You guys are gonna have to forgive me if I come across a bit *off* in the next few days or so.
To be brief, it’s currently 00:39am, and April 25th. It’s been 2 years since my grandfather died. He was basically my dad. I called him dad. My grandparents have always had custody over me. They are my parents.
My grandad had kidney failure and was on dialysis since March 10th 2010. He was told he would only have 2-3 years if he was lucky. My grandad, being the stubborn and determined person he was, made it a whole 12 years going in and out of hospitals 4 times a week. Just so he could live to see me grow up, to be here for my grandma and I, and to make sure I didn’t grow up without a father figure in my life.
He wasn’t a typical father figure by any means. He was in an electric wheelchair the whole time I knew him. He had limited mobility. I grew up learning from him, however, that life’s pleasures can still be enjoyed.
He introduced me to the beauty of film and music. He taught me how to do algebra when I was 7 for no reason at all. He taught me how to play sudoku from the age of 9. He let me watch the same films over and over again. He stayed up with me when I was sick. He encouraged me to sing in public. He was brutally honest with me. He taught me how to defend myself, to stick up for myself.
Late 2021, when I was 19 and in my second year of university, he started to think about stopping his dialysis treatments. And by April 2022, the pain he was in, the fact he couldn’t eat, couldn’t sleep, was constantly tired, became too much. He made the bravest decision, and for the first time in his life, a selfish one.
I watched him over the course of 5 days deteriorate, cry in excruciating pain. I watched him die slowly. It shouldn’t have happened the way it did, and we were promised as much. But eventually he let go after days of fighting to savour every last moment of life.
The day before he died, his eyes, which were usually clouded in grey, shone the brightest blue I’ve ever seen in my life. I remember leaning over him into his line of sight. And I will never forget the smile on his face when he saw me. That’s how I choose to remember him. That was the moment I anointed him my Bright Eyed Superman.
He taught me that good people exist. Because he was one of them. The most selfless, caring, loving, generous human being I’ve ever had the privilege of knowing.
And I got to call him my dad. How lucky am I?

I did not have annoying medical inaccuracy on my bingo card for this week's episode of My Stand-In, then again I don't know what I've expected. Inaccuracy is like the Spanish Inquisition. Bane of my existence, the thorn under my nail.
Joe's mom gets hospitalised due shortness of breath. The doctors diagnose pulmonary oedema, caused by end stage renal disease / kidney failure. So far we're good, and I appreciate chosing something outside the usual cancer/heart disease pool.
But then the doctor tells Joe, that his mother has septicaemia and requires lifelong hospital treatment to stay alive.
First of all, what an ESRD patient has is called uraemia not septicaemia. Those are very different things. Idk if the mistake is on the scriptwriter's part or on the translator's, and I'd be grateful if someone who spoke Thai could tell me!
Second of all, dialysis is usually done by outpatient treatment! Although Joe's mom would have to visit a dialysis station every three days for the rest of her life, she would be able to live her life outside of the hospital! And that is without considering alternative treatments like peritoneal dialysis or a kidney transplant.
The two options for my second points are either the Doylist bad writing (bleh) or the Watsonian the doctor is trying to scam Joe. Which is ridiculous, but in this series, a crazy sublot like this might fly...

Cat update under the cut
My cat has basically tried to speedrun kidney failure. He went from "early kidney disease" last month (which we'd been monitoring for about a year", to "advanced kidney disease" sometime this month. The test results came back yesterday, and he was immediately checked in to the local animal hospital.
He has improved in the last 24 hours, though he's still a ways off from "fine".
I just got back from visiting him, and easily spent 3/4 of an hour giving him chin scratches. I also got him to eat, and left some of his home-food with the animal techs. (He's a picky little shit.)
They're hoping he'll continue to improve over the next 24 hours. However, the fluid treatment he's getting at the hospital isn't something that can be administered at home by a regular joe. So it's a matter of if he can get well enough AND stable enough to transition to home care without rapidly going downhill again.
(His specialist worked it into conversation yesterday that even though successful kidney transplants have been done in cats 20 years ago... No one in Australia does them or feline dialysis. I was like "... My mind hadn't gone there yet, but thanks? For letting me know?")
If the upward trend continues, I'll get another update sometime tomorrow morning 🤞

Mount Sinai Hospital Treatment Report: Johnathan S. MacNamara
Date of Admission: September 9, 2012
Date of Discharge: November 17, 2012
The patient was admitted to the Emergency Room at 6:24 AM, 09/05/12 and evaluated to be in critical condition almost immediately. He exhibited signs of septic shock, including: lung failure, kidney failure, liver failure, a blood pressure of 68/43 mm Hg, and a heart rate of 134 BPM. In addition, he showed signs of infection stemming from an abdominal laceration approximately 30 inches in length and on average approximately 2 inches deep. The exhibited signs included: seizures, a consistent 105° F fever, swelling around the wound, pus in and around the wound, and human fecal matter smeared across the abdominal region. The lattermost sign was not so much a symptom as an obvious cause.
The patient's wounds were immediately cleaned with heavy disinfectants and a tracheostomy was preformed. He was attached to a mechanical ventilator and the blood-oxygen levels slowly stabilized. Patient was admitted to the ICU for further treatment at 7:36 PM, 09/05/12. Injuries upon admission were mapped as follows:

1. Bullet entry and exit through right deltoid. 2. Large laceration across midsection. Shows signs of heavy bacterial and viral infection. 3. Second-degree burn in the shape of greek letter omega. 4. Electric burns on lower right back. Presumably from high-voltage taser or cattle prod. 5. Intense electric burns in genital and inner-thigh areas. 6. Hamstrings lacerated. Presumably to constrict movement of legs.
Upon admission to the ICU, the patient was administered a standard dose of penicillin. There had been no signs of consciousness since arrival to the hospital, however the patient did appear to be in a survivable state. Within the unit, he was attached to a dialysis machine and given a steady IV of saline solution and another one that contained water-soluble vitamins and nutrients. Once the infection showed no further sign of spreading, the patient was given an emergency liver transplant. This occurred on 09/06/12.
The transplant succeeded, and the liver immediately began to filter blood and further cleared the infection. The decision was made to keep the patient on the dialysis machine until the infection and subsequent sepsis was cleared, and after a partial recovery from the first transplant occurred.
After 57 hours of total unconsciousness and unresponsiveness, the patient was declared to be in a coma caused by toxic-metabolic encephalopathy. He was assumed to wake up within the month.
The tracheostomy ventilator was removed on 09/23/12 and replaced with a laryngoscopic ventilator. This was changed once more on 10/03/12 when it was replaced with a mask ventilator.
The genital burns required a skin graft, but otherwise healed normally without infection or complications.
At 1:58 PM on 10/09/12, the patient began to react to outside stimuli. He moved when touched and made audible responses to pain. Over the next few days, he began to react to auditory and olfactory stimuli in addition to touch-based reactions increasing in consistency. By 10/12/12, the patient could follow simple commands and had managed to open his eyes a few times.
On 10/15/12 at approximately 12:30 AM, the patient awoke in a frenzied state. He attempted to rip out IVs and pulled off the ventilation mask and appeared to be unaware of where he was. It took excessive physical force, but he was eventually restrained and slowly calmed. He fell back asleep soon after, but showed greater signs of healing by that point. The next afternoon, he gave an official record of what happened to him, which provided a more accurate method of how to treat him. He consented to a kidney transplant and went into surgery on 10/18/12.
There were no complications during the surgery, and the donor was the wife of the patient, Rosalind M. Schaffer. She didn't suffer any complications from the donation either.
After the surgery, he was moved from the ICU and into the recovery wing with a private room.
The patient was dosed on standard pain medication over recovery, and showed no infectious flare-ups over recovery. He was monitored for lasting impacts of his wounds or disease, and had gained a noticeable tremor in both his hands. He underwent physical therapy while in the hospital's care, allowing him to regain full motion in his legs and to lessen his hand tremors.
The patient was discharged at 8:14 PM on 11/17/12 with a clean bill of health and prescribed a pain medication for as long as needed. It was also recommended that the patient stop smoking, as his lungs had previously failed once and it could be a major risk factor. The patient obviously did not follow the advice, as he was seen smoking a cigarette the second he left hospital grounds.

The Doctors said he was lucky they caught it when they did, considering the common early detection signs and symptoms for his condition hadn't occured. Luck had nothing to really do with it, Emily had been the one to convince him to get tested. First goading him to try and prove her wrong. And when that tactic didn't work, guilt tripping him with full on puppy dog eyes pleading, for her sake and peace of mind.
They told him that he had Polycystic kidney disease (PKD), Autosomal dominant polycystic kidney disease (ADPKD) to be exact. That his condition is a genetic disorder that causes cysts to grow in the kidneys, where they can disrupt functioning.
His high blood pressure had been noted in his charts by his regular physician with the suspected underlying cause of the stress due to a combination of his job and age. Yet unfortunately because of this slight oversight with his hypertension, his condition had progressed to the beginning stages of kidney failure.
Dave knew this immediately spelled desk duty for him. That his time as a field was done and his agent status may soon to have effectively come to an end. He had always assumed he would have retired this time due and work related injury or died a noble death on the job. It pained him knowing this could be the beginning of the end for him and his career.
The doctors with his input immediately came up with a plan of action. They decided to start him on medications and dialysis once a week, hoping that would be sufficient. That within a few weeks they would reevaluate, having a clearer picture and better handle on the needs and limitations of his condition.

He couldn't lie or decide to up and leave them with no explanation. In their line of work they lost too many people and were left with to many regrets. He knew they would never forgive him if he kept this a secret. If he chose to disappear and slowly die on his own without them, out of pride or fear.
David Rossi was a lot of things, but he wasn't stupid, he knew he needed their support if he had any hopes of beating this thing. So he invited them over, for what might be the last team dinner he hosted for awhile. He wanted to have one more meal as a family with some semblance or sense of normalcy, knowing his news could wait until after dessert to be announced.
Everyone had been shocked and rendered momentarily speechless, worried for him upon learning of the life altering news he received. No one expected the nerve wracked Reid to be the first one to find words to fill the silence that had overtaken the room.
With his diagnosis, Spencer informed them all that typically his disease was caught a decade of two earlier and managed through diet, excercise and medication. That it had a fairly decent outcome and life expectancy if he followed the treatment plan.
He spouted off some other statistics most instantly forgot, too distracted by their own thoughts and feelings on the matter. But they still understood the gist of the genius's intended spiel. He was just as worried and distraught as the rest of them, try to make sense of the information they learned. But most of all afraid of the possibility of losing a piece of their family.
Once Reid finished speaking, he couldn't hold off the tears he had been trying to keep at bay. No one in the room was completely dry eyed as they all tried to digest the news. Slowly they all began to ask clarifying questions of the man of the hour about his condition and next steps going forward.
Hours later, Emily had been the last to leave, only showing the depth of her true emotions to him. Dave held onto her a little longer than usual, needing her strength, afraid to let go knowing he couldn't stop whatever came next for him. That this wasn't some nightmare he could simply wake up from.
As their embrace broke, he asked her what made her force him to get tested. He was hoping she would say something profound and wise, that would resonate with in him, and help him make sense of things.
Yet, she simply told him, it was a gut feeling she had, especially with his abnormal headaches as of late. That to her, something about him just seemed off, that he didn't seem himself with his usual laser sharp level of focus and observation. She couldn't explain it beyond that, beside knowing she needed him to be okay.
While her words hadn't provided him the type of clarification he had secretly and niavely hoping for, nor could she fix the predicament he found himself in, what she said provided another form of relief from his pain.
He wasn't surprised she of all people had noticed, Emily was the best profiler he had ever had the pleasure of working with. With her extreme empathy and gut instincts, she had always been the one looking out and supporting others, making sure they were ok. She had always been the first person to notice something was wrong and to begin trying to make it right.
He was just glad to have someone who obviously cared that deeply for his well-being in his life. For her sake and the team's, he wanted to bravely fight until he ran out of options. He hadn't given up against an enemy before, he wasn't about to start now.
