Let's Talk about Xyrem.
Xyrem ("oral sodium oxybate" or the sodium salt of gamma-hydroxybutyrate) is used in the treatment of narcolepsy, as well as (sometimes) idiopathic hypersomnia. Even if you don't have narcolepsy or any related conditions, you may find this run-down interesting. Here's why:
Gamma-hydroxybutyrate is roofies. That's right. Date rape drug. Right here.
The drug is so tightly controlled that there is one pharmacy in all of the United States that can fill it. Doctors must be approved and participate in a special program to even prescribe it.
Nobody really knows how it works in the treatment of narcolepsy.
I was prescribed Xyrem quite some time ago - at this point, nearly a year back. It took six-plus months of insurance, doctor's office, and central pharmacy wrangling to get the drug to my door. The whole time, I wondered: what should I expect from Xyrem? How do I know if it's working? How do I know if it's not working? What's it like? Lists of side effects and contraindications are readily available online, but I couldn't find a single detailed testimonial. This bothered me.
I've been on Xyrem for about a month and a half now. Here's what I can tell you about it.
You have to wake up at night to take a drug that's meant to improve your sleep. Everybody I explained this to found it funny. The standard practice is to split your dose in two - to take one half at bed, and the other half two to four hours later. If you're on Jazz Pharmaceuticals brand Xyrem and not the generic, they send you a tiny little alarm clock with a light on it to facilitate this. I have never needed it.
It takes 3 weeks to titrate up to the "full" dose, which is also the maximum dose. From there, you can titrate back down to a lower dose if you're experiencing unpleasant side effects. I'm in the process of doing this myself.
It doesn't necessarily knock you out. From the way the drug is described, one might get the impression that the moment it kicks in, you're going to be unconscious. I didn't find this to be the case. Your mileage may vary; I don't always fall asleep at all on the first dose, but it does at least get me sleepy enough to fall asleep on the second.
You have an unusual amount of agency in how you take Xyrem. This surprised me, especially given how tightly controlled possession of this drug is. For example, I metabolize Xyrem really fast. If I take it in two doses, I will sleep a maximum of 6 hours. I take the same amount of medicine and split it into three doses instead to compensate for how fast I metabolize it. That way, I'm more likely to sleep about 8 hours. This isn't just accepted, it's encouraged. You can even take a bigger dose first and a smaller one second, or vice-versa. The only hard and fast rule is: do not go over the max dose.
If you take it with alcohol, or within 4-6 hours of alcohol, it could kill you. A lot of drugs warn you not to take them with alcohol. I cannot stress enough that if you have ignored that warning in the past: do not ignore it here. Do not. Xyrem is a powerful CNS depressant. Alcohol is a CNS depressant. It really can kill you.
It works(?) Like many drugs that act on the brain, nobody is really sure how Xyrem works. It doesn't affect the most common (known) cause of narcolepsy (a lack of orexin/hypocretin). It's theorized that the drug acts on GABA receptors in a way that "consolidates" the fragmented sleep architecture of narcolepsy.
Narcolepsy can be thought of as an autoimmune disorder of sleep architecture. The sleep architecture of a narcoleptic is irregular, both within itself and from night to night. People with narcolepsy tend to have less of the deep sleep stages than they should. Narcoleptics also have a high percentage of stage 1 (light) and REM sleep. It's theorized that excessive REM occurs because it is of poor quality/does not serve its intended function, so the brain spams REM in an attempt to compensate. A diagnostic trait of narcolepsy is the ability to enter REM within 8 minutes of falling asleep - if sleep architecture is normal, this does not occur. While not all people with narcolepsy have cataplexy, cataplexy itself is actually REM intrusion into waking life. The narcoleptic brain is that screwed up about REM. Xyrem appears to regulate shifts between sleep stages and reduce the nightly percentage of REM sleep. I used to dream nightly. Subjectively, I do not dream at all on Xyrem.
The only difference between Xyrem and Xywav is salt. A full 9g dose of Xyrem contains 1,640mg of sodium. The maximum sodium intake recommended by the American Heart Association is 2,300mg. One of the few things I saw said about Xyrem prior to taking it was that it was disgustingly salty. It is very, very salty. I don't mind it, though. I've seen it said that Xywav tastes much worse, but I can't attest to that.
Subjective experience
Xyrem comes Priority Air Mail in a sizable cardboard box. An adult with ID must be present to sign for it. The first month's prescription comes with a light-up alarm clock. This kit and all subsequent kits come with:
The medicine, in however many bottles are required
A number of syringes, marked with common doses
A number of pill bottles
You put water in the pill bottles. They tell you to put about 60ml, but as far as I can tell, this is to make the saltiness tolerable. I made a little game of this - I try to put the same amount of water in each pill bottle, gauged by nothing but sound. I've gotten pretty good at this. I have my nightly dose split 3 ways. After adding the drug to the water, I close each bottle and swirl it a bit. I don't know if this actually does anything.
Xyrem works best if you're already tired when you take it. Hilariously, I have ADHD in addition to narcolepsy. Nighttime sleepiness isn't a thing I Do naturally. Consequently, the first dose of Xyrem only puts me to sleep about half the time, and it takes a while even when it does. I know myself well enough to know that if I wait until I'm actually "sleep for the night" tired to take it, I might be up until 3 or 4 AM. Instead, I take the first dose at around 11 PM. Even if it doesn't put me to sleep, it DOES make me sleepy enough that the dose I take 2-3 hours later will definitely work.
I was very careful to set alarms the first week or so of taking Xyrem, but I've never needed them. For reasons that are unclear to me, I always wake up when Xyrem is fully metabolized. Without more Xyrem, there is no urge to go back to sleep. When I've run out of doses for the night, I'm up for the rest of the day. There's no napping.
Some people have pretty nasty side effects with Xyrem. Headache and nausea are the most common. I had both of those, once each. The headache lasted all day but was otherwise unremarkable; the nausea was genuinely awful. The only persistent side effect I have, is tremors.
While I was waiting for Xyrem, my sleep specialist put me on Adderall. Nobody would prescribe this for ADHD, but you'll do it for narcolepsy? Sure, whatever I'll take it. I mention this because I thought it was possible that Adderall was causing tremors. I ran a little experiment: I took Xyrem but no Adderall one day and still had tremors. I took Adderall, but no Xyrem the night before, and the tremors subsided. It's definitely the Xyrem. While this is a known possible side effect, I can't find any information on how or why Xyrem, a CNS depressant, would cause something that seems very much the opposite of a depressed central nervous system. I am currently titrating back down from the max dose in an attempt to see if a lower dose will mitigate the tremors. If that doesn't work, I'm not... entirely sure what to do. Xyrem is a weird drug. It's strange not to dream at all; it's strange to wake up twice a night and still get better sleep than I ever have. Executive function has improved considerably, given that both neurological issues that cause executive dysfunction are being treated. Still: the tremors are, I will not lie, distressing. Not as bad as they were on Wellbutrin, which I was forced to discontinue! But - disruptive and distressing, nonetheless. I'm hoping that the tremors will stop eventually, or that dose adjustment will help.
Overall: would recommend if you have narcolepsy. You wouldn't think that a drug that obligates you to wake up multiple times a night could improve your sleep! Well, bucko, if your sleep architecture is already so disordered that you have narcolepsy: it can.

Ignoring his inflammatory rhetoric, do you think Bernie's *POLICIES* are actually that extreme? I've read them over and, if he had a softer approach, they seem like they'd be sound policies to me
The problem with Bernie is that it's hard to know, in actual practice/reality, what his policies are. Yes, on paper, everyone knows what they are, and it's not a week if he's not writing yet another op-ed in the Guardian about Workers' Rights or Climate Justice or whatever. But like.... what good does that do anyone? Everyone who reads the Guardian probably agrees with those things already, and that includes me -- I read it, I financially support it, etc., but it's not where I come for policy actions or where I want to see US senators spending all their time. (Like, Bernie, aren't there other people you could be talking to, who could actually do something about this?) It's the definition of preaching to the choir, where he's regurgitating a boilerplate piece of progressive ideology but not actually doing anything about it or attempting to reach an audience who doesn't already agree with him. (Plus the Guardian, unfortunately, is always also willing to let his ex-campaign manager, David Sirota, bash the Democrats for something, but never mind that.)
Likewise, Bernie's actual voting record in the Senate isn't always a match with the things for which he has (very loudly) advocated, and he's a multi-millionaire old white man from Vermont (one of the whitest states in the Union) who has often seemed interested in preaching socialism for everyone else but resisting any scrutiny or participation in that for himself. He has gotten a lot of mileage and built a disproportionately influential political career out of championing so-called leftist progressivism/socialism, but as I keep saying about him, he never seems to do anything about it. "Tax billionaires" or "save the planet" are extremely broad-brush statements that everyone in the liberal camp can mostly agree on. And no, I wouldn't say those positions are particularly extreme; they're pretty much mainstream Democratic ideology at this point. Indeed, I think Bernie gets unwarranted traction out of positioning himself as the "radical" alternative to the Democrats, when most of the things he says are now basically part of the party platform and have been adopted or explored in some shape or form. Just because they can't actually be implemented at the moment, whether due to legislative roadblocks or otherwise, doesn't mean that they're not moving in that direction.
Likewise, Bernie's favorite hobbyhorse of Medicare for All is often used by his fans to irrationally bash the Democrats, as if we don't have universal healthcare -> quod erat demonstrandum, Democrats Are Neoliberal Shills. I've written many posts about the state of the healthcare debate in America and how passing even a much-watered-down Affordable Care Act cost Obama control of Congress for pretty much the rest of his presidency. So if Bernie and co. want to offer a roadmap for how to pass another major healthcare reform/overhaul that goes even further than the ACA -- trust me, everyone's listening and wants to know how to do that. That is neither extreme nor particularly, at least among Democrats, controversial in the way it was in 2009, when we still had Blue Dog Democrats in red states like Nebraska and South Dakota. But a) we have the united fascist bloc of Republicans who would object and obstruct it on principle, and b) we DON'T have enough Democrats to just pass it by fiat and have that be the end, because that's not the way things have worked in the history of anything.
So basically: if progressives want to endlessly harp on the Democrats for not magically pulling Medicare for All out of a hat, they're welcome to do that if, and only if, they can produce a credible policy/platform/program of action to actually get it passed that the Democrats could be following and aren't (and "they don't care enough about this and could fix it if they wanted!" is not that). That way, they could actually show that the reform is empirically possible and the Democrats are not carrying it out. But flatly ignoring all the political realities and blaming them for not producing a miracle in an extremely adverse legislative and political climate does not count as good-faith engagement and isn't directed toward any constructive end. It's just performative gesturing to show that they are Better Than The Establishment, something something something, and Bernie is usually one of the chief offenders in this regard, despite actually being part of the Establishment for decades.
I will give Bernie credit for two things: he has voted for all the major Democratic legislative packages with the relative minimum of selfish hostage-taking/sabotaging such as that carried out constantly by Manchin and Sinema, and he quickly shut down any talk of running (yet fucking again) in 2024, in order to support Biden's re-election bid. But as far as policies go, he's still not shown me that he either has a concrete plan to carry them out, that he knows what they are aside from broad-brush, vague and general talking points, or that he will put in the work to get them achieved, so yeah.

OBDSTAR DC706 Unbiased Customer Reviews
As we know, OBDSTAR DC706 ECU programmer is designed for vehicle ECM, TCM and body/other system for ECU flasher. It can read ECU info, R/W Eeprom/Dflash/Pflash/MAP, reset ECU (Virgin), calibrate mileage, read IMMO pincode, and recover ECU, etc via OBD and bench. What is the actual use effect? Let’s see the related customer reviews.
Tumblr media
Review 1:
Tool arrived. All work fine!!! Cadillac 6L45 TCM clone ok. GM Acdelco E39a clone ok.
Review 2:
GM TCM T87 and T87 you can do it yourself, full reading and writing. That was only available for flex, B-flash. Now i finished it with dc706. Amazing!
Review 3:
The DC706 is a good tool especially for the money. It is a bit clunky to work with the files. Otherwise, it’s a very formidable tool. We use it often on the DPS6 Ford TCMs as it is easy to connect when working mobile.
Review 4:
9GV OBDSTAR reading fine with everything included nothing separate!!! Very good device!
Review 5:
Audi Temic VL381 0AW gearbox clone ok. The dc706 obdstar did it perfectly. Not everything is flex or dim. And not fc200 either.
Review 6:
Fiat IAW 9gv is done, same as IO terminal without opening it. Amazing!
Review 7:
OBDSTAR DC706 is a good tool for PSA whatever.
Review 8:
The first test was a Guzzi miu g4 motorcycle engine control module on bench.
Review 9:
DC706 for the win on 2016 Chevrolet Colorado BCM. D70F3558/24c66.
Review 10:
Tested on DQ250 02E year 2016, two DQ200 2010 and 2012.
All works perfect, highly recommended!
Review 11:
I picked up this tool a few weeks ago and have spent some time testing its capabilities. The DC706 is a tablet based ECM, TCM, and BCM cloning tool, advertised as being able to read and write memory areas (eeprom, D-Flash, P-Flash) of supported control units, as well as some pin code extraction, all through OBD or bench mode (mostly bench).
The tool this most closely resembles is IO Terminal. I am not labeling this as comparison because I have never used IO Terminal, nor do I wish to open a debate on who cracks or pirates who's software, that's above my pay grade. I'll stop short of calling this a review, because I haven't fixed any cars with it yet, I've only tested it on various modules.
I had been considering the purchase of IO Terminal for some time for in house use. As an independent repair facility, where I see the benefit of this type of tool would mostly be on the GM global A stuff, TCM's in particular. As we all know, GM's transmissions are a weak link. One of the running jokes in the shop when a 6L80 equipped GM is towed into the shop… is it here for its 100,000 mile transmission replacement? Thus far we have been able to sell a factory or Jasper remanufactured transmission only requiring SPS2 programming. But as these vehicles age I envision used replacements will become an option for some based upon financial means and vehicle depreciation. Also in today's world of hit and miss module availability, this type of tool can provide another option. Tooling and software from Kess V2, Flex, PCMflash and the like are not financially practical for sporadic in house use, and I'm not likely to market module cloning as a sublet service anytime soon. The cost of IO terminal hardware and the various software packages can add up and has kept me on the fence about purchasing it, unsure of the return on investment.
I saw this dc706 ecu tool from OBDStar and it seemed intriguing. OBDStar makes some immobilizer and odometer correction tools, as well as motorcycle and marine scan tools. This new tool offered from OBDStar and has been on the market for about 6 months.
Checking the module coverage list it was pretty much identical to IO Terminal. The price point was good at about $1,400 for the full software version (ECM, TCM, and Body/other) plus another $100 for their P003 adapter for certain modules. It seems like the subscription fee will be a couple of hundred dollars a year to keep it current. So I decided to roll the dice and give it a try.
The first thing I noticed after activation of the tool was a version update that now includes ZF 8HP TCM's on all makes, including Dodge and GPEC-4 PCM's in boot mode. In addition, I turned the tool on a few days ago to discover another update that amongst other things included GPEC-2, 2A, and 3 PCM's. It's encouraging to see regular updates and expanded coverage.
I gathered a bunch of modules to test: E39A and E92 GM PCM's, 2 FCA RF Hubs, a Ford continental BCM, a GM Bosch BCM, and a GPEC-2 PCM. Guide and pin out diagrams are clear and accurate. Performed all reading and writing on all of these modules easily and successfully. VIN's were visible on eeprom and d-flash sectors, as well as pin codes.
Deciphering some of the verbage while using can be a challenge, but already having Chinese tablet based scan tools from Launch, Autel, and Topdon, I consider Chinenglish a second language. An example of this in photo number 2 while reading a GM Bosch BCM in bench mode, it gave me pause when I first saw it, but I speculate it was telling me not to wake up the BCM with ignition source voltage during read and write operations. I proceeded and successfully read and write the BCM.
No tool like OBDSTAR DC706 ECU tool perfect, I'm sure holes and flaws will be discovered, but at this point my confidence level is pretty high. I just hope it comes through when I need to use it “for real”.
I wanted to share this for those who maybe in a similar situation, it may be worth considering this tool.

Depending on how expensive your flight is, you can get more points. These JetBlue Points do not have a predetermined expiration date, which is a major bonus. Users can also rapidly buy Jetblue points and miles or trade them.
There are numerous methods to earn TrueBlue points, including traveling, shopping, dining, and making regular transactions. It's also simple to redeem them for reward travel when the time comes. Since TrueBlue points never expire, they will be available when you need them.
When you make trip reservations with JetBlue Vacations and Paisly, increase even more points. Thanks to our dining, shopping, retail, and credit card partners, you may even earn points without leaving your house.
Because they encourage loyalty and will reward you for choosing to fly with them frequently, Jet Blue Airlines is renowned for having the greatest frequent flyer programs. What You Need to Know About Plumber Costs Near Me
Before you decide to hire a plumber, you should know the costs involved. These expenses may include trip fees, hourly rates, and union dues. However, the costs are not uniform across states. Generally, the rates will be higher in high-income areas. In low-income areas, the charges will be lower. Regardless of the location, it is important to request an estimate before hiring a plumber. Hourly rates Plumbing labor costs vary widely across the country and are not uniform. For example, plumbers in high-income areas may charge higher rates. Meanwhile, plumbers in lower-income areas may charge less. Before hiring a plumber, make sure to get several estimates and ask about emergency service rates. The cost of hiring a plumber can range anywhere from $45 to $200 per hour. Generally, plumbers bill out higher rates to cover their overhead. Apprentice plumbers, on the other hand, charge less than the full-time rate. However, they should be supervised by a master or journeyman plumber. Apprentices will pay their supervisor's hourly rate. Hourly rates vary by region and workload. Trip fees Plumbers can add a trip fee to the bill when they travel from one job to another. This fee typically amounts to one hour's labor plus mileage. It's important to understand that these fees are not flat and may vary from company to company. The standard hourly rate for a plumber is $75, and you can expect to pay more if you live in a rural area. When calling a plumber, make sure to ask about trip fees. Most plumbing companies charge an additional $50 to $300 for travel time. This fee covers the gas and time the plumber spends in traveling to your home. You should also ask whether the plumber will charge you an additional trip fee if you are calling them out of their regular hours. Experience Plumbers play a key role in the construction process. They install and repair water and piping systems in a variety of settings, including commercial and residential buildings. Their work requires concentration and an in-depth knowledge of various construction problems. They must also have a solid grasp of math and science. In addition to the above-mentioned skills, plumbers must be able to work efficiently under a variety of conditions. Experienced plumbers have the expertise to deal with more difficult plumbing issues than others. They will consider every aspect of the job and come up with a long-lasting solution that won't cause future problems. A plumber with a lot of experience will be able to identify problems and fix them quicker and easier without creating a mess. They can also help you save money by avoiding costly and frequent repairs. Union dues Plumbers are members of the United Association of Plumbers (UAP), which offers many benefits to its members. These include medical, dental, vision, and retirement benefits. It also offers member assistance programs to help members with a variety of issues. In addition to these benefits, UAP members can also participate in a retirement program and receive training from the organization.
youtube
Union dues pay for the operations of the union, including staff salaries, office rent, and arbitration costs. They also fund the union's communications and organizing efforts, as well as member trainings. Additionally, many unions set aside a portion of their budget for a strike fund, which goes directly to the members in case of a strike. Job type Plumbers perform a variety of plumbing jobs, including the installation and repair of pipes and fixtures. They also work on sewage systems and gas lines. The types of problems they usually fix include burst pipes, clogged pipes, noises, and smells. You can also call a plumber for help if you're experiencing water pressure issues, low hot water, or malfunctioning appliances. Plumbing services can vary in price, depending on the job type. Some plumbers charge hourly, but most charge a flat rate for their services. This flat rate can include parts and other fees that may be involved in the project. Many plumbers charge a flat rate for standard projects, like installing a toilet. However, these plumbers charge higher rates for more complex tasks. Locality One of the biggest factors that affect the cost of hiring a plumber is where he or she lives. In more about Candu Plumbing of Chatsworth , plumbers are more likely to be experienced and earn a higher hourly wage. These factors increase the overall price of plumbing services. It's a good idea to use a plumbing cost calculator to get an idea of what you should expect to pay in your area. The average hourly rate for plumbers varies, but a general rule of thumb is $45 to $150. This cost range can be more or less depending on the plumber's expertise, geography, and the materials he or she uses. Plumbers are also more expensive if they have their own business and supervise others. Experience of plumber
Tumblr media
Plumbing is a crucial part of any home, carrying water into and waste out of it. As long as there are plumbing systems in your home, you will need a plumber to fix them at some point. You should be prepared for the inconvenience that plumbing work can cause. Best Plumbers in Chatsworth CA can prepare by removing valuable items from the home and moving them elsewhere. It is also a good idea to prepare for water shutoff during the work. Before you hire a plumber, it's important to know how much they charge. You can get a referral from a trusted friend, but you can also price shop. You should try to be as thorough as possible. It can help you find the best plumber at a reasonable price.
