Hi, thank you sm for running this blog
I wanted to ask if you maybe had any resources to figure out if like, natal surgeries to "fix" intersex traits were done to someone?
I don't have acess to a doctor atm and I'm worried I might have genital scarring. Ik it's a legally mandatory practice here if the kid is intersex, but I can't find any resources on my own
Thank you so much again, sorry if my language isn't appropriate, I'm still new to this
Content note: discussion of genitalia and surgery
Hi anon,
Hope you're doing well. Unfortunately, there aren't a ton of resources out there, but I can try to share what I have. A lot of this will depend on what kind of variation you have and what kind of surgery might have happened.
One thing that can sometimes be a sign that you've had intersex surgery is if you had to see a urologist a lot as a kid, or if your medical records show that you went to a urologist as a baby. If you ever remember having any unexplained surgeries that might have been explained as for removing a cancer risk, or for fixing urinary function, or things like that, sometimes intersex surgeries are put in the medical record under those terms instead.
This article has a photo of an intersex adults scars from a surgery they had as a kid. This study has diagrams of clitoroplasty (tw for cissexist language, medical talk, and genital surgery diagrams). The intersex reddit has some people sharing photos or discussing scars in a way that might be helpful. If you have genital scar tissue, another way to notice it might be if you have a lot of unexplained pain down there or issues with urination such as frequent UTIs. In general, if you want to search to do your own research, the most common types of intersex surgery are gonadectomy, clitoroplasty, clitoral recession, vaginoplasty, hypospadias repair, and sometimes phalloplasty.
Sending all the good wishes your way, anon, and please feel free to come back if you have any other questions we can help answer.

It's commonly understood that the best age for hypospadias repair surgery is between 6-18 months for optimal results. However, success rates can vary based on factors such as the type and severity of hypospadias, the patient鈥檚 physiological conditions, the surgeon鈥檚 expertise, and post-operative care.
So, is it possible for an adult with a history of multiple failed hypospadias surgeries to have success with a redo surgery in adulthood?
Yes, there are certainly chances of success, although it's not 100% guaranteed. Here are the key factors that can influence the outcome:
Experienced Hypospadias Surgeon: It's crucial to find a surgeon who specialises in hypospadias repair and can evaluate the probable reasons for past failures to plan an effective surgery.
Surgeon鈥檚 Expertise in Redo Surgeries: Redo hypospadias surgeries in adults require different techniques compared to surgeries in children. An experienced surgeon with a track record of successful redo surgeries is essential.
Severity and Type of Hypospadias: The degree of hypospadias and the individual鈥檚 tissue health play significant roles. Adult tissues are different from those of children, and the position of the urethra must be carefully considered.
Physical and Psychological Preparedness: The individual鈥檚 overall physical health, mental preparedness, and positive attitude can greatly impact the success of the surgery. Effective communication and coordination with the surgeon are vital.
Medical History and Current Health: The patient鈥檚 complete medical history, including previous surgical techniques used, and current health conditions are crucial. This helps the surgeon tailor the surgical approach for the best possible outcome.
While challenges exist, with the right surgeon and careful planning, successful hypospadias repair in adulthood is achievable. Consulting a specialized urologist will provide a better understanding of the specific factors at play and the potential for a successful聽outcome.

Best Pediatric Urologist in India: Meeting Your Child Needs
Tumblr media
When it comes to your child's health, especially concerning sensitive and complex issues like urological conditions, finding the right pediatric specialist is paramount. Pediatric urology is a specialized field that focuses on diagnosing, treating, and managing urinary and genital problems in children. India is home to several world-class pediatric urologists who provide expert care tailored to each child's unique needs. This blog aims to guide you through understanding the importance of pediatric urology, recognizing common conditions, and finding the best pediatric urologist in India for your child.
Understanding Pediatric Urology
Pediatric urology encompasses a wide range of conditions affecting the urinary tract and genital organs in children. These conditions can be congenital (present at birth) or acquired, and they vary in severity from minor issues to complex disorders requiring surgery. Pediatric urologist in India are specially trained to handle these delicate conditions, providing care that is both effective and compassionate.
Common Pediatric Urological Conditions
Hypospadias: A congenital condition where the urethral opening is not located at the tip of the penis. Surgical correction is often required.
Vesicoureteral Reflux (VUR): A condition where urine flows backward from the bladder to the kidneys, increasing the risk of urinary tract infections (UTIs) and kidney damage.
Undescended Testes (Cryptorchidism): A condition where one or both testes fail to descend into the scrotum, which may require surgical intervention.
Posterior Urethral Valves (PUV): Abnormal folds of tissue in the urethra of boys that can obstruct urine flow, leading to kidney damage if untreated.
Urinary Tract Infections (UTIs): Recurrent UTIs may indicate an underlying urological issue that needs evaluation.
Spina Bifida and Neurogenic Bladder: Conditions that affect bladder function due to nerve damage or developmental abnormalities.
Hydronephrosis: Swelling of a kidney due to a build-up of urine, often detected prenatally.
Importance of Specialized Care
Children are not just small adults; their bodies are still developing, and their medical needs differ significantly from those of adults. Pediatric urologists undergo extensive training to understand these differences and to provide age-appropriate care. They are skilled in performing delicate surgeries on infants and children and are adept at using minimally invasive techniques to reduce recovery times and minimize discomfort.
Choosing the Right Pediatric Urologist in India
India boasts several renowned pediatric urologists, many of whom are affiliated with top hospitals and medical institutions. When selecting a pediatric urologist for your child, consider the following factors:
Qualifications and Experience: Ensure that the urologist is board-certified and has extensive experience in treating pediatric urological conditions.
Hospital Affiliation: Look for specialists affiliated with reputed hospitals known for their pediatric care facilities.
Specialized Training: Prefer urologists who have undergone specialized training or fellowships in pediatric urology.
Patient-Centered Approach: Choose a doctor who prioritizes patient comfort, provides clear communication, and involves you in the decision-making process.
Advanced Technology and Techniques: Opt for specialists who use the latest diagnostic and surgical technologies to ensure the best outcomes.
Positive Reviews and Testimonials: Seek recommendations from other parents or read reviews to gauge the doctor鈥檚 reputation and success rates.
Conclusion
Choosing the right Pediatric urologist in India is crucial for ensuring your child receives the best possible care. With India's growing expertise in pediatric urology, parents have access to world-class specialists and cutting-edge treatments. By considering factors such as qualifications, experience, and hospital affiliation, you can find a pediatric urologist who will provide the compassionate and expert care your child needs.

If your child is experiencing any urological issues, don鈥檛 hesitate to seek the advice of a specialist. Your child鈥檚 health and well-being are of utmost importance, and finding the right pediatric urologist can make all the difference.

My talk at the Hypospadias International Society (HIS)meeting
Tumblr media
I had the great privilege to take part in the HIS meeting held in Brazil a few months ago. I couldn鈥檛 go there physically due to some family commitments. So, I attended the virtual meeting for 3 days. In the meeting, I delivered a talk on 2-stage repair for complex聽failed hypospadias cases聽with the use of oral mucosal graft. I was among the very few doctors from India to attend this meeting and deliver a talk at this prestigious meeting.
Tumblr media
Hypospadias聽is one of the most common congenital anomalies. Unfortunately, there is not much awareness in the general public or even among many doctors about this problem. There are several reasons for this:
Hypospadias聽is an external anomaly and is not usually life-threatening. So, most people think that it is a minor problem.
Many doctors also don鈥檛 realise that聽hypospadias surgery聽is a technically demanding operation and has to be only done by the best surgeon.
Most people have little awareness about the far-reaching complications of聽hypospadias,聽which not only affects urination but can potentially disrupt the normal adult life of the person and the family.
The social and psychological stigma associated with聽hypospadias. Being a genital problem, most parents are not ready to share the information with family and friends and take their help to search for the聽best doctor for hypospadias surgery.
Once the child grows older, he becomes shy and doesn鈥檛 discuss the problem with the parents anymore.
Most parents and many doctors think that聽hypospadias聽is a minor problem and seek a solution from the local doctors; if the surgery is successful, it is ok. But more often than not, the operation may result in failure, and only then do the parents start looking for an expert. This results in the child undergoing multiple operations, including more complex reoperations, which could have been avoided if an expert surgeon had been involved from the beginning.
Thus,聽hypospadias聽remains a common and neglected problem. In addition, many surgeons of many specialities perform聽hypospadias surgery, and there are hundreds of techniques for hypospadias repair. This makes things more complicated for parents to choose the best surgeon and for family physicians to refer these cases to the聽best surgeon for hypospadias!
The purpose of the聽Hypospadias International Society (HIS)聽was to disseminate proper knowledge about聽hypospadias correction techniques聽among its members. Many enthusiastic surgeons from across the globe participate in its annual meetings. The latest meeting in Brazil was attended by over 100 hypospadialogists. Various聽techniques of hypospadias repair,聽including primary and complex聽failed hypospadias repairs,聽were discussed by experts. The pros and cons of various techniques were debated by experts in a scientific atmosphere. The long-term follow-up and聽results of hypospadias repairs聽were also discussed.聽
Tumblr media
My talk in the HIS meeting was about the most聽complex cases of hypospadias, which come after multiple failed operations elsewhere. These cases are very complex and require the expertise of the聽best聽hypospadias surgeon. Typically, a 2-stage (sometimes 3-stage) repair is performed utilising the skin taken from inside the oral cavity (mouth) as a graft to prepare the deficient urinary tube (urethra). We have done more than 100 such cases over the past 8 years in Hyderabad, with over 90% success of such complex operations. Our patients come from聽all over India, including Andhra Pradesh, Maharashtra, Karnataka, Tamil Nadu, Kerala, Orissa, West Bengal, Bihar and UP. We also performed聽complex redo hypospadias repairs聽on聽international patients聽who underwent their initial failed hypospadias operations in other countries.
My talk about the technique and results of聽complex hypospadias reconstructions聽at the HIS meeting was highly appreciated. I received positive messages from many surgeons across the world congratulating us for our work and the good results. However, an ideal situation is where no child requires such complex reconstructions for聽failed hypospadias. This is possible if the聽initial hypospadias surgery聽is performed by experts only. When choosing the聽best hypospadias surgeon聽for their child, parents should spend some time identifying the best surgeon with the聽best results in hypospadias repair. Remember, even the best surgeon can have complications (like the best driver can have an accident), but the complications will be a small percentage, usually minor and easily treatable. I discussed a few tips for parents in my earlier blog about how to choose the聽best hypospadias surgeon聽for their child. That blog may be referred to by those interested in knowing more about this.

Hypospadias (urine passage ending short of the end of the penis)
Intersex (incomplete or otherwise abnormal development of the genital organs)
Kidney stones
Neurogenic bladder from spinal cord lesions, such as myelomeningocele
Re-do urology surgery
Rhabdomyosarcoma of the genitourinary system
Testicular tumors
Undescended testicles
Ureteropelvic junction obstruction (blockage of urine flow from the kidneys)
Vesicoureteral reflux (backup of urine from the bladder toward the kidneys)
Wilms tumor and other kidney tumors in children
Research areas in pediatric urology:
Doxazosin as the main treatment for dysfunction
Penile and Primary Bladder Reconstruction for Surgical Repair of Classic Bladder Extrusion
The association between attention deficit / hyperactivity disorder (ADHD) and urinary incontinence and nocturnal enuresis
A study of urinary tract infections (UTIs) and vesicoureteral reflux, both with void dysfunction and transient urodynamic dysfunction in childhood (when these two abnormalities occur together)
Individualized chemotherapy combined with surgical rules in genitourinary neoplasms
Why the pediatric urologist:
Children are not the only little adults. They never know what bothers them. They cannot always answer medical questions and are not always patient and cooperative during medical examinations.
Pediatric urologists are trained to focus on the special needs of children and their parents, including sensitive and embarrassing issues related to the genitalia. They know how to assess and treat children in a relaxed and cooperative way. Additionally, pediatric urologists often use devices specifically designed for children. This will help create a comfortable and non-threatening environment for your child.
If your pediatrician recommends that you see your pediatrician, you can be assured that you have extensive treatment options, the most comprehensive and comprehensive training, and a wealth of experience treating children and treating pediatric urinary tract disorders.

Surgical management of tumors and malignancies of the kidneys, bladder and testicles
Evaluation and management of urological tract problems identified before birth
Evaluation and management of urinary tract problems associated with neurological conditions such as spina bifida.

Diagnosis and treatment by our urologists:
Urinary Tract Infections - An infection that affects any part of the urinary system. It causes an increased need to urinate, pain and fever when urinating.
Hypospadias: a congenital condition in children whose urethral opening is in the wrong place. This requires surgical correction.
Vesicoureteral reflux (VUR): A disorder in which urine goes in the wrong direction and returns to the kidneys.
Bedwetting: Commonly known as nocturnal enuresis, your pediatric urologist must identify the cause of your child's bladder loss at night to correct it.
Hydronephrosis: barrier that obstructs the bladder and causes urine to flow back to the kidneys. Due to this obstruction, children may have difficulty urinating and may experience abdominal pain.
Ureteropelvic junction (UPJ) obstructions: An obstruction of the kidneys that prevents the flow of urine from the organs. Severe cases require surgery to get out of the blockage.

Micropenis is an unusually small penis. A common criterion is a dorsal erect penile length of at least 2.5 standard deviations smaller than the mean human penis size, or smaller than about for an adult when compared with an average erection of 12.5 cm. The condition is usually recognized shortly after birth. The term is most often used medically when the rest of the penis, scrotum, and perineum are without ambiguity, such as hypospadias. Micropenis occurs in about 0.6% of males.

I think I might be intersex? I'd kill for your opinion. I was born with a penis but had surgery as a baby (I'm to understand it was falling into my body) and it's always resided internally in its own cavity. I've heard of intersex folks that have an identical situation, any adivce/thoughts? I've been at a loss with this (and doctors don't take you seriously when you're overweight so they've been no help).
Hi anon! Sorry it took so long to answer.
I'm sorry you've been having to deal with discrimination from doctors--that's so messed up and you deserve better.
The first thing that comes to mind for me is that might have been a sign of hypospadias or epispadias, and that the surgery you had as a child might have been hypospadias or epispadias repair. Hypospadias is a condition where the urethra is located on the underside of the penis, and epispadias is when the urethra doesn't develop into a full tube and is located in a different place on the penis. A lot of times, hypospadias and epispadias also come along with congenital chordee, which means that the penis is curved and bent in a different direction due to bands of fibrous tissue. Undescended testicles (cryptorchidism) are also often associated with them. Hypospadias and epispadias are both considered intersex variations. I'm not too sure what that aftermath of that surgery looks like or potential complications as an adult, but that's something that really comes to mind to me as a possibility for you.
Another possibility might be Penoscrotal Transposition, which is a variation where people's penis is located below or in the middle of the scrotum. I'm less familiar with what surgery looks like for that, or what the possible after effects might be.
There are also several different intersex variations that could cause different genital configuration, but those would be probably less likely unless you also had more symptoms like growing breasts during puberty, micropenis, infertility, or other signs of estrogen based puberty.
I hope some of that might be relevant, and if any followers have any other suggestions please add on! (As always, disclaimer that we can't diagnose or provide medical advice.)

While hypospadias correction surgeries are typically performed between the ages of 6 to 18 months for optimal success, it's important to note that adults of varying ages, including those in their late 20s, 40s, and beyond, may still opt for repair treatment.
However, there are certain challenges associated with hypospadias repair treatment in adults:
Tissue Hardness and Complications: The hardness of tissues in adults can make the surgery more complicated and extend the recovery time.
Psychological Readiness: Mental preparedness is crucial for undergoing surgery, but adults may face psychological challenges and struggle with low self-confidence levels, which can be significant hurdles to overcome.
Increased Risk of Bleeding: Adults may have larger penile sizes and experience erections, which can increase the risk of bleeding during surgery and impact wound healing.
Limited Surgeon Expertise: Since most surgeons primarily deal with paediatric hypospadias cases, their expertise in handling adult cases may be limited, posing potential challenges during treatment.
Multiple Surgeries and Long Recovery: Depending on the severity of the condition, multiple rounds of surgeries may be required. However, the long recovery time associated with adult surgeries can prolong the treatment process.
Despite these challenges, adults considering hypospadias repair treatment should consult with experienced urologists to discuss their options and weigh the potential risks and benefits. With proper care and guidance, successful outcomes can still be achieved in adult cases of hypospadias.

What Is Pediatric Urology?
Children frequently suffer from Genito-urinary disorders. Infants are not able to comprehensively express the discomfort due to these disorders. In such a scenario, a trained and experienced聽pediatric urologist聽is required.聽Pediatric urologists聽are the doctors that diagnose and treat various Genito-urinary disorders in children.
The genito-urinary system includes ureter, kidney, bladder, and reproductive organs. It has been estimated that of all the disorders requiring surgery in children, almost 50% of them are associated with the Genito-urinary system. These disorders may include both congenital as well as acquired. A聽Pediatric urologist聽in India聽is an expert in handling the simple as well as complex cases of the Genito-urinary tract in infants and children.
How Pediatric Urology Is Different From Adult Urology?
Pediatric urology聽is quite different from adult urology. The symptoms of pediatric disorders are different from those of adult urology. Most of the urological disorders are congenital disorders and requires altogether a different treatment approach. Most of these problems can be managed with close observation and may not require any intervention.聽Pediatric surgeons聽are now well-equipped to precisely diagnose and manage various conditions without any surgery. Minimally invasive surgery also plays an important role in minimizing complications.
What Are The Various Pediatric Disorders?
Following are the various聽pediatric urological聽conditions:
Hypospadias聽: Hypospadias is the condition in which the opening of the urethra is not present on the tip of the penis. It is rather present on the underside of the penis.
Undescended testicles:聽Medically known as cryptorchidism, it is the condition in which either one or both the testicles failed to descend.
Vesicoureteral reflux:聽This condition involves the flowing of urine in the wrong direction.
Bladder exstrophy:聽When the bladder protrudes outside the abdominal wall, the condition is termed as bladder exstrophy.
Pelvic-ureteric junction obstruction:聽This condition is characterized by impairment in the flow of urine at the junction of pelvis and ureter.
Epispadias:聽When the urethra opens on the upper side of the penis, the condition is termed as epispadias.
Other conditions:聽Various other pediatric urological conditions include Testicular torsion, Ureterocoele, Horseshoe kidney, Duplex kidney, Ectopic ureters, and Absent kidney or renal agenesis.
What Are The General Symptoms Of Pediatric Disorders?
Symptoms of pediatric disorders are presented based on the type of Genito-urinary disorder. However, common symptoms include:
路 Painful urination
路 Fever
路 Blood in urine
路 Thin urine stream
路 Abdominal pain
路 Swelling in testis
路 Urinary tract infection which may frequently recur
路 Pus in urine
路 Increased urine frequency.
