sharing a head with nonhumans is so interesting.
i’ve never really cared about bats, to think about them more than distant admiration for what they do for the ecosystem. i don’t know much about them at all actually
we have twins in here who are young girls, beasts, & partially bats. it’s not befitting of them to call them littles when they’re more animal than child and denounce those sorts of labels. they’re so interesting… they are medical alters and especially love attending procedures but also just learning the ins and outs of surgery. most people might find that weird or grim or ‘bad’ but they keep our body calm and our heart rate low when we go in for surgeries. they’re very respectful about it so to speak but they’re just LOUD and EXCITED about it like little kids and overstimulated animals.
last night they were talking together for about an hour about laparoscopies and how they would want to probe ‘a body’ (a living, safe, healthy one, but with not really any thought toward the individual involved), and chittering about how excited they were seeing pictures of the inside of our abdomen, and how they want to eat lots of shredded beef and little birds.
and they could not sleep with the lights on like i usually do. it made them dizzy and disoriented. and they tossed and turned and said their skin crawled and that they couldn’t close their eyes tight enough. but in complete black with the windows open they wrapped themselves in a thin sheet, super taut around the back but a little loose around the arms, and swaddled themselves like that.
i heard bats chirp outside like i always do — but it was the first time i actually noticed it. they talked to themselves about how happy the sounds made them and briefly i felt as if bodily we were flitting about in the open air, darting after bugs. and our fingers felt sooo long, but the way our arms tucked around our body felt so correct.
it was just so weird and interesting.. i was only a passenger but i’ve never felt so viscerally like an animal that i’m just not

probably not Exactly what u were talking about, (but jsyk if you do ever have major abdominal surgery): you CAN ask your surgeon for a recording. they will take a video of it if you ask them to. my friend had major spinal surgery as a teenager and they gave her a DVD of the recording! :)
I didn't get a video, but my surgeon did print out some photos (from a laparascopic procedure) of my guts on nice paper! I plan on framing them at some point and hanging them up :)
#i knew it was laparo and asked for photos when i first met her#and she was actually so excited that i wanted to see#anonymous#the curious clown#i am all about getting photos and videos of procedures done

How bariatric surgery in India helps in treating fatty liver diseases
Obesity is a condition where excess body fat may adversely affect health.
A common tool for evaluating obesity is the Body Mass Index (BMI), which compares weight to height. BMI is determined by dividing a person’s weight in kilograms by the square of their height in meters (BMI = weight (kg) / height² (m²)).
The World Health Organization defines obesity as a BMI of 30 or higher, although the classification can differ according to various health standards.
Obesity is linked to following health issues:
1.Type 2 diabetes
2.Cardiovascular Diseases
3.Strokes
4.Certain types of cancer
5.Joint disorders
Nonalcoholic Fatty Liver Disease (NAFLD) is one such disease that can be caused due to obesity.

What is Nonalcoholic Fatty Liver Disease (NAFLD) ?
NAFLD, or Nonalcoholic Fatty Liver Disease, is a condition where excess fat builds up in the liver, similar to how fat accumulates around your waist. This fatty buildup can disrupt the liver’s function and is linked to health problems like diabetes and heart disease.

How obesity causes Nonalcoholic Fatty Liver Disease (NAFLD) ?
Obesity is a major risk factor for a serious liver condition called Nonalcoholic Fatty Liver Disease (NAFLD). NAFLD develops when excess fat accumulates in the liver, similar to how fat builds up around the waist. This fatty buildup can disrupt the liver’s normal function, potentially leading to complications like diabetes and heart disease. While the exact cause of NAFLD remains under investigation, it’s likely connected to how our bodies store and process fat. Doctors diagnose NAFLD by measuring the fat content in the liver, often using tests alongside a Body Mass Index (BMI) as a reference point. Because excess fat storage is a key factor, people who are obese are significantly more likely to develop NAFLD. Early diagnosis and lifestyle changes are crucial for managing NAFLD and reducing the risk of future health complications.
How weight loss through bariatric surgery helps in treating NAFLD?
Bariatric surgery addresses NAFLD by promoting significant weight loss, which is crucial for improving the condition. By reducing overall body fat, this procedure decreases the fat stored in the liver, easing pressure and aiding its function. Moreover, bariatric surgery can lead to hormonal changes that improve insulin sensitivity and fat metabolism, further alleviating stress on the liver. In some instances, significant weight loss achieved through surgery can also reduce liver inflammation and potentially reverse some damage caused by NAFLD.

Hernia Treatment in Pune: What You Need to Know
Hernia is a common medical condition that can cause discomfort, pain, and limitations in daily activities. Fortunately, Pune has emerged as one of the leading cities in India for advanced hernia treatment, offering expert surgical care and world-class medical facilities. Whether you're looking for a reliable hernia surgeon in Pune or seeking the best hospital for hernia surgery in Pune, this guide provides everything you need to make an informed decision.
What Is a Hernia?
A hernia occurs when an internal organ or tissue pushes through a weak spot in the surrounding muscle or connective tissue. Hernias most commonly occur in the abdominal region, and the most prevalent types include inguinal (groin), umbilical, femoral, incisional, and hiatal hernias.
Symptoms of Hernia
The symptoms of hernia vary depending on the type and severity, but common signs include:
A visible bulge in the abdomen or groin area
Pain or discomfort, especially when bending, coughing, or lifting
A feeling of heaviness in the abdomen
Weakness or pressure in the affected area
In some cases, nausea and vomiting (if the hernia becomes strangulated)
If you experience any of these symptoms, it's crucial to consult a hernia surgeon in Pune for timely diagnosis and treatment.
Why Choose Pune for Hernia Treatment?
Pune boasts a combination of highly skilled surgeons, modern hospitals, and personalized patient care. Many of the city’s medical facilities are accredited and offer both open and laparoscopic hernia repair options. Moreover, Pune’s strategic location and cost-effective healthcare attract patients from across India and abroad.
Finding the Best Hernia Surgeon in Pune
When it comes to hernia surgery, the expertise of the surgeon plays a critical role in the success of the treatment. The best hernia surgeons in Pune are known for:
Extensive experience in handling various types of hernias
Proficiency in minimally invasive laparoscopic surgery
High patient satisfaction rates
A multidisciplinary approach to diagnosis and recovery
Association with reputed hospitals and clinics
Before choosing a surgeon, consider scheduling a consultation to discuss the procedure, recovery expectations, and possible risks.
Best Hospital for Hernia Surgery in Pune
Choosing the right hospital is just as important as selecting the right surgeon. The best hospital for hernia surgery in Pune typically offers:
State-of-the-art operating rooms
Advanced diagnostic imaging (CT scans, ultrasounds, etc.)
In-house anesthesiologists and post-op care teams
Hygiene and infection control protocols
Transparent pricing and insurance support
Hospitals in Pune like Ruby Hall Clinic, Jehangir Hospital, Laparo Obeso Centre, and others are renowned for their surgical expertise and infrastructure. These hospitals are equipped to handle complex hernia cases and provide comprehensive post-operative care to ensure a smooth recovery.
Types of Hernia Surgery
There are mainly two types of hernia surgeries available in Pune:
Open Hernia Surgery In this traditional method, a larger incision is made to push the bulging tissue back into place and repair the muscle wall using sutures or a mesh.
Laparoscopic (Keyhole) Surgery A less invasive option, this technique involves making small incisions and using a camera to guide the repair. It typically results in faster recovery, minimal scarring, and reduced post-op pain.
Discuss with your hernia surgeon in Pune to decide which option is best suited for your condition.
Recovery After Hernia Surgery
Recovery time depends on the type of surgery performed, your overall health, and adherence to post-operative instructions. Most patients can resume normal activities within 1–2 weeks after laparoscopic surgery, while open surgery may require 4–6 weeks.
Hospitals in Pune offer dedicated follow-up programs, including physiotherapy and diet consultations, to help you recover quickly and safely.
Tips for a Smooth Recovery
Follow your surgeon’s instructions strictly
Avoid heavy lifting or strenuous activity during the recovery phase
Maintain a balanced diet to prevent constipation and straining
Keep the surgical site clean and dry
Attend all follow-up appointments
Cost of Hernia Surgery in Pune
The cost varies depending on the hospital, type of surgery, surgeon’s fee, and duration of hospital stay. On average, hernia surgery in Pune can range from ₹60,000 to ₹1,50,000. Many hospitals offer insurance coverage and financing options to make the treatment accessible.

Portal Hypertension: Symptoms, Diagnosis, and Surgical Options Explained
Portal hypertension is a condition where there’s increased blood pressure in the portal vein — the major vessel that carries blood from the digestive organs to the liver. This condition is often caused by liver damage (such as cirrhosis) and can lead to severe complications if left untreated.

Symptoms of Portal Hypertension
Common signs and symptoms include:
Abdominal swelling (ascites)
Enlarged spleen (splenomegaly)
Visible abdominal veins
Vomiting blood due to bleeding from esophageal or gastric varices
Fatigue or confusion in severe liver dysfunction
Early detection is key to managing the disease before complications arise.
Diagnosis
Doctors use several methods to confirm portal hypertension:
Ultrasound with Doppler to measure blood flow in the portal vein
Endoscopy to detect varices (swollen veins)
CT or MRI scans for liver and vascular imaging
Liver function tests and blood workSurgical and Interventional Options
When medications aren’t enough, surgical or interventional procedures are considered:
✅ TIPS (Transjugular Intrahepatic Portosystemic Shunt):
A minimally invasive procedure that creates a new pathway for blood flow through the liver, relieving pressure.
✅ Devascularization Surgery:
Used when shunts aren’t possible; it controls bleeding by tying off veins.
✅ Shunt Surgery:
Such as splenorenal or mesocaval shunts, these reroute blood flow to reduce pressure in the portal system.
✅ Liver Transplant:
For patients with advanced liver failure and uncontrolled portal hypertension.
Expert Care at Laparo-Obeso Centre
Dr. Shashank Shah, an expert in advanced gastrointestinal and metabolic surgery, offers precision laparoscopic techniques to manage portal hypertension with better recovery and reduced risks.
Conclusion: Portal hypertension requires expert care and timely intervention. If you or a loved one experiences symptoms, consult a specialist promptly. With modern surgical options, life-threatening complications can often be avoided.

What don’t you like about the OR?
Hello 🌸
I dont like anaesthesiology. I don’t mind the surgical procedure. I think there are specialties I would like to learn more. The techniques and the skills required for a surgery can be cool and I wouldn’t mind. I’m good with needles and I like doing more practical stuff like drenages, thora/laparo/arthocentesis, veins, arteries etc i like that. But just the part of the anaesthesia where you gotta maintain the patient stable and sedated… i don’t like it. And it’s fair, it’s not the job i apply for and it’s not the career id want to pursue!
Plus I met some unpleasant personalities that I didn’t really vibe with lol
Mind you, the OR is a completely different place in a hospital and most of the things work differently than any other room. It takes time to adjust and understand what you’re supposed to do and WHEN.
There are members of the team that recognise the role. Im a resident, but im still a doctor. I always specify im an emergency medicine resident not an ICU/anaesthesia resident and most of the time they understand you’re there to learn and that you are just doing it for a brief period of time. It’s normal that you’re like a fish on a tree lol but there are also some others that are more hostile, passive aggressive and generally less inclined to appreciate your presence. I dont mind criticism, because that’s how I learn. But i mind judgement when it doesn’t come with an explanation.
Most of the time I had to learn things by myself because nobody would take 2 minutes to give directions. I’m obviously not talking about the medical procedure or conditions, that’s something I can only learn by studying. But I wouldn’t have minded a tip or two when you ask me to dilute remifentanyl or that i can literally cut my nail by opening the electrolytes bottle in the wrong way, or that if i put electrodes like this or like that is more efficient, or that if i close the patient eyes like this and not like that, it won’t hurt them … everyone has their techniques so when use one from the other they will tell you you’re doing it wrong but if you do their way with someone, then you’re wrong again! It was exhausting and i was annoyed more often than not
But obviously there’s so much more lol
i tried to learn as much as possible. Above all im not particularly satisfied with the results hahah yeah i improved my manual ventilation skills, yes i succeeded in 70% of intubations but it wasn’t as satisfying for me as it usually is at the end of a rotation

"How Bariatric Surgery Improves Health Beyond Weight Loss"
When we think of bariatric surgery, weight loss is often the first benefit that comes to mind. However, the true impact of this transformative procedure extends far beyond the numbers on the scale. Bariatric surgery not only helps shed excess weight but also improves overall health, tackles chronic diseases, and enhances quality of life in ways that can be life-changing.
Here’s a closer look at how bariatric surgery benefits your health beyond weight loss.
1. Improvement in Type 2 Diabetes
Bariatric surgery has shown remarkable results in managing and even reversing type 2 diabetes. Procedures like gastric bypass and sleeve gastrectomy improve insulin sensitivity and regulate blood sugar levels. For many patients, this can mean reducing or completely stopping diabetes medications shortly after surgery.
2. Reduction in Cardiovascular Risks
Obesity is a significant risk factor for heart disease, hypertension, and stroke. Bariatric surgery not only helps reduce excess weight but also improves cholesterol levels and lowers blood pressure. This combination dramatically decreases the risk of cardiovascular complications.
3. Relief from Sleep Apnea
Sleep apnea, a condition where breathing repeatedly stops during sleep, is common in individuals with obesity. After bariatric surgery, many patients experience a significant reduction in sleep apnea symptoms, with some no longer needing CPAP machines to assist their breathing.
4. Joint Pain and Mobility
Excess weight puts undue stress on joints, leading to conditions like arthritis and chronic pain. Losing weight through bariatric surgery alleviates this pressure, improving mobility and reducing joint pain, which allows patients to lead a more active lifestyle.
5. Boost in Fertility and Hormonal Health
For women, obesity can cause hormonal imbalances that affect fertility and menstrual cycles. After bariatric surgery, hormonal health often improves, making it easier to conceive and maintain a healthy pregnancy.
Mental Health and Quality of Life
Obesity is not just a physical challenge — it takes a toll on mental health as well. Many patients report feeling more confident, energetic, and emotionally resilient after surgery. The reduction in weight, combined with better health outcomes, often leads to an improved outlook on life and greater self-esteem.
Take the First Step with an Expert
If you’re considering bariatric surgery, it’s vital to choose an experienced surgeon who prioritizes your overall health and long-term results. Understanding the link between obesity and infertility and finding solutions through bariatric surgery
Obesity is a medical condition in which excessive fat is accumulated on the body that presents a risk to health. Obesity is measured using the Body Mass Index (BMI). A body mass index (BMI) of over 25 is considered overweight, and over 30 is considered obese.
In today’s world obesity has become a global pandemic that is affecting millions of people. While the physical health risks associated with obesity are well-known, such as diabetes, heart diseases and joint problems, its impact on fertility is often overlooked. Obesity can result in fertility issues for both men and women, making it a significant concern for those trying to conceive. The purpose of this blog is to explore the connection between obesity and infertility, explore the underlying mechanisms, and provide guidance on how to tackle this issue.
What is infertility?
Infertility is a disease of the male or female reproductive system defined by the failure to achieve pregnancy after 12 months of regular sexual intercourse.
The Multifunctional Role of White Adipose Tissue: More Than Just Fat
When we think about body fat, we often think that it merely stores energy. However, white adipose tissue (WAT) is a vital endocrine organ that releases special proteins called adipokines, which play key roles in regulating various body functions, such as energy, immune response and reproductive health.
What Are Adipokines?
The proteins called adipokines are produced by fat cells and are involved in cell physiology as signalling molecules. Leptin, adiponectin, resistin, visfatin, omentin, and ghrelin are a few examples of well-known adipokines. These proteins play a crucial role in regulating our immune system’s response, the way our reproductive system works, and how our body handles carbohydrates and lipids. On the other hand, imbalanced adipokine levels — either too high or too low — can result in metabolic problems and inflammation.
How obesity affects fertility in women?
The Impact of Body Fat on Reproduction:
The amount of white adipose tissue we have can significantly impact our reproductive health. Here’s how:
1. Necessary for Normal Reproductive Function:
Sufficient body fat is essential for puberty, hormone production, and maintaining pregnancy. However, too much body fat can cause reproductive problems.
2. Excess Fat and Reproductive Disorders:
Women with too much body fat (obesity) can develop ovulation disorders and polycystic ovarian syndrome (PCOS). In turn, PCOS triggers the release of inflammatory factor TNF (bad) and reduces adiponectin secretion (good).
The regulation of glucose and fat metabolism is supported by adiponectin. Elevated insulin levels and insulin resistance may be caused by reduced adiponectin levels in obese women leading to the development of type 2 diabetes.
Elevated levels of leptin, one of the primary adipokines, from excessive body fat can disrupt ovarian function and the early stages of pregnancy, leading to poor reproductive outcomes.
Metabolic and reproductive health is also influenced by other adipokines, resistin, visfatin, and chemerin, which are associated with insulin resistance, type 2 diabetes, and impaired ovarian hormone production.
How bariatric surgery helps in treating infertility in women?
Bariatric surgery is an obesity treatment that involves weight reduction, adipokine balance, and improved reproductive health. Reducing body fat mass normalizes adipokines levels, restoring balance and improving insulin sensitivity. Bariatric surgery can also improve hormonal imbalances that cause conditions such as polycystic ovarian syndrome (PCOS). Weight loss after surgery can also reduce insulin resistance and overproduction of androgens by insulin-stimulated ovarian cells. This improves ovarian function and hormone balance, improving the chances of pregnancy in women who suffer from infertility due to obesity.
How Obesity affects fertility in Men?
Obesity can significantly lower a man’s fertility by messing with his hormones. Fat tissue, especially around the belly, turns testosterone, the key sperm-making hormone, into estrogen, the female sex hormone. This double blow reduces testosterone and creates an unfriendly environment for sperm development. On top of that, obesity disrupts the brain’s signals to the testes; further hindering testosterone production and harming sperm health.
How treatment of obesity through bariatric surgery leads to treatment of fertility in men?
By reducing abdominal fat, bariatric surgery reduces the conversion of testosterone to estrogen, improving testosterone levels and sperm production, thereby increasing the chances of conception.
Overall Health Benefits of Bariatric Surgery:
● Beyond fertility improvements, bariatric surgery offers comprehensive health benefits by reducing the risks of obesity-related conditions such as type 2 diabetes, cardiovascular disease, and metabolic syndrome.
● These health improvements contribute to better overall reproductive health and pregnancy outcomes in women who undergo surgery.

Infertility is a disease of the male or female reproductive system defined by the failure to achieve pregnancy after 12 months of regular sexual intercourse.
The Multifunctional Role of White Adipose Tissue: More Than Just Fat
Adipose tissue is divided into two types: white adipose tissue (WAT) and brown adipose tissue (BAT), which are easily distinguished by their colour. The white and brown adipocytes that make up these depots have different physiological characteristics. WAT, which is responsible for energy storage, endocrine communication, and insulin sensitivity, is the body’s greatest adipose tissue volume. In contrast, BAT utilises energy to generate non-shivering heat, which is essential for maintaining body temperature. Additionally, diet-induced thermogenesis and whole-body fat oxidation are facilitated by BAT activity.

Obesity can lead to elevated male hormone levels (hyperandrogenemia) and problems with ovulation (anovulation), but not all obese women experience these problems. Some are fertile and don’t have hormone imbalances. The main problem is often high insulin levels and insulin resistance, which accompany obesity and can alter hormone production.
