6 Leonard A. Lauder Passes Away At 92 The beauty and fashion worlds mourn the death of the legendary Leonard A. Lauder, who passed away at the age of 92 on June 14. Lauder was chairman emeritus of The Estée Lauder Companies Inc., which announced his death on Sunday. During his career at the company—originally founded by his mother Estée Lauder—Leonard was a leading businessman in cosmetics, joining the brand in 1968 and serving as president from 1972-1995, CEO from 1982-1999, and chairman from 1995-2005. During his tenure, he was celebrated for launching multiple brands under the Lauder roster, including Aramis, Clinique, and Lab Series—plus the acquisitions of Aveda, Bobbi Brown, Jo Malone London, La Mer, and M-A-C. Lauder’s accomplishments also included launching Lauder’s first research and development laboratory, as well as the company’s international sales growth. As chairman emeritus, Lauder continued to lead at the company and aided in beginning its ELC Cares Employee Relief Fund during the COVID-19 pandemic. Space Station Science Highlights: Week of January 20, 2020
ISS - Expedition 61 Mission patch. Jan. 24, 2020 The week of Jan. 20, crew members aboard the International Space Station worked on a number of scientific investigations, including studies of using probiotics to improve immune health and of complex plasmas, as well as measurements of black holes and Earth’s upper atmosphere. NASA astronauts Jessica Meir and Christina Koch conducted their third spacewalk together on Jan. 20, completing replacement of batteries that store and distribute solar power on the space station. The crew also continued operations for the departure of the Cygnus cargo craft, currently planned for Jan. 31, and NASA astronaut Andrew Morgan and Luca Parmitano of ESA (European Space Agency) prepared for Saturday’s fourth and final spacewalk to repair the Alpha Magnetic Spectrometer (AMS-02).

Image Credit: NASA. Now in its 20th year of continuous human presence, the space station provides a platform for long-duration research in microgravity and for learning to live and work in space. Experience gained on the orbiting lab supports Artemis, NASA’s program to go forward to the Moon and on to Mars. Here are details on some of the microgravity investigations currently taking place: Studying complex plasmas The crew took photos of the Plasma Krystall-4 (PK-4) system and packed hard drives for return to Earth on a Soyuz supply craft. A collaboration between the European Space Agency (ESA) and the Russian Federal Space Agency (ROSCOSMOS), PK-4 studies complex plasmas, or low temperature gaseous mixtures of ionized gas, neutral gas, and micron-sized particles. When the micro-particles become highly charged, they interact strongly with each other and can form self-organized structures called plasma crystals. Plasmas occur throughout the universe, from the interstellar medium to the heat shields of spacecraft re-entering Earth's atmosphere. Understanding how plasma crystals form in microgravity could shed light on plasma phenomena in space and lead to new research methods and improved spacecraft designs. Analyzing different influences on complex plasmas also could lead to improvements in industries that use plasmas on Earth. Beneficial bacteria Probiotics are live microorganisms that aid human digestion. The Probiotics investigation studies whether these beneficial bacteria improve the human intestinal microbiota and immune function. Some species of harmful bacteria may grow stronger in microgravity, while the human immune system becomes weaker in space, leading to increased risk to the health of astronauts on long-duration missions. Incorporating probiotics into food for future space missions could provide basic nutrition and help safeguard the health of crew members. The investigation also could shed light on changes that take place in bacteria during spaceflight, benefiting efforts to reduce infections. The Neutron star Interior Composition Explorer (NICER) provides high-precision measurements of neutron stars and other X-ray astrophysics phenomena through observations in the electromagnetic band in which these stars radiate significant portions of their energy. Since July of 2019, NICER has been observing a recent outburst of the X-ray binary EXO 1846-031, last detected in 1985. The mission measured the most ever high frequency quasi-periodic oscillations (QPOs), oscillations related to matter orbiting at a significant fraction of the speed of light within a few hundred kilometers of the event horizon, the boundary beyond which even light cannot escape and, thus, nothing can be observed. The data provide evidence confirming the object as a relatively lightweight black hole binary, only three to four times the Sun’s mass.

Caitlin sat in her room, alone and tired. She felt as if the gray walls were mocking her, impeccably smooth. Too smooth.
She knew she was on thin ice with work, and staying up until 4 am wouldn’t help her but she couldn’t sleep.
Her mind... it was racing. She wasn’t used to it at all. It had started out small, when she saw a bird sitting in a tree outside her work. She couldn’t take her eyes off of it. There was something mesmerizing about the way it flew before she just shook her head and returned to work, attempting to shrug the feeling off.
It continued, she found herself wanting to coo at the toddler as he held his mother’s hand every morning on the way to school.
When the screen in her room would report news, she found herself sad when a death was announced.
Then she met you.
You had recently started a job with the same company, working as bio-engineers. More specifically, geneticists.
“EnLight Corp only wants to take away what’s natural! So let’s give in to what we all want to do”
Your task? To find a permanent solution. Eradicate the world of emotions. The pill had been developed 75 years ago during WWII. It was designed for soldiers so nothing would be compromised by our only weakness. Feeling.
“Keep our country alive. The solution is right here. Ingestible, sterile, safe. No more will we have to worry. I have been working with EnLight and together, we will win this war.”
Conspiracy theorists say that in the beginning of WWII, FDR and EnLight Corp. began to give civilians the pill but nobody has any record of that. To everyone else, it started to get pumped into society around 2010. It seemed every advertisement was pushing for this “enlightenment.”
Of course, nobody told the truth. EnLight and the government did whatever it took. They lied. Claiming that the lack of emotion was merely a temporary side effect for a pill meant to be an effective and easier way to vaccinate.
Over time, people stopped feeling, so they stopped caring. They took the pills, not realizing that as time moved on, they were being isolated. Government started to do whatever they wanted. Mercenaries hired by them soon littered the streets, eradicating the world of the sickly homeless. And people didn’t care. People continued to watch as these “Visionaries” (as the media called them) took lives away to serve EnLight.
“These mercenaries, they hide behind their masks and shamelessly pull their triggers over and over! We are being silenced.”
Nobody cared when they moved everyone out of their homes and into bland complexes. Nobody cared when they started to take away kids to raise as Visionaries. Nobody cared while the government and ELC continued to work together, slowly ridding you and everyone else of any differences. People wore the same white top and black pants every day, they ate from the same menu, they watched the same news program at the same exact time.
Slowly, that became life. To the population of your country, nobody else existed. You became sheltered.
People tried to fight it at first. Caitlin Snow watched as her own mother began to work at ELC. It pushed them apart as Carla stopped caring. Cait watched her dad get shot when he tried to escape with her. What scared Caitlin the most was that she didn’t cry at all. There was a tug in her chest but— after that? Nothing. Two weeks of being lied to and taking those pills and changes were already happening

You had become the leader of a rebellion. But nobody else had to find out.
You took this job as a cover. You wanted inside information. And you were gonna get it.
When you met Dr. Snow, she was polite. Shaking your hand and greeting you that morning. Nothing really changed for a while. You would greet each other in the morning, but things were completely professional.
You had to keep the act up. Anyone to get caught would immediately be taken to isolation where they’d take away your pills. You’d slowly gain emotions back and it drives every person to insanity, almost always resulting in suicide.
But you just couldn’t help yourself. The chestnut haired co-worker was somehow enrapturing despite the lack of emotions.
You really didn’t mean to fall in love.
“They are stripping us of something beautiful. Today, I found myself desiring a partner. Not just to repopulate— it was more! You read about this in your books but it’s true. I felt love. Real. Fresh, undiluted, pure.”
You knew Caitlin skipped her pills. You could see in her eyes when she would feel. The way she froze when your hand brushed hers.

Abstract
Fitness landscapes are often described in terms of ‘peaks’ and ‘valleys’, indicating an intuitive low-dimensional landscape of the kind encountered in everyday experience. The space of genotypes, however, is extremely high dimensional, which results in counter-intuitive structural properties of genotype-phenotype maps. Here we show that these properties, such as the presence of pervasive neutral networks, make fitness landscapes navigable. For three biologically realistic genotype-phenotype map models—RNA secondary structure, protein tertiary structure and protein complexes—we find that, even under random fitness assignment, fitness maxima can be reached from almost any other phenotype without passing through fitness valleys. This in turn indicates that true fitness valleys are very rare. By considering evolutionary simulations between pairs of real examples of functional RNA sequences, we show that accessible paths are also likely to be used under evolutionary dynamics. Our findings have broad implications for the prediction of natural evolutionary outcomes and for directed evolution.

Ever since they were first introduced in Sewall Wright’s foundational paper1, fitness landscapes have become an enduring and central concept in evolutionary biology2,3,4,5,6. In particular, a low-dimensional picture of fitness ‘peaks’ and fitness ‘valleys’ has played an important role in shaping intuition around evolutionary dynamics. A key prediction is that a population must typically traverse an unfavourable valley of lower fitness to move from one fitness peak to another. But, as already pointed out by many since4,7,8,9,10,11, the space of genotypes is typically extremely high dimensional. As illustrated in Fig. 1, what appears to be a fitness valley in a lower-dimensional landscape could be easily bypassed when dimensions are added9,10,11.
Three key open questions are: (1) does the low-dimensional picture of fitness valleys hold for realistic high-dimensional genotype spaces? And if we define accessible paths of point mutations between a low fitness phenotype and a high fitness phenotype as those with monotonically increasing fitness, (2) what properties of biological systems facilitate their presence and (3) are such paths sufficiently common that they can easily be found by an evolving population?
One way forward is to consider empirical fitness landscapes, where much recent progress has been made5,12, particularly for molecular phenotypes5,13,14,15,16,17,18,19,20,21. This body of work has yielded important insights, such as the role of local epistatic interactions in sculpting evolutionary paths22,23,24. Nevertheless, ruling out high-dimensional bypasses is difficult in empirical studies because genotype spaces, which grow exponentially as KL for alphabet size K and genotype length L, are almost always unimaginably vast25. They are also highly connected since distances are linear; two genotypes are at most L point mutations away, but are connected by up to L! shortest possible paths given the L mutations may occur in any order. For example, even for a very short L = 20 strand of RNA, there are up to 20! ≅ 2 × 1018 paths between any two genotypes. Empirical landscapes can typically only ever sample a small fraction of the full genotype space, so what appears to be an isolated fitness peak, may in fact be accessible via pathways not included in the experiment.
A different strand of work, which can in principle address questions of global accessibility, has focused on model genotype-to-fitness landscapes3,6,10,11,26,27. If fitness is assigned randomly to genotypes, as in Kingman’s ‘house of cards’ model28, then the probability of finding accessible paths is small. If instead there are correlations between fitness and the genotypes, then, depending on details of the model, accessible paths can indeed be common11,29. These correlations are often expressed in terms of ruggedness: a more rugged model has fewer correlations between genotypes and fitness, and so is less navigable. While much progress has been made in this literature, it is not clear how well these models capture the true correlations of biological fitness landscapes.
Here we take a different approach, and build on recent advances showing that many realistic genotype-phenotype (GP) maps share generic structural features that can enhance navigability30,31,32. In contrast to the genotype-to-fitness models studied by others (above), we consider the genotype-to-phenotype-to-fitness map by inserting the GP map as an additional intermediate step that provides the non-random organization of the mapping from genotypes to fitness. This means correlations in fitness are naturally incorporated as a consequence of the GP map, rather than through an assumption explicitly parameterized.

The idea of "leys" as straight tracks across the landscape was put forward by the English antiquarian Alfred Watkins in the 1920s, particularly in his book The Old Straight Track. He argued that straight lines could be drawn between various historic structures and that these represented trade routes created by ancient British societies. Although he gained a small following, Watkins' ideas were never accepted by the British archaeological establishment, a fact that frustrated him. His critics noted that his ideas relied on drawing lines between sites established at different periods of the past. They also argued that in prehistory, as in the present, it was impractical to travel in a straight line across hilly or mountainous areas of Britain, rendering his leys unlikely as trade routes. Independently of Watkins' ideas, a similar notion—that of Heilige Linien ('holy lines')—was raised in 1920s Germany.
During the 1960s, Watkins' ideas were revived in altered form by British proponents of the countercultural Earth Mysteries movement. In 1961, Tony Wedd put forward the belief that leys were established by prehistoric communities to guide alien spacecraft. This view was promoted to a wider audience in the books of John Michell, particularly his 1969 work The View Over Atlantis. Michell's publications were accompanied by the launch of the Ley Hunter magazine and the appearance of a ley hunter community keen to identify ley lines across the British landscape. Ley hunters often combined their search for ley lines with other esoteric practices like dowsing and numerology and with a belief in a forthcoming Age of Aquarius that would transform human society. Although often hostile to archaeologists, some ley hunters attempted to ascertain scientific evidence for their belief in earth energies at prehistoric sites, evidence they could not obtain. Following sustained archaeological criticism, the ley hunter community dissipated in the 1990s, with several of its key proponents abandoning the idea and moving into the study of landscape archaeology and folkloristics. Belief in ley lines nevertheless remains common among some esoteric religious groups, such as forms of modern Paganism, in both Europe and North America.
Archaeologists note that there is no evidence that ley lines were a recognised phenomenon among ancient European societies and that attempts to draw them typically rely on linking together structures that were built in different historical periods. Archaeologists and statisticians have demonstrated that a random distribution of a sufficient number of points on a plane will inevitably create alignments of random points purely by chance. Khnopff came from an upper middle class Belgian family of lawyers. In 1875 he began to study law at the University of Brussels, but his true passion was literature. He avidly read Flaubert, Baudelaire and Leconte de Lisle, and his circle of friends included many young writers of the day, such as Emile Verhaeren, Max Waller, Iwan Gilkin and Georges Rodenbach. It was not long before he abandoned his law degree and began studying painting under Xavier Mellery. It was in Paris at the Exposition Universelle of 1878 that he discovered the works of Gustave Moreau, John Everett Millais and Edward Burne-Jones, which were to have a decisive influence on his own artistic output. Khnopff made his artistic debut in 1881, at the Brussels Salon held by the group L'Essor. In 1883 he was a founding member of the Belgian avant-garde group Les XX and later also of La Libre Esthetique. He exhibited at the annual Salons of the Rose + Croix in Paris, at the exhibitions of the Vienna Secession, and also started to exhibit regularly in England. The Pre-Raphaelite painters Hunt, Watts, Rossetti, Ford Maddox Brown and Burne-Jones became friends. In 1895 he became a correspondent for the English periodical The Studio, whose Studio-Talk-Brussels column he edited until 1914; he also regularly contributed to the journal of the Vienna Secession, Ver Sacrum.
For Khnopff, the experience of an ideal work of art was a quasi mystical experience, which allowed one to escape the world through imagination. Typically, his paintings are highly enigmatic and have multiple layers of meaning. To achieve this, Khnopff would deliberately isolate the motifs that interested him within a composition, and rearrange them like props on a stage. By choosing and ordering fragments of images on the basis of highly personal analogies, he eliminated every reference to real space and time. Similarly to Odilon Redon (see lots 257-259) Khnopff wanted his works to be contemplated and perceived intuitively rather than analysed and fully understood. It is for this reason that he has often been referred to as a painter of the invisible. For Khnopff, the image was merely the physical expression of the philosophical objective he had set himself, namely to seek out 'ideal beauty'.
Khnopff found the personification of ideal female beauty embodied by his sister, Marguerite, and he depicted her numerous times (see lots 251, 254 and 256). His depictions of her often heighten her androgynous features. Indeed, androgyny is central to Khnopff's work and representative of his belief that women embodied the duality and ambiguity of the world.
Khnopff very much saw himself as the genius artist who has to renounce the dangers of sensuality in order to fulfil his spiritual and artistic calling. This idea was widespread in symbolist circles at the turn of the century and found expression in the many depictions of Oedipus and the Sphinx (see lot 254). This self-imposed isolation is also expressed by one of Khnopff's maxims, 'My soul is alone and nothing influences it. It is like glass enclosed in silence, completely devoted to its interior spectacle' (quoted by Verhaeren in his memoirs), and is behind the introspective, meditative aspects of many of his works, notably his depictions of unattainable femmes fatales or saintly virgins, and especially works such as Avec Verhaeren. Un Ange, whose stillness is reminiscent of Baudelaire's poem Beauty:
I am fair, O mortals! like a dream carved in stone,
And my breast where each finds death in turn
Is made to inspire in the poet a love
As eternal and silent as matter.
On a throne in the sky, a mysterious sphinx,
I join a heart of snow to the whiteness of swans;
I hate movement for it displaces lines,

Executed circa 1909, the delicate colour harmonies of the present work illustrate the technical experimentation that continued to characterise Khnopff's work into maturity.
Mysticism and eroticism were two prevalent artistic themes at the turn of the century. Khnopff often portrayed women as sphinx-like creatures, mysterious figures harbouring untold secrets - at once vague and defined, aloof and sensual, fragile and powerful, soft and cruel.
According to Gisèle Ollinger-Zinque 'representations of women play a central role in Belgian Symbolism: the figure of Woman is seen as an embodiment of the duality and ambiguity of earthly existence. For Khnopff, Woman is by turns the angel, muse and friend who flies to Man's salvation. Yet she is also the depraved temptress, the femme fatale, the living symbol of Péladan's Le vice suprême. Angel or Demon, she is always alone, isolated from the world and unattainable. She speaks to no one and Love, the very symbol of communion, is denied her.' (Six symbolist works, Patrick Derom Gallery, Brussels, 2005, p. 23).
Commenting on Femme Mysterieuse, Gisèle Ollinger-Zinque writes: 'This striking, idiosyncratic work, obsesses and is very intense. Around 1909, Khnopff executed a number of works which depict women covered in jewellery.... The faces of these women are enigmatic, their eyes are closed, and nothing can trouble the meditation of these inaccessible princesses of dreams.' (Fernand Khnopff, Tokyo, Hymenia, Nagoya and Yamanashi, 1990, p. 169).
Traditionally, in the 'language' of flowers, cyclamen were a symbol of diffidence. In the nineteenth century flowers given by a man to a woman had a clear meaning. While red roses for example spoke of passion and love, the message of cyclamen was 'Votre beauté me désespère' (your beauty makes me despair). The cyclamen in the present work are thus representative of the woman's unattainablility.
The stasis of Khnopff's female protagonists is a reflection of their introspection and trance-like state. He believed that an abandonment of consciousness was at the source of dreams, and was fascinated with sleep and hypnotic states. Christina Georgina Rossetti (5 December 1830 – 29 December 1894) was an English writer of romantic, devotional and children's poems, including "Goblin Market" and "Remember". Dot Porter, Curator, Digital Research Services at the University of Pennsylvania Library, offers a video orientation to Ms. Codex 1645, an 18th-century copy of a technical treatise on the painting of miniatures, in print from 1673. Includes information on the reduction of images and mixes of pigments for landscapes, skies, varieties of flowers, skin, faces, drapery, and jewels. Slightly more than half the manuscript (through p. 138) closely matches early printed editions; the remainder of the text presents material from other sources, including preparations and uses of gold leaf, varnish, lacquer, and gold and silver inks, and instructions for marbling paper and tinting parchment. It was written in Nancy, France, in 1744.

• Bệnh nhân cần điều trị bằng thuốc kháng viêm không steroid (NSAID) liên tục- Chữa lành loét dạ dày do dùng thuốc NSAID. – Phòng ngừa loét dạ dày và loét tá tràng do dùng thuốc NSAID ở bệnh nhân có nguy cơ.
• Điều trị kéo dài sau khi đã điều trị phòng ngừa tái xuất huyết do loét dạ dày tá tràng bằng ��ường tĩnh mạch.
• Điều trị hội chứng Zollinger Ellison
* Trẻ vị thành niên từ 12 tuổi trở lên:
• Bệnh trào ngược dạ dày thực quản (GERD)
– Điều trị viêm xước thực quản do trào ngược
– Điều trị dài hạn cho bệnh nhân viêm thực quản đã chữa lành để phòng ngừa tái phát
– Điều trị triệu chứng bệnh trào ngược dạ dày –thực quản (GERD).
•Kết hợp với kháng sinh trong điều trị loét tá tràng do Helicobacter pylori.
Liều lượng – Cách dùng
Loét tá tràng 20 mg/ngày x 2-4 tuần. Loét dạ dày & viêm thực quản trào ngược 20 mg/ngày x 4-8 tuần.
Có thể tăng 40 mg/ngày ở bệnh nhân đề kháng với các trị liệu khác. Hội chứng Zollinger-Ellison 60 mg/ngày.
Dự phòng tái phát loét dạ dày, tá tràng 20-40 mg/ngày.
Chống chỉ định:
Tiền sử quá mẫn với esomeprazole, phân nhóm benzimidazole hay các thành phần khác trong công thức.

Esomeprazole ức chế CYP2C19, men chính chuyển hoá esomeprazole. Do vậy, khi esomeprazole được dùng chung với các thuốc chuyển hoá bằng CYP2C29 như diazepam, citalỏpam, imipram, imipramine, clomipramine, phenytoin…, nồng độ các thuốc này trong huyết tương có thể tăng và cần giảm liều dùng.
Tác dụng phụ:
Các phản ứng ngoại ý do thuốc sau đây đã được ghi nhận hay nghi ngờ trong các chương trình nghiên cứu lâm sàng của esomeprazole. Không có phản ứng nào liên quan đến liều dùng. Thường gặp (>1/100,1/1000, 1/10.1000,Các phản ứng ngoại ý được ghi nhận đối với hỗn hợp racemic (omeprazole) và có thể xảy ra với esomeprazole: Hệ thần kinh trung ương và ngoại vị: Dị cảm, buồn ngủ, mất ngủ, chóng mặt. Lú lẫn tâm thần có thể hồi phục, kích động, nóng nảy, trầm cảm và ảo giác chủ yếu ở bệnh nhân mắc bệnh nặng. Nội tiết: nữ hoá tuyến vú. Tiêu hoá: Viêm miệng và bệnh nấm Candida đường tiêu hoá; Huyết học: giảm bạch cầu, giảm tiểu cầu, mất bạch cầu hạt và giảm toàn bộ tế bào máu. Gan: tăng men gan, bênh não ở bệnh nhân trước đó mắc bệnh gan nặng: viêm gan có hoặc không có vàng da, suy gan. Cơ xương: Đau khớp, yếu cơ và đau cơ. Da: Nổi mẩn, nhạy cảm ánh sáng hồng ban da dạng, hội chứng Stevens-Johnson hoại tử biểu bì gây độc (TEN), rụng tóc. Các phản ứng ngoại ý khác mệt mỏi, phản ứng quá mẫn như: phù mạch, sốt, co thắt phế quản, viêm thận kẽ.Tăng tiết mồ hôi, phù ngoại biên, nhìn mờ rối loạn vị giác và giảm natri máu.
Chú ý đề phòng:
Phụ nữ có thai & cho con bú. Cần loại trừ bệnh ác tính trong trường hợp nghi ngờ loét dạ dày.
Thông tin thành phần Esomeprazole
Dược lực:
Thuốc ức chế bơm proton.
Dược động học :
Phân bố Thể tích phân bố biểu kiến ở trạng thái hằng định trên người khoẻ mạnh khoảng 0,22 L/kg thể trọng. Esomeprazole sodium gắn kết 97% với protein huyết tương. Chuyển hóa và bài tiết Esomeprazole sodium được chuyển hóa hoàn toàn qua hệ thống cytochrome P450 (CYP). Phần chính của quá trình chuyển hóa Esomeprazole sodium phụ thuộc vào men CYP2C19 đa hình thái, tạo thành các chất chuyển hóa hydroxy và desmethyl của Esomeprazole sodium. Phần còn lại của quá trình chuyển hóa phụ thuộc vào một chất đồng dạng đặc hiệu khác, CYP3A4, tạo thành Esomeprazole sodium sulphone, chất chuyển hóa chính trong huyết tương. Các tham số dưới đây chủ yếu phản ánh dược động học ở những cá nhân có men chức năng CYP2C19, là nhóm người chuyển hoá mạnh. Tổng độ thanh thải huyết tương khoảng 17L/giờ sau khi dùng liều đơn và khoảng 9L/giờ sau khi dùng liều lặp lại. Thời gian bán thải trong huyết tương khoảng 1,3 giờ sau khi dùng liều lặp lại 1 lần/ngày. Nồng độ và thời gian tiếp xúc (AUC) tăng lên sau khi dùng lặp lại Esomeprazole sodium. Sự tăng này phụ thuộc theo liều và đưa đến kết quả là có mối liên hệ không tuyến tính giữa AUC và liều dùng sau khi dùng liều lặp lại. Sự phụ thuộc vào thời gian và liều dùng này là do sự giảm chuyển hóa ở giai đoạn đầu qua gan và giảm độ thanh thải toàn thân có lẽ do sự ức chế men CYP2C19 của Esomeprazole sodium và/hoặc chất chuyển hóa sulphone. Esomeprazole sodium thải trừ hoàn toàn khỏi huyết tương giữa các liều dùng mà không có khuynh hướng tích lũy khi dùng 1lần/ngày. Sau khi dùng liều lặp lại 40 mg tiêm tĩnh mạch, nồng độ đỉnh trung bình trong huyết tương là khoảng 13,6mcmol/L. Nồng độ đỉnh trung bình của thuốc ở dạng uống tương ứng trong huyết tương là khoảng 4,6mcmol/L. Có thể ghi nhận một tỷ lệ tăng nhẹ (khoảng 30%) về mức tiếp xúc theo nồng độ và thời gian sau khi tiêm tĩnh mạch so với dạng uống. Các chất chuyển hóa chính của Esomeprazole sodium không ảnh hưởng đến sự tiết acid dạ dày. Khoảng 80% Esomeprazole sodium liều uống được bài tiết qua nước tiểu dưới dạng các chất chuyển hóa, phần còn lại qua phân. Ít hơn 1% thuốc dạng không đổi được tìm thấy trong nước tiểu. Các nhóm bệnh nhân đặc biệt Khoảng 1-2% dân số không có men chức năng CYP2C19 và được gọi là nhóm người chuyển hóa kém. Ở các cá nhân này, sự chuyển hoá của Esomeprazole sodium được xúc tác chủ yếu bởi CYP3A4. Sau khi dùng liều lặp lại Esomeprazole sodium 40 mg dạng uống, 1lần/ngày, mức tiếp xúc theo nồng độ và thời gian trung bình ở người chuyển hóa kém cao hơn khoảng 100% so với bệnh nhân có men chức năng CYP2C19 (nhóm người chuyển hóa mạnh). Nồng độ đỉnh trung bình trong huyết tương tăng khoảng 60%. Sự khác biệt tương tự cũng được ghi nhận đối với Esomeprazole sodium tiêm tĩnh mạch. Những ghi nhận này không ảnh hưởng đến liều dùng Esomeprazole sodium. Sự chuyển hóa của Esomeprazole sodium không thay đổi đáng kể ở bệnh nhân cao tuổi (71-80 tuổi). Sau khi dùng liều đơn Esomeprazole sodium 40 mg dạng uống, mức tiếp xúc theo nồng độ và thời gian ở phụ nữ cao hơn nam giới khoảng 30%. Không ghi nhận có sự khác biệt về mức tiếp xúc theo nồng độ và thời gian giữa các giới tính sau khi dùng liều lặp lại 1lần/ngày. Sự khác biệt tương tự cũng được ghi nhận khi dùng Esomeprazole sodium đường tĩnh mạch. Những ghi nhận này không ảnh hưởng đến liều dùng Esomeprazole sodium. Sự chuyển hóa của Esomeprazole sodium có thể bị suy giảm ở bệnh nhân rối loạn chức năng gan từ nhẹ đến trung bình. Tốc độ chuyển hóa giảm ở bệnh nhân rối loạn chức năng gan nặng, dẫn đến làm tăng gấp đôi mức tiếp xúc theo nồng độ và thời gian của Esomeprazole sodium. Vì vậy, không dùng quá liều tối đa 20 mg ở bệnh nhân rối loạn chức năng gan nặng. Esomeprazole sodium hoặc các chất chuyển hóa chính không có khuynh hướng tích lũy khi dùng 1lần/ngày. Chưa có nghiên cứu nào được thực hiện trên bệnh nhân giảm chức năng thận. Vì thận chịu trách nhiệm trong việc bài tiết các chất chuyển hóa của Esomeprazole sodium nhưng không chịu trách nhiệm cho sự đào thải thuốc dưới dạng không đổi, người ta cho là sự chuyển hóa của Esomeprazole sodium không thay đổi ở bệnh nhân suy giảm chức năng thận.
Tác dụng :
Esomeprazole sodium là dạng đồng phân S-của omeprazole và làm giảm sự bài tiết acid dạ dày bằng một cơ chế tác động chuyên biệt. Thuốc là chất ức chế đặc hiệu bơm acid ở tế bào thành. Cả hai dạng đồng phân R-và S-của omeprazole đều có tác động dược lực học tương tự. Vị trí và cơ chế tác động Esomeprazole sodium là một chất kiềm yếu, được tập trung và biến đổi thành dạng có hoạt tính trong môi trường acid cao ở ống tiểu quản chế tiết của tế bào thành, tại đây thuốc ức chế men H+K+-ATPase (bơm acid) và ức chế cả sự tiết dịch cơ bản lẫn sự tiết dịch do kích thích. Tác động lên sự tiết acid dịch vị Sau 5 ngày dùng liều uống Esomeprazole sodium 20 mg và 40 mg, độ pH trong dạ dày > 4 đã được duy trì trong thời gian trung bình tương ứng là 13 và 17 giờ trong vòng 24 giờ ở bệnh nhân trào ngược dạ dày thực quản có triệu chứng. Tác động này giống nhau bất kể Esomeprazole sodium được dùng đường uống hoặc đường tĩnh mạch. Khi dùng AUC như là một tham số đại diện cho nồng độ thuốc trong huyết tương, người ta đã chứng minh được có mối liên hệ giữa sự ức chế tiết acid với nồng độ thuốc và thời gian tiếp xúc sau khi dùng Esomeprazole sodium dạng uống. Tác động trị liệu của sự ức chế acid Khi dùng Esomeprazole sodium 40 mg dạng uống, khoảng 78% bệnh nhân viêm thực quản do trào ngược được chữa lành sau 4 tuần và 93% được chữa lành sau 8 tuần. Các tác động khác có liên quan đến sự ức chế acid Trong quá trình điều trị bằng thuốc kháng tiết acid dịch vị, nồng độ gastrin huyết thanh tăng đáp ứng với sự giảm acid dịch vị. Tăng số tế bào ELC có lẽ do tăng nồng độ gastrin huyết thanh đã được ghi nhận ở một số bệnh nhân khi điều trị dài hạn với Esomeprazole sodium dạng uống. Sau thời gian dài điều trị bằng thuốc kháng tiết acid dịch vị, nang tuyến dạ dày được ghi nhận xảy ra tương đối thường xuyên hơn. Những thay đổi này, là kết quả sinh lý của sự ức chế mạnh lên tiết acid dịch vị, thì lành tính và có thể phục hồi được.
Chỉ định :
Esomeprazole sodium được chỉ định trong bệnh trào ngược dạ dày-thực quản trên bệnh nhân viêm thực quản và/hoặc có triệu chứng trào ngược nặng như là một liệu pháp thay thế cho dạng uống khi liệu pháp dùng qua đường uống không thích hợp.
Liều lượng – cách dùng:
Thuốc uống:
– Loét tá tràng 20 mg/ngày x 2-4 tuần.
– Loét dạ dày & viêm thực quản trào ngược 20 mg/ngày x 4-8 tuần. Có thể tăng 40 mg/ngày ở bệnh nhân đề kháng với các trị liệu khác.
– Hội chứng Zollinger-Ellison 60 mg/ngày.
– Dự phòng tái phát loét dạ dày, tá tràng 20-40 mg/ngày.
Thuốc tiêm:
Bệnh nhân không thể dùng thuốc qua đường uống có thể điều trị bằng dạng tiêm với liều 20-40mg, 1 lần/ngày.
Bệnh nhân bị trào ngược thực quản nên được điều trị với liều 40mg, 1 lần/ngày.
Ðể điều trị triệu chứng bệnh trào ngược, bệnh nhân nên được dùng liều 20mg, 1 lần/ngày. Thời gian điều trị qua đường tĩnh mạch thường ngắn và nên chuyển sang dùng thuốc đường uống ngay khi có thể được.
Cách sử dụng
Ðường tiêm:
Liều 40mg Dung dịch pha tiêm nên được tiêm tĩnh mạch trong khoảng thời gian tối thiểu 3 phút.
Liều 20mg: Nửa phần dung dịch pha nên được tiêm tĩnh mạch trong khoảng thời gian tối thiểu 3 phút. Nên loại bỏ phần dung dịch không sử dụng.
Ðường tiêm truyền:
Liều 40mg: Dung dịch pha tiêm nên được truyền tĩnh mạch trong khoảng thời gian từ 10 đến 30 phút.
Liều 20mg: Nửa phần dung dịch pha nên được truyền tĩnh mạch trong thời gian tối thiểu 10-30 phút. Nên loại bỏ phần dung dịch không sử dụng.
Trẻ em: Esomeprazole sodium không nên dùng cho trẻ em vì chưa có dữ liệu. Người suy chức năng thận: Không cần điều chỉnh liều ở bệnh nhân suy chức năng thận. Do kinh nghiệm điều trị cho bệnh nhân suy thận nặng vẫn còn hạn chế, nên thận trọng khi dùng thuốc trên các bệnh nhân này.
Người suy chức năng gan: Không cần điều chỉnh liều ở bệnh nhân suy chức năng gan từ nhẹ đến trung bình. Ðối với bệnh nhân suy gan nặng, không nên dùng quá liều tối đa 20 mg Esomeprazole sodium mỗi ngày.
Người cao tuổi: Không cần điều chỉnh liều ở người cao tuổi.
Hướng dẫn sử dụng:
Tiêm: Dung dịch tiêm được pha chế bằng cách thêm 5 mL dung dịch NaCl 0,9% dùng đường tĩnh mạch vào lọ thuốc chứa Esomeprazole sodium. Dung dịch tiêm sau khi pha trong suốt và không màu hoặc vàng nhạt. Sự phân hủy dung dịch pha chế phụ thuộc vào độ pH cao và vì vậy thuốc chỉ nên pha với dung dịch NaCl 0,9% dùng đường tĩnh mạch. Dung dịch sau khi pha không nên pha trộn hoặc dùng chung một bộ dây truyền với các thuốc khác. Dung dịch đã pha nên được kiểm tra bằng mắt thường xem có phần tử lạ và biến màu trước khi dùng. Chỉ sử dụng dung dịch trong suốt. Dung dịch đã pha nên được dùng trong vòng 12 giờ sau khi pha. Không bảo quản ở nhiệt độ lớn hơn 25 độ C. Dung dịch đã pha nên được tiêm tĩnh mạch trong khoảng thời gian tối thiểu là 3 phút. Nên dùng nửa thể tích pha nếu chỉ dùng 20mg. Phần dung dịch không sử dụng nên được loại bỏ.
Tiêm truyền:
Dung dịch tiêm truyền được pha chế bằng cách hòa tan thuốc Esomeprazole sodium chứa trong 1 lọ với một thể tích lên đến 100 mL dung dịch NaCl 0,9% dùng đường tĩnh mạch. Dung dịch tiêm sau khi pha trong suốt và không màu hoặc vàng nhạt. Sự phân hủy dung dịch pha chế phụ thuộc vào độ pH cao và vì vậy thuốc chỉ nên pha với một thể tích xác định dung dịch NaCl 0,9% dùng đường tĩnh mạch. Dung dịch đã pha không nên pha trộn hoặc dùng cùng một bộ dây truyền với các thuốc khác. Dung dịch đã pha nên được dùng tách biệt với các thuốc khác.
Dung dịch đã pha nên được kiểm tra bằng mắt thường xem có phần tử lạ và biến màu trước khi dùng. Chỉ sử dụng dung dịch trong suốt. Dung dịch đã pha này nên được dùng trong vòng 12 giờ sau khi pha. Chống chỉ định :
Tiền sử quá mẫn với hoạt chất chính Esomeprazole sodium hoặc với các chất khác thuộc phân nhóm benzimidazoles hoặc bất kỳ tá dược nào của thuốc này.
Tác dụng phụ
Các phản ứng ngoại ý do thuốc sau đây đã được ghi nhận hay nghi ngờ trong các thử nghiệm lâm sàng của Esomeprazole sodium dùng đường uống hoặc đường tĩnh mạch và sau khi lưu hành dạng uống của thuốc trên thị trường. Thường gặp (> 1/100, Nhức đầu, đau bụng, tiêu chảy, đầy hơi, buồn nôn/nôn, táo bón. Ít gặp (> 1/1000, Viêm da, ngứa, nổi mề đay, choáng váng, khô miệng, nhìn mờ. Hiếm gặp (> 1/10.000, Phản ứng quá mẫn như phù mạch, phản ứng phản vệ. Tăng men gan. Hội chứng Stevens Johnson, hồng ban đa dạng, đau cơ. Các phản ứng ngoại ý sau đây đã được ghi nhận đối với đồng phân racemic (omeprazole) và có thể xảy ra với Esomeprazole sodium: Hệ thần kinh trung ương và ngoại vi: Dị cảm, buồn ngủ, mất ngủ, chóng mặt. Lú lẫn tâm thần có thể hồi phục, kích động, nóng nảy, trầm cảm và ảo giác chủ yếu ở bệnh nhân mắc bệnh nặng. Nội tiết: Nữ hóa tuyến vú. Tiêu hóa: Viêm miệng và bệnh nấm Candida đường tiêu hóa. Huyết học: Giảm bạch cầu, giảm tiểu cầu, mất bạch cầu hạt và giảm toàn bộ tế bào máu. Gan: Bệnh não ở bệnh nhân trước đó mắc bệnh gan nặng; viêm gan có hoặc không có vàng da, suy gan. Cơ xương: Ðau khớp, yếu cơ. Da: Nổi mẩn, nhạy cảm ánh sáng, hoại tử thượng bì nhiễm độc (TEN), rụng tóc. Các phản ứng ngoại ý khác: Mệt mỏi. Phản ứng quá mẫn như: sốt, co thắt phế quản, viêm thận kẽ. Tăng tiết mồ hôi, phù ngoại biên, rối loạn vị giác và giảm natri máu. Tổn thương thị giác không phục hồi được đã được ghi nhận trong một số rất hiếm trường hợp bệnh nhân mắc bệnh trầm trọng đã dùng omeprazole đường tĩnh mạch, đặc biệt khi dùng liều cao, nhưng vẫn chưa xác định mối quan hệ nhân quả giữa việc dùng thuốc và biến cố ngoại ý này.

In Arc the Lad 2, Elc is capable of using swords, spears or axes. Despite the spear being Elc’s default weapon, as well as his signature weapon in every appearance afterwards, Arc 2 always portrays Elc using the sword during action cutscenes. Even the Elc clones (during the Chimera Labs segment) are shown with swords rather than spears.

Made in the sound lab class with Daniel using audio recorded in the local grocery store using Zoom recorders hired from the ELC.
Already quite familiar with using Audition but was nice to formally learn how to utilise the program’s features, especially the effects.
I like the effect of the ‘heartbeat’ but would like to learn how to improve upon the sounds quality, and perhaps remove the white noise in the background.
