I cannot believe I didn't post this one here! Drawn last year, the biggest page of my fish anatomy and histology atlas.
Technically, transverse sections are more useful for looking at symmetry, so you can see if something is bilateral, but sagittal sections are just so cute! I love seeing a whole fish on a slide.

The vet called me a little while ago with the histopathology results. IT WASN’T CANCER! I was so scared that the mass was malignant and am so grateful that it wasn’t. That’s the good news. The bad news? Still no definitive answer. The doctor is going to continue looking for answers, see if she can find any similar cases.
The report is under the cut for anyone interested in reading it.
Chloe has an appointment tomorrow morning at 11 to have her sutures removed, and her blood count, temperature, and weight rechecked.
History:
Proximal medial aspect of right hind limb. Fever of unknown origin for past year, mild stomatitis, proactively treated with azithromycin in case of Bartonella. Borderline anemic. Mass rapidly appeared over the weekend with a flattened plaque on the dorsal aspect. Purulent discharge was noted today.

Received: A 4.5 cm x 3.0 cm skin biopsy, with a 3.5 cm x 3.3 cm ulcerated mass.
HISTOPATH REPORT:
MICROSCOPIC FINDINGS:
Haired skin: Ulcerative dermatitis and cellulitis, necrotizing, pyogranulomatous and lymphoplasmacytic, chronic, locally extensive, marked
COMMENTS:
Histologic findings revealed severe ongoing inflammation and granulation tissue formation. No neoplastic populations are observed. Areas of inflammation extend to the deep specimen margin. The extensive degree of necrosis present is suggestive of an infarction. The underlying cause is not evident in this sample. Possible causes include a venomous bite, previous trauma, infection, or a chemical/thermal burn that caused vasculitis, leading to subsequent ischemic necrosis. Special stains are pending to further rule out fungal and acid-fast agents. Results will be forwarded in an addendum. Aerobic culture and sensitivity on fresh tissue may be indicated to further rule out an antibiotic-resistant pyoderma.
MICROSCOPIC DESCRIPTION:
Haired skin and subcutis: An extensive area of the epidermis and dermis has undergone coagulative necrosis, admixed with abundant eosinophilic necrotic coagulum, layers of poorly preserved neutrophils, edema, and streaming nuclear debris. Surrounding areas of necrosis are coalescing aggregates of numerous macrophages, lymphocytes, plasma cells, and fewer neutrophils, admixed with granulation tissue. In less affected areas, anagen hair follicles and sebaceous glands are intact.

GMS and Fite-Faraco stains did not reveal the presence of fungal or acid-fast bacterial etiologic agents.

buhay pa me hehe its been 10 days since my surgery the first few days were rough masakit everywhere pre tsaka sobrang emotional ko naluluha lang ako every time surprisingly naman mabilis yung recovery ko wow although hanggang ngayon hoarse pa rin yung voice ko hindi ako makakanta nang ayos hays ang hirap tuloy magconcert sa banyo kapag naliligo ako :( charot hahaha andddd hindi me naglalabas labas kasi nabbobother ako ayokong pinagtitinginan yung tahi.....dkdjsisjwjsjw
ayun hinihintay ko pa yung histopath results para isabay ko na sa follow up check up ko wooo hope everything goes well

Bartonella henselae
Case Report:
23M veterinary student from home with a cat, presents with a one year history of neck swelling, 1 month of fevers and lethargy. Also he lives in Karachi.

The fevers are a/w chills and rigours that response to paracetamol and have no particular pattern to them.
He doesnt have any arthralgias or arthritis or rash.
They find pretty sizable lymph nodes on exam in the cervical chain and inguinal regions.
He starts developing hypotension and they start him on some vasopressors and meropenem.
The fevers don't respond, and someone decides to start azithromycin, which it does respond to.
He's also worked up for IE given the chronicity of events.
Eventually on biopsy of the lymph node, the lab finds bartonella hensalae.
Microbiology + transmission:
aka cat scratch fever, so this is why history is so important to infectious diseases physicians.
it's actually disease of cats that can spread to their humans via bites and scratches. Or the cat licks a wound etc.
and unfortunately also via arthropod vectors and mossies
after entry into a host, it's main target cells are CD34s, immune cells, and then alters the host immune system
it's interesting special power is that it can predispose hosts to other pathogens in this manner

Historical trivia
the fun bits
genus of Bartonella is named for a Peruvian scientist (Alberto Barton, also had interest in brucellosis and leishmaniasis --> other tropical diseases taht cause fevers of unclear origin), he isolated the bacteria from patients during an outbreak among railway works in South america. This was 1905.
reminds me of love in the time of cholera, when people still wrote physical letters.

species of bartonella henselae is actually named for Diane Henselae, a researcher from Oklahoma, who collected samples during an outbreak there in the mid 1980s.
rare for someone who discovered a species named for them to be both alive and a woman, and I can't find much about her online.
there are other bartonella species that cause historically significant diseases like trench foot (bartonella quintana, transmitted by lice) and carrion's disease (bartonella bacilliformis, with a high mortality rate). another post for another day.
Clinical features
incubation period: up to 10 days
initial: rash at site of injection or intro of pathogen, from there it travels to local lymph nodes causing lymphadenopathy about 1-3 weeks later

Bsymptoms - low grade fevers/malaise/fatigue
epidemio: occurs most often (for a rare disease) in kids < 15
Complications
infection affecting the eye (neuro-retinitis = visual changes, irritation and photophobia), liver, spleen, brain/spine (transverse myelitis, encephalitis etc), bones and heart valves (IE)
Increased risk groups for complications
HIV/AIDs, immunocompromised hosts (i.e. transplants), mortality is an issue in this group, as disseminated disease a possibility
small risk of IE in groups with RFs for develop this (prosthetic valves, damaged valves, unrepaired congential heart disease etc)
Investigations
challenging, no gold standard test as the sens and spec is variable for each individual one, so it's a combination really, of history exam and a variety of confirmatory tests
PCR (variable spec/sens), serology (indirect and often negative in early stages, can also be positive for years post treatment and doesn't differentiate from other bartonella species), cultures are definitive but it can take 21 days for anything to grow (it's fastidious)
histopath of lymph nodes helpful - stains: silver stain or Warthin Starry stain (silver nitrate) which is kind of a reverse starry starry night used for spirochetes like helicobacter.
in IE, it can be culture negative, which can confuse diagnosis

Management
mild disease is self resolving
first line: few days azithromycin or doxycycline can reduce symptoms and is indicated for enlarged lymph nodes to reduce size or unresolving LAD >1 month
in case of eye infection or IE expect longer duration of hterapy, like doxy and rif for 4-6 weeks
will also respond to bactrim, cipro, rif and gent
Prevention from the CDC:
avoid strays, wash hands after petting cats, keep strays from your cat, avoid owning new kittens who are < 1 yr if you are immunocompromised
avoid getting scratched..which is kinda hilarious but can see why

Hi, do you know of any good charts (or could you at least point me in the right direction to find them) with common IHC and stains and what they stain for in veterinary histopath? I’ve started to attend pathology journal club at my school, and I think I would get a lot more out of them if I had any idea about these stains since so many articles focus on them. I’d like to start getting familiar with them! I tried to ask one of the residents for one but all they told me was that these charts do exist, did not offer to share any. Thank you in advance!

I'm not really aware of any charts, per se, but if you can get your hands on a copy of the textbook "Tumors in Domestic Animals" (Meuten, picture below) there's a whole chapter just on IHC. This includes lists of common IHC for diagnosis and prognosis, including a table of what species they work in and some information about the use and localization of each. Then you just need to look at what particular IHC is being used. For example, there's a huge number of pancytokeratin options. AE1/AE3 is common, but it's not what everyone uses, and different cell types will express different cytokeratins.
If you can't find something in there, the internet can probably fill the gaps. Pathology outlines is for human medicine but is a nice quick resource for the basics of IHC and special histologic stains. Hope this helps!

Exactly the date of the release of the MTLE result. We're expecting it to be later this afternoon or evening.
A day or 2 before the exam
I wasnt able to make an update duringg these days because we already went to the place where we stayed annnd I crammed alot of notes + closed door exams. It was mind wrecking. Last minute I was studying.
Day 1
After finishing the first subjec, I already felt that it's over. I cried when I saw Kath sa hallway. But I had no choice but to continue. Luckily, CM was a bit easy. Pang palubag loob kung baga.
Day 2
I find ISBB and Hema quite alright. Histopath, wow, save the best for last talaga. As someone who didn't study HTMLB, in short, alay, grabe ang daming histopath na tanong, about 90% ay histopath then the rest ay totoo ngang very out of medtech na.
BE aftermath
I honestly felt scared, and fear dominated over faith. My heart wanted to scream. My eyes felt like waterfall. I was able to sleep naman after since we drank jacoke right after day 2.
Waiting Game
Eto yung probably mas mahirap pa kaysa sa BE. A lot of pipol were rationalizing, twitter, messenger, facebook, reddit, discord. Gosh, it's everywhere!! But yknow what? I cant stop looking at those things. The more I read and look, the more I realize the chance of me passing is very slim. Heck, i cant even remember the questions and even if I did, I cant recall my answer!
As a distraction, I just cleaned our house, bathed my dog, and tried to get involved in family business but at the end of the day, the thought of BE result kept lingering. I just prayed on it. Pray and pray and pray.
Release of Result
Grabeee yung pag release ng result, I was asleep. It was around 1:30 am of March 27, 2024. Jawie was the first one to congratulate me. And I quickly jumped into my laptop to see my name. And viola, page 60, number 2916. There, my name is on the list of passers! Wala sa topnotchers, haha chariz.
Tbh, I didnt know what to feel cause some of my friends, my dear ones, weren't on the list. I cant even post my name sa messenger nor sa facebook because I dont wanna make them feel behind, although ang dami na nakapag post sa batchmates ko. Still, I dont wanna be the one to cause them to feel that way. Hanggang ngayon di ko alam kung ano mararamdaman ko. Heaven knows how thankful I am, I really am. St. Joseph of Cupertino, St. Jude of Thaddeus interceeded for me. I cant thank them enough for allowing me to pass the BE.
Pero yung thought na hindi nakapasa iba kong kaibigan, really saddened me. Jinkx, Tevs, Tofu, and Ally, may you guys keep on going. Keep on striving. You may be saddened by today's result, but that should not be the reason for you to stop pursuing. I believe in you guys. Kayang kaya niyo! I will always be right here for you. And when the time is right, the Lord will it happen.
At this moment, mas nangingibabaw ang lungkot kaysa saya. But nonetheless, I owe it all to God, who never ceases to fail me. Who gave me all the strength from the start until the very end of the waiting game.

on this day last year:
joycee and i had a birthday breakfast at mcdo in laguna near turbina before heading back to manila.
went straight from turbina to tondo medical center because i was on duty at the histopath section
didn't enjoy my day because histopath was such a boring section (lol)
on this day:
date with hb at dca and went grocery shopping at sm hypermarket
quick trip to the dentist because of my fucking wisdom tooth (wasn't able to enjoy my food because of this) lol
scheduled for a surgery on friday
still happy i get to celebrate with my fam
this day's better than last year :D

I told my mom I'm not in the mood to eat. Palagi naman. Hahaha. Gumawa pa ng paraan. She handed me soju and fries, although hindi ko man lang natikman o nakain, kasi wala talaga akong gana, apple lang nakain ko and banana the whole day. Grabe sobrang frustrating and truly draining for me 'tong araw na 'to. Sobrang lumalabo na mata ko and sobrang sakit na ng ulo ko due to this setup, magkaka scoliosis na ata ako sa pag-aaral, I have this bad posture din kasi. I watched lecture videos of those two subjects that I'm gonna take an exam tomorrow, all in all? 5 videos! It was more than 5, pero I think kaya ko na yung iba on my own, recap nalang naman, since naquiz naman na namin, inulit ko lang talaga. Sana makasagot talaga ako bukas. My ears are ringing and hindi pa ako nakakapikit man lang. Sobrang pagod. I thought today would be my rest day since bukas pa ang exam ko, but I was wrong. I literally woke up to study!
I am still in shock about my exams that happened yesterday na hanggang ngayon wala pa ding scores, sobrang hirap. Kahapon pa lang ubos na utak ko. Tapos sa halip na magpahinga lang ako today, I watched the lecture vids, hopefully na may na-retain sa utak ko. Geez, ang interesting naman ng pinanood ko, kasi may autopsy and biopsy na kasama which is pretty cool for me hahaha, kita mo internal organs and how they make the incision, plus the different terminology in time and diagnosis ng mga sakit, ultimo pati pala pagkamatay, nalalaman kung anong oras posibleng namatay? Interesting, indeed! Baka kaya din ako walang gana pagkain, dahil sa mga nakita ko hahaha. Ang ganda ng forensic, pero insights lang ang inaaral namin, since we're more focus and specific about how cell dies and their mechanism, particularly sa tissues, its either physiological or pathological. Kumbaga, inalam lang namin yung terms and definitions. Hahaha. Change of heart for forensic, Steph? Think about it!!!!!
Plus, BB, hahahaha, I love immunology and serology, pero ang hirap intindihin paminsan minsan ng blood banking, sa sobrang dami. Sa tingin ko naman makakasagot ako, intro palang naman, pero nakakalito talaga, hahahaha. Kaya ko naman 'to. Kakayanin! I have to and I need to!!!!!

My exams for tomorrow. Kaya ko? Kaya. Everythings falling in to place, sana magtuloy-tuloy na. Ayos na din kami ng bestfriend ko, kahit hindi pa completely, we'll get there.
