Need a Neck Fix? Coming Right Up! - Hyperdiluted Radiesse
The Aging Neck
The aging neck has bothered people for as long as necks have aged. In recent times, surgery was the only option to fix the skin laxity and crepe-like look that accompany a well-lived life full of birthdays and sun. Likewise, sun-damaged or sagging skin on the chest and arms has also been next to impossible to treat. Of course, we still try, with creams, which might help temporarily (as in hours), and laser treatments, which are great for pigmentary concerns but lacking in regards to the neck lines or arm skin laxity.
Enter Radiesse. You might be thinking, “Wait, Radiesse has been out for years. How can this be a new treatment and why haven’t I heard of it being used in these other areas before?” Well, while I don’t have a good answer as to why we in the medical aesthetic community hadn’t thought of this sooner, I will tell you what we know now about its novel use in the neck, chest, and even arms!
About Radiesse and its Typical Use
We have known for years is that Radiesse has both volumizing (or filling) properties and biostimulatory (collagen regenerating) properties. We have always used it primarily as a filler, realizing that people would get a secondary benefit of some improved skin in the areas injected but not necessarily making that a focal point. But what if we wanted to use the biostimulatory properties as a primary goal instead of the filler properties? One thing about Radiesse that makes it great for restoring lost volume in the cheeks is that it is a bit on the thicker side, but to inject it into neck lines, for example, would be difficult and we would expect visible bumps to show. For that reason, we haven’t used Radiesse solely for skin remodeling or for any reason in the neck, chest, or arms. 
Why Didn’t We Think of This Before?
So far in this post, you have learned that we have a filler/ biostimulator historically being used primarily as a filler because it’s a thick gel and will not easily spread out. But what if we want it to spread out so we can make much more use of it’s biostimulating, collagen regenerating properties? The solution? Make it less thick!
Isn’t it funny how the simplest ideas can sometimes yield some of the biggest triumphs? In this case, when we add simple saline and lidocaine at various dilution rates (depending on the area being treated), then we get an easily spreadable biostimulator capable of remodeling skin over a larger area and not so much a filler anymore. Bingo. We might use 1 part Radiesse to 1, 2, or 3 part(s) saline/lidocaine and now a single syringe of Radiesse can treat the underlying skin of the entire neck, for example. As a rule of thumb, 1 syringe of Radiesse will treat one or two hand-sized areas depending on the ratio used for a given area.
I do not even need to describe the benefits, as pictures tell a thousand words. But I will say this and I’m sure you’ll agree: incredible.
FAQ
Results of collagen remodeling take time. With Hyperdiluted Radiesse, results are seen as early as 4 weeks, which means you will see very little, if anything, before one month’s time.
Results typically peak at month 4 (four).
Stability is achieved at month 9 (nine), meaning the results you still see at month 9 is the new you! It is the long-term, persistent new collagen that you get to keep.
More than one treatment may be necessary to achieve optimal results. Two treatments are typical, though severe sun damage may require more.
A cannula is used during the treatment, and therefore, there is typically very little, if any bruising. Because it is being injected into a larger surface area, there is little to no swelling, either.

Why Radiesse for Cheeks is Still Hot in 2020
Radiesse dermal filler (Merz Pharmaceuticals) has been around for over 14 years. Made of calcium hydroxyapatite in a water-based gel, Radiesse breaks down into calcium and phosphorus (already found in our bodies), so it is a naturally-based filler option with a nice longevity profile (12—18 months). Radiesse was initially FDA approved for the long-lasting correction of moderate to severe facial wrinkles and folds, and since then many practitioners have found many uses for it in the medical aesthetics field. 
Why I Love It
I personally love Radiesse as a structural filler, as it is thicker than its hyaluronic acid (think Juvederm (Allergan)) counterparts and provides a nice lift when used in the cheeks, for example. I also frequently use it in the jaw and the temples. What do we subconsciously look at, besides wrinkles, that makes us instinctively know that someone is of a certain age?
Your brain loves to classify things and there are lots of things that clue our brain in to an approximate age of a person we are looking at.
A few subtleties, from the top, down, excluding wrinkles:
Thinning hair/receding hairline - This generally doesn’t happen until later in life so is typically a sign of aging. PRP treatment is already showing promising results in treating this condition.
Sunken temples - no one comes into the clinic exclaiming, “My temples have sunken!” But I know that if there is a concavity to the temples, the person looks older. Fill in the temples (using a filler like Radiesse), and in a subtle, yet obvious way, the patient typically agrees they look younger yet wouldn’t be able to put their finger on the reason if they didn’t know.
Hooded eyelids - although many people (myself included) have low-riding brows, excess skin on the upper lids gives away age.
Under-eye bags - these can come in the form of a tear trough, or depression from the inner corners of the eye diagonally downward about an inch; or they can be full-blown herniated fat pads. The former can be treated with filler (I prefer Belotero for this) and the latter must be corrected by surgical intervention.
Flattened cheeks (as fat pads fall forward) - This happens to 100% of people. If there is one thing a person needs first, filler-wise, it is almost universally cheeks that need restoring. Typically we notice that a person’s cheeks, usually right as a continuation of the tear trough mentioned earlier, start to flatten and the weight of the cheek dips below the line of the nose at the bottom. When we see this, it is obvious a person needs cheek restoration.
Smile lines from the corners of the nose to the corners of the mouth (nasolabial lines) - This is often corrected by treating the cheeks. At the very least, it is made better by correcting the cheeks and sometimes I save a little bit for this area itself. In my opinion, if you go to a place and the practitioner sees a line and wants to fill the line, you will not get the best, most natural result. I like to ask where this line comes from. It comes from the cheek falling, so if you just fill the line and still have the cheek falling forward onto it, it won’t look natural. Naturally, the cheek is up, holding everything up when there aren’t nasolabial folds to be seen, so it only makes sense that if we put things back to where they used to be, it will look natural, because those structures used to naturally be there.
Downward turn of mouth corners - Often I see patients who have had prior filler procedures, yet the provider did not correct the downward turning lip corners. This is an injustice, in my opinion. The mouth makes up a third of the area of the things on the face, so if the corners turn downward, the person is going to look like they are worried or upset, unless of course, they are able to walk around with a full smile on their face the entire day. Anything pointed downward must be corrected if the goal is a more youthful look, whether it is glabellar lines, tear troughs, the break in the cheeks, or the downward turning of the mouth corners (or “marionette lines” as they are sometimes called, after the puppets that can open and close their mouths when the puppeteer commands).
Jowls - Certainly this is universally a sign of aging. There used to only be surgical fixes for this, but nowadays we have a couple of nonsurgical options. If it is mild, we can often use fillers (like Juvederm Voluma or Revanesse Versa) placed in front of the jowls to reduce their appearance. If it is more moderate in severity, we now have PDO threads, which are pretty great in the right hands. PDO threads are textured, absorbable suture material that has been around for over 30 years, often used in heart surgeries. They are placed right under the skin, so they are less invasive than Botox or fillers, which are placed in the muscle and sometimes right over bone, respectively. One day someone had an idea, “what if we stick these in the face?” and in this case, it turned out to be a good idea. And so the PDO movement was born.
Vertical neck bands - These generally only exist in people who display signs of aging. Often they can be corrected by the neuromodulators Botox/Dysport/Xeomin/Jeuveau as they usually originate from a unique muscle called the platysma muscle. The platysma is the only muscle that is integrated within the skin and other muscles and doesn’t cause movement by directly connecting two bones. You are using your platysma when you draw out the corners of the mouth to the side and downward. Vertical bands are often easily treated with neuromodulators.
Those are most of the rest of the signs of aging on the face besides wrinkles. Personally, I think it’s the structural things that give it away the most - cheeks, temples, downward turning lip corners.

Why doesn’t my upper teeth show when I smile after I did Botox around the eye?
There are muscles that allow you to smile (black arrows point to some of these on picture below) that attach to the circular muscle around the eye, the orbicularis oculi (circled in red) that are treated when you get get your crow’s feet taken care of. Injections are typically placed in a few spots around this muscle (blue X’s). We know Botox relaxes muscles. If even a little was placed too low or migrated (this is very rare) downward (such as in a path indicated by green arrows), then those strap muscles couldn’t effectively lift the corners of the mouth and you might not be able to fully smile for up to 3–4 months.
This should not routinely happen. If you hear about this happening more than once — ever — at a particular practitioner’s office, you may want to consider going elsewhere next time as either the injections are being misplaced (too low) or the way they reconstitute the Botox is too dilute (same amount of the drug in a greater-than-normal amount of saline), which could increase the risk of product migration.
It is Imperative to see a skilled, knowledgeable practitioner when you get treated. One who understands facial anatomy and knows how all this works.
