The Sleep Room by Jon Stock – haunting accounts of horrific medical abuse

A book about psychiatrist William Sargant’s unethical treatments at a London hospital in the 1960s is all the more powerful for its vivid patient testimonies

Achild of 14 is forced to walk on to a stage and strip to her underwear. Tiny and mute beneath the stacked rows of medical students, she is paraded for their benefit by a consultant psychiatrist some 44 years her senior. It is 1966 – the peak of Swinging 60s’ hedonism, liberalism and youthful counterculture – but in a locked psychiatric ward in London’s Royal Waterloo hospital, unspeakable violations are being inflicted upon patients.

The perpetrator-in-chief, William Sargant, is the subject of thriller writer Jon Stock’s first nonfiction book, The Sleep Room: A Very British Medical Scandal. One of the most notorious figures in British psychiatry, Sargant initially wished to be a physician. He pivoted to psychiatry after one of his earliest pieces of research met with a humiliating reception at the Royal College of Physicians, causing him to suffer a nervous breakdown and spend time in a psychiatric hospital himself. At this time – the 1930s – effective psychiatric treatments were virtually non-existent. Serious mental illness usually led to lifelong incarceration in an asylum. But the therapeutic nihilism of psychiatry was shifting towards optimism. Psychiatrists began experimenting with so-called “heroic” therapies, such as putting patients into insulin comas or giving them electroconvulsive therapy (ECT) to try to “reset” their brains.

Sargant firmly believed that a broken brain was no different to any other damaged organ or limb, and best fixed with aggressive physical treatment. Not for him the namby-pamby chitchat of Freud’s “sofa merchants” and their spurious talking cures. Rather, psychiatric illnesses such as depression, anxiety and schizophrenia could all be cured with excessive doses of drugs and electricity, or, if they failed, with surgical lobotomy. Sargant’s patients were sequestered away behind locked doors on the top floor of the hospital. The most infamous part of his ward was a six-bedded area known as the Sleep Room. Here, the patients, nearly all of whom were female, were drugged into long-term stupors, being roused from their beds only to be fed, washed or given innumerable doses of ECT. A typical “narcosis” treatment comprised three months of near-total unconsciousness, after which time the patient had often been reduced to a “walking zombie” with permanent memory loss.

The Sleep Room is peppered with haunting first-hand accounts of horrific treatment at the hands of William Sargant. The actor Celia Imrie, for example, the aforementioned child, was admitted under Sargant’s care because she was close to death from anorexia nervosa. She recounts being forced to drink such large doses of chlorpromazine – the first antipsychotic - that she dribbled, shook uncontrollably and found her hair in clumps on her pillow. She was injected daily with enough insulin to make her drowsy, weak, sweaty and near comatose. She remembers other women around her having huge rubber plugs jammed between their teeth before the high-voltage electricity of ECT was sent through their temples, and their bodies “shuddered and jerked” with the “scent of burning hair and flesh”. Sometimes a patient would reappear on the ward with their head thickly bandaged, scarcely able to walk after being lobotomised. Amid the moans, screams and stale stench of sleep, Imrie recalls the nurses reporting her own resistance to the drugs to Sargant, to which he ominously responded: “every dog has his breaking point”.

To say that these stories are difficult to read is an understatement. Even as someone who learned, as a medical student, about the unspeakable mid-century vogue for lobotomy – permanently subduing patients by gouging out parts of their frontal lobes – some of the accounts made my skin crawl. It is to Stock’s great credit that he places patient testimony centre stage, allowing several patients to tell their stories at length in their own, unedited words.

Sargant himself is depicted as a sinister, all-powerful, “simian” monster who ran his ward as a personal fiefdom in which he could pursue, unchecked, his conviction that effective treatments worked by physically erasing and reprogramming disturbed minds. A darker benefit of his Sleep Room was the absolute power it gave him over his charges. Once rendered unconscious, a patient could be subjected to treatments they might otherwise never have consented to, such as scores, or even hundreds, of doses of ECT. As Sargant himself put it: “What is so valuable is that they generally have no memory about the actual length of the treatment or the number of ECT used.” Before long, colleagues had nicknamed him “Bill the Brain Slicer” and “Sargant the Shock”. His fascination with techniques for brainwashing led to speculation that he may have collaborated with the CIA’s MKUltra programme into mind control, though no definitive proof of this is provided by Stock.

At least five of Sargant’s patients died during narcosis. The dangers of artificially prolonged sleep included deep vein thrombosis, pressure sores, infection and paralysis of the gut, all of which could be fatal. Though the disproportionate number of female patients under his care was not, in itself, evidence of misogyny, his treatment of particular individuals locates him firmly within that psychiatric tradition. He was perfectly willing, for example, to regard female independence as synonymous with madness and in need of social correction. One young woman was dispatched to Sargant by her wealthy businessman father after she fell in love with an “unsuitable” man in the 1960s. Sargant “helped” the situation by subjecting her to months of narcosis, ECT, antipsychotics, antidepressants and the “truth drug”, sodium amytal, after which she was successfully returned to her family with all memories of her boyfriend completely erased. Obedience had been achieved through physical obliteration of the mind.

While there is no denying Sargant’s lack of medical ethics, and his authoritarianism and reliance on dogma as opposed to clinical evidence, The Sleep Room can present him as almost cartoonishly villainous. With eyes “like washed black pebbles”, “sadomasochistic stubbornness” and a “prodigious enthusiasm for pill popping”, he sweeps the wards like a demented demigod, barking orders to terrified staff and patients alike. This portrayal unhelpfully flattens very real and complicated issues over how best to treat serious mental illness. Patient consent – the bedrock of all good medical practice – cannot always be obtained when a patient’s illness means they lack capacity to make decisions for themselves. Even today, with a modern emphasis on the rights of patients, the responsibilities of clinicians and the need for humanity in psychiatry, enforced treatment is sometimes the only way to prevent a severely unwell psychiatric patient harming or killing themselves. ECT, for example, although used much less frequently than in Sargant’s day (and rightly so), can be a life-saving last resort, as in patients with catatonic depression. With more nuance and less condemnation, Stock could have interrogated more richly the complexities of how best to treat disabling and life-threatening psychiatric illness.

Climate Changed: Fiona demonstrated wild hurricane future, and need to adapt

As she stood near the remnants of flattened homes in Port aux Basques, N.L., Denise Anderson said the thought of continuing to live next to the ocean is hard after a deadly storm foreshadowed the violence of weather to come.

"I grew up in this area, I wanted to come back to this area, but now I'm not so sure I want to," she said two days after post-tropical storm Fiona damaged the home where she has lived for three years, destroyed her neighbours' houses and swept one local woman out to sea.

Across the East Coast, similar emotions about the way climate change is altering life can be heard, as residents rebuild their homes and cope with weeks without power, and political leaders are asked how they'll prepare the coastlines and power grids to meet the next gale.

About 200 kilometres to the south across the Cabot Strait, in Reserve Mines, N.S., Reggie Boutilier pointed out a missing portion of his roof and wondered when the next storm would come. "It's only early in the hurricane season, and I'm thinking we're off to a bad start," he said the day after Fiona hit.

The scientific predictions on what's to come aren't reassuring.

Canada's Changing Climate, a federal summary of climate science released in 2019, said fossil fuel emissions are likely increasing the intensity of tropical storms that form in the southern Atlantic and head north to the Canadian coast

Blair Greenan, a federal scientist at the Bedford Institute of Oceanography who worked on the report, said in an interview that water temperatures off the Maritimes have gone up 1.5 C over the past century, adding a potent source of increased energy for the storms.

Anya Waite, a professor of oceanography at Dalhousie University, said the "sobering" reality is the warmer water shoots heat and moisture into storms like Fiona, giving them a longer duration and, often, a wider path.

While utility spokespeople referred to Fiona as "historic" in their news releases, Waite -- also the science director of the Ocean Frontier Institute -- says storms of this magnitude will become increasingly common. "We will be getting storms that have a lot more longevity because of the surface water being so much warmer," she said.

A "perfect trifecta" of conditions -- general sea-level rise over the past century created by melting glaciers, storm surges and lower barometric pressures during storms -- is also increasing the likelihood of coasts being swamped during hurricanes, she added.

"In terms of adaptation ... one of the main things is we will just have to move away from the coast," she said. "We love the coast so much that people are clinging to their last rock as it goes under. We can't do that."

Peter Bevan-Baker, the leader of the Prince Edward Island Green Party, saw an altered landscape as he drove around the Island last Friday, with thousands of trees down, farmers' barns destroyed and beaches that define the Island suddenly washed away. "The Island is changed forever," he said in an interview.

Meanwhile, thousands of people remained without power nearly two weeks after the storm hit, and complaints rose about the lack of basics such as heat, electricity, gasoline and even food for seniors in provincially operated buildings.

 Yet, during briefings last week, the privately owned utilities Nova Scotia Power and Maritime Electric, which serves P.E.I., dismissed the suggestion that power lines should be buried, saying underground lines would cost up to 10 times more without eliminating the risk of outages.

Bevan-Baker said these kinds of "standard" answers don't recognize the changing climate realities. "I understand burying lines is an enormously expensive proposition, but so is rebuilding if it's a storm like this every few years," he said.

Blair Feltmate, head of the Intact Centre on Climate Adaptation at the University of Waterloo, said that while further studies on how utilities should adapt may be useful, the time for action arrived with the 170 kilometre-per-hour gusts that buffeted the region.

Endless scenario planning can become "a substitute for action," he said in an interview.

He said where housing or infrastructure was destroyed close to the shore, the rebuild needs to occur further inland. More crucially, modelling is needed on potential coastal damage throughout the Atlantic region, in order to set rules on building that take climate adaptation into account.

Solutions will vary. In some instances, higher seawalls will protect towns; in others, development may have to retreat, while tidal flats and marshes are created to absorb some of the sea's fury, Feltmate said.

Bevan-Baker points out that in P.E.I., there are close to 30,000 undeveloped lots near the coast, and yet there's still no provincewide land-use plan taking into account future storm surges.

Joanna Eyquem, a geoscientist who also works with the University of Waterloo climate adaptation centre, said the providers of key infrastructure -- whether utilities, railways or ports -- "really need to step up to the adaptation challenge" and consider climate change in all they're doing, something that is still not universal in Canada.

By contrast, in the United Kingdom, most similar organizations and companies report climate adaptation progress every five years, in addition to making mandatory climate-related financial disclosures annually, she said.

Feltmate said ordinary citizens have to act as well. His studies show many homeowners in flood-prone areas still don't have generators to run sump pumps if the power goes out and haven't graded their land to slope rainfall away from the buildings.

While some of the adaptation is costly, Feltmate points to research indicating that for each dollar spent -- whether in cutting trees around power lines or creating power grids that are more decentralized -- there are savings of five to six dollars in averted damage.

After prior severe storms, such as Juan in 2003 and Dorian in 2019, similar messages were delivered, and governments in the region briefly seemed attentive to the changing realities. But during election campaigns that followed, climate adaptation policies were only sketched out broadly and the focus shifted back to ailing health systems.

Will this time be different, after roofs are replaced, harbours rebuilt and freezers restocked? There are signs that even if officials are slow to change course, the urgency is sinking in at ground level.

In Burnt Islands, N.L., fisherman Murray Hardy gestured around his basement after shovelling out the mud deposited by Fiona's tidal surge, saying he'll prepare for the next hurricane by emptying out the space and replacing gyprock before mould sets in.

"What am I going to do? You got your home," he said, when asked if moving was an option. "I expect more of this. All they talk about is global warming and the tides and such. I'll just clean all this out."

The [United Packinghouse Workers of America] takes its advocacy outside the plant and sues a Waterloo tavern owner for failing to serve Blacks. It was one of many tactics the union used to desegregate the city. One of its most effective strategies involved white workers going from tavern to tavern to order food and drinks. Their Black coworkers came in next. When the businesses refused to serve the Black workers, the white workers walked out. From the late 1940s through the ʼ60s, the union handled discrimination complaints at other workplaces, pressured hotels to desegregate, boycotted stores that wouldn’t hire Blacks and convinced the local newspaper to stop identifying race in crime articles only when the suspect was Black.

Jimmie Porter, a locally heralded civil right activist, was central to the union’s integration efforts. A native of Mississippi, he observed that while the racism in the North wasn’t as blatant, it also wasn’t too different from what he’d left. “I pretty well knew where I stood in Mississippi, and here, I had to be told and reminded,” he said in an oral history interview. “They had conditioned most of the Blacks who lived here to never look at how well they should be doing compared to whites who they had gone to school with, but to measure themselves by their country cousin.”

Anna Mae Weems becomes one of the first Black women to integrate Rath’s sliced bacon department, a bastion of white women working in a pristine environment. Born in Waterloo, Weems couldn’t understand why, after graduating from high school, she couldn’t get the jobs that her white classmates were getting. The union recruited her to further challenge the race and gender barrier at Rath. She soon became the shop steward for the bacon line.

It had been a long fight to get there. Black workers had often been assigned to the dirtiest jobs in the packinghouse. Black women were overrepresented in hog casings departments, where they “flushed worms and feces from the animal’s intestines,” one historian wrote. Meanwhile, Black men were frequently assigned to the kill floor, though the position had unexpected advantages. Whenever there was a dispute, the workers could stop the line, threatening to let the hog carcasses rot until the company resolved their grievance.

1956            

Rath’s employment peaks at nearly 9,000 workers. Thanks to the jobs at the packinghouse and at other factories, thousands of Black people moved to Waterloo from the South during the Great Migration. As Rath became an increasingly popular brand, the union ensured that the workers’ economic fortunes rose with it. By the mid-1960s, wages were the equivalent of $24 to $32 an hour in today’s dollars, helping create a Black middle class.

1967            

An upstart company, Iowa Beef Packers, introduces a product known as “boxed beef,” transforming the meatpacking industry. Instead of sending sides of beef to butcher shops, IBP workers stood side-by-side, each making a specific cut to disassemble a carcass moving down a conveyor. “We’ve tried to take the skill out of every step,” IBP’s president had told Newsweek in 1965. The new process sped up production and allowed the company to move its plants from cities into rural areas where livestock was plentiful and unions were scarce. Most large meatpackers would follow suit.

1968

The UPWA merges with the more conservative Amalgamated Meat Cutters as corporate power grows in the changing meat industry.

1979            

The meatpacking union joins an organization of retail and grocery clerks to form the United Food and Commercial Workers. Some meatpacking workers found themselves battling with their union as much as their employers. At some plants, members of old UPWA locals tried to push back against wage cuts, but the UFCW leaders sided with the meatpackers. “It was like a shot of whiskey. When we was the UPWA, we was little but powerful,” a union leader told oral historians. “Then we joined the Amalgamated and we got like a mixed drink. Now it looks to me like we’re a shot in a quart of Squirt.”

1985                

After years of financial trouble, Rath shuts its doors, contributing to an economic tailspin in Waterloo that deeply affects the Black community. Simultaneously, the 1980s farm crisis had taken a toll on Waterloo’s other big employer, John Deere, which laid off thousands. As the last ones in, Black workers were now the first to go, erasing hard-fought economic gains.

The civil rights movement had spurred the desegregation of Waterloo’s schools, but as in other cities, it prompted white flight. Without good-paying jobs, many middle-class Black families also left for opportunities elsewhere. Those who stayed faced bleak prospects. “You could have a master’s degree and be in Waterloo, and if you were Black, it was hard for you to find a job,” said the Rev. Belinda Creighton-Smith, senior pastor of Faith Temple American Baptist Church.

1988            

IBP announces its plan to build the world’s largest hog-slaughtering plant in Waterloo, promising 1,500 jobs for the struggling city. Many hoped it would provide work for hundreds of laid-off Rath employees, but some leaders had their doubts. The company had a reputation for mistreating workers and had been fined by the Labor Department for failing to report injuries. Willie Mae Wright, the only Black city council member at the time, was among those skeptical of IBP. But after meeting with community members, she said in an interview, she “went along with it knowing that people didn’t have jobs.” City officials approved the IBP plant.

1990            

IBP’s slaughterhouse opens to much excitement in Waterloo. But many of IBP’s initial hires don’t stay on the job for long. Some told community leaders they were overwhelmed by the speed of the processing lines, which left their hands numb. After several years, few in the local workforce wanted to work there.

1996

IBP looks elsewhere for workers. It recruits homeless people from shelters and under highway overpasses. It hires labor agencies to find workers from the U.S.-Mexico border, and appeals to California farmworkers who want out of the hot fields and a lower cost of living.

IBP also runs a recruiting operation in Mexico, buying ads on local radio stations and turning pharmacies, stores and car washes into application centers. The company eventually charters buses to transport workers directly from Mexico to its plants. While IBP insisted the workers were authorized, dozens were detained in two immigration raids on the Waterloo plant....

2018            

A financial news site, 24/7 Wall St., ranks the Waterloo-Cedar Falls metro area the worst place for Black people in America. The Black unemployment rate is nearly five times higher than for whites, and Black residents own homes at less than half the rate of white residents, the report notes. Despite the economic gains that meatpacking jobs had provided a generation earlier, Waterloo remains largely segregated, with a historically Black neighborhood bounded by railroad tracks on three sides. And many in the Black community haven’t fully recovered from the 1980s economic downturn.

2020                

An outbreak at the Tyson plant makes Waterloo one of the country’s biggest COVID-19 hotspots. The disease disproportionately affects the city’s immigrants, refugees and communities of color — a demographic heavily employed by Tyson. “This is their first attempt to get a slice of this American apple pie and then for it to be so bitter for them is a travesty,” said state Rep. Ras Smith, who represents the city’s east side. “I don’t want Tyson to overshadow what Waterloo is.”

Jordan’s King Is His Own Worst Enemy (Foreign Policy)

There’s much more evidence of the monarch’s poor governance than a foreign conspiracy against him.

A century ago, Sharif Hussein bin Ali had big dreams for his Hashemite dynasty when he was king of the Hejaz and emir of Mecca and Medina, Islam’s holiest sites. But ever since the time of Lawrence of Arabia, when the Hashemites were Britain’s main regional allies during World War I and led the Arab revolt against the Ottoman Empire, the dynasty has been in steady decline. And with the ongoing dispute among Hussein’s descendants in Jordan, the family may have reached a new low.

The Hashemite dynasty has faced myriad challenges over all those decades, both externally and internally. Brothers in the line of succession have often been dumped for sons, but never did the family wash its dirty linen in public—until this month, when an internal rift became public gossip.

On April 3, Jordan announced that it had foiled a conspiracy to unseat its monarch and destabilize the country. Foreign entities, top officials claimed, were colluding with Prince Hamzah to topple King Abdullah II. Two weeks later, the palace still has not shared a shred of evidence, and it’s becoming increasingly clear that the tale doesn’t add up.

More likely is that we are watching the oldest story in the world: a succession battle playing out between royal siblings. Jordan’s monarch placed his half-brother and former crown prince under house arrest to remove the challenge to his throne, along with 18 alleged co-conspirators. But rather than a seditious prince, the whole episode has revealed the authoritarian streak of an insecure king.

Jordan’s tribes have historically owed allegiance to the Hashemites in part due to their religious lineage as descendants of the Prophet Mohammad, who, too, hailed from the House of Hashim. Their support is essential for the dynasty, but they increasingly feel marginalized and disaffected. The United States, which give billions of dollars in aid to the country, have officially backed the king in the feud. But they have been forced to take note of mounting repression in Jordan under Abdullah’s leadership.

Abdullah sold himself to the West as a Harley-Davidson-driving, laundry-washing, pro-democracy monarch, but he has in fact consolidated power inside the palace, gagged the press, arrested protesters, and dragged his feet on devolving actual power to the legislature. The Hashemites, who were once seen as the more modern monarchs, the most Westernized, are coming to be seen as rulers of just another authoritarian Arab state.

According to Reporters Without Borders, Jordan ranks 128th out of 180 nations—below Afghanistan—in press freedom. Freedom House, a U.S.-based nonprofit that conducts research and advocacy on democracy, political freedom, and human rights, demoted Jordan’s status from “partly free” to “not free” in the last year. Abdullah’s Jordan is not Syria or even Saudi Arabia—yet—but those who disagree with the state run the risk of a knock on the door from the intelligence services.

No one believes Abdullah intends on meaningful political reforms, and his economic reforms have produced more allegations of corruption than positive economic results. He unleashed austerity measures to procure loans from the international community and went on a privatization drive that some international observers applauded. But these measures came at the cost of losing support from the kingdom’s tribes.

Tariq Tell, a professor of political studies at the American University of Beirut and an expert on Jordanian politics, noted that the nationalist tribes had been critical of the neoliberal economic reforms that had come to dominate policymaking under the king. “The networks of East Bank tribes have been eroding since the privatization drive,” he said. “Their children are not getting the same jobs and benefits.” As their share of the pie, state jobs, and benefits shrank and discontent set in, Hamzah saw an opportunity to curry favor with this traditional support base. He began reaching out to tribal figures, making appearances at weddings and funerals.

Little is known about the prince’s economic and political ideology and how it compares with his brother’s approach to governance. Hamzah has voiced the concern of the masses but so far has not offered any solutions on how he intends to save a country devoid of resources and flooded with refugees. His biggest asset seems to be his looks, as he bears a close resemblance to his father, the long-ruling and fondly remembered King Hussein bin Talal. Nevertheless, his popularity has nonetheless risen since his arrest.

He is ambitious and was reportedly preferred by Hussein as a successor over his elder brother, a choice that however proved too difficult to reconcile with Jordan’s constitution. His consolation position as crown prince, next in line to the throne, was removed by Abdullah and passed to his own son in 2004. That must have hurt, but it still does not prove that he was plotting a coup against the king.

According to Tell, no one believed a coup was in the works. “Information coming out of the palace is very contradictory,” he said. “The latest events seem connected to a dispute over succession that has been going on since the removal of Hamzah as crown prince. It seems the king wanted to end it.” Adnan Hayajneh, a professor of international affairs at Jordan’s Hashemite University, said the palace’s claims have left him befuddled. “From a political science perspective, I can’t make sense of how foreign powers were involved,” he said. “The implication that Israel must be involved does not make sense because they have good ties with Jordan. Why would they want to destabilize Jordan? And even though the Saudis and Emiratis have sidelined Jordan lately, they also don’t want to destabilize the country.”

Among those arrested for allegedly plotting the coup, just two were connected to Saudi Arabia. But experts say these men are not linked in any way to the prince. Bessma Momani, a professor of political science at the University of Waterloo and a senior fellow at the Ontario-based Centre for International Governance Innovation, said the arrest of Bassem Awadallah, a Jordanian-Saudi dual national and advisor to Saudi Arabia’s crown prince, Mohammed bin Salman, was tactical. “The tribes despise Awadallah and see him as synonymous with corruption and elitism,” Momani said. “But he has no link to Hamzah. Awadallah’s arrest was a distraction.”

The palace’s insinuation is that Israel and Saudi Arabia want Jordan to become an alternative homeland for Palestinians currently residing in the West Bank as part of a broader deal that replaces the Hashemites as the custodians of Al-Aqsa Mosque in Jerusalem with the Al Saud family. Since Abdullah won’t play ball, they want Hamzah to launch a coup by way of a popular uprising. But analysts disagree and call it conjecture.

“The idea has been floated periodically over the past half a century or so without ever being taken that seriously, certainly not by Arab governments,” said Tobias Borck, an associate fellow at the London-based Royal United Services Institute. “It is often suggested that Saudi Arabia or the UAE now actually see this as a feasible policy option. I do not believe that. I have never heard a Saudi or Emirati policymaker seriously argue for it.”

At the heart of the king’s insecurities is the protest movement locally described as Hirak. In 2011, as the Arab Spring engulfed the region, Islamists of the Muslim Brotherhood and members of Jordan’s tribes took to the streets.
