Can you trust AI Answers about your health?
During the summer of 2020, when the entire world was focused on the pandemic, getting treatment for other health issues became a challenge.
I started experiencing a lot of back pain, but I shrugged it off as an injury and took Advil to cope.
A week later, I thought I had food poisoning.
I tried an at-home service where they pumped me full of IV fluids.
I went to urgent care. They sent me to get scans. I paid hundreds of dollars out of pocket to get them quickly.
The imaging place never sent the scans to the urgent care.
A day later, I had the worst chills. It was July, in Phoenix. Most likely 100 F. I went outside and was still freezing.
At that point, I was taken to the ER. I ended up in the ICU in one of those rooms they zipped up in plastic.
While most of the focus was on COVID, I had something else: a large kidney stone. The kind that doesn’t pass on its own.
I was in septic shock and acute renal failure according to discharge papers.
The hospital stay itself wasn’t too long, but the treatment with specialists took three months to complete.
Surgeries during COVID were extra special because if you tested positive, your surgery was delayed.
Two years later, in 2022, I ended up in the same hospital for the same reason.
And now, I’m a few days into aggressive antibiotic treatment for my kidneys, yet again. Wondering if I make it to the next followup or have to Lyft off to the ER.
So what does this have to do with Google AI Answers?
In 2020 and 2022, I spent a lot of time perusing Google Search results on kidney stones.
Now, I get AI Answers above at the top of SERPs (search engine results pages).
This wouldn’t be a bad thing if one could trust the AI to accurately summarize its sources.
That’s the big if.
In the first screenshot, you’ll find an AI Answer from Google Search results for the phrase how to pass kidney stone.
The first mistake involved an error with paraphrasing a source with legitimate information.
Because I can assure you after four years of seeing urology specialists, no one has ever suggested drinking two quarts or liters of urine.
The basil leaves suggestion, on the other hand, is suspect. I’ve never come across that as a suggestion. It would have stood out, because I have a lot of basil growing around the garden.
I don’t even remember that website from my previous Google searches. I had to check Wikipedia and other sources to find out what the company even was.
Much like the search quality raters and AI Answer checkers do...
In the second screenshot, Microsoft Bing with Copilot offered ads for supplements and advice from five sources, two of which are the MSN Health Hub.
The hub includes a section where you can Ask a health professional questions.
In the third screenshot, ChatGPT using GPT-4 with browsing offered a concise response based on its training data, but claimed not to have access to external sources.
In the fourth screenshot, Perplexity provided the best AI Answer with 19 sources I recognized from my previous research into this health issue.
Moral of the story:
You can’t trust generative AI with your money or your life issues.
But if you do, start with the right AI Answer engine. One that cites sources you trust and doesn’t suggest drinking your own pee.
Think of Perplexity as a better starting point for more in-depth research that you can discuss during your next doctor’s visit.

I teach patho to nursing students. Every semester since spring of 2020, I have taught about Covid. And every semester to have to change my lecture to keep up with new data. I was teaching Covid this semester and I told them that if nothing else, understand the science.
I explained how Covid attaches to ACE2 receptors in the nose, throat, heart, kidneys, and gut first. That’s why you see a runny nose, sore throat, myocarditis and acute kidney failure first. There aren’t as many ACE 2 receptors in the lungs, so Covid hits the lungs later.
That’s why you see lag time between initial infection and respiratory symptoms. More importantly, I explained that Covid is not just a lung disease, it is a massive systemic inflammatory response to the virus. Covid also upregulates a protein called bradykinin, causing clots.
If nothing else, I told them to think of Covid as huge inflammatory process that causes injury, scarring and clots to form everywhere. That’s why we are seeing so many strokes, MIs, weird liver failure in peds, bowel ischemia, and long Covid. It all points to chronic inflammation. There is no immunity as the variants keep changing, kind of like the flu, which is why we get flu shots every year. We’ve just let this run rampant, so a slew of Covid variants keep popping up, rendering treatments less effective.
The worst part of this was when one student said that their parents had gotten Covid and asked how to keep them safe from the complications. Another student piped in and said she had Covid 3 times and was she at risk too? It was my moment of reckoning. I realized that there wasn’t much I could say except to protect themselves with masks, vaccinations, and to try not to get Covid again. I told them that if they had Covid, they were at higher risk of long Covid and lantern complications. There was nothing to be done.
Except to protect themselves going forward. After that there was silence. One of my students then said how disappointed in government and public health leaders he was. Another told me she was disappointed in her professors and her school for not being more vigilant.
I told them that as long as they understood the patho of the disease, they could guard against misinformation. I told them to educate their families and patients about the science, not the rumors. This left me feeling so sad. What have we done?
If I can teach this to first semester nursing students, why couldn’t we have taught this to everyone? People do not understand the disease and no one bothers to explain it. Perhaps that’s why we have failed so miserably.
We need to backtrack and explain the disease before we start talking about immunity debt (a non issue) and endemicity. My students are so young. It’s heartbreaking.

Someone sent me lilies again! I know they mean well, but people really need to stop sending me flowers that can kill my cat. Luckily, I checked before taking the bouquet into my apartment, so my cat wasn’t exposed to them this time.
PSA: Don’t send lilies or bouquets that contain them to cat owners. They’re common in sympathy bouquets, but you should check any bouquet you send to cat owners for lilies. You may say it’s on the cat owner to check the bouquet, but plenty of cat owners don’t know that lilies are very toxic to cats. Consuming any part of a lily, its pollen, or even the water they were in can cause is an acute kidney injury that will kill a cat without medical treatment. Even in the best case scenario where the cat gets prompt treatment and survives without any issues, the cat has to be hospitalized for a couple of days, which is unpleasant for the cat, and the owners have a huge veterinarian bill. I paid $4,000 dollars upfront in an NYC pet hospital, and was reimbursed $3,000 by my insurance. I’m lucky that I’m in a position where I can afford this, but many people can’t, and I hate to think about what would happen in that scenario.
To the cat owners, know which flowers are toxic to cats. Lilies are just one of them. The ASPCA has a list of these plants. They also have an animal poison control center, which does charge a fee of about $95. You should absolutely call them if you think your cat may have consumed a lily or other poisonous plant. They will tell you what you need to do and work with the veterinary team at the hospital that you take your cat to devise a treatment plan. Their number is (888) 426-4435. RDA – The Recommended Dietary Allowance (RDA) for vitamin D is 600 international units (15 mcg) daily for adults through age 70 years and for children 1 to 18 years of age. Vitamin D intake and effective sun exposure are often inadequate in older adults. In older adults, particularly those with osteoporosis, we suggest supplementation with vitamin D. We administer 600 to 800 international units daily.
●Causes of vitamin D deficiency and resistance – Vitamin D deficiency can be caused by unusually low sun exposure combined with lack of vitamin D-fortified foods or malabsorption. Alternatively, impaired hydroxylation of vitamin D in liver or kidney can prevent metabolism into the physiologically active form. Rarely, genetic defects may cause the end organs to be unresponsive to vitamin D, as in hereditary vitamin D-resistant rickets (HVDRR).
●Vitamin D intoxication – Vitamin D intoxication generally occurs after inappropriate use of vitamin D preparations. Prolonged exposure of the skin to sunlight does not produce toxic amounts of vitamin D3, due to photoconversion of previtamin D3 and vitamin D3 to inactive metabolites. Symptoms of acute intoxication are due to hypercalcemia and include confusion, polyuria, polydipsia, anorexia, vomiting, and muscle weakness. Long-term intoxication can cause bone demineralization and pain. In children, the hypercalcemia can cause brain injury.
●Tolerable upper intake level – The Institute of Medicine (IOM, now called the National Academy of Medicine [NAM]) has defined the "tolerable upper intake level" (UL) for vitamin D as 100 mcg (4000 international units) daily for healthy adults and children 9 to 18 years (table 1).
