GREEN OWNER SURRENDER NOTES – BASIC INFORMATION: Princess is a 4 year old black and white female large mixed breed dog Previous owner got princess from a friend and had her ever since she was 6 weeks old. She is surrendering because she has a new baby. She previously lived with 2 adults and 1 child. Around strangers, Princess is cautious and will need to sniff you out first. She is shy but will warm up to you quickly. Princess has been around a 9 year old child. She is friendly and respectful. Princess has been around other dogs in dog parks she is usually tolerant. Princess has not been around cats. Her behaviour is unknown. Princess does not resource guard food bowls toys or treats. She will bark if someone unfamiliar approaches the yard. Princess has never bit another animal or person before. She is housetrained, and has a medium energy level – friendly, affectionate, and playful. Other Notes: During storms or fireworks, Princess is usually relaxed and is unbothered by it. Princess is tolerant of baths. She loves to be held and isn’t bothered by being restrained. She isn’t bothered if she gets her nails trimmed or her paws are touched. She has a medical issues. She has skin allergies. She was last seen by a vet back in the summer of 2018 and she was checked out. For a New Family to Know: Princess is a Medium energy, Friendly, affectionate, playful and mellow dog. Previous owner's favorite thing about Princess is that she is a good spirited dog and is super gentle with people. Princess loves to play with ropes and play tug of war especially. She has been kept indoors only but outside for walks. She has been to dog parks and gets along with other dogs. She eats Natural Balance food since she does have skin allergies. Princess sleeps inside of a crate in the living room. She knows how to sit stay, go to the bathroom, come and lay down. Princess may tend to pull on the leash when men walk her but she will be more gentle with females. She has been left off the leash and will wander but come back to you when called. INTAKE NOTES – DATE OF INTAKE, 1/2/2019: Upon intake, Princess had a Tense body. She was lip licking and was whale eyed. When offered treats she began to rub up on the treats and my hand. Owner is unaware of why this is happening. She has been to the vet a few times to get her checked out but stated the vets have never given her a concrete answer for her skin issues. She has several dry spots around her body and some are crusted with dry blood. She also is bleeding from her neck." Subjective: Alert, walks well on leash. Observed Behavior - Slightly tense, allows handling. Evidence of Cruelty seen – None. Evidence of Trauma seen – None. Objective: BAR-H, MMs pink and moist, BCS 5/9. EENT: AS crusty and erythematous, mildly stenotic. Eyes clear, no nasal or ocular discharge. Oral Exam: Clean adult teeth. PLN: No significant enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, no coughing or sneezing. ABD: Not distended, no masses palpated. U/G: Female intact, no spay scar noted. MSI: Skin of entire body is erythematous, hypotrichotic and covered with crusted papules. Round pink erythematous alopecic nodules at the lateral aspect of both carpi. Small cracked lesion on the lateral aspect of the right front foot which is bleeding. Skin free of parasites. Ambulatory x 4. CNS: Mentation appropriate - no signs of neurologic abnormalities. Severe dermatitis, otitis externa AS - R/O allergies to food vs. environmental triggers Prognosis: Good with proper workup and treatment; poor quality of life otherwise Plan: 1.
