Abstract
Background: There is no effective therapy for many neurological disorders associated with significant neurological damage such as kernicterus. Patients with Kernicterus (Chronic bilirubin encephalopathy) which is a neurological dysfunction resulting from exposure of the brain to severe hyperbilirubinemia mostly during the neonatal period continue to experience significant disability. There is no known therapeutic intervention that can obviously improve this condition. The aim of this paper is to describe a novel therapeutic approach which in two months produced marked improvement of the neurological dysfunction caused by kernicterus.
Patients and Methods: A girl with kernicterus was not speaking and not saying any word. She was lacking the balance (coordination) without obvious muscle weakness. She was unable to maintain the sitting posture on a chair for few minutes. She was unable to maintain straight standing posture when supported on chair at all. She had difficulty in holding things. The girl was treated with a novel therapeutic approach including two courses of intramuscular cerebrolysin and intramuscular citicoline.
Results: After treatment the first month of treatment, speech development was initiated, and she was saying few words. She was able to sit normally on the chair and maintaining the sitting posture indefinitely. She was able to maintain more straight stable standing posture without holding a chair and with the ability to hold things at the same time indicting improved coordination. She also developed improved ability to hold small things like a pen. After the second month of treatment, the girl was able to stand alone and was making few steps slowly holding furniture. After treatment the second course of treatment, the girl was able to stand alone and walk rapidly holding furniture. Treatment was not associated with any side effects.
Conclusion: The novel use of intramuscular cerebrolysin and citicoline in a patient with kernicterus was safe and effective.

Introduction
Kernicterus is a neurological dysfunction resulting from exposure of the brain to severe hyperbilirubinemia mostly during the neonatal period. In kernicterus, hyperbilirubinemia results in deposition of bilirubin in the grey matter of the brain causing neurotoxicity associated with mass-destruction of neurons by apoptosis and necrosis resulting in irreversible brain damage, and chronic neurological disorder. Clinical manifestations of kernicterus may include severe motor disability with inability to walk. There is no effective therapy kernicterus [1,2]. The aim of this paper is to describe a novel therapeutic approach which in two moths produced marked improvement of the neurological dysfunction caused by kernicterus.
Patients and Method
A girl was first seen at the age of about five years during November,2018 because of delayed development with lack of body control, abnormal movements, and no speech. Her birth weight was about three Kilogram, and she developed severe neonatal hyperbilirubinemia of 26 mg/dL, and was treated by phototherapy and exchange transfusion. The family consulted many physicians in Iraq, and when no obvious improvement could be achieved nor any hope of improvement was given, the family took their daughter to India. During March,2017, the girl was seen by Dr Rakesh Kumar Jain who was a senior consultant at the neurology center of Fortis Memorial Research Institute in Gurgaon India. Dr Jain performed a 21-channel electroencephalography on the girl which showed intermittent short and sharp waves from bilateral focal regions with no ictal events. The electroencephalography report stated that drug induced EEG showed normal sleep pattern with intermittent bilateral frontal sharp waves. Dr Jain also performed a study of the short latency auditory evoked potential on the girl. The report of this study stated that the stem auditory evoked response showed normal latencies of waves with hearing threshold of 20 dB bilaterally. Dr Jain sent the girl to the Modern Diagnostic Research Center for brain MRI which was performed at the 22nd of March, 2017, by Dr Sneha Thakur, a consultant radiologist. The brain MRI detected no significant abnormality. Dr Rupal Gupta, a consultant ophthalmologist at the Fortis Memorial Research Institute examined the girl on the 30th of March, 2017, and prescribed eye glasses for the girl. After visiting many physicians in Iraq and India, the only beneficial therapeutic intervention for the girl was the correction of her refractive error with eyeglasses.
When the girl was seen at the pediatric neuropsychiatry clinic at the Children Teaching Hospital of Baghdad Medical City during November,2018, she was not speaking and was not saying any word. The girl was lacking the balance (co-ordination) without obvious muscle weakness. She was unable to maintain the sitting posture on a chair for few minutes and was also unable to maintain straight standing posture when supported on chair at all (Figure 1). She also had difficulty in holding things.

The girl was treated with a novel therapeutic approach consisting of two courses.
The First Course Included
Cerebrolysin 5ml given by intra-muscular injections every third day on the morning, and she received 20 doses over two months. Citicoline 500 mg given by intra-muscular injections every third day on the morning, and she received 20 doses over two months. Cerebrolysin and citicoline were given on two different days.
The Second Course Included
Cerebrolysin 5ml given by intra-muscular injections every fifth day on the morning, and she received 10 doses over fifty days. Citicoline 500 mg given by intra-muscular injections every fifth day on the morning, and she received 10 doses over fifty days. Cerebrolysin and citicoline were given on two different days.
Results and Discussion
After treatment the first month of treatment, speech development was initiated and she was saying few words. She was able to sit normally on the chair and maintaining the sitting posture indefinitely (Figure 1). In this paper, we have reported that the novel use of intramuscular cerebrolysin and citicoline in a patient with kernicterus was safe and effective. The use of cerebrolysin and citicoline was beneficial in the treatment of various childhood neuro-psychiatric disorders including developmental and pervasive developmental disorders, brain atrophy, kernicterus, and cerebral palsy [3-5]. Cerebrolysin is a peptidergic medicine which contains mainly biologically active neuropeptides including brain-derived neurotrophic factor, glial cell line-derived neurotrophic factor, nerve growth factor, and ciliary neurotrophic factor. It has a nerve growth factor like activity on neurons, and growth promoting efficacy in different neuronal populations from peripheral and central nervous system. Cerebrolysin has a direct neurotrophic effect, and obvious neuroprotective properties against many types of lesion in vitro and in vivo. The therapeutic effects of cerebrolysin have been considered to be similar to the pharmacological activities of naturally occurring nerve growth factors. The safety, tolerability, and efficacy of neuroreparative cerebrolysin therapy have been established in clinical trials included adults with stroke and Alzheimer s disease.

The Neuroreparative Effects of Cerebrolysin have Been Attributed to [3-5]:
a) Inhibition of apoptosis.
b) Improving synaptic plasticity and induction of neurogenesis.
c) Augmenting the proliferation, differentiation, and migration of adult.
d) subventricular zone neural progenitor stem cells, contributing to neurogenesis.
e) Induction of stem-cell proliferation in the brain.
Citicoline (cytidine diphosphate choline) is a mononucleotide made of ribose, pyrophosphate, cytosine and choline is a watersoluble naturally occurring substance that is generally grouped with the B vitamins. It is also considered a form of the essential nutrient choline. It is a safe substance with generally minor side effects which may include digestive intolerance after oral administration [6]. An accumulating research evidence suggests that citicoline is endowed with interesting pharmacological properties that can make it useful in the treatment of various disorders that has no universally accepted effective treatment including neurological conditions such as Parkinson’s disease, brain ischemia, hemorrhagic stroke, Alzheimer’s disease; and ocular condition such as glaucoma, nonarteritic ischemic neuropathy and amblyopia [6].
The Neuro-Protective Effects of Citicoline Were Attributed to the Followings [6]:
a) Preservation of cardiolipin and sphingomyelin
b) Preservation of arachidonic acid content of phosphatidylcholine and
c) phosphatidylethanolamine.
d) Partial restoration of phosphatidylcholine levels.
e) Stimulation of glutathione synthesis and glutathione reductase activity.
f) Reduction of phospholipase A2 activity.
g) Increasing glucose metabolism in the brain.
h) Increasing cerebral blood flow.
Reducing oxidative stress and preventing excessive inflammatory response in the brain by inhibiting the release of free fatty acids and reducing blood brain barrier breakdown. Enhances cellular communication by increasing the availability of neurotransmitters, including acetylcholine, norepinephrine, and dopamine. Lowering increased glutamate concentrations and increasing the decreased ATP concentrations induced by ischemia. Citicoline increases dopamine receptor densities, and therefore could improve memory impairment resulting from poor environmental conditions. Citicoline could also improve focus and mental energy and could be useful in the treatment of attention deficit disorder. Citicoline has also been shown to improve visual function in patients with glaucoma [7].
Conclusion
The novel use of intramuscular cerebrolysin and citicoline in a patient with kernicterus was safe and effective.

Living with LHON: Adapting to sight loss at 15 years old
When faced with a devastating diagnosis, a family has two choices – to give up or make it their mission to adapt and carry on. 
Ana Velosa’s son Tomás, who was diagnosed with the rare genetic sight-loss condition Leber’s hereditary optic neuropathy (LHON) at just 15, chose the latter. And she couldn’t be prouder of all he has achieved.
We speak to Ana about the lack of disease education and psychosocial support for people with LHON in her native Portugal, and why Tomás is her hero.
Life must go on
Tomás was enjoying a family holiday, in August 2016, when he first started to experience symptoms of LHON. At the time, his blurred vision and difficulty focusing the right eye were chalked up to tiredness.
But when the problem continued back home, Ana took Tomás to see an optometrist, and that is where the journey began.
“We thought he may need glasses – we never thought it would be more complicated than that,” said Ana.
The optometrist said there was no lens that would fix the problem. On the advice of a paediatrician friend, Tomás went to hospital where he was admitted with suspected intercranial pressure.
Ana explained: “They mentioned many possible conditions. First, they said that it may be a tumour, in his brain. Then that it may be some bacterial or viral disease and they gave him medication for that.
“They also mentioned MS, but there was nothing concrete. It was only towards the end of his hospital stay that they started to mention the possibility of LHON.
“But we were still far from realising what was happening – that he was going to lose his central vision.”
On 20 September 2016, a little over a month after first experiencing symptoms, Tomás was diagnosed with LHON.
The rare genetic disease is passed down the maternal line and affects around two in every 10,000 people in Europe. The eye condition tends to leave sufferers reliant on their peripheral vision within months of the onset of symptoms.
This is what happened to Tomás, who, within four months of that family holiday, had completely lost his central vision.
Ana, who was not aware that she carried the faulty gene that leads to LHON, said: “This is a process similar to loss. In his case, it was a loss of his vision.
“He still has his peripheral vision, so is able to walk on the street and orientate himself, but he is not totally autonomous. As parents, we feel that loss as well and it is not easy to deal with. We also have a feeling of impotence because it is incurable.”
The diagnosis was made just as Tomás was moving into his second year of secondary school, but he hasn’t let it hold him back.
It took some time for him and his family to adapt to this new reality, but Tomás is now at university studying business management.
Ana said: “It demands a certain amount of time to accept, but he is doing it with a great deal of courage and valour. He really has the will to overcome it.”
But that’s not to say it has been easy.
Lack of support
The Velosa family is extremely grateful that Tomás’ diagnosis was made relatively quickly, thanks to the specialists based at their local hospital.
Sadly, this is not the case for everyone facing LHON in Portugal.
“We were extremely lucky,” said Ana, “not all doctors are qualified to diagnose this type of thing.
“Many physicians have no knowledge of this condition particularly in ophthalmology, which is where people will normally go if their children can’t see well. You won’t go to a neuro-ophthalmologist.”
To prevent delayed diagnosis, the country is in desperate need of an LHON referral centre, she said.
Staffed by specialist physicians, it would see patients and doctors collaborate with similar units and researchers around the world, as well as offer healthcare professional education.
“It would make diagnosis easier and would be a great step towards helping patients.”
Speeding up diagnosis is only part of the solution for people living with this condition in Portugal, however. There is extensive unmet need among people living with LHON and their families, who are often left to cope alone.
“There is a lack of practical help in everyday life. The hospitals are not equipped to offer that help. It means that in terms of practical matters, such as technological solutions, we need to find these ourselves,” Ana said.
“Help from those support services, such as social workers and psychologists is absolutely essential to offer real support to patients and their families.”
It’s not just medical support that is lacking. The country does not have an LHON patient support organisation and disease education and information in its native tongue is thin on the ground.
“We feel more loneliness, because it is a rare disease and there is little information. As far as I know, there is no information in Portuguese.
“What we have read was written and translated by doctors and other researchers from abroad.”
A dedicated patient advocacy group would have the potential to transform the experiences of people living with LHON and their families by offering services such as peer support, she believes.
“We would be able to share experiences, ideas and feelings, thus reducing the feeling of loneliness,” said Ana.
“It may be easier to access European and worldwide congresses to keep more up-to-date with research, be able to exchange information about practical aspects of life, and even put some pressure on the State to obtain better financial support.”
Tomás’ journey
It would also work to fight the stigma attached to the condition and help people living with LHON feel more included in society.
“There are still a lot of preconceptions and ignorance in our society towards people with a rare disease or disability.
“Inclusion is what places people in a position of equality with others,” Ana said.
This can be achieved by making specific adaptations that allow individuals to participate in social, academic or professional life.
Such adaptations have been a big part of Tomás’ success story. At school, a special status granted by the Portuguese Ministry of Education gave him extra time in lessons and allowed exam conditions to be adapted to his needs.
Tomás has also utilised technical solutions, such as a mobile phone with a higher resolution and writing using computer hardware and software rather than with pen and paper.
His university is also working with him to offer specific adaptations that allow him to be included and integrated into the course and student life.

Crucially, Tomás’ attitude, determination and perseverance have been the keys to his success.
“He is very special, a true and marvellous hero. He never gave up. He makes a huge effort to keep studying and is doing the best he can at university,” said Ana.
“It is our son who takes us by the hand. He is the one who has demonstrated that he does not want to stop, that he is not giving up and that life has to go on.”
Tackling the unmet medical, psychosocial and support needs of Portugal’s LHON community would help even more people follow his example and achieve their dreams.

Abstract
Peripheral facial palsy (PFP) is often idiopathic; a secondary systemic cause is noted in less than 1.2% of cases. Primary Sjogren’s Syndrome (PSS) remains an exceptional and often insufficiently known etiology of facial paralysis, and only a few sporadic cases are reported in the medical literature. These inaugural forms of the disease represent a real diagnostic challenge for clinicians. We report an original observation of isolated and recurrent PFP as inaugural symptom of PSS in a 53-year-old female with no notable pathological history.

Introduction

Primary Sjögren’s syndrome (PSS) is an autoimmune disease characterized by focal lymphoid infiltration of the exocrine glands, mainly manifested by dry ocular and oral syndrome [1]. During the course of the disease, one-third of the patients develop extraglandular lesions: pulmonary, neurological, renal, hepatic and cardiac, which signal the systemic nature of this disease and condition its prognosis [2,3]. The prevalence of this connective tissue disease is estimated at 1-3% of the general population [1,4] and can reach as much as 4.8% in Europe [2]. The neurological manifestations of this disease are observed in 15-20% of cases and are highly polymorphic and far dominated by peripheral neuropathy [5-8].
The spectrum of PSS-associated neurological manifestations may include: multiple mononeuropathy, symmetrical axonal sensorimotor polyneuropathy, sensory ataxic neuropathy, painful sensory neuropathy, cranial neuropathy, autonomic neuropathy, radiculoneuropathy, aseptic meningitis, encephalopathy, psychiatric symptoms, chorea, seizures, chronic myelopathies, multiple sclerosis-like syndrome, and cognitive impairment [5-8]. Neurological involvement may be the first manifestation of PSS in approximately 25% of cases [7]. Of all the possible neurological manifestations of PSS, peripheral facial palsy (PFP) remains exceptional and unusual [9], and only a few sporadic cases were reported in the medical literature [9-12]; these inaugural forms of the disease represent a real diagnostic challenge for clinicians. We report an original observation of isolated and recurrent PFP as inaugural symptom of PSS.
Case Report
A 53-year-old Tunisian patient, with no notable pathological history, was referred to our department for recurrent PFP. She was treated by her family doctor for three episodes of PFP occurring within a year and a half without any other associated abnormalities. This PFP was right in two times and left in one time. The etiological investigations requested by his treating physician, including a specialized ENT examination and a cerebral CT-scan, were without abnormalities. The diagnosis of idiopathic PFP (Bell’s palsy) was retained. The patient was treated with short course corticosteroid and vitamin therapy but without improvement.
In somatic examination, the patient was apyretic, with correct conscious, respiratory, and hemodynamic status. We noted a marked dryness of the skin and the tongue.
Laboratory tests revealed a marked biological inflammatory syndrome with erythrocyte sedimentation rate at 68 mm/H1, C-reactive protein at 18 mg/l, and polyclonal hypergammaglobulinemia at 24.6 mmol/l. the other basic tests were within normal limits: blood count, creatinine, serum calcium, ionogram, fasting glucose, transaminases, muscle enzymes, lipid parameters, and thyroid hormones. Immunological exploration showed positive anti-nuclear autoantibodies at 1/640 with positive anti-SSA antibodies at 50 IU. The specialized ophthalmologic examination objectified xerophthalmia with bilateral filamentous keratitis and a positive Schirmer’s test.
Biopsy of the accessory salivary glands showed a stage 3 chronic sialadenitis according to the Chisholm classification. Thus, the diagnosis of PSS was retained based on the following criteria: xerophthalmia, xerostomia, positive ophthalmological tests, stage 3 chronic sialadenitis, and positive anti-nuclear and anti-SSA antibodies. Cerebrospinal fluid analysis and cerebromedullary MRI did not show signs suggestive of specific neurological involvement of PSS (neuro-Sjögren). Similarly, other systemic complications of PSS and a possible lymphomatous transformation were eliminated by specific investigations. At the end of this assessment, the diagnosis of a recurrent and isolated PFP revealing PSS was retained. The patient was treated with systemic corticosteroid therapy at a dose of 1 mg/kg/day and hydroxychloroquine at a dose of 400 mg/d, salicylic acid at a dose of 100 mg/d, and symptomatic treatment of xerophthalmia and xerostomia, with favorable evolution. No recurrence of the PFP has been noted for five years now.
Discussion
PFP is often idiopathic (Bell’s palsy or FRP); a secondary systemic cause is noted in less than 1.2% of cases [13]. PSS remains an exceptional [11] and often insufficiently known etiology of facial paralysis [11,14]. Indeed, only a few sporadic cases have been reported in the literature [9-12,15]. In large series of PSS, this neurological manifestation remains unusual: in fact, only two cases of PFP associated with PSS were found in the series of Teixieira F et al of 93 patients followed for PSS [14], no case has was noted in the European series of 392 patients with PSS of whom 74 had neuro-Sjögren’s [8], and no case of PFP was reported in the Ye W et al series of 566 patients with PSS of whom 184 had neurological signs [6]. PSS-associated PFP can be uni- or bi-lateral [12], episodic or most often recurrent [11], isolated or more often integrated in the framework of a complex neurological attack [11] and can remain for a long time the only manifestation of the disease [11,15]. PFP associated with PSS usually responds well to systemic corticosteroids and others specific therapies for this autoimmune disease.
These forms of isolated and SSP-revealing cranial neuropathies are often ignored and neglected by clinicians, and thus sometimes responsible for a very important diagnosis delay [16]. The exact pathophysiology of this neuropathy is not very well known. It appears to be multifactorial involving vasculitis, autoimmunity, inflammation, and cryoglobulinemia [4,5]. A reported case of PSSassociated PFP involved, in addition to these classic pathogenic factors, a vitamin B12 deficiency [11]. Finally, it should be kept in mind that a PFP for rapid onset and/or progression during PSS should raise concerns about the lymphomatous transformation of this disease [17].
Conclusion
Peripheral facial palsy is an exceptional and uncommon neurologic complication of PSS. The isolated and inaugural forms are often ignored by health practitioners and represent a real diagnostic challenge. It is thus necessary to evoke the diagnosis of PSS in front of any PFP that does not proven, particularly if recurrent and in elderly. Early diagnosis and adequate management can improve the prognosis of this disease, especially that a lymphomatous transformation can be announced by a PFP arising and/or progressing rapidly.
