Defining a Health Information Exchange

The United States is facing the largest shortage of healthcare practitioners in our country's history which is compounded by an ever increasing geriatric population. In 2005 there existed one geriatrician for every 5,000 US residents over 65 and only nine of the 145 medical schools trained geriatricians. By 2020 the industry is estimated to be short 200,000 physicians and over a million nurses. Never, in the history of US healthcare, has so much been demanded with so few personnel. Because of this shortage combined with the geriatric population increase, the medical community has to find a way to provide timely, accurate information to those who need it in a uniform fashion. Imagine if flight controllers spoke the native language of their country instead of the current international flight language, English. This example captures the urgency and critical nature of our need for standardized communication in healthcare. A healthy information exchange can help improve safety, reduce length of hospital stays, cut down on medication errors, reduce redundancies in lab testing or procedures and make the health system faster, leaner and more productive. The aging US population along with those impacted by chronic disease like diabetes, cardiovascular disease and asthma will need to see more specialists who will have to find a way to communicate with primary care providers effectively and efficiently.

This efficiency can only be attained by standardizing the manner in which the communication takes place. parc central residence siteplan  Healthbridge, a Cincinnati based HIE and one of the largest community based networks, was able to reduce their potential disease outbreaks from 5 to 8 days down to 48 hours with a regional health information exchange. Regarding standardization, one author noted, "Interoperability without standards is like language without grammar. In both cases communication can be achieved but the process is cumbersome and often ineffective."

United States retailers transitioned over twenty years ago in order to automate inventory, sales, accounting controls which all improve efficiency and effectiveness. While uncomfortable to think of patients as inventory, perhaps this has been part of the reason for the lack of transition in the primary care setting to automation of patient records and data. Imagine a Mom & Pop hardware store on any square in mid America packed with inventory on shelves, ordering duplicate widgets based on lack of information regarding current inventory. Visualize any Home Depot or Lowes and you get a glimpse of how automation has changed the retail sector in terms of scalability and efficiency. Perhaps the "art of medicine" is a barrier to more productive, efficient and smarter medicine. Standards in information exchange have existed since 1989, but recent interfaces have evolved more rapidly thanks to increases in standardization of regional and state health information exchanges.

History of Health Information Exchanges

Major urban centers in Canada and Australia were the first to successfully implement HIE's. The success of these early networks was linked to an integration with primary care EHR systems already in place. Health Level 7 (HL7) represents the first health language standardization system in the United States, beginning with a meeting at the University of Pennsylvania in 1987. HL7 has been successful in replacing antiquated interactions like faxing, mail and direct provider communication, which often represent duplication and inefficiency. Process interoperability increases human understanding across networks health systems to integrate and communicate. Standardization will ultimately impact how effective that communication functions in the same way that grammar standards foster better communication. The United States National Health Information Network (NHIN) sets the standards that foster this delivery of communication between health networks. HL7 is now on it's third version which was published in 2004. The goals of HL7 are to increase interoperability, develop coherent standards, educate the industry on standardization and collaborate with other sanctioning bodies like ANSI and ISO who are also concerned with process improvement.

In the United States one of the earliest HIE's started in Portland Maine. HealthInfoNet is a public-private partnership and is believed to be the largest statewide HIE. The goals of the network are to improve patient safety, enhance the quality of clinical care, increase efficiency, reduce service duplication, identify public threats more quickly and expand patient record access. The four founding groups the Maine Health Access Foundation, Maine CDC, The Maine Quality Forum and Maine Health Information Center (Onpoint Health Data) began their efforts in 2004.

In Tennessee Regional Health Information Organizations (RHIO's) initiated in Memphis and the Tri Cities region. Carespark, a 501(3)c, in the Tri Cities region was considered a direct project where clinicians interact directly with each other using Carespark's HL7 compliant system as an intermediary to translate the data bi-directionally. Veterans Affairs (VA) clinics also played a crucial role in the early stages of building this network. In the delta the midsouth eHealth Alliance is a RHIO connecting Memphis hospitals like Baptist Memorial (5 sites), Methodist Systems, Lebonheur Healthcare, Memphis Children's Clinic, St. Francis Health System, St Jude, The Regional Medical Center and UT Medical. These regional networks allow practitioners to share medical records, lab values medicines and other reports in a more efficient manner.

Seventeen US communities have been designated as Beacon Communities across the United States based on their development of HIE's. These communities' health focus varies based on the patient population and prevalence of chronic disease states i.e. cvd, diabetes, asthma. The communities focus on specific and measurable improvements in quality, safety and efficiency due to health information exchange improvements. The closest geographical Beacon community to Tennessee, in Byhalia, Mississippi, just south of Memphis, was granted a $100,000 grant by the department of Health and Human Services in September 2011.

Vienna….oder Wien

I am now a week into my vacation and I've finally found the time and motivation to write. Sorry for the hiatus, but as life does it got busy. But, despite how often I feel I've told the story, not everyone has heard the continuation of my real estate adventures.

As I previously mentioned, I made two appointments. One at the Vienna U.S. Consulate, and the other in Frankfurt.  A week prior to my appointment in Vienna I received an automated email from the Vienna Consulate, requesting that I postpone my appointment if it is not an emergency. Later that same day I received an email from the Frankfurt Consulate, stating they flat out cancelled my appointment as (I assume) all notary services were considered non-emergency and thus postponed…for you by the Consulate...period.

That second email freaked me out a bit, as at that point I had already booked my train tickets and Airbnb in Vienna. I decided to simply soldier on with the whole ordeal. And that's why you would find me Monday morning the 15th of June on the train headed to Vienna. 

Concerned with the border crossing I had my passport, aufenthaltstitel (residency permit), both my negative Corona test results (in paper form) and a copy of my appointment in Vienna. Prepared, with everything within reach, I waited. The only reason I noticed that I had crossed the border was because I got a notification from my phone carrier that my service switched to roaming…

After checking into my Airbnb I wandered around town the whole afternoon.  I ate some kind of bread breakfast hash looking thing (I thought the soaked bread was potatoes at first) for dinner, served politely to me by someone in a penguin suit as traditional cafes tend to do in Vienna.  I definitely had some culture shock that first evening. Everything was german and somehow also not, and I could feel it.  

The next morning everything went smoothly. I got up early, made it to my appointment an hour early because, well in Frankfurt it had taken me almost an hour to get through security, only to note that the Vienna U.S. Consulate was on the 4th floor of a mostly Marriott hotel building.  Security took me literally 2 minutes, and despite me being there an hour early there was NO ONE around. They processed my request and I was outta there in maybe 20 minutes.

The real story starts when I tried to overnight it with FedEx or some such equivalent.  In the US they are at almost every post office type place, and the Post Office was also one of the suggested locations by the U.S. Consulate. I get there (random Post office) and they tell me the fastest they can do is 2 to 4 days, probably more like 5 because of Corona.  I ask where the nearest FedEx or UPS (or an equivalent), is and they inform me that I must go to the Airport. 

Ok, no big deal right? For those Americans reading my blog remember first that I have no car.  So I take the subway to the train station (10 minutes), the local fast train to the Airport (20 to 25 Minutes). At the airport I assume it's inside, only to find out it's in the Cargo North terminal.  No problem, they say, there is a bus that goes there. I go to take the bus but they tell me...nope it's closed due to Corona so I have either the choice of walking out there (30 to 40 minutes) or taking a taxi. The weather was nice, and it felt super weird taking a taxi after all this way, so I walked out there. I finally reach the Cargo terminal and I am the only non-employee there. It took me about 20 minutes to get my documents all shipped and paid for. Then I had the entire trip back.  All in all it took me about 4 to 5 hours simply to overnight my documents with travel and wait times included.

I find it super hilarious that the hardest part of my entire journey was overnighting my documents. It was also super humid outside (side note: the words humid and Gay (homosexual) sound incredibly similar in German and I constantly say right now that it feels really gay outside…) so when I got back to my Airbnb I rinsed off the stickyness…

The rest of my stay in Vienna was uneventful and fun.  Had some Schnitzel, Vienna breakfast, really, really dark and strong coffee (that's their thing I guess), some Grüner Veltliner (Austrian white wine) and some Austrian craft beer. Final opinion on Vienna...it feels like a european Portland. Weird, much more artsy, with crazy awesome architecture and friendly people.  Everytime I ended up sitting alone for dinner people would randomly talk with me.  Definitely was strange, but also super nice.

I definitely didn't see enough of the museums, but I was there long enough to get a good impression. Vienna is a pretty interesting place, and I hope I get to return someday.  

Right now I am at what is called a "Viereckig Bauernhof", or a four sided homestead/farm near Leipzig in the former East half of Germany.  I'm already quite enjoying my stay here, and will get you a complete story later in the week.  So far it includes chickens, Elvis, a southern dinner, a garden and a crazy Russian Motorcycle. Stay tuned.

Conversa Health raises $12M Series B for its digital health platform

It’s no surprise that the coronavirus pandemic is accelerating the digital health space and so it’s also no surprise that a lot of the startups in this area are currently getting funded. Conversa Health, a Portland, Oregon-based startup that provides a virtual care and communication platform for personalized healthcare, is among these startups. The company today announced that it has raised a $12 million Series B round led by Builders VC and Northwell Ventures, the venture arm of Northwell Health, one of the largest healthcare providers in New York, with 23 hospitals and 800 outpatient facilities.

With this round, which also saw the participation of P5 Health Ventures, Nassau Street Ventures and Ohia’s UH Ventures (the venture arm of Ohio’s University Hospitals), Conversa has now raised a total of over $26 million.

In addition, the company also today announced that it has appointed Murray Brozinsky as its new CEO. Conversa co-founder — and now former CEO — West Shell III will become the company’s executive chairman.

“I used to say that virtual health is inevitable,” Brozinsky told me. “And the reason is that we can talk about the economics and how healthcare is moving to a value-based reimbursement system and how we’re gonna break the bank in spending 20% of GDP and our need to improve the patient experience. So you can see how it would become the predominant way that we practice healthcare in this country. And with COVID, that has now become the catalyst for it to become a tipping point. So we’ve seen an acceleration of virtual care and we fully believe that the major health systems are going to implement platforms like this, in many cases, our platform as the first line.”

The goal, Brozinsky told me, is for Conversa’s platform to become the digital front door for a health system. The company’s automated chat-based platform can help triage patients and see if they need a virtual or in-person visit with a doctor, for example. But healthcare systems can also use it to check in on patients and gather data about them, either by asking for it or through connected devices. Indeed, as Brozinsky noted, in many ways, Conversa is a data company.

One new line of business is the company’s Employee HealthCheck services, which allows employers to screen workers before they return to work, using a simple Q&A process. In the current environment, where businesses are very much responsible for creating and maintaining a safe work environment, that’s indeed a very timely launch (and health care providers, too, are using the company’s system to screen patients and their loved ones before they arrive at their facilities).

“We think it’s going to be incumbent on employers to continue to screen their employees for COVID,” said Brozinsky.  “And then on the heels of that, we’re seeing a lot of — and this has been an issue, but now it’s become a bigger issue — which is mental health. So a lot of PTSD, certainly from healthcare workers, stress and anxiety for lots of people in the environment. So we’ve got programs on the heels of COVID that are helping to screen for mental health.”

He also noted that while a lot of employers have launched wellness programs, the activation rates for these have remained very low, all while the nature of work is changing rapidly as people work remotely and are often scared to come in to work.

“Even before COVID-19, we have been expanding our work with Conversa throughout our organization over the last few years as they are a critical component of Northwell Health’s vision for virtual health, further strengthening the provider-patient relationship through personalized, insightful engagement,” said Joseph Schulman, Senior Vice President, Population Health, Business Transformation, for Northwell Health. “We have been successfully using Conversa to scale our communications and care for thousands of COVID-19 patients with programs focused on lab results, quarantine, antibody tests and more. Conversa has been an extraordinary partner.”
