MP Shah Hospital Joins CaReMe Global Alliance

  A Landmark Partnership

MP Shah Hospital in Nairobi, Kenya, has achieved a historic milestone by becoming the first hospital in Africa to join the CaReMe Global Alliance. This groundbreaking collaboration, spearheaded by global pharmaceutical company AstraZeneca, is poised to reshape the landscape of cardiovascular, renal, and metabolic (CVRM) disease care in Kenya. The alliance seeks to improve outcomes for patients grappling with conditions like diabetes, heart failure, and chronic kidney disease.   A Unified Approach to Healthcare The CaReMe Global Alliance is dedicated to transforming CVRM care. Instead of targeting each condition individually, the alliance champions an integrated approach that treats all three comorbidities as one. Already, this innovative strategy has demonstrated its potential to enhance clinical outcomes, reduce costs, and optimize resource utilization in partner hospitals worldwide. This move signals a significant step towards a more sustainable healthcare model.  

The Leading Cause of Mortality

CVRM diseases, which affect the heart, kidneys, and pancreas, collectively stand as the leading cause of death on a global scale. The pernicious nature of these conditions lies in their ability to amplify the risk of further complications. In Kenya, the prevalence of Non-Communicable Diseases (NCDs), which encompass CVRM conditions, is on the rise. These diseases now account for over 50% of inpatient admissions and 40% of hospital deaths, consuming a substantial portion of the healthcare budget.   A Lifeline for Kenyan Healthcare This new alliance aims to proactively address the mounting burden of CVRM diseases in Kenya. During its inaugural year, the partnership sets forth ambitious goals, including the engagement of 400 patients in the iCaReMe registry. Notably, key objectives encompass the seamless integration of cardiology, diabetic, and renal clinics, as well as the holistic management of patients. Moreover, the collaboration will involve the assessment of outcomes based on the CaReMe Model approach. Furthermore, beyond the immediate benefits to patients, this groundbreaking collaboration underscores Kenya's unwavering commitment to pioneering advanced healthcare solutions. By doing so, it positions the nation at the vanguard of medical innovation in Africa, setting a remarkable precedent for the region.

Monkeypox Vaccine

There seems to be a high demand and low supply of monkeypox vaccine in the United States and due to this, there have been American citizens traveling to Vancouver to receive the free vaccine. Recently the BC government has put a stop to the Americans receiving vaccines in Canada. I feel mixed on this predicament. I can understand why the government is concerned for their own supply of vaccines for their citizens. However, as a gay or bisexual man that is at high risk of being exposed to monkeypox, I can also imagine the fear they must be feeling having to be told there is not enough for them to protect themselves. Also, with us being neighbors to the United States, if the monkeypox is spreading rapidly down there, is it not in our best interest to use some of our supply to help the United States get their vulnerable individuals protected in hopes of also having less being spread up into Canada? I am trying to understand both sides. The topic of monkeypox may also be more sensitive coming off of the aftermath of the covid-19 pandemic and all the controversy and stress we have endured through that. I just hope our countries can try to work together to protect the vulnerable people from monkeypox and not make it a power struggle.

Non-Communicable Diseases (NCDs) and Injuries Director Ministry of Public Health Kabul Afghanistan by Said Habib Arwal* in Crimson Publishers: Medical Rehabilitation

Evidence, including from the Afghanistan mortality survey (AMS), identifies the increasing mortality due to the major NCDs and injuries, which have a major impact on the health of communities, the health system, and the economy of Afghanistan, as a major challenge.

The determinants of non-communicable diseases are multifactorial and often are outside the control and influence of the health sector. The four main groups of non-communicable diseases share four main risk factors: tobacco use, unhealthy diet, harmful use of alcohol and insufficient physical activity. To reduce these risk factors, changes in the lifestyles and behaviors of individuals and families is necessary. Because of the communal nature of the Afghan society, there is a need for interventions in community settings to be at mosques, villages, schools or workplaces, to promote and encourage healthy behaviors to prevent non-communicable diseases.

Moreover, the prevention, detection, and management of non-communicable diseases are important to decrease suffering and reduce deaths from the mentioned diseases. These all can be done through a coordinated multi-sectoral national response. The Afghan Ministry of Public Health as the steward of the health sector has assumed the responsibility to tackle the burden of non-communicable diseases, in view of the demographic and epidemiologic transition envisaged in the near future. The MoPH has taken some steps toward initiating a comprehensive response to non-communicable diseases.

Establishment of the Department of Non-communicableDiseases within the structure of the MoPH is an important first step in this regard. The department, since its initiation, has prepared a vision to tackle non-communicable diseases through evidence-based interventions adapted to the local context and situation. Development of a national non-communicable diseases the strategy is an indispensable step, which provides clear guidance to the department on priority interventions and actions to take in order to reduce the burden of these diseases and improve the quality of life for the people of Afghanistan.

The interventions outlined in this document will guide many public and private sector individuals and organizations involved in the prevention, and control of non-communicable diseases.

There have been many current events going on around the globe that have been linking to the topics I have been learning about in my global health class. The war between the Ukraine and Russia has been a brutal battle and now we are seeing that the Ukraine has become a breeding ground for communicable diseases such as Polio, HIV/AIDS, Covid-19 and Tuberculosis. The individuals are all crowding together to hide, flea or battle for their country. The close proximity as well as the decrease in sanitation and health services is causing infections to spread amongst them. #communicablediseases

As the war continues, a LGBTQ couple proudly fights for their country as Ukrainian soldiers. They have attached a unicorn symbol to their uniforms to symbolize their community and send a clear message. During the war between Russia and the Ukraine, many occurrences have resulted in damage to water infrastructure. Due to this, over 6 million people in the Ukraine are struggling to find drinking water. This puts them at high risk of infections and illness. Clean water is a necessity to life, this needs to be resolved. #Cleanwater

During the war another basic necessity of life has been greatly impacted. Over 1800 schools and universities have been damaged during the battles. The effects of the war will negatively impact the education system in Ukraine for a long time. It will take a huge amount of money, resources and time to rebuild the schools and universities. The children could have benefitted from the normalcy and safety of attending school during the war but for many, that is no longer an option. #educationisaright
