New world news from Time: COVID-19 Cases in Africa Surpass 1 Million — But the True Toll is Likely Higher
(JOHANNESBURG) — Africa’s confirmed coronavirus cases have surpassed 1 million, but global health experts say the true toll is likely several times higher, reflecting the gaping lack of testing for the continent’s 1.3 billion people.
While experts say infection tolls in richer nations can be significant undercounts, large numbers of undetected cases are a greater danger for Africa, with many of the world’s weakest health systems. More than 22,000 people have died of COVID-19.
The World Health Organization calls the milestone a “pivotal point” for Africa as infections in several countries are surging. The virus has spread beyond major cities “into distant hinterlands” where few health resources exist and reaching care could take days.
Immediately knowing they were at a disadvantage, African nations banded together early in the pandemic to pursue badly needed testing and medical supplies and advocate for equitable access to any successful vaccine. Swift border closures delayed the virus’ spread.
But Africa’s most developed country, South Africa, has strained to cope as hospital beds fill up and confirmed cases are over a half-million, ranking fifth in the world. The country has Africa’s most extensive testing and data collection, and yet a South African Medical Research Council report last week showed many COVID-19 deaths were going uncounted. Other deaths were attributed to other diseases as people avoid health centers and resources are diverted to the pandemic.
It’s all a warning for Africa’s other 53 countries of what might lie ahead. While dire early predictions for the pandemic have not played out, “we think it’s going to be here at a slow burn,” the WHO’s Africa chief, Matshidiso Moeti, said Thursday.
Just two African countries at the start of the pandemic were equipped to test for the virus. Now virtually all have basic capacity, but supplies are often scarce. Some countries have a single testing machine. Some conduct fewer than 500 tests per million people, while richer countries overseas conduct hundreds of thousands. Samples can take days to reach labs. Even in South Africa, turnaround times for many test results have been a week or longer.
“We are fighting this disease in the dark,” International Rescue Committee expert Stacey Mearns said. In addition, Africa has just 1,500 epidemiologists, a deficit of about 4,500.
African nations overall have conducted just 8.8 million tests since the pandemic began, well below the Africa Centers for Disease Control and Prevention’s goal of 13 million per month. Countries would love to increase testing if only supplies weren’t being snapped up by richer ones elsewhere.
Africa CDC director John Nkengasong said estimating the true number of cases on the continent is “very tricky.” Some 70% of infections are asymptomatic, he has said. Africa’s young population also might be a factor. Without a dramatic increase in testing, “there’s much we don’t know.”
But some experts are making their best guesses.
Africa likely has at least 5 million infections, said Ridhwaan Suliman, a senior researcher at South Africa’s Council for Scientific and Industrial Research. He believes the true number in South Africa alone is at least 3 million. The country has conducted far more tests than any other in Africa — more than 3 million — but in recent days about 25% have come back positive. Because of shortages, South Africa largely limits testing to health workers and those showing symptoms.
Experts see South Africa as an indication of what’s to come elsewhere.
Sema Sgaier, an assistant professor of global health at Harvard and director of the Surgo Foundation, thinks the number of infections across Africa could be more than 9 million. The U.S.-based Institute for Health Metrics and Evaluation puts the number at more than 8 million. And Resolve to Save Lives, led by Tom Frieden, former director of the U.S. Centers for Disease Control and Prevention, estimates it could be 14 million.
For Resolve to Save Lives senior vice president Amanda McClelland, the more worrying number is not the overall cases but the health workers infected across Africa — now about 35,000. That affects care for everyone on a continent whose shortage of workers has been called catastrophic.
Reflecting the pandemic’s diverse nature across Africa, just five countries account for 75% of confirmed cases: South Africa, Egypt, Nigeria, Ghana and Algeria. Nigeria alone could have had close to 1 million cases by now if Africa’s most populous country hadn’t acted quickly, the Africa CDC’s Nkengasong said.
Still, with insufficient testing, people live with the fear that loved ones may have had the virus without knowing for sure.
In Burkina Faso, Yaya Ouedraogo lost his uncle and cousin in April. Both were in their 70s with a history of high blood pressure and diabetes, and both had complained of shortness of breath, fever and body pain, he said.
“They had all the symptoms of coronavirus, but in certain areas no one was investigating it and they didn’t get tested,” he said.
The WHO Africa chief has said officials don’t think the continent is seeing a “silent huge epidemic,” with thousands dying undetected, but she acknowledged under-reporting of cases.
“What we’d like to see — to be able to be really confident — is higher testing rates,” Moeti told reporters last week, and she criticized the “very distorted global market” in which richer countries have the bulk of testing materials while poorer ones scrape by on just hundreds of tests a day.
Moeti also worries about a related danger for which even less data exists: the number of deaths from diseases such as malaria, HIV and tuberculosis as resources are diverted to COVID-19.
Whatever Africa’s real coronavirus toll, one South African church has quietly been marking the country’s “known” number of deaths by tying white ribbons to its fence. The project’s founders say each ribbon really stands for multiple people.
Already, the Rev. Gavin Lock wonders about what to do when the length of fence runs out. Maybe they’ll change the ribbons’ color to represent 10 people, or 50.
“It’s a work in progress,” he said.

Author Mary Parlange moved from Vancouver to Melbourne in late 2017, as her husband Marc took an academic position at Monash University. When scouting for a home in their new city, the couple bid on some contemporary houses, but ‘hadn’t really found the right place.’ After scrolling though real estate apps, Mary stumbled across the Salter House, designed by Walter Burley Griffin and his wife Marion Mahoney Griffin in 1924.
Mary describes ‘when we walked in it just felt right, like Goldilocks finding the right bed.’ Their offer was accepted and as the reality set in, Mary reveals ‘we looked at each other and said “what have we just done?” At the time, we knew almost nothing about the Griffins or the history of the house. We just knew this could become a home.’
The ‘home-making’ process was not a straight forward or simple one, with their builder Stuart McLean reminding Mary and Marc ‘you can’t open a can of worms without expecting to find any worms.’ The budget and timeline were revised countless times to accommodate the essential excavation and re-stumping of bedrooms, reinforcing of walls, rewiring, gutting and refitting of bathrooms, and all closets, built-in shelving and door hardware being replaced. Floors were repaired, sanded, stained and finished, and fireplaces were updated.
In addition to these extensive structural changes, interior walls were painted matte white, against an exterior grey. The landscaping was also completely redesigned by Sam Cox, introducing Castlemaine slate, and native planting ‘more in line with Griffin’s landscape philosophy.’
Mary describes the joy of moving through the home, where different ceiling heights create a sense of opening out, and inviting in. The iconic Griffin designed knitlock concrete tiles create a curved ribbing effect, and give the home a distinctive and timeless aesthetic.
For such a unique house, it is almost uncanny how well the couple’s furniture and artwork sits against the Griffins’ design. Beloved items include the bed in the guest room, forged in Pojoaque New Mexico, which originally belonged to Mary’s mother. The home tells a story of the family’s globe-trotting life, with pockets of folk art from their travels peppered through the property. Mary particularly highlights the combination of her parents dining table, paired with the Hans Wegner sofa and chair (restored by Angelucci 20th Century). She explains that everything fits together in this house ‘like it was pre-ordained.’
This stunning renovation was a major undertaking, ‘from the time we moved to Melbourne to when we moved into the house, we lived in seven different apartments and AirBnbs’ Mary admits. But the process has not only delivered a beloved home, Mary has also developed a new passion for the life and work of the Griffins. She explains ‘we have lived all over the world, and this has – by far – been the biggest adventure and most rewarding result.’

Songs of freedom: In jail, Burkina inmate makes first album

Known as Johnyto, the 25-year-old is serving two years for being an accomplice to robbery at an overcrowded prison known for its abysmal conditions in Bobo-Dioulasso, Burkina’s second city.
Here, he is one of 700 inmates, some of whom are looking on curiously as he works the synthesizer, the mixer and the amplifiers.
“School didn’t work out because I wanted to make music, but well, that didn’t take off … Many things happened before I wound up here,” admits the young singer, whose real name is Jonathan Sougue.
“But one day, I hope to become a real star and forget this life.”

And in March, Johnyto finally got the break he was looking for when he entered an audition run by Burkinabe reggae star Freeman Tapily who was looking for an inmate who could record an album behind bars.
In what could have been dubbed “Prison’s Got Talent” or perhaps “X-Factor Behind Bars”, four people in his jail vied for first place, and Johnyto, with his mix of rap and ballads, won.
Several months later, Tapily’s team came into the prison and set up a basic recording studio where Johnyto spent three days with them recording five songs, shooting a music video and taking a series of publicity shots.
MUSIC FOR REHABILITATION
The project is being jointly financed by the Burkinabe Office of Author’s Rights and by African Culture, an organisation owned by Tapily which invites well-known Burkinabe singers and musicians to perform at concerts inside prisons for free as a way of helping with the inmates’ rehabilitation.
“These projects are close to my heart,” Tapily told AFP.
He started the initiative in 2010 with a concert performed in front of 1,400 inmates at the MACO central prison in the capital, Ouagadougou.
“Now it’s 2018 and there are roughly 2,000 prisoners. It’s going up!” he said.
But what he really wants to see is the numbers falling — in a sign that the inmates are completing their sentences, getting out and getting on with normal lives.
“For the number to fall, we have to get on with rehabilitation. With rehabilitation, we will fight crime and repeat offences, that’s our battle,” he said.
“And in terms of rehabilitation, music can do a lot.”
“I’M SORRY”
Tapily has a lot of confidence that Johnyto can make it as a professional musician.
“Me, I make my living from music … and I think Johnyto’s talent is far greater than mine,” he told AFP.
“I think he can go beyond rehabilitation – he’ll definitely do that – but I think he could even help other people do the same,” he said, suggesting the young musician could even hire some of them to work with him.
The album, which is due to be released in December, has songs on it with lyrics in both French and Dioula, a local language also spoken in Ivory Coast and Mali.
And Johnyto has dedicated the main track – “I’m Sorry” – to his mum.
“I didn’t know how much you loved me / I’m sorry, yes mama, I’m sorry / I’m sorry to have caused you so much misery,” he sings.

Once the album is finished, Tapily’s team will burn 100 copies that Johnyto can distribute and sell himself, like most Burkinabe musicians do.
Although the prison administration itself doesn’t get involved in funding such external activities, it is happy to encourage them and to ensure the events go off as smoothly as possible.
FROM SEWING TO STOCKBREEDING
For Pascal Yabre, regional head of prison security, rehabilitation of the inmates is a top priority.
“The aim is to enable those prisoners who have been here once to leave and never come back,” he said.
Across this West African nation, prisons are now offering inmates training in a wide range of skills, from gardening to livestock farming, to tailoring, carpentry and soapmaking.
Activities vary from one place to another, but for now, only a small percentage of prisoners are involved.
“The problem is that we often lack resources, which means we don’t manage to put them on at all the prisons,” Yabre admits, citing financial difficulties at the prisons’ authority.
For Tapily, the funding shortfall is easily explained.
“We are in a poor country where everything is a priority so the state can’t allow itself to spend too much on the prisons,” he says.
“If I was president and had to choose between building a new hospital and a new jail, I would choose a hospital!”

(JOHANNESBURG) — Africa’s confirmed coronavirus cases have surpassed 1 million, but global health experts say the true toll is likely several times higher, reflecting the gaping lack of testing for the continent’s 1.3 billion people.
While experts say infection tolls in richer nations can be significant undercounts, large numbers of undetected cases are a greater danger for Africa, with many of the world’s weakest health systems. More than 22,000 people have died of COVID-19.
The World Health Organization calls the milestone a “pivotal point” for Africa as infections in several countries are surging. The virus has spread beyond major cities “into distant hinterlands” where few health resources exist and reaching care could take days.
Immediately knowing they were at a disadvantage, African nations banded together early in the pandemic to pursue badly needed testing and medical supplies and advocate for equitable access to any successful vaccine. Swift border closures delayed the virus’ spread.
But Africa’s most developed country, South Africa, has strained to cope as hospital beds fill up and confirmed cases are over a half-million, ranking fifth in the world. The country has Africa’s most extensive testing and data collection, and yet a South African Medical Research Council report last week showed many COVID-19 deaths were going uncounted. Other deaths were attributed to other diseases as people avoid health centers and resources are diverted to the pandemic.
It’s all a warning for Africa’s other 53 countries of what might lie ahead. While dire early predictions for the pandemic have not played out, “we think it’s going to be here at a slow burn,” the WHO’s Africa chief, Matshidiso Moeti, said Thursday.
Just two African countries at the start of the pandemic were equipped to test for the virus. Now virtually all have basic capacity, but supplies are often scarce. Some countries have a single testing machine. Some conduct fewer than 500 tests per million people, while richer countries overseas conduct hundreds of thousands. Samples can take days to reach labs. Even in South Africa, turnaround times for many test results have been a week or longer.
“We are fighting this disease in the dark,” International Rescue Committee expert Stacey Mearns said. In addition, Africa has just 1,500 epidemiologists, a deficit of about 4,500.
African nations overall have conducted just 8.8 million tests since the pandemic began, well below the Africa Centers for Disease Control and Prevention’s goal of 13 million per month. Countries would love to increase testing if only supplies weren’t being snapped up by richer ones elsewhere.
Africa CDC director John Nkengasong said estimating the true number of cases on the continent is “very tricky.” Some 70% of infections are asymptomatic, he has said. Africa’s young population also might be a factor. Without a dramatic increase in testing, “there’s much we don’t know.”
But some experts are making their best guesses.
Africa likely has at least 5 million infections, said Ridhwaan Suliman, a senior researcher at South Africa’s Council for Scientific and Industrial Research. He believes the true number in South Africa alone is at least 3 million. The country has conducted far more tests than any other in Africa — more than 3 million — but in recent days about 25% have come back positive. Because of shortages, South Africa largely limits testing to health workers and those showing symptoms.
Experts see South Africa as an indication of what’s to come elsewhere.
Sema Sgaier, an assistant professor of global health at Harvard and director of the Surgo Foundation, thinks the number of infections across Africa could be more than 9 million. The U.S.-based Institute for Health Metrics and Evaluation puts the number at more than 8 million. And Resolve to Save Lives, led by Tom Frieden, former director of the U.S. Centers for Disease Control and Prevention, estimates it could be 14 million.
For Resolve to Save Lives senior vice president Amanda McClelland, the more worrying number is not the overall cases but the health workers infected across Africa — now about 35,000. That affects care for everyone on a continent whose shortage of workers has been called catastrophic.
Reflecting the pandemic’s diverse nature across Africa, just five countries account for 75% of confirmed cases: South Africa, Egypt, Nigeria, Ghana and Algeria. Nigeria alone could have had close to 1 million cases by now if Africa’s most populous country hadn’t acted quickly, the Africa CDC’s Nkengasong said.
Still, with insufficient testing, people live with the fear that loved ones may have had the virus without knowing for sure.
In Burkina Faso, Yaya Ouedraogo lost his uncle and cousin in April. Both were in their 70s with a history of high blood pressure and diabetes, and both had complained of shortness of breath, fever and body pain, he said.
“They had all the symptoms of coronavirus, but in certain areas no one was investigating it and they didn’t get tested,” he said.
The WHO Africa chief has said officials don’t think the continent is seeing a “silent huge epidemic,” with thousands dying undetected, but she acknowledged under-reporting of cases.
“What we’d like to see — to be able to be really confident — is higher testing rates,” Moeti told reporters last week, and she criticized the “very distorted global market” in which richer countries have the bulk of testing materials while poorer ones scrape by on just hundreds of tests a day.
Moeti also worries about a related danger for which even less data exists: the number of deaths from diseases such as malaria, HIV and tuberculosis as resources are diverted to COVID-19.
Whatever Africa’s real coronavirus toll, one South African church has quietly been marking the country’s “known” number of deaths by tying white ribbons to its fence. The project’s founders say each ribbon really stands for multiple people.
Already, the Rev. Gavin Lock wonders about what to do when the length of fence runs out. Maybe they’ll change the ribbons’ color to represent 10 people, or 50.
“It’s a work in progress,” he said.
