not working. What we mainly disagree on are the reasons—and the best strategies to fix it.
The fact is, if Obamacare isn’t available in 2018, or is so expensive no one can afford it, millions of Americans will have no health insurance.
I asked my readers for ways to reduce and control this risk, and they responded.
Here are some of their ideas.
Health Insurance Hacks
I suggested looking for short-term medical coverage if Obamacare becomes unavailable in your area. Reader Mike E., a Colorado insurance broker, added some details.
Temporary insurance will probably continue to be available since it's not under the ACA restrictions. But there are several caveats:
Not only will it not cover preexisting conditions, many of the temp insurance carriers will deny any coverage if you show preexisting conditions on your application form, even common things like moderate hypertension. Leaving such details off the application is a risky prospect, because the carrier can deny all claims if they find you've falsified information. And if they're receiving major claims, they'll probably look for a reason to deny them.
The biggest downside is the total benefit limit. As far as I know, there are no plans available with a limit above $2 million. Nonetheless, temp insurance is a good option if you can qualify.
Some readers may be in a position to change their permanent residence (e.g., to a second home). Traditionally, the choice of which to use has been based on state tax rates, but now access (and cost) of health insurance may be a bigger factor.

Food and eating habits spring from geography. For instance, tropical island cultures eat lots of fish and fruit because that’s what is handy. Subarctic cultures eat a lot of fat from whale blubber and walrus.
Here in the US, our native foods are carb-heavy grains. We eat lots of corn and wheat because our Great Plains are the ideal environment for growing them, and we have a river system to ship them.
Alas, the refined grains we eat are turned into sugars by the human body. Add to that a car-based culture where people don’t walk as much, and we’re set to gain weight.
Worse, you can be obese and malnourished at the same time: according to a 2015 Guardian article, 85% of Americans don’t consume enough of the most important vitamins and minerals necessary for proper physical and mental development.
So we develop diabetes, joint problems, heart disease, and more. And in the last decade, the costs of treating these conditions have exploded.
Quick personal anecdote: About eight years ago, I was 40 pounds overweight. What helped me was a doctor-supervised low-carb diet called Ideal Protein. Over time, it changed my eating habits, so I lost those pounds and kept them off.
Yoga helped too. Your head needs to be in the right place, or nothing will work.
If you’re struggling with weight loss, stick with it. Losing even a few pounds will help reduce your health risks.
Healthcare Options Abroad

One alternative would be to look at other countries. Particularly if you have relatives you can stay with or want to vacation in an area. Friends have had prostate and carpal tunnel surgery in Colombia at a cost less than the co-pay on their insurance. Their experience has been as good as any friends’ with similar surgery in the US.
I’ve heard of this. Some hospitals in Thailand, for example, reportedly deliver US-quality treatment at a fraction of the price. The savings are enough to pay travel expenses, too. This will no doubt become more popular if Obamacare falls apart.
