ID: a video showing a weeks worth of heart rate numbers that start out high, ranging from 72 to 158, then dropping drastically to a resting rate in the 50s with the highest being 99.
The video then moves on to show a month’s worth of similar data, showing wild swings in heart rate from the low 40s up to the mid hundreds before evening out at the end of the week.
——
So, uh, I think the amitriptyline is managing my Hyperadrenergic POTS. Quite spectacularly, actually.
I’ve never been able to stay on blood pressure meds because they block epi-pens from working but also because they made my MCAS worse, but amitriptyline inhibits the uptake of norepinephrine (among other things) which is what causes Hyperadrenergic POTS.
My elevated norepinephrine levels are caused by my MCAS. Amitriptyline is a multi-level mast cell inhibitor.
And as you can see from the above video it has dropped my heart rate significantly. All those high heart rate days with the massive swings between high and low? Those were normal days where just sitting up in bed gave me a resting heart rate in the 80s and 90s. That’s been my life for as long as I can remember.
That one day where my heart swings between 59 and 99? That 99 was me walking up a steep hill in high 80s heat with 70% humidity while carrying a gallon of water back from the store.
I currently have a resting heart rate of 58 even after drinking caffeine and not having had any salt yet today.
I think this bitch is managing my MCAS/h-POTS. Holy shit.

Hi! I know this is potentially not really in your wheelhouse, but do you have any thoughts on the idea that ME/CFS, POTS, EDS, ADHD, autism, PMDD and MCAS tend to cluster? It's always felt possibly kind of spurious and psychiatry-pilled to me, and also like something that might have interesting clinical/cultural precedents, but I haven't been able to find much about it.
i don't think it's surprising at all that chronic illnesses 'cluster'---they're often poorly understood, diagnosed by exclusion, and characterised by overlapping symptoms. like there are probably some shared mechanisms going on that cause many of these conditions, or underlying causes in common. as it stands many chronic illnesses are basically descriptions of symptom clusters, meaning that what causes them or creates the dysfunction is unknown thus far, so it really shouldn't be surprising to see people whose symptoms suggest multiple of these diagnoses.
wjere like autism and ADHD are concerned i do think the connection is much more spurious. offhand i would posit that, rather than evidence of some kind of biological susceptibility of certain 'neurotypes', we might be seeing the physical toll of chronic stress for people dx'd with socially alienating disorders; people who are chronically ill being more likely to pick up psych diagnoses as a kind of collateral result of constant contact with the medical system; the alienation and stress of being chronically ill resulting in various kinds of social and functional difficulties that are dx'd as psych disorders; or the effects of, say, sleep disturbances related to chronic illness causing difficulties dx'd as psychological disorders, or vice versa. that's speculative on my part though.
also you do have to keep in mind where the data come from that show relationships between various diagnoses. as in like, what country with what degree of common access to healthcare and withwhat sorts of billing practices that interact in what way with diagnostic codes. like a diagnosis is often as much about creating an official record of some accommodation need (drugs, welfare eligibility, mobility aids, etc) as it is about any kind of transcendent medical truth. again, we're talking about conditions that are often under-studied and poorly understood and sometimes treated as a kind of dumping ground for 'mysterious' or 'difficult' patient presentations. so, observations of this nature should really be interpreted in general as indicating at least as much about how a given medical system functions as about how the human physiology works.

I honestly think it's nothing personal. Someone just saw your post and felt pity because the writers often don't do this much research when it comes to the show, so OP and those reblogging the post probably thought you'd do yourself a favor if you stopped obsessing over the timeline. What they failed to notice, though, is that this is not really the focus of your analyses; you don't care about the timeline, you care about the aviation stuff. It's your passion so it's normal that you want to see it represented well or at least somewhat believable in the show. This is not a crime.
If you worry about potentially irritating people, you could always add a disclaimer on top of the post that this is just an info dump about the real world, but the show ultimately does what it wants to do so no one here should feel obligated to subscribe to your theories.
I think there are a lot of people actually appreciating your input to our fandom, but there will obviously be some who don't care and get irritated. It's normal. I get irritated all the time when I scroll on my "For you" page here on Tumblr just because people headcanon Buck and Tommy in a way that I don't. That shouldn't stop them from writing what they want, though. It just says that I'm not their target audience. I think the same's with you. If someone's really this annoyed because you write about something you're passionate about and it collides with what they believe to be true, let them. Add a tag that they could block if they don't want to see it. But don't let them dictate what you write about.
Sorry, this got really long, but I really want you to know that it's okay to be yourself here, even if some people don't appreciate it. There will always be those who don't, but there will also be those who do. I think your input is great for fanfic writing purposes at least. If you help just one person with your knowledge, I think it's enough to feel satisfied. Also, if Buck was real, I bet that your blog would be one of his favorites because he could actually learn some cool stuff about his boyfriend's profession.
Ultimately, this is your mental health and no one here can force you to do something you don't feel comfortable with. If their comments made you feel like you shouldn't impose, it's a shame but if helps your mental health it's fine. I'll miss your analyses and I'm sure plenty of people will too. But we'll have to accept it because there's a real person behind this screen and their comfort should be prioritized on this blog. But if a part of you feels like you don't want to stop, please listen to it before you make this decision and remember that it's okay to be fully and unapologetically yourself.
I actually love long posts. And this is so lovely, I teared up a bit reading it yesterday, so thank you.
I think you put into words very accurately what I wasn't able to. I made my first ever post the day 7x09 aired. (It was actually my 30th birthday.) Something happened that week, a Singapore B777 encountered severe turbulence over Myanmar, injuring 104, killing 1. The media immediately pointed out the jet "plunging 6000 ft in 3 minutes" using FlightRadar24 data. I'm not talking about tabloids, I'm talking about BBC, Reuters, Al Jazeera, reputable news agencies. Aviation experts contacted the media upon reading the articles and told them a descent rate of 2000 fpm is actually standard, more so when there are injured passengers in need of urgent medical attention. Even then the media didn't walk back their sensationalized statements until a week later. That combined with the widespread attention Boeing has gotten after the MCAS scandal and the door plug blowout earlier this year, this kind of clickbait-y reporting with numerous misconceptions always gives me the ick.
Then, as you remember, the tide on BuckTommy turned drastically around that time, when a subsection of the fandom realized Tommy was staying. And that's when I started seeing takes like he only "drove the helicopter", Hen was the hero, Tommy did nothing and didn't deserve a medal. As you can see I was already in that mindset, being irritated by aviation misconceptions, I really wanted to hit back with my measly little knowledge about flying. I was already seeing people over here getting annoyed by the constant negativity and fighting, then I had an idea, why not try explaining the 5 years vs 7 years at Air Ops conundrum by bringing the intensive training for LAFD pilots into it? I mean, the introduction Tommy gave Buck during the Harbor Tour was lifted verbatim from the real LAFD website. The writers didn't even have to google the "training takes 2 years" part, it was on the same page, they just had to scroll down a little and read a few more lines.
I just thought it would be a fun little hc/potential background story for Tommy, showing how hard it is to be a firefighting pilot. On top of that, competency and dedication towards one's goal is just super hot. Even the CAL FIRE air tanker stuff I wasn't trying to persuade others that the invisible string theory is wrong. I just saw and loved the idea that Tommy always "knows a guy". We only saw one example in canon of that, him being friends with the Las Vegas fights promoter, so I was like, wait, he knew a guy with CAL FIRE too! Maybe also sneaking in some fun facts about the power and danger of water dropping in an urban environment. I never intended to nitpick, I'm just trying to bring 2 things I love together. I mean real CAL FIRE didn't even start operating C-130 until last year, and I don't give a damn about it.
I wrote about my fear of one day being mistaken as one of the Tommy bashers because my current overfixation coincides with what the haters decide to fuss about that day. I saw pushback against looking too much into the timeline first 2 times I posted about Tommy's pilot career. As I said, I planned to keep writing about things I enjoy until the day comes that I unknowingly walk into the bear trap. Honestly, I feel like this time is it.
Like many BuckTommy enjoyers, I've been a little on edge since the hiatus started. We've got attacked, harassed and bombarded daily by a thousand different reasons Tommy supposedly won't be back for S8. Then I saw those planes, I saw that hangar, and I was so excited. I had so many ideas and speculations to play with. The 8x02 title came out, I finally got a couple aviation related asks, I was so pumped. I wasn't even thinking about CAL FIRE at the time, someone asked if it was possible Tommy flew that plane realistically, so answered it. And hours later, that post popped up, explicitly mentioning my theory, with 200+ notes. It was just... a little overwhelming.
I'm not even mad at those who think I obsessed over these things too much. I just... wasn't prepared for that. I thought, comparing to the months of bad faith discourse, I was just talking planes, helicopters and clouds, that should be quite tame? I did not expect it to be actually controversial. I just don't feel the same happiness nerding out about aviation in this fandom afterwards. Maybe I just need some time to process and perhaps tone it down? I don't know. I still have many ideas in my head and those asks in my inbox.
But once again, thank you for taking the time to read my ramblings. You have no idea how good that makes me feel.

Hi and welcome to another episode of Nerd Knowlege with Kari!
In this episode we'll be talking about the Boeing 737 MAX 8, the MCAS system and why said system was even installed, why it failed.
Tumblr media
(this ist the actual Cockpit of a 737 max 8)
One lovely day someone at Boeing woke up, saw that Airbus (the Europen counterpart) had decided to renew its engines, and went: "Oh shit we have to do that too". And they did, just not well.
The Boeing 737 Max 8 failed 33 out of 89 tests, which is a lot.
With a too-high AoA (angle of attack) at take-off caused by the weight of the new engines, they could lead to possible danger.
So Boeing decided that a system that corrects the aeroplanes nose if needed. And so the MCAS ( or Maneuvering Characteristics Augmentation system [say that 5 times in a row I dare you]).
One would argue that pilots would need proper training for a new system, but not Boeing. Why?
Because it's cheaper to give pilots a one to two-hour iPad course. And I'm not even joking, that is literally the training they got.
Now what made it fail?
The MCAS system relied on one single sensor, and that sensor gave wrong AoA data, causing the system to automatically push the nose downwards.
The pilots could push up the nose as much as they wanted, the system would always push it back down. The pilots did not know how to fix this problem which then led to two horrible crashes.
I would like to add that having the finance guys take over the company was a bad decision.
I hope you're doing good!
Logging off,
Kari

Rambling about my latest medical appointment below the cut.
TLDR: MyChart tattled on me to my allergist about the second opinion I booked, allergist added extra tests (serotonin) without talking to me about them, and I don't know what to do next so I'm just writing it all out in the hopes the answer comes to me
I knew Epic/MyChart shared some data across providers without the user linking accounts and without explicitly asking for consent, but I wasn't aware just how much data was visible. Test results - don't like it, but on some level I can understand why its helpful for doctors to be able to see those. However, turns out that they can also see any appointment you have booked, even if you haven't even seen the provider yet. So my allergist was fully aware that I booked a second opinion. Great. 🙃
He was actually a lot nicer (to my face at least) at this appointment though!
Last appointment he just did skin prick allergy testing, said there was nothing interesting, and left me totally hanging (while my health declined) for the four months until my next appointment. He had sooner availabilities, he just said to book a 4 month follow-up.
So in the meantime I had talked with my regular provider, who told me to get a second opinion and sent a referral to an allergist near me. Since waitlists are fucking long, I also asked what would be the next best bet if it truly wasn't in the realm of things allergists cover, and he also sent a referral to rheumatology.
I recently had that rheumatology appointment (I need to make a catch-up post sometime soon). Unsurprisingly, she also told me to get a second opinion and recommended someone she knew a few hours away from me. I'm still between the first appointment & follow up, so while the bloodwork results have come back recently, I haven't gotten to talk with her about them yet.
Which means I had appointments booked with three different allergists. #1 is the guy I have been seeing #2 is the first referral I got, but it's just to someone nearby, my provider doesn't know them #3 was actually recommended to me and my rheumatologist has sent patients there before
#3 seems like the best option, but they're booked out until May. #1 sucks, but I figured it was better to continue care than to cancel and go it alone for 4 more months. I was on the fence about if I should try #2 (February) or just wait until #3, but figured I'd make that decision after my appointment with #1.
Anyway, he was nicer at the appointment. He actually indicated he wasn't sure what to do next instead of just dropping me. He mentioned possibly passing me forward to the Mayo Clinic (😬), he said I should think about it and message him if I want a referral to otolaryngology (why do I need to message him?), and he said he'd do testing for MCAS (which he brought up at our first appointment, but kept leaving out) just to cover all the bases. He said he was fine with the second opinion and strongly encouraged me to go with allergist #2 over #3 - he said private practice allergists aren't any good.
I did blame the second-opinion on the rheumatologist a little. I mean, what else can you expect from a doctor if you turn up and say 'it looks like an allergy, but it isn't'? He implied she was just shooting off random tests, mentioning a very slightly positive one that, again, I haven't had a chance to talk with the rheumatologist about. He said he wasn't comfortable just ordering tests if he didn't have suspicion for it. I've never asked him to? He's the one who brought up MCAS in the first place, I thought this was going to be an easy case of allergic asthma 😑
He asked more about my breathing. I keep saying that usually it's not short on the inhale and it's not short on the exhale, it's just short, but I don't think it's sticking. He keeps bringing up Vocal Cord Dysfunction (VCD) which typically causes stridor and difficultly breathing on the inhale. Asthma, again - typically, causes wheezing and difficulty on the exhale. My stridor/wheezing only occurs when things are going particularly badly, but it's always been both on the inhale and the exhale. I've tried the VCD breathing exercises he gave me, they didn't help (made my breathing worse at the beginning TBH). I've tried the ones that F's doctors have given her (she has both asthma and VDC, so it's not as if I'm uneducated here), they also didn't help. If I did just have VCD that was extremely sensitive to irritants, you would think it would have been notable on the methacholine challenge he had me do... Ah fuck I'm gonna have to get a pulmonologist, aren't I?
On the way out, he recommends I bring a copy of my tests to the second opinion visit and asks me to have them send him a copy of their notes. I ask for a copy of my skin testing since that's never been uploaded to MyChart. He tells me she'll be able to get it online. ??? So why am I bringing her paper copies of everything else and why can't I see it?! I let it slide, but I'll definitely put in a written request at some point.
Finish appointment, don't get given a date for a follow up, get blood drawn, go home.
At home I pull up the little appointment summary and find out he's testing my serotonin levels?! He had not mentioned it at all during the appointment, but he's ordered one blood test and one urine test to check serotonin levels. I knew I was getting blood and urine tests done, but I was told it was for MCAS, so I was obviously very concerned when I saw serotonin added to the list. I don't need a (pseudoscientific) anxiety diagnosis added to my chart by an allergist if my serotonin comes back low. Having read about it, there's justification for the test because I have some symptom overlap with carcinoid syndrome, but I'm still very uncomfortable with having it slid in there without anyone telling me.
Great, a blood test for cancer! I said I was open to trying things just to rule them out. I still maintain that this seems triggered by fragrances, but I have considered that something else going on could upset my immune system and cause it to go wild on a regular allergy. I love data, data is my best friend. This isn't even costing me anything because we've hit the out of pocket limit already. I did the stupid methacholine challenge, I'm willing to participate with even the less fun testing. So why wasn't it mentioned? None of the options I can come up with are great: 1. He's hopeful for low levels so he can just mentally categorize me as an anxiety case 2. He didn't mention it because he thinks "cancer" might make me freak out 3. He didn't think mentioning it/allowing some kind of consent was important
He was questioning me about the flushing during the appointment, so it seems incredibly unlikely that it was a last minute addition. From what I can tell, the urine test is usually done with a handful diet/med restrictions because certain things can mess up the results - but I did not receive those instructions. It's possible that the plan is to just do it & retake if there are issues, but I'm collecting urine for 24hrs, I'd like to just do it right the first time.
Other than those it's tryptase (to establish a baseline), IgE, histamine (blood), histamine (urine), and leukotriene E4. I'm somewhat surprised about urine histamine instead of n-methylhistamine, but maybe I'm misunderstanding how fragile histamine is. F's surprised about the lack of a prostaglandin test.
Also, I have VCD in my chart now... No referral request message required 😑
He takes all his notes in writing - so the actual appointment notes won't be uploaded for a week or so 🙃
