Biden Administration Adds Insulin to Drug Price Negotiation List in Major Blow to Big Pharma

The Biden administration pleasantly stunned health care reform advocates Tuesday by including short-acting insulin in its list of 10 drugs for which Medicare will negotiate lower prices, power vested in the White House by the Inflation Reduction Act.  The IRA was passed in the face of one of the heftiest barrages of lobbying in congressional history, with the pharmaceutical industry spending more than $700 million over 2021 and 2022 — several times more than the second- and third-ranking industries — much of it aimed at stopping the legislation, watering it down, or undermining its implementation.  The industry succeeded in narrowing the scope of the new law, with only 10 drugs eligible for negotiation in 2023. But the effort to stop the administration from using that authority to target blockbuster drugs failed spectacularly, as the White House list includes medications that seniors spend billions on in out-of-pocket costs alone, never mind the fees paid by Medicare itself. But it was the inclusion of the broad swath of ingredients and medical devices needed to make insulin that sets the White House move apart and which will likely have the most far-reaching impact.  “This was an unexpected victory in a long fight against an illegal cartel of three corporations who have raised their insulin prices in lockstep,” said Alex Lawson, executive director of Social Security Works, referring to Eli Lilly, Novo Nordisk, and Sanofi. “The inclusion of insulin in the list of negotiated drugs shows that the Biden White House isn’t fucking around.” Lawson’s organization has advocated for drug price negotiations for years, often drawing the ire of the White House and congressional leaders in the process. Joe Biden’s insulin category covers a wide variety of products, including Fiasp, Fiasp FlexTouch, and Fiasp PenFill — rapid-acting insulin products — as well as commonly prescribed insulin products NovoLog, NovoLog FlexPen, and NovoLog PenFill. Major insulin makers, under severe public pressure, have in recent months announced price cuts to their products, though they could unilaterally raise prices absent this government intervention. “We did not expect to see short-acting insulin on this list. We were caught a little off guard,” said Shaina Kasper, policy and advocacy director for T1International, an organization dedicated to defending the interests of diabetes patients. Broadly speaking, she said it was “the result of years of dogged organizing and advocacy by patients with diabetes and caregivers.” The IRA laid out specific criteria for which drugs could be included, and the White House went after the 10 that added up to the highest spending among drugs that met that criteria. Kasper said her organization will continue to push for executive action to drive down drug prices — the Biden administration has other authorities it could draw on — as well as legislation at the state and federal level.

Liều lượng – Cách dùng

Dược lực:

Hiệu quả làm giảm glucose huyết của insulin là do làm quá trình hấp thu glucose dễ dàng theo sau sự gắn kết insulin vào các thụ thể trên tế bào cơ và tế bào mỡ, đồng thời ức chế sản xuất glucose từ gan.

Dược động học :

– Hấp thu: Khi uống insulin bị thuỷ phân ở đường tiêu hoá làm mất tác dụng. Dùng đường tiêm bắp hấp thu nhanh hơn tiêm dưới da. Trong máu, insulin có thời gian bán hủy vài phút. Do vậy, thời gian tác động của một chế phẩm insulin được xác định chỉ do đặc tính hấp thu của nó. Quá trình này chịu ảnh hưởng bởi nhiều yếu tố (ví dụ liều insulin, đường tiêm và nơi tiêm), các yếu tố này giải thích sự khác biệt đáng kể trong cùng một bệnh nhân và giữa các bệnh nhân khác nhau. Thời gian tác động trung bình sau khi tiêm dưới da: Bắt đầu: trong vòng nửa giờ. Tác động tối đa: từ 1 đến 3 giờ. Thời gian tác động kéo dài: khoảng 8 giờ. – Chuyển hoá: Insulin chuyển hoá ở gan bằng phản ứng thuỷ phân dây nối peptid và cắt cầu disulfid làm mất hoạt tính. – Thải trừ: insulin thải trừ qua đường nước tiểu.

Tác dụng :

– Trên chuyển hoá glucid: insulin có tác dụng làm giảm glucose máu do insulin giúp glucose dễ xâm nhập vào tế bào đặc biệt là các tế bào gan, cơ và mô mỡ bằng cách làm giàu chất vận chuyển glucose ở màng tế bào. + Cơ chế làm giảm glucose máu của insulin do insulin kết hợp với receptor, phức hợp insulin-receptor sẽ tự phosphoryl hoá tạo tín hiệu dẫn truyền tới nang dự trữửtong tế bào, các nang sẽ di chuyển tới màng tế bào, hoà vào màng tế bào và hướng chất vận chuyển glucose ra ngoài màng tế bào do đó làm tăng cường vận chuyển glucose làm cho glucose vào tế bào với tốc độ nhanh. Khi nồng độ glucose nội bào cao sẽ thúc đẩy insulin ra khỏi receptor, những chất vận chuyển glucose lại được thu hồi vào những nang bọc kín để trở lại kho dự trữ ở nội bào. Insulin còn thúc đẩy tổng hợp và ức chế phân huỷ glycogen bằng cách kích thích glycogen synthetase và ức chế glycogen phosphorylase. Khi thiếu insulin, tế bào không sử dụng được glucose và glucose cũng không chuyển thành glycogen để dự trữ ở gan được. Hậu quả là tăng glucose máu và glucose niệu. _ Trên chuyển hoá lipid: tăng tổng hợp và dự trữ lipid ở gan, ngăn cản phân giải mỡ và ức chế tạo các chất cetonic nhờ ức chế hoạt tính của lipase nhạy cảm với hormon, làm nồng độ acid béo tự do và glycerol trong huyết tương. – Trên chuyển hoá protid: thúc đẩy đồng hoá protid bằng cách làm acid amin dễ xâm nhập vào tế bào để tổng hợp protein. Đặc biệt ở thành mạch, insulin tham gia vào tạo glycoprotein cấu trúc, do đó nếu thiếu insulin thành mạch dễ bị tổn thương.

Chỉ định :

Ðiều trị bệnh đái tháo đường týp I ( đái tháo đường phụ thuộc insulin ). Điều trị đái tháo đường týp II khi các thuốc chống đái tháo đường tổng hợp không còn hiệu quả. Thuốc được chỉ định để ổn định bệnh đái tháo đường ban đầu và đặc biệt dùng cho những trường hợp cấp cứu của bệnh đái tháo đường. Trẻ em gầy yếu, kém ăn, suy dinh dưỡng, nôn nhiều và rối loạn chuyển hoá đường( thường truyền glucose kết hợp với insulin). Gây cơn shock insulin để điều trị bệnh tâm thần( tạo cơn hạ glucose huyết đột ngột và mạnh).

Liều lượng – cách dùng:

Liều lượng thay đổi tùy theo từng cá nhân và do bác sĩ quyết định phù hợp với nhu cầu của từng bệnh nhân. Bệnh nhân đái tháo đường không nên ngưng điều trị insulin trừ khi có sự chỉ dẫn của bác sĩ. Nhu cầu insulin trung bình hàng ngày trong điều trị bệnh đái tháo đường thay đổi từ 0,5 cho đến trên 1,0 UI/kg cân nặng, phụ thuộc vào mỗi bệnh nhân. Ở những bệnh nhân đái tháo đường kiểm soát chuyển hóa tốt sẽ làm chậm lại quá trình xuất hiện và tiến triển các biến chứng của đái tháo đường giai đoạn muộn. Vì vậy, kiểm soát chuyển hóa tốt bao gồm theo dõi glucose, được khuyến khích. Ở những bệnh nhân lớn tuổi, mục đích chủ yếu của điều trị là làm giảm triệu chứng và tránh tình trạng hạ đường huyết. Thuốc thường được tiêm dưới da vào thành bụng, vùng đùi, vùng mông hoặc vùng cơ delta cũng có thể được dùng. Tiêm dưới da vào thành bụng bảo đảm sự hấp thu nhanh hơn so với những vị trí tiêm khác. Tiêm vào nếp gấp da được véo lên để làm giảm thiểu khả năng tiêm vào cơ. Chỉ có dung dịch tiêm insulin người là được tiêm tĩnh mạch nhưng chỉ do bác sĩ thực hiện. Sau khi tiêm, nên giữ kim dưới da ít nhất 6 giây. Giữ chặt nút đẩy cho đến khi rút kim khỏi da. Ðiều này bảo đảm thuốc đã vào hết và hạn chế khả năng máu hay các dịch khác trong cơ thể chảy vào kim hay vào ống insulin. Nên thay đổi vị trí tiêm trong vùng tiêm để tránh sự loạn dưỡng mỡ. Ðể tránh nguy cơ lây bệnh, mỗi ống tiêm chỉ sử dụng cho một người. Thuốc tác dụng nhanh và thường được dùng kết hợp với insulin tác dụng vừa hay insulin tác dụng kéo dài. Việc tiêm insulin nên được thực hiện 30 phút trước bữa ăn chính hay bữa ăn phụ có carbohydrate.

Chống chỉ định :

Hạ đường huyết. Quá mẫn cảm với insulin người hay bất kỳ tá dược nào của thuốc.

Tác dụng phụ

Hạ đường huyết là một tác dụng không mong muốn thường xảy ra trong điều trị bằng insulin. Những triệu chứng của hạ đường huyết thường xuất hiện đột ngột, bao gồm đổ mồ hôi, da xanh và lạnh, bồn chồn, run, cảm giác lo âu, mệt mỏi bất thường, hay nhầm lẫn, khó tập trung, lơ mơ, đói dữ dội, giảm thị lực tạm thời, nhức đầu, buồn nôn và đánh trống ngực. Hạ đường huyết nghiêm trọng có thể dẫn đến bất tỉnh và có thể làm suy chức năng não tạm thời hay vĩnh viễn hoặc thậm chí tử vong. Phù và bất thường về khúc xạ có thể xảy ra khi bắt đầu dùng liệu pháp insulin. Những triệu chứng này thường thoáng qua. Các phản ứng quá mẫn cảm tại chỗ (đỏ, sưng và ngứa ở chỗ tiêm) có thể có trong khi điều trị với insulin. Những phản ứng này thường thoáng qua và biến mất khi tiếp tục điều trị. Các phản ứng quá mẫn cảm toàn thân thỉnh thoảng xuất hiện. Những phản ứng này nghiêm trọng hơn và có thể gây phát ban toàn thân, ngứa, đổ mồ hôi, rối loạn tiêu hóa, phù mạch thần kinh, khó thở, đánh trống ngực và hạ huyết áp. Những phản ứng quá mẫn cảm toàn thân rất dễ đe dọa tính mạng. Loạn dưỡng mỡ có thể thấy ở chỗ tiêm do không thay đổi vị trí trong vùng tiêm.

pump break

last night I decided to take off my pump for a little longer than usual... it’s still off. I have a back up lantus pen that I used in combination with a nova rapid pen. I had to buy insulin full cost today (because I’m a Canadian currently living and working in Australia) which was $250 for 5 boxes of nova rapid penfill. That hurt financially and emotionally tbh. So last night when I ran out I decided to try a day without. I found an equation that said for basal it’s around 0.25 * your weight in kilos. So I did that and added a few units and so far so good fam. My bg range for the day has been 4.9 - 8.8. Think I might pump break for a few more days - kinda feels nice without anything attached to me. Maybe once I’m home in February I might take a longer break off the pump and go back on pens for a bit. I usually switch back and forth every few years. What do you folks think? Anyone else try a pump break?
