These 2 covered it all really well but I want to acknowledge that the dark urge is also disabled, not only by having survived severe brain damage, but also by being Bhaalspawn, an example of having an extremely vilified disability along with severe trauma.

I will not draw any comparisons to real life disabilities because they don't exist, and to avoid vilifying complex human experiences. I affirm that, in real life, disabled people, even those who experience symptoms relating to violent thoughts, are much more likely to be victims than perpetrators.
That being said, this is a fantasy curse/disability characterized by near uncontrollable violent urges, loss of self control, losing time, and literally being controlled against your will. This is a fantasy disability that I find personally compelling as someone who manages some similar symptoms.
The dark urge has persistent intrusive thoughts that they are capable of recognizing as socially unacceptable and can manage through careful thought and effort. But they are still treated as a monster by some of the party members early on, especially depending on early choices and willingness to talk about their condition. This mirrors stigma that many disabled people experience if they decide to be honest about their symptoms, with or without precedent.
Dark urge's journey with their primary disability is about chosing to overcome their "evil nature" through great effort or embracing that nature to the detriment of everyone around them, including themself. With careful planning and deliberate effort, it is possible to play dark urge in a way that almost completely mitigates the most awful things the urge is capable of during a normal playthrough. Their good ending involves successfully managing their intrusive thoughts, curing themself of their curse, and ending the cycle of generational trauma, thereby earning the respect and even admiration of their skeptical party mates, and then going on to choose their own path.

I don’t know who needs to hear this today, but intrusive thoughts are basically your brain’s (sometimes very upsetting) way of saying “If there were two guys on the moon and one of them killed the other with a rock would that be fucked up or what?”
I’ve personally found that adding the “would that be fucked or what?” part in myself really helps put the more disturbing thoughts we sometimes get into perspective. Helps me say “yeah thar sure would be fucked up” and move on with my day.
It’s not a secret desire, it’s not something that only occurs to you because you’re a bad person. It’s just your brain deciding to process the fact that it knows an uncomfortable thing exists in the world by feeding it to you in an absurd “what if” with you as the main character.

Ok I’m having a slight mental breakdown about the sun the last couple days (idk what’s happening but every time the sun touches me I get these horrible intrusive thoughts like “I’m not wearing sunscreen, my skin is going to peel off my body, I’m going to get melanoma and die of sun cancer”) so to reassure myself and spread awareness here are the signs of melanoma and when to get it checked by a doctor:

I think the most fucked up thing about intrusive thoughts is it's really difficult to discuss them without discussing their content, because without a discussion of their content it's impossible to get across just how distressing and debilitating they are. "Oh, you have unpleasant thoughts sometimes? Yeah we all have that, I guess."
But if you discuss their content there's this huge risk of people just pulling away in disgust. "Oh god, you have THOSE sorts of thoughts? They should lock freaks like you the fuck up!" As if they think people act on every single thought they have.

I’m having a particularly bad episode of OCD today and wanted to remind everyone as well as myself.
Your intrusive thoughts and urges do not reflect who you are as a person.
Your intrusive thoughts and urges do not make you sick or evil.
Your intrusive thoughts and urges do not mean you are a despicable human being.
They do not mean any of the above, or any of the other lies your brain might conjure. (You’re twisted. You’re perverted. You deserve death. You deserve to never be happy. To always be alone. To always feel guilty. And the list goes on).
What your intrusive thoughts and urges do mean is that you’re struggling, and it’s hard, and sometimes you want to give up and disappear forever.
But your intrusive thoughts and urges do not define you. They do not have control over you, they do not have charge of your brain. They are not going to drag you under for the rest of your life, even if it feels like that right now. You will overcome this, and you will pound those stupid thoughts into the earth, no matter how long it takes.
You are not your intrusive thoughts.
Your intrusive thoughts are not you.

You cannot pretend you give a shit about people who suffer from OCD or other compulsory disorders, and then turn around and declare people have to feel guilt about their thoughts to be a good person.
No one has to punish themselves physically, verbally or mentally for having intrusive thoughts, no matter what those thoughts are about.
And expecting people to do grand gestures to prove they really are sorry for having intrusive thoughts is vile and abusive. No one has to perform remorse for your entertainment.
