Please don’t take me talking about Iyami’s canon fondness for feminine clothing as me lauding it as LGBT+ representation! While I do think that he’s a pretty neat early example of a GNC manga character (Osomatsu-kun started being published in 1962!) and probably one of the first to achieve so much recognition and popularity in Japan, Fujio Akatsuka was a very progressive man for the time he was born and raised in and there was no shortage of other cool early LGBT+ themes and characters in his work, even in other aspects of Iyami’s character!
For example, Iyami has married another one of Akatsuka’s characters, an openly gay man, in at least two separate stories from the 70s and has shared what might very well be the first kiss on the lips between two men in manga history with him!

I’m the original creator of this iceberg, and ever since its creation I haven’t been happy with it as the information has been unclear, deleted or poorly explained. Oh! Did you know! Iyami’s zansu originated as a form of speech associated with women! More specifically, an archaic, shortened form of de gozaimasu, associated with the madams of old-timey high class adult entertainment businesses. This, like most of Iyami’s speech quirks, comes directly from Tony Tani, one of the comedians Akatsuka took inspiration from when creating him. Tony Tani’s influence is VERY visible on Iyami, from his flamboyant stage persona, his tendency to sprinkle English words in his speech (he introduced himself as “number one handsome boy” in English, for example) to his signature pencil mustache! In fact, Iyami’s voice in the 80s adaptation of the Osomatsu-kun anime is a VERY spot on Tony Tani impression by Kaneta Kimotsuki! Just listen to one after the other:

It’s so weird that even though I know they’re sextuplets, I still think Karamatsu is the one who looks most like their father (probably because of the acne and eyebrows when they were young).
But I know Karamatsu won’t grow up to look like their dad,
because the running joke in the JP fandom is knowing that Karamatsu’s gonna grow up half-bald as an adult lmaooooo~
