Access to healthcare is a fundamental right, but millions of Americans face significant barriers in receiving the care they need. For those living in rural areas, urban centers with shortages of healthcare providers, or communities with high levels of poverty, obtaining basic medical services can be challenging. This is where health centers play a critical role, serving as essential lifelines for underserved communities across the United States.

What Are Health Centers?
Health centers, particularly those funded by the Health Resources and Services Administration (HRSA), are community-based organizations that provide comprehensive healthcare services. They operate in areas that are designated as Medically Underserved Areas (MUAs) or Health Professional Shortage Areas (HPSAs), where healthcare is otherwise scarce. These centers provide care regardless of a patient’s ability to pay and offer a sliding fee scale based on income.
Addressing Healthcare Disparities
One of the most important roles that health centers play is reducing healthcare disparities. Individuals in underserved communities often face a lack of access to medical facilities, higher rates of chronic disease, and poorer health outcomes. Biden Turbocharges Clean Energy in Underserved Communities

Major developments aimed at tackling energy inequality are happening. The Biden administration is significantly boosting incentives and resources to drive clean energy deployment in underserved communities across America. The U.S. Department of the Treasury and Internal Revenue Service announced today that applications will open on May 28 for the second year of the Low-Income Communities Bonus Credit Program - a key provision of President Biden's Inflation Reduction Act providing up to a 20 percentage point tax credit boost for solar and wind projects located in low-income areas.   Deputy Treasury Secretary Wally Adeyemo said: "This groundbreaking incentive created by President Biden's Inflation Reduction Act is creating jobs and opportunity while lowering energy costs for communities that were long underinvested in." "In the program's first year, we saw sky-high demand for these clean energy investments."  
Boosting Capacity and Equity Provisions
Program Enhancements for 2024: Over 2.1 gigawatts of total capacity available, up from 1.8 GW in 2023 325 additional megawatts rolling over from very high first-year demand Expanded eligibility criteria and subcategories for equitable distribution At least 50% of capacity reserved for projects meeting additional criteria   Targeting Low-Income Residential Areas The program allocates federal tax credits to qualifying solar and wind facilities under 5 megawatts located in low-income residential areas, on tribal lands, or providing direct economic benefits to lower-income households. It aims to lower energy burdens while catalyzing private investment in historically underserved communities.   Ensuring Investments Reach Most In Need This second program year also incorporates key enhancements based on lessons learned in 2023. Most notably, at least half of the available credits in each category are now reserved for projects meeting additional socioeconomic and community benefit criteria beyond just geographic location. "The program's impact will grow even more in its second year thanks to the increased available capacity and new provisions to ensure investments reach the communities most in need of affordable clean power," said Senior White House Climate Advisor John Podesta.  
Turbocharging Clean Energy Investment
This incentive structure has already turbocharged private capital flowing into community solar projects, rooftop installations, microgrids and more across America's urban cores and rural towns. According to a recent Princeton study, low-income areas received over 5 times more proposed solar investments in 2023 compared to 2022 baselines.   Concerns Over Equity Remain However, some environmental justice advocates argue the program's impact has been inequitably distributed so far, with wealthier communities capturing a disproportionate share of the tax credits. The new 50% set-aside aims to address those concerns. "While a step in the right direction, we need to see much more from this administration in terms of actually delivering affordable clean energy to the frontline communities who have disproportionately borne the brunt of fossil fuel pollution for decades," said Juan Jhones-Lugo, Energy Justice Director at The Oak Collective.   Helping Families Navigate Incentives The Treasury Department will host a public webinar on May 16 providing additional details and guidance ahead of the May 28 application opening date. All submissions within the first 30 days will be treated equally to ensure fair access. But beyond the tax credits themselves, the Biden administration is deploying a multi-pronged strategy to ease clean energy access and lower costs for American families - especially in underserved communities.   Multi-Faceted Approach Key Administration Clean Energy Cost-Saving Initiatives: $20 billion Greenhouse Gas Reduction Fund to finance community lending State home energy rebate programs launching this summer, worth up to $8,000 per household Expanded tax credits of 30% for residential solar, heat pumps, efficiency upgrades Nonprofit coalition campaigns for consumer education and awareness Renewed focus on bridging financing gaps for low and middle-income families   Driving Affordability and Sustainability "We're using every tool available to help families across America save money by tapping into affordable clean energy," Adeyemo stated. Rotation 2: Emergency Medicine
I survived the rest of my Behavioral and Mental Health rotation (truth be told, I actually ended up quite enjoying it by the time all was said and done), and passed my first EOR with flying colors! It is not lost on me that psych is said to be among the easiest, and my current rotation (emergency med) is among the hardest.
I gave up my apartment for clinical year, moved everything to storage, and sent my doggo to live with my parents in GA. I miss her more than I could have ever imagined (that’s a lie, I knew it’d be bad), and can’t wait to see her at the end of this month. Because I knew my rotations could be virtually anywhere, I felt it financially responsible (I first typed easiest, but had to change- there is nothing easy about living out of a storage unit/your car) to give up my apartment, save on paying double rent, and go wherever my clinical coordinator could send me. I think that’s working out in my favor in terms of experience.
I am currently in very rural Ohio, at an underserved 240 bed private community hospital, where I learning more than I ever thought possible. I am wrapping up week two (I am working this weekend), and I have already sutured multiple times, as well as diagnosed some pretty wild stuff. I love my preceptors (I have about 5 in total), and everyone here is such a family. They are all excellent teachers, and really want me to do well. They also continue to push me out of my comfort zone and force me to learn, which I love.
The hospital puts me up in housing, so I am living with three other medical students, which is great. Since there isn’t much to do around here, I have been studying a lot in my free time and I think that will set me up well for the EOR. I am going through the major systems first (I just reviewed cardio, and wouldn’t you know the next day I was able to catch and diagnose a 3rd degree heart block- as it turns out, STUDYING WORKS!) so I am feeling so motivated that my studying is actually paying off in what I am doing every day.
Next, I need to focus on really learning meds and side effects for all of the major disease states. Some I know a lot of the “main meds,” but not 2nd or 3rd line... and some I am embarrassingly bad at. But now that I am seeing them so often, I think this will help.
The only thing that I find particularly challenging about this rotation is any time we see a subarachnoid hemorrhage (believe it or not, I’ve seen two already). After an autopsy was done on my best friend, it was determined that she’d had an aneurysm and died from a SAH. It helps to know that even if her husband had been home, the likelihood that he might’ve been able to save her before she suffered major permanent brain damage was slim. But there is this part of me that feels like if she had just called me and told me how she was feeling, that she didn’t feel right, and explained her symptoms, we might have been able to save her and Baby Blair. The rational part of me recognizes that this is a ridiculous thought, and that I cannot absorb any blame for something so tragic. However, the part of me that misses my best friend with every fiber of my being can’t help but wonder if I could have done more had I only known.
This post deteriorated rather quickly, and while I would usually apologize for that, I am working on saying ‘thank you,’ instead of ‘sorry.’ So thank you for letting me feel my feels, however irrational they may be, and letting me get it out into the universe.
I am so bummed that this rotation will come to an end at the end of the month. I have a 5-day weekend after my EORs/callback, during which time I am going to drive the 500 miles home to see my dog, get hugs from my parents, and sleep in a bed that is more mine than any bed I have slept in for the last 2 months.
And then, I will head back to Ohio for my third rotation, which is critical care. This is an elective rotation for me, and I am so thrilled to be stepping back into an ICU. I miss my days as a nutrition support dietitian, and cannot wait to learn all about managing ICU patients as a mid level provider!

The reason why I do what I do
I made it to Bangkok, yay! And it was absolutely beautiful, despite the heat and humidity (although my skin definitely appreciated getting away from the dry Utah air). First stop was the Grand Palace, home of the Kings of Siam (now Thailand) since 1782 and a very popular tourist attraction.

As an almost-graduate PA student, the objective of this international elective rotation was to observe the different fields of medicine and get a taste of how they’re practiced in this country. My classmates and I spent one week at Sirindhorn Hospital in Bangkok. We attended lectures about Thailand’s tropical diseases, toured several departments and watched their healthcare teams in action, and even enjoyed an afternoon of traditional Thai dancing and aerobics.

For one of those days, we were asked to follow the home health team. This was a group of providers who visited the homes of patients that are unable to travel to the hospital, and they would provide care directly at their bedside. This was already a unique experience for me because all my previous rotations in PA school had only been in outpatient or inpatient facilities. So, we left the crazy city of Bangkok and drove to the quiet outskirts, to a single place that I will never forget.
There was a patient—an elderly woman alone in a bed, completely nonverbal and immobile, connected to a feeding tube and urinary catheter—whose residence was under a bridge over a still local river. Yes, in order to access her home, you had to cross over a wobbly wooden plank into a small nook, supported by more unsteady floorboards. The roof, being the bottom of the concrete bridge itself, was so low that you could not stand up straight. It was dim and musty, with debris and dismantled furniture to the left, chickens to the right, pots and pans in front, tangled wires above, and barking dogs below. The only slight movement of air was provided by a fan next to her bed, accompanied by the muffled static sound of a small tube TV.

I was completely stunned by the sights of this patient’s living conditions. I couldn’t fathom the thought that she’s dealt with this every day and had no one even living with her. However, I felt grateful that this lady, who had several medical conditions including poorly controlled diabetes, routinely received wound care from the home health team. The providers worked so quickly and efficiently to clean up her foot, and they exhibited such a positive aura the entire time. Thus, I was comforted knowing that patients like this woman had not been forgotten and that she would continue to be in good hands. I felt thankful that the home health staff was so willing to share this part of their journey with us. Nevertheless, I can’t imagine how many other people are still out there in similar conditions flying under the radar.

My heart lies with these underserved communities, especially those outside of the U.S., who don’t have reasonable access to health resources. As I had mentioned in my very first blog post, my goal is to continue reaching out to the medically underserved as a PA in the near future—my duties in these regions definitely don’t stop here in Thailand. I want to keep lending a hand to those who really need it, even if I am just one person, and embody my program’s mission. Healthcare is a right, not a privilege. And that is exactly the reason why I do what I do.
