Part 1 of Intraoperative Resuscitation

And there I was, lying on the operating table, intubated, with an open incision over my sternum, my heart having stopped beating, and with the ventilator breathing in my place, the worst thing I could have hoped for happened: cardiac arrest. I was dead.

One of the surgeons began performing CPR on my lifeless chest. The ventilator continued to breathe for me. This continued for a while until my heart began to move erratically and uncontrollably.

There, another surgeon called for the defibrillator paddles, applied a conductive gel, and placed them on my exposed chest. I could hear him say, "Charging to 150 Joules." All that remained was to wait for the cardioversion to take effect. When the paddles were ready, he said, "Preparing shock, no one touches the patient," and then said, "Clear!" At that moment, my body shook from the electric shock that ran through my heart, but it was still beating erratically.

The same surgeon would repeat the action, but this time he said, "Charging to 250 Joules." It had to work this time for the surgery to continue. Then, the paddles were already charged, and he warned the rest of the staff that no one could touch me, and immediately afterward he said, "Clear!" My body received another electric shock directed at my heart, but with the same result: my heart was still fibrillating.

The same surgeon would administer another shock, but this time he said, "Charging to 300 Joules." I was only expecting the worst, and with the paddles charged, he said, "Clear!" once more. My body shook violently from the electric shock directed at my heart, but it was the same result: ventricular fibrillation.

He prepared another 360-joule shock. My body began to shake violently again, but this time my heart stopped: asystole, I was dead again. The surgeons performed CPR on my chest again, injected adrenaline, took me off the ventilator, and air was delivered through an Ambu Bag connected to my endotracheal tube. The same cycle was repeated.

Will I be able to leave from the OR alive, or will I end dead?

Kotone's cardioversion

Kotone Shiomi was feeling very dizzy when she was on her dancing routine. Unable to even stand up by herself, she went to a nearby clinic with help of her friend Fuuka. The doctor made her lay down on a bed and immediately connected a set of AED pads on her, as she knew what was happening. Kotone was suffering from Ventricular Tachycardia, which could result in a cardiac arrest if untreated. Her heart was racing between 160-180 bpm, so Tae Takemi, the doctor, sedated her and immediately started a cardioversion, to be able to get a sinus rythm again. Three shocks were needed but, in the end, Kotone's heart went back beating as usual.

Alice was laying on the bed, her heart had just started to beat again. The monitor was showing a sinus rhythm while she was breathing through the ventilator. All of a sudden the alarm started and the monitor showed asystole again. The doctor immediately entered the room and started cpr. After a few cycles he removed the hospital gown and left Alice's chest completely naked. The resuscitation continued with cpr and lot of defibrillation until the nurse finally regained a sinus rhythm.
