Celery in sauce (prose, cooking)

Cut one celery stalk into 10cm pieces and boil for 3-4 minutes. After cooling, cut into 3cm pieces. Mix 15cc of medium-thick sauce, 15cc of vinegar, 8cc of soy sauce, 8cc of olive oil, and one bay leaf. Put everything in a ziplock bag, knead well, and leave in the refrigerator for about 2 hours. The sauce mixture will soak in, the celery aroma will remain, and the pieces will become soft.

MC y EA: Paciente conocida en la institución por HACP el 03/06/2023 y consecuencia de esta múltiples intervenciones y complicaciones. Consulta porque desde hace 15 días viene con fiebre subjetiva, astenia, adinamia, malestar generalizado, dolor abdominal y salida de material purulento por un punto del tercio superior de la cicatriz de la laparotomía. Ha tomado acetaminofén sin mejoría por lo que decide consultar

Revisión por sistemas: niega.

AP: Choque hemorrágico Junio 2023. Herida por arma cortopunzante penetrante en abdomen (03/06/2023): Herida gástrica transfixiante en el cuerpo gástrico de 8 cm en cada cara. Herida de la raíz del mesenterio hacia yeyuno proximal con sangrado de vasos arteriales y venosos, hematoma en raíz del mesenterio con sangrado activo por lesión de vasos yeyunales de la mesentérica superior. Herida de mesocolon trasverso. Herida de conducto de quilo. Desgarro transversal de fascia y músculo recto abdominal izquierdo con sangrado de vasos musculares. Sepsis de origen abdominal. Peritonitis secundaria por S. mitis. Trastorno de ansiedad. Alérgicos: niega.

Procedimientos:

09/06/2023 Desempaquetamiento + lavado peritoneal + eventrorrafia, rafia gástrica sin fugas, salida de líquido claro (linfa?) cerca a la ligadura de vasos yeyunales en donde se dejó dren.

06/06/2023 Laparotomía exploratoria + Desempaquetamiento abdominal + exploración vascular retroperitoneal + ligadura de vasos mesentéricos + reparación de sangrado vascular de rama yeyunal de arteria mesentérica superior + cierre de defectos del meso, intestino delgado y mesocolon + parche de omento sobre estómago anterior + empaquetamiento de la raíz del mesenterio + laparostomía con malla sobre bolsa de viaflex y cierre de piel.

03/06/2023 Laparotomía exploratoria, ligadura de vasos venosos y arteriales, ligadura de conducto de quilo, gastrorrafia, exploración retroperitoneal y empaquetamiento abdominal con laparostomía.

Subjetivo: ligero dolor abdominal, sin disnea. Sin emesis.

Signos vitales: Presión arterial (mmHg): 121/81, Presión arterial media(mmHg): 94, Frecuencia cardíaca(LPM): 67

Buenas condiciones, sin dificultad respiratoria. ORL sano. Cardiopulmonar normal. Abdomen con cicatriz en línea media, 2 puntos de pequeñas dehiscencias en epigastrio y mesotastrio, sin eritema ni secreción anormal en el momento. No masas ni megalias. Extremidades: sin edemas.

Análisis y Plan de manejo: Paciente conocida, con antecedentes y diagnósticos anotados. Ahora dolor abdominal y probable absceso de pared que ya se solicitó drenaje percutáneo.
