These scenes are both anime original, but why do you think SP added these scenes to signify SNS are not brotherly? In the manga the only time Naruto talks about the brother thing near the end is in 694 when he says he and Sasuke aren’t real brothers but thinks they can fix things between them. (Regarding this I saw some antis saying that Naruto is just stating that he and Sasuke aren’t blood related.)
But do you think SP added these scenes because they were informed by the editors team that SNS are not actually meant to be brotherly? Or do you think they just wanted to randomly add them in? I kind of hope it’s not the latter since I like SNS but I’m not really sure and I’m interested in your take on it.
tbh idrk either. the anime team does pretty much what they want don't they? I mean just look at imp episodes (vote2) the anime added a bunch of stuff that totally missed the point of the whole battle. at least naruto saying 'they're not brothers' is canon. I think they just needed to stretch the episodes out tbh. is it possible that Kishimoto/editors asked for that to be put in? possibly... possibly not.
I guess the vote2 scene was included to show how they both reject the label brother cuz of the upcoming 'what just friendship mean to you' scene. as for the boruto one, Kurama asking naruto if he and sasuke are brothers... that's kind of off isn't it? considering since Kurama's been through it all... idk. then naruto calls itachi a 'wonderful brother' what??? that says it all right there. it's anime filler, they do what they want.

Why Naruto in Boruto Episode-220 is OOC???
I recently answered some asks about SNS being OOC in recent Boruto episode. And some of my SNS friends here commented that it’s not. I should have arranged and inserted all the panels for that post and explained my view point in a better way. Again, this post’s intention is not to prove that whoever didn’t agree with me are wrong.... It’s quite the opposite. 
I just don’t want us all to become some hypocrite or make Sasuke’s character to be assassinated in the worst way possible just because of that single episode and that stupid Extra Scene. Because, that episode totally shat on Sasuke’s character development which started from Shippuden to Gaiden to the latest episode. And it also shat on all the development about Naruto in the worst way possible.
Let me explain why. 
So... the scene starts like this

Sasuke is saying, “Even though I couldn’t able to kill him back then.... I intend to keep up my end of the bargain, this time”....
Sasuke is being very practical here... Because he has his priorities. And you should know what his priorities are!!! If you don’t, then I will let you know later in this post.

Right after that Naruto gets pissed off and do this.... (Which still looks Extremely OOC)... But let’s not get in to it.... By Naruto saying this, it makes Sasuke looks insensitive. It totally gives off the vibe that Naruto is the parent here and Sasuke couldn’t understand it. We can totally wear a Thick SNS spectacles and say it’s not and claim that they are arguing like 2 parents here... But it’s not really... It totally places Sasuke away from Naruto and Boruto...
Because right after this.

Now.... All these dialogues were not in the Manga at all... Here Naruto is saying “But while I’m still alive, I won’t allow that to happen (to kill him)..... NO MATTER WHAT ANYONE SAYS....”.... Anyone means Sasuke included as well..... 
If I see this scene as a separate entity... Then Naruto is acting like a Good Father and providing a Warmth which is missing from the Manga. I agree... 
But from the facts that was already established in the Manga and Gaiden, This is absolutely CRINGE AND ANTI-SNS.... Because here Naruto is putting Boruto above Sasuke.... Well, if this was the same dynamics they established in the Manga.... It won’t be a problem at all.... But it’s totally opposite...... 
Let me tell you why...
Ikemoto is not some great Manga Artist or anything... That Manga is totally a shit. I agree. And I’ve always said that I totally take any extra scene they give us in the Canon Episodes... And those extra scenes should help elevate a bond between two characters. Not to downgrade another bond. That extra scene with Kurama in previous episode was THE BEST and WELL DONE FROM ANIME’S PART.... It gave a good farewell for our beloved Kurama. But this above extra scene totally shat on SNS, Sasuke and ESPECIALLY KAWAKI. 
How these extra Scenes were OOC when it comes to the bond between NARUTO AND KAWAKI??? 
I’ve mentioned this in many posts.... Naruto likes/prefers Kawaki than Boruto. It doesn’t mean that he don’t like his Son or anything... It’s just a matter of preference. But Naruto’s preference for Kawaki is almost equivalent to his preference for Sasuke in Shippuden/Part 1.

Kawaki block the attack and saves Naruto and Hima. And from here on now... You will see a Naruto whom you already saw somewhere else in Naruto series....

Now this Delta girl gets angry because Kawaki is some precious Vessel that ‘Kara’ Organization constructed using their experiments.

Now.... Right After Delta mentions that Kawaki is their, ‘Vessel’.... Naruto’s inner demon awakens...

Just like How Naruto awakened Kurama Chakra for Sasuke in Land of the Waves.... Naruto gets extremely angry..... With similar ANGRY EXPRESSION....

As well as Delta....... “This Guy.... is being overcome with Anger over Kawaki...?”
BOTTOMLINE: Naruto got EXTREMELY angry because this girl treated Kawaki like a Vessel not primarily because she tried to hurt his kids..... And they stole our Land of the Waves dynamics and gave it to Naruto and Kawaki.... After this Delta died. 
And this is not the only time....
When Jigen came to Konoha to take back Kawaki... Naruto was hellbent on protecting him... Even though Jigen is extremely strong... Kawaki himself was asking Naruto to not butt in!!!

“No can do!!! It’s the Hokage’s Job to protect everyone in the Village.... Without Exception... And that includes You, Kawaki....”
This is the second time someone came in search of Kawaki to Konoha. And this time also Naruto never planned on giving Kawaki to Jigen.

This is the third time, someone came to Konoha to take back Kawaki... This time it’s Isshiki. This Amado guy was suggesting to kill Kawaki whereas Naruto never budges. He simply says with an Anger ridden face, “I ain’t gonna allow that!!”
And after Kurama’s death... that is after Naruto got Nerfed....

Kawaki himself was feeling so bad that he was some troublesome outsider... Because everyone was looking for him like Delta, Isshiki, Jigen.... But Naruto refused to offer him up.... KAWAKI HERE IS SULKING LIKE A KID......

But Naruto just grabbed him like some Laundry Bag and walked up carrying him like a sack of potatoes saying, “Who cares...?? We’re going home!!”.... Why is Naruto saying the same thing what he told to Sasuke in VoTE1????

And calling him as his, “A Stupid Son” no less.... Let me tell you.... Naruto never did such a gesture to his own son in this Manga. He never hugged him... Whereas Naruto already hugged Kawaki.

Now Naruto is simply assuring that Kawaki’s place is in Konoha. [[Why am I getting SNS vibes here??]].... Naruto is giving a Pillow Talk to Kawaki for like 3 pages in this chapter.

The point I am trying to make here is that Naruto & Kawaki’s bond stole many SNS tropes... Which I don’t mind at all... Rather, I am very happy for that. 
Now... All of a sudden, Naruto acting up like a proper emotional lovey dovey father for Boruto is just OOC... At the expense of pulling Sasuke’s Collars and putting down Sasuke in the very next scene.... 
And do you know what’s even more OOC???

I’ll show you another Biggest Hypocrisy we are unconsciously doing here as SNS fans.... 
How these extra Scenes were OOC when it comes to SNS and SASUKE ESPECIALLY???
I am one of the very few person in this SNS fandom to support Sasuke’s decision to accept Konoha as his Home and I loved this development... Even though Majority of this Fandom feels it’s OOC.... I accept this because Kishimoto wrote it that way and to my mind it also feels right... And this is Canon.

Sasuke lost his most beloved person, Itachi for the sake of this Village. And Sasuke accepted his Ideals and decided to protect this Village. He lost many things here and yet Sasuke made the right decision to see the Bigger Picture... 
From Itachi and Minato’s lives, we all might have observed one thing... That is, They put the Village above their own Blood. Minato made Naruto into a Jinchurikki to save the village.... Itachi killed his own parents to save the Village.... 
So, this place carries lot of importance for the characters in this world....
SASUKE’S PRIORITY IS NARUTO AND KONOHA.... 
Few Months ago... I answered an ask... And the Question is, Who do you think sasuke love the most between naruto and sarada ?

My answer to that response was Naruto and I provided my reason considering all the facts from Boruto Manga.
I will copy paste the most important part here...
There is an undeniable fact that Sasuke spends more time with Boruto than he spends with Sarada. He is his Master and they have pretty good bonding.
But even towards Boruto, Sasuke has his priorities. Since Boruto was given a karma seal, he might turn into an Otsutsuki monster anytime. And Boruto was worried about this, to which Sasuke said this.“Don’t worry. I swear to stop you with all my strength, Even if I have to kill you”…
Well, this is getting real complicated now!!!! You may wonder, ‘How dare Sasuke can kill Naruto’s son? If Naruto knew about this, he will not forgive Sasuke. He will even kill Sasuke’…. Atleast this was how fans reacted for that chapter. But Naruto proved them all wrong.
Gaara asking, “Would you be able to take down your own son? “
Naruto says, “I’ll do what needs to be done as Hokage”
And this Amado also tells Naruto, “I spoke to Sasuke earlier, but It did seem like he was prepared to if necessary”
So, Naruto will kill Boruto if he threatens the safety of the village someday. And Naruto also knew about Sasuke’s decision regarding this.
You may now ask, ‘Hey how does any of this have to do with Sarada?”
Well, if Sasuke can able to kill his best friend’s Son, can’t he be able to kill Sarada too, if necessary (I mean if she poses any threat to the village someday)???? And I am 100% sure, Sasuke will not play father card if it comes to that.
So, for Sasuke, both Boruto and Sarada are equal. He almost treats Boruto as his own son. So, there is that.
Am pretty sure.... Sasuke would not hesitate to kill Sarada if it is necessary.... Because he is just like Itachi. He even tried to kill his own Daughter (mistakenly) in Gaiden for which he didn’t show a big regret or something... 
Now, Naruto playing a Caring Father Card for Boruto (Which is not even in the Manga)..... shat on Sasuke’s ideal to accept Konoha as his home... NARUTO COMES OFF AS A BIGGEST HYPOCRITE TO PROTECT HIS BLOOD FAMILY WHILE SASUKE LOST HIS ENTIRE BLOOD FAMILY FOR KONOHA..... Am not saying Naruto shouldn’t care for Boruto.... But not at the expense of portraying Sasuke as this emotionless Moron just because he put the safety of the Village above Boruto’s life.... Sasuke looks like a fucking Clown here!!! Whereas Naruto was changed into a good Father!!! Shit!!! That one extra scene changes the entire context of the story... 
God!!! This is too bad.... In every angle!!!!

This dude abandoned his Family for a decade and still distancing Sarada at the end of Gaiden. Because Forehead poke means distancing, lying and bla bla.... 
Whereas he was too close with Boruto and almost acts like a GOOD father in Boruto movie. I won’t be saying he doesn’t care about Sarada or anything. I am just saying that, he always gives importance to his priorities.... And his priority is Naruto and Konoha..... Not Sarada. I don’t even have to provide any proof here.

Whereas all of a sudden, Naruto started to go “My Dear Son” mode for a son he had with Hinata.... and this even projects Sasuke as a Bad Family man who abandoned his Daughter for a decade.... and still putting a distance between him and her...And we all are pretty okay with it.... 
Why should Sasuke’s role revolve around Naruto and his son when Naruto himself putting Sasuke down here??? “No matter what anyone says”... Here Naruto puts Sasuke along with the level of Gaara and other Kages.... This is totally an Anti-SNS.
And this scene is not even in the Manga.
NARUTO PUTTING DOWN SASUKE & KONOHA FOR THE SAKE OF BORUTO IS ABSOLUTELY OOC..... IT BREAKS EVERYTHING THAT WAS ESTABLISHED IN SHIPPUDEN, GAIDEN AND BORUTO MANGA & MOVIE....
AND WHAT’S EVEN WORSE IS AFTER THAT SCENE, BORUTO GOT A FUCKING MAGICAL PILL WHICH GAVE HIM A TEMPORARY CURE.... 
THEN WHAT’S THE PURPOSE OF THESE EXTRA SCENES ANYWAY???
AND SHIKAMARU SOMEHOW WAS SHOWN AS THIS UNDERSTANDING GUY...
Anyways... 
To Conclude....
This whole Manga is going in a path where Naruto has a special place for Kawaki.... And Sasuke has a special place for Boruto... While both N & S keep each other as their priorities even in this shitty Emotionless Manga. In Short, Boruto and Kawaki are treated as an unofficial children of SNS... Which is very evident from the panels I’ve attached above.
When Sasuke was willing to put the Village’s safety as the priority to the point of doing long missions of 12 years and even went to the extent of not meeting his own Daughter, not able to recognize his own daughter and even went to the point of killing her....  But Naruto acting like a good caring father shows that he is an hypocrite who expects others to lose their family time for this village’s safety but Naruto is somewhat not ready to lose his family member for the sake of the village... Such an hypocrite!!! [[It hurts my SNS heart to write this]]... AN HYPOCRITE FOR A SCENE WHICH WAS NOT IN THE MANGA... 
I know it hurts every Father to take down their own child.... His pain could’ve been handled in a million different ways... I would’ve preferred to see Sasuke as the one to ease Naruto’s pain and make him come to the conclusion in a more organic and Kishimoto’s trademark sentimental way than this Abomination.
Naruto putting down Sasuke along with Gaara is just .... just.... just.... intolerable!!!! 
That scene could have been easily avoided or replaced with some other character... 
I am saying this again for the 1000th time, Naruto can care for Boruto all he wants.... He can even cry and have nightmares about his own son turning into some Otsutsuki Monster which shows how Naruto is struggling to come to a decision... Which are all fine... I don’t mind seeing the bond between Naruto and Boruto... But not at the expense of putting down Sasuke and treating him like a Clown.... 
And I am saying this for the millionth time.... Naruto and Sasuke can have arguments all they want like an old couple... Hell!!! I would pay any money for that. I want them to be like this...

Hello! YES Sasuke is a total mum in...almost every sense in and for Boruto. He scolds the kid, he teaches the kid, he takes him to the battlefield without asking permission to his parents, he cares about him on the battlefield, he sacrificed himself for the kid in the battlefield (this in the anime canon episodes ) and said he was like his father , he saw tiny Naruto picture in Boruto, and...i could go on
But the most important is that only he, NOT HIS MUM stupid Byakugan princess Hinata, could make Boruto reflex about his bond wit his father. Sasuke was able to make Boruto introspect on how much naruto fought for being the man he is then, and told to Boruto how he trained, his past and the person he was when they were genins. Something hinata couldnt do bc if that were the case then all this drama about father-son thing wouldnt have happened.
Because the only thing she did was what? OH yeah STALKING. "And then Naruto talking about Boruto as if they’re his son…" Totally yes. Take it as you want, as anti-SNS tend to take them, brothers, friends, rivals, etc minus lovers.
But you do not compare your child with your best friend or your brother about whom he resembles the most. Because they were speaking in terms of behaviour sence not even physical sense. Like maybe you could say this to a brother since well...genetics. But this is discarded to discuss with a friend or best friend bc how you would say "Boruto is more like you?" if he doesn't even have sasuke features? And when you talk about behaviours is about the ones they get and learn at home. This is only a thing Naruto could discuss with HInata since she raised the child. But he is discussing it with Sasuke so...who is the real mum here? And about Mitsuki and Boruto sun and moon thing i see it a bit of random tbh xD For me is already random that Orochimaru has a kid and he is walking around there as nothing happened but anyway...It's random since in the anime and manga these 2 have no chemistry at all and in the manga and canon eps such as Mitsuki and Sarada have no relevance at all so its pointless . It's a bit random they put this symbolism into a kid from Orochimaru and not Sarada or even Kawaki for example. Maybe to dismiss SNS? i dont know xD But the kid is very nice and he cares for Boruto so thumbs up jaja xD PD: sakura stans do not interact.

WARNING UNREASONABLE SHITPOST
recently, I see so much hate everywhere. magnetic storms or what but fandoms are getting rlly nasty when clashing with each other.
I think it is important to understand, that we shouldn't be bashing others, whatever we or they might like, as long as it is not offensive or reasonably immoral - and I stress, REASONABLY! some people believe homosexuality is immoral and that is just /warning! offensive language/ dumb.
as a huge fan of SNS I see a lot of anti's this days - antisakura, antiending, antinarusasu, antisasunaru, antinaruhina, antisasusaku, antiboruto and whatever. and - ugh - as much as I myself hate to admit that.... expectant drums sound.... nobody cares!
I mean - it's Kishimotos work. it's his idea, it's his story, it's his vision and - sadly - we don't get to decide whether he was wrong or not.
whether he should have ended the story with SNS,
whether he should have made female characters more prominent,
whether he should have killed Sakura off,
whether he should have made Naruto a better father,
whether he should have done many other things we all want him to do,
because - no matter how much we love the story or the characters - we are not the ones who created them, not the ones who understand them the most.
I mean, I have written myself a couple of paragraphs on how much I hate what is going on in Boruto - so yeah hi hypocrite! - but I sincerely mean well. yeah it pains me a bit to watch Boruto cuz of nostalgia I feel for Naruto series but I keep on watching, because I can't cut out that part of my heart that is responsible to crave more of Naruto universe even if I hate it.
and I am probably going to keep criticizing every damn thing - like, have you heard the Kara soundtrack? omfg I facepalmed the whole episode after that - and I wish it was better, but I am not going to bash characters for being created that way.
bc you know what? people are shitty. good people are shitty. bad people are shitty. everybody is shitty. so I guess made up characters get to be shitty too.
I mean, bashing Sakura for not having same problems as Naruto and Sasuke and thus not always getting them? - duh! remember yourself at 12, 13, 14, 15, you were a fucking whiner and you know it. and what were your problems? not getting a laptop you wanted? being punished for bad grades? getting into the fight with your parents for being unreasonable?
yeah, like you never said stuff like I wish I didn't have parents. I remember a moment after the fight I wrote a note to my parents before going to sleep that literally said ' sorry you got such a bad daughter I wish you didnt have to deal with me' and then went to sleep. in the morning - having completely forgotten it - I was woken up by my crying mom who said it was the stupidest thing on earth I knew I hurt her with it. and I regret it. it was dumb and shitty and I normaly consider myself a good person, but that was a huge shit I pulled. and I can't take it back. but I was like 10 and I didn't know better at the time.
why would I hold Sakura to higher standards then I hold myself?
then people bash her for being mean to Naruto - well, duh again - KIDS ARE MEAN. she got older and managed to see Naruto for who he was, but people still see her as a bully.
well, if we are not ready to accept peoples mistakes if they have managed to live up to them, I dont want to be a part of that 'we'
next I see a lot of bashing for Sakura being a bad mom - ugh, duh again /what is it, third time?/! how come? yeah she works a lot, since she is a single mother and all, but she genuinly cares for Sarada, even if she gets a bit overprotective at times. she doesn't have a very working relationship with Sasuke - well sure, he is never home!
then there's bashing Naruto and Sasuke for being bad fathers - you might think they'd learn to appreciate a family after being orphans.........
well where do I start. Sasuke is clinically afraid of losing loved ones after he has lost so much, so he is much better off not having many bonds and it is obvious as day. it doesn't make what he does right or good or wrong, it's just who he is. Naruto... well, lets start with the fact that he never had a role model. he had Iruka - at times, he had Jiraya for three years, but he never really had a family. he was a good dad - as we can see - when kids were little and he wasnt a hokage...
but now... he might work too hard, he might overdo it a bit at times, but if you think that he struggled his whole life to get appreciation why would you think he'd stop now? he just wants to manage everything, but he cant, and it kills him but he doesnt stop and its beautiful. yeah, its unhealthy and stupid but beautiful, because that is what the core of his character is. thats what this world has shaped him to be.
BUT THEN AGAIN THAT IS JUST MY PERCEPTION. I am pretty fucking sure Kishimoto and authors had their own reasons for it that are justified and totally okay because these are his characters who he can do whatever he wants to.
just like we people can.
and I can go on, because there is just SO MUCH HATING AND BASHING I just can't take it anymore.
I mean this whole war of sasunaru and narusasu - why would you care who fucks whom as long as both enjoy it and dynamics in bed dont influence dynamics in the relationship.
I mean... fanfics are not real. hell, even Naruto is not real, but fanfics exist entirely separately from the anime (most of them don't even get personalities right) and it is OKAY. they can exist separately to please those who are unhappy with how things go.
but please please pleeease stop bashing everything just for the sake of it. and I don't mean you are not entitled to feel and I am not telling what you should feel but there is so much more wholesomeness and awesomeness and beauty in portraying and seeing Sakura as a supportive friend and Sasuke as a helpless child inside and Naruto as a strong person who just really can't catch a break. and Boruto and Sarada as a way to make SNS canon in some twisted way.

Most of the people jumping ship right now are those who were fairly low-expectations with regards to reform of the shinobi system and various societal issues. The people who cared deeply about that stuff rejected Boruto from the outset. The people who are going “that’s it, even I can’t take this��� at this point are jumping ship due:
1. slowness and lack of progress in the Boruto anime
2. incessant and cringe-worthy copying/call-backs to original Naruto
3. increasingly ridiculous “ninja tech” such as NINJA LIGHTSABERS and NINJA MACHINE GUNS and NINJA MECH SUITS
The specific people on my dash who are now saying “that’s it, fuck this garbage” are mostly citing #2, from what I’m seeing. Stuff like making Team Konohamaru “team 7″, having Boruto and Sarada near-kiss in a total call back to the SNS kiss, having their first mission be a C-rank gone wrong, etc etc etc.

But, after S7 TBPH, D stans are especially hilarious to me now bc Dani's smeagol death and dark arc arent coming, they've been in motion this entire time and now it's coming to the forefront. All the 'spoilers' are D fans trying to cover there arses with fanfic bs and excuses for actual dark arc spoilers.

7/8) I know that her stans have become desensitised to the possibility that she will actually face true consequences because she's never really had to since S1, and as a result of D never facing them, they have come to truely believe that she's done nothing wrong. I think D&D did this on purpose. S8 will be the game changer and D&D will pull out no stops in closing off the ends they created in S3-S7 (I don't think they like D as much as people think they do).

Etilefrine Could Improve Response to Standard Medical Therapy in Chronic Hepatitis C Egyptian Patients with Cirrhotic Refractory Ascites: A Randomized Pilot Study-Juniper Publishers

Ascites is a frequent complication of cirrhosis that accounts for over 75% of episodes of ascites. Patients with cirrhotic ascites have marked splanchnic vasodilation and arterial hypotension with subsequent activation of vasoconstrictive and anti-natriuretic mechanisms. One of the most serious complications in cirrhotic patients with ascites is the occurrence of refractoriness that is the inability to resolve ascites by the standard medical treatment. The aim of this study is to evaluate the effects of etilefrine on systemic hemodynamics, renal function and control of ascites in chronic hepatitis C (CHC) patients with cirrhotic refractory ascites receiving standard medical treatment (SMT) with low sodium diet and maximal diuretic doses of 160mg/day of furosemide and 400mg/day of spironolactone.
Methods: A total of 50 CHC patients with cirrhotic refractory ascites were prospectively studied after 1 month administration of SMT (n=25) or SMT plus etilefrine (n=25), in a randomized controlled study.
Results: A significant increase in 24-h urinary output, urinary sodium excretion, mean arterial pressure (MAP), and decrease in body weight, plasma renin activity and plasma aldosterone concentration (P<0.05) was noted after 1 month in the SMT/etilefrine group. Furthermore, the effective diuretic doses and the need for large-volume paracentesis were significantly reduced in the SMT/etilefrine group compared to the SMT group after 1month of therapy. No significant changes in the aforementioned parameters were noted in the SMT group, except that MAP was significantly decreased. There was no significant change in the score of the model for end-stage liver disease (MELD) in the SMT/etilefrine group; however, there was significant deterioration in the MELD score in the SMT group.
Conclusion: These results suggest that the addition of etilefrine to SMT improves the systemic hemodynamics and enhances water and sodium excretion, providing better control in patients with refractory cirrhotic ascites treated with SMT alone.

    Introduction
Ascites is the most common of the three major complications of cirrhosis; the other complications are hepatic encephalopathy and variceal hemorrhage [1]. Approximately 50% of patients with "compensated" cirrhosis develop ascites during 10 years of observation [1]. Ascites is the most common complication of cirrhosis that leads to hospital admission [2]. Development of fluid retention in the setting of cirrhosis is an important landmark in the natural history of chronic liver disease: approximately 15% of patients with ascites succumb in 1 year and 44% succumb in 5 years [3]. Many patients are referred for liver transplantation after development of ascites. Refractory ascites develops in approximately 5-10% of all cases of cirrhosis- related ascites and carries a high mortality rate [4]. The available therapies for patients with refractory ascites are repeated large volume paracentesis, trans jugular intra hepatic portosystemic shunts, peritoneovenous shunts, and liver transplantation [5,6].
The mechanism by which refractory ascites develops in cirrhosis is related to splanchnic vasodilatation followed by maximal activation of the sympathetic nervous system (SNS) and the renin - angiotensin- aldosterone system (RAAS) [7-10]. Splanchnic and peripheral vasoconstrictors (octreotide, midodrine, and terlipressin) increase effective arterial volume and decrease activation of the renin-angiotensin system with resultant increase in renal sodium excretion [6]. Vasopressors causing splanchnic vasoconstriction have been used in hepatorenal syndrome [11-13] for the prevention of post-paracentesis circulatory dysfunction [14-16], improving circulatory and renal function in patients with cirrhotic ascites [17,18] and for control of ascites in patients with refractory ascites [19]. Combined use of midodrine with standard medical therapy (SMT) was found to improve the systemic hemodynamics without any renal or hepatic dysfunction and is superior to SMT alone for the control of refractory cirrhotic ascites [20]. Clonidine, a centrally-acting presynaptic ɑ2- adrenergic receptor agonist, when given with spironolactone has been shown to cause rapid mobilization of ascites by significantly decreasing the sympathetic activity and renin- aldosterone levels [21,22]. Synthetic vasopressin-V2 receptor antagonists are being evaluated for mobilization of ascites by increasing the excretion of solute-free water [23-25]. Etilefrine is a sympathomimetic agent with a potent stimulating effect on peripheral ɑ-adrenoceptors and a mild agonist effects on β1- and β2-adrenoceptors. It has a potent vasoconstrictor effect. It has a stimulant effect on the cardiovascular system where it raises blood pressure to normal, improves cardiac performance and tissue perfusion. Etilefrine is indicated in hypotension and, hypotensive circulatory disorders [26-28]. There are no published reports on the combined use of etilefrine and SMT in patients with refractory cirrhotic ascites. Therefore, this study is designed to investigate whether the long-term use of etilefrine would improve systemic hemodynamics and control of ascites in patients with cirrhosis and refractory ascites.
    Methods
The study protocol was approved by the local ethics committee and was conducted in accordance with the principles of Helsinki declaration. Written informed consent was obtained from all patients before enrollment in the study. A total of 58 chronic hepatitis C patients with refractory cirrhotic ascites were evaluated for inclusion in the study between November 2015 and May 2016. Eight of the enrolled patients were lost during the study period for different personal unidentified reasons. Accordingly, 50 CHC patients with cirrhosis and refractory ascites with stable renal function (creatinine level <1.5 for at least 7days), attending Gastroenterology and Hepatology Department of a specialized hospital were prospectively included in the study. Diagnosis of cirrhosis was based on clinical, biochemical and ultrasonography findings with or without liver biopsy [29,30]. Inclusion criteria were as follows: presence of refractory ascites; patients less than 60 years of age and no treatment with drugs known to affect systemic or renal hemodynamics within one week before initiation of the study.
Exclusion criteria were as follows: presence of marked hepatic encephalopathy, GIT bleeding, hepatorenal syndrome, hepatocellular carcinoma, bacterial peritonitis, portal vein thrombosis, arterial hypertension, diabetes, intrinsic renal or cardiovascular disease. Refractory ascites is defined as
A.  fluid overload that is unresponsive to sodium- restricted diet and high-dose diuretic treatment (400mg/d of spironolactone and 160mg/d furosemide),
B.   buildup of fluid that recurs rapidly after therapeutic paracentesis or
C.    development of diuretic-related complications that exclude the use of an effective diuretic dosage. Patients were randomized to either SMT alone (n=25) or SMT plus oral etilefrine (n=25). Etilefrine is active ingredient of EffortilR produced by Chemical Industries Development, Egypt under license of Boehringer Ingelheim International, Germany. Etilefrine was blindly given orally at a dose of 5 mg/8h. SMT was defined by dietary restriction of sodium (≤2g/day, starting at least 7 days before the start of the study), treatment with a combination of a loop diuretic (furosemide 160mg/d) and a distally-acting aldosterone antagonist (spironolactone 400mg/d) and repeated large volume paracentesis (LVP) along with intravenous albumin (8g/L of ascitic fluid removed).
During the study period, diuretic doses were reduced by a 40mg for furosemide and100 mg for spironolactone for a mean weight loss >0.8kg over 4 days from the previous weight. Large- volume paracentesis was performed when ascites is tense and symptomatic. Frequency of paracentesis sessions over the 4 weeks preceding the study was obtained from patient files. All patients were subjected to baseline clinical and biochemical workup including, body weight, mean arterial blood pressure, 24-h urinary output, 24-h urinary sodium excretion, liver function tests and renal function tests. These parameters were assessed at baseline and at weekly intervals for 1 month. Measurement of mean arterial blood pressure (MAP) was calculated as diastolic blood pressure+((systolic blood pressure-diastolic blood pressure)/3). Three measurements were taken each 1-hour apart and the mean was calculated. Plasma renin activity and plasma aldosterone concentration were evaluated at baseline and after 1 month (endpoint). Plasma renin activity (PRA) was measured by radioimmunoassay using RIA plasma renin activity kit (Diasorin, Stillwater, MN, USA). Plasma aldosterone concentration (PAC) was measured by radioimmunoassay using ALDO-RIACT aldosterone kit (Cisbio, Parc Marcel Boiteux, France). Diuretic requirements were assessed at baseline, at weekly intervals and at endpoint. Patients were instructed to undergo tapping when become symptomatic.
Outcome measures
The primary endpoints of the study were partial or complete control of ascites. Complete response was defined as the elimination of ascites (as assessed by clinical examination and abdominal ultrasonography); a partial response was defined as the presence of ascites not requiring paracentesis; and absence of a response was defined as the persistence of ascites requiring paracentesis [31]. Secondary endpoints include alteration of diuretic requirements, changes in the scores of end-stage liver disease, liver and renal function, and frequency of other complications of cirrhosis (e.g., encephalopathy, upper GIT hemorrhage or development of hepatorenal syndrome) after 1 month of therapy.
Statistical analysis
Data were analyzed using SPSS for MS-Windows (version 17.0, SPSS, Chicago, IL, USA). The baseline patient characteristics (clinical as well as biochemical) were compared between two groups (SMT or SMT plus etilefrine) by using Kruskal-Wallis ANOVA, Chi-square test or Fisher's exact test as appropriate. Intra-group comparisons were done using multiple repeated- measures analysis of variance. The paired t-test was performed to detect mean and standard deviation of prevalues (baseline) and post values (endpoint at 1 month) of the same variable of the same patients. The results were reported as mean values ±SD. A p-value of ≤ 0.05 was taken as significant.

The demographic characteristics and baseline clinical and biochemical parameters were similar between SMT and SMT/ etilefrine groups (Table 1). Baseline body weight did not differ significantly between the two groups (p>0.05). There was a significant decrease in mean body weight in SMT/etilefrine group at 1-month as compared to baseline (p<0.05) however; it did not change in the SMT group (Table 2) (Figure 1).

Baseline MAP did not differ between SMT and SMT/etilefrine groups (p>0.05). There was a significant increase in mean arterial pressure in SMT/etilefrine group at 1-month as compared to baseline (p<0.05) and a significant decrease (p<0.05) in the SMT group (Table 2) (Figure 2). Baseline urine output did not differ between SMT and etilefrine groups (p>0.05). The urine output was significantly higher in the SMT/etilefrine group (p<0.05) but not the SMT after 1 month of treatment as compared to baseline (Table 2) (Figure 3). Baseline urinary sodium excretion was comparable in the SMT and SMT/etilefrine groups (p>0.05). Urinary sodium excretion significantly increased in the SMT/ etilefrine group after treatment at 1-month as compared to baseline (p<0.05); however, it did not change in the SMT group (Figure 4).
Baseline values for plasma renin activity (Table 1) were similar in both treatment groups (p>0.05). Plasma renin activity significantly decreased at 1-month (p<0.05) only in the SMT/etilefrine group with no change in the SMT group compared to baseline (Figure 5). Baseline plasma aldosterone concentrations did not differ between the two groups (p>0.05). Plasma aldosterone concentrations decreased significantly in the SMT/etilefrine group at 1-month as compared to baseline (p<0.05); however, it did not change in the SMT group (Figure 6). Baseline values for serum creatinine in both SMT and SMT/ etilefrine groups were similar (p>0.05, (Table 1). There was no significant change in serum creatinine in both groups after 1-month treatment as compared to baseline (p>0.05, (Table 2).
Baseline serum bilirubin and INR were similar in both groups (p>0.05, (Table 1) but there were significant increase in their values at 1 month only in the SMT group (p<0.05, (Table 2). Baseline MELD score was similar in both treatment groups (p>0.05; (Table 1). There was a significant deterioration in MELD score in SMT group at 1 month (p<0.05) with no change in the SMT/etilefrine group as compared to baseline (p>0.05. The need for LVP (≥5L) was significantly reduced in the SMT/etilefrine group (p<0.05; (Figure 7) at lmonth compared to baseline value. No significant change was noted in the SMT group. Fifteen patients from the SMT group required LVP compared to only six in the SMT/etilefrine group over the 1-month period of therapy.

As depicted in Table 2, diuretic requirements were significantly declined from baseline in the SMT/etilefrine group (p<0.05) with no change in the SMT group. There was higher rate of partial response to treatment and better control of ascites in the SMT/etilefrine group (p<0.05; (Table 3) (Figure 8) compared to SMT group at 1 month of treatment. Mild abdominal pain that subsided on its own was noted in three patients in the SMT group. In the SMT/etilefrine group, mild headache was developed in two patients, which disappeared with time without discontinuation of therapy.
Follow-up
The 1-month morbidity and mortality of the study is depicted in Table 4. In SMT group, encephalopathy developed in two patients, upper gastrointestinal bleeding, spontaneous bacterial peritonitis (SBP) and renal failure developed in one patient each. One case of SBP was recorded in the SMT/etilefrine group. The 1-month mortality was two in the SMT group and was related to sepsis during the follow-up period. No mortalities were recorded in the SMT/etilefrine group.
    Discussion
Splanchnic arterial vasodilatation induced by nitric oxide [32] and glucagons [33] leads to disturbance of systemic hemodynamics reflected as reduced arterial blood pressure, reduced vascular resistance, and decreased effective blood volume with activation of potent vasoconstricting systems such as the sympathetic nervous system, the renin-angiotensin- aldosterone system, in addition to nonosmotic release of vasopressin [34-36]. This results in renal vasoconstriction, avid sodium and fluid retention with development of ascites [37]. The administration of arterial vasoconstrictors has been associated with improvement in systemic hemodynamics and renal function in cirrhotic patients with ascites [38]. The efficacy of vasoconstrictors in advanced cirrhotic ascites might be related to failure of the activated endogenous vasopressor systems to counteract the arterial vasodilatation [39], probably due to reduced arterial reactivity to vasopressors [40]. Administration of intravenous arterial vasoconstrictors such as metaraminol [41], norepinephrine [42,43], angiotensin II [44] and terlipressin [45] in cirrhotic ascitic patients has been associated with improvement of systemic hemodynamics without harmful effects on renal function. Midodrine, a potent peripherally acting oral a-adrenergic receptor agonist, either alone [19,20,46] or in combination with octreotide and albumin [47] has been used to improve renal hemodynamics in cirrhotic patients with ascites. In these studies, midodrine-induced splanchnic vasoconstriction improved systemic hemodynamics with better control of ascites without any renal or hepatic dysfunction.
Combined use of midodrine with tolvaptan, an aquaretic vasopressin V2 receptor antagonist, has been found to control ascites and improve response to diuretic therapy. Rai et al. [48], reported that midodrine (by causing splanchnic vasoconstriction, increasing effective arterial blood volume and improving renal perfusion) and tolvaptan (by increasing free water clearance) acting at different sites in combination were more effective in combating increased renal sodium retention and refractoriness to diuretic therapy and better controlled ascites. There are no studies in literature on the short- or longterm use of combination of etilefrine and standard medical therapy in patients with cirrhotic refractory ascites, therefore the results of the present study will be compared to previous studies in which different vasopressor agents were used for the control of refractory ascites.
In our study, we compared the changes in systemic hemodynamics (MAP), renal excretory function (24-hour urinary sodium excretion, 24-hour urinary output), plasma renin activity and plasma aldosterone concentration in patients with cirrhotic refractory ascites after 1-month treatment with SMT alone or in combination with etilefrine. Changes in models for end-stage liver disease scores, the need for paracentesis and diuretic requirements were also compared in both groups.In the current study, there was a significant increase in the mean arterial pressure (MAP) with etilefrine while there was no change in the SMT group. Etilefrine-induced increase in MAP may be related to reducing venous pooling and counteracting reflex arteriolar vasodilatation [28].
In agreement with our results, Kalambokis et al. [17,38] reported an improvement in circulatory function manifested as a significant increase in MAP after short-term use of midodrine [17] and chronic combined use of midodrine with octreotide [38] in patients with nonazotemic cirrhotic ascites. Similar results were reported after long-term use of midodrine with SMT [19,20,48], and combined use of midodrine with SMT and clonidine [20]. On contrary, Oda et al. [49] reported that a 3-month course of midodrine produced no change in MAP in cirrhotic patients with refractory ascites. In comparison to baseline values, our results showed a meaningful improvement in renal hemodynamics and function reflected as a significant increase in twenty-four-hour urine output and urinary sodium excretion in the etilefrine/SMT group but not in the SMT group. These findings agree with those observed with previous studies employing midodrine plus SMT [19,20,48] , combined use of midodrine with SMT and clonidine [20], and combined use of midodrine with SMT and tolvaptan [48]. In another study, a single dose of terlipressin (vasopressin V1 receptor agonist) showed marked increase in urinary sodium excretion in patients with and without refractory ascites [18]. The results of our study disagreed with a previous study [50] that found no change in 24-h urine volume after two-week midodrine therapy.
In the present study a significant decrease in plasma renin activity (PRA) and plasma aldosterone concentration was noted only in the etilefrine/SMT group after one-month therapy compared to baseline. This effect is possibly related to etilefrine-induced suppression of the renin-angiotensin- aldosterone system. Similar results with other vasoconstrictors were reported by Singh et al. [19, 20] and Rai et al. [48] after long-term use of SMT/midodrine regimen [19,20,48], combined SMT/midodrine/clonidine therapy [20] or combined SMT/ midodrine/tolvaptan therapy [48]. In an earlier study, no change in PRA was noted in patients with refractory ascites maintained on a 3-month course of midodrine therapy [49]. In the current study, significant reduction in mean body weight was observed in patients receiving etilefrine plus SMT compared to those treated with SMT alone. This comes in concordance with previous findings of decreased body weight by midodrine [38,50]. The reduction in body weight may be related to a drop in fluid accumulation by etilefrine-induced vasoconstriction with reflex inactivation of the renin-angiotensin-aldosterone system (RAAS). Similar explanation were reported in previous studies in which the authors observed a sig-nificant reduction in plasma renin and aldosterone con-centration and a trend toward a reduction in the volume of ascitic fluid removed by paracentesis following the administration of midodrine [38,50]. No change in body weight was reported in a number of previous studies utilizing different vasoconstrictors including midodrine [19,20,48].
Rate of response to treatment, measured as need for large- volume paracentesis (≥5L) and reduction of ascites with SMT alone or combined SMT/etilefrine therapy was measured. There was higher rate of response to treatment, reflected as a significant decrease in the number of times of paracentesis in the combined SMT/ etilefrine therapy at 1 month. There was no significant change in rate of response to treatment in the SMT group. These results come in harmony with some previous studies in which the vasoconstrictor midodrine was used with various daily doses [20,48,50]. In another study, midodrine along with octreotide and albumin given for 1 month showed lesser requirement of paracentesis in eight patients with refractory ascites [47]. In agreement with previous studies evaluating midodrine [17,19,20,48], midodrine and clonidine [20], midodrine plus octreotide [38] or midodrine plus tolvaptan [48] in cirrhotic patients with ascites, our results did not show significant change in hepatic function or MELD score in the combined SMT/etilefrine group. A significant deterioration in MELD score was noted in the SMT group at 1 month. In one previous study [38], combined use of SMT with midodrine and tolvaptan showed significant improvement in MELD score at 1 and 3 month of therapy. In another pilot study, significant deterioration in the MELD score was observed during treatment with midodrine at 1 month of therapy [47]. Diuretic needs for furosemide and spironolactone were significantly reduced in the SMT/etilefrine group at 1 month compared to baseline. Diuretic doses were reduced by increments of 40 mg (furosemide) /100mg (spironolactone) for each ≥0.8kg mean decrease in body weight from the previous weight over 4 days of therapy according to the criteria of International Ascites Club [51].
The reduction of diuretic requirements and subsequent enhancement in diuretic response may be related to etilefrine- induced improvement in renal perfusion and/or its inhibitory effect on RAAS. No significant change in diuretic needs was noted in patients receiving SMT alone. Reduction in diuretic needs with better control of ascites was reported in one earlier pilot study [20]. There was higher rate of partial response to treatment (defined as ascites requiring no paracentesis) and better control of ascites in the SMT/etilefrine group compared to SMT group at 1 month of treatment (76 versus 40%). Etilefrine use was well tolerated by the majority of patients. Only two patients developed mild headache that resolved spontaneously within few days without discontinuation of treatment.
    Conclusion
To our knowledge, this is the first study of long-term use of combined SMT/etilefrine therapy in patients with cirrhotic refractory ascites. In patients receiving combined SMT/ etilefrine therapy, we observed a significant increase in MAP, 24-h urinary output, 24-h urinary sodium excretion and a significant reduction in body weight, plasma renin activity and aldosterone concentration. There was a considerable reduction in the need for large volume paracentesis and diuretic therapy. Large multicentre, randomized-controlled trials are required before combined SMT/etilefrine therapy can be routinely recommended.

What Remains Hidden
                        I spent the least enjoyable portion of my week in a termination interview for an employee, with whom I had worked for a number of years.  The man had run afoul of our 10-point no fault attendance policy.  The employee, who is a friend, had scored an impressive array of points in all sorts of amazing circumstances within a relatively short period of time.  He was like the Michael Jordan of attendance infractions – raining in the points in one and two point weekly episodes.  His stories ranged from the incredible to the improbable, but as usual he and his union representative focused in on the couple of points that might be in doubt so that the man could remain an employee of our company.

I encouraged both of them to pursue their grievance, because I respected the excellent work that the employee had done for us previous to his most recent run of total unreliability, poor workmanship and serial distraction.  My friend, an ex-con and recovering addict, had been a great talisman for human potential and second chances until his struggles in the last eight months.  
But really, I explained to them both, gaining my friend an extra couple of weeks of employment really seemed like a lot of bother and paperwork prior to his inevitable final flameout with the next one or two point incident.  I posited that the employee needed to fix whatever the root cause was that was making him the second most prolific scorer of attendance points in the history of our facility.  My friend’s visage flickered brighter momentarily and he blurted out, “You mean I should do something to resolve all this car trouble I’ve been having.”  
“No,” I replied, “that is probably not what you need to take care of.”  What was hidden remained concealed; my intervention failed.  We finished up the required paperwork and the transacted our final goodbyes.  After he left, I prayed to Pam to intercede for my friend at whatever level she has access. In the short term, his future looks pretty bleak.
My bereavement meeting with widows and widowers in the Hall at 6PM of Thursday was a much brighter chapter of my week.  The meeting doesn’t take long, but it is a good chance to review the ups and downs of my life with some other people who are walking similar paths.  This week we discussed how to get through the holidays, a uniquely challenging season for people in our circumstance.
In our discussion of family vendettas that sometimes occur at Thanksgiving, it struck me that hidden things can contribute to family friction as well.  Like rocks submerged in navigable waters, underlying and unspoken issues can ruin family harmony and destroy relationships.  One person described a death of a father that occurred two days before a family gathering several years ago.  A couple of siblings immediately engaged in an argument that has separated them for the several years since.  Was the father previously acting in a way that prevented the full exercise of an otherwise underlying bitterness in the family?  Were the siblings, like my daughter Abby and I did after Pam’s death, choosing to argue over some unimportant minutia as a distraction from their overwhelming grief.  The road to reconciliation is different depending on the underlying true cause of the disagreement, a cause that must be brought to light for resolution.
Unspoken issues and unconscious motivations are ubiquitous to the human condition.  I met two friends for lunch the other day, and in our discussion it became clear that both men were being shaped by the Holy Spirit in particular ways without an apparent root cause.  Through our conversation over several hours, more and more puzzle pieces were added to their stories so that a vague idea of what God was trying to accomplish became more discernable.  
The first man works for law enforcement in one of the most violent neighborhoods of our local area, yet he is walking ever closer to the Lord in his daily life.  While waiting for our other friend to arrive, he and I discussed a recent shooting that he had witnessed and how our reaction to the tragedy differed so greatly from the laughing perspective of a person who had uploaded video of the same incident onto YouTube (http://www.youtube.com/watch?v=DvEKq5CeGJo&sns=em). (Violent content)  
My friend had been driven to pray the Hail Mary aloud during the ordeal.  By contrast, for the person filming the car-jacker dying in a hail of bullets, the incident was pure entertainment.  I have struggled with the purpose of why the Holy Spirit moved my friend to his quiet act of piety for several weeks, but in the course of our conversation it became apparent to me that my friend’s outlook on his whole career had been altered by being inspired to make a few simple prayers.  He has spent the ensuing time considering the plethora of poor corpses that he seen over the years and has begun to pray for their many souls.  Before the incident my friend had protected his sanity at work with a Kevlar vest of emotional detachment.  Now the policeman was allowing the plight of perpetrators and victims alike to touch him in a very human way. I expect that the Holy Spirit is preparing him for additional service to the community in a new role.  Instead of merely maintaining the simple peace of the community; his future will involve leading others to the profound peace of Christ.
My other friend arrived later and described the gravity like pull he feels in his life towards devotion to the Virgin Mary.  Although, he was Catholic as a young child, his family made a few years long foray into the Protestant faith.  He returned only in recent years carrying much of the same anti-Marian baggage that I also lugged around for thirty or so years.  Lately, my friend has been feeling irresistible urges to pray to Mary for a very specific thing that have been subsequently granted in dramatic fashion. He has also received a free copy of Tim Staples’ new book, Behold Your Mother, under unusual circumstances. Through seeming coincidence, he was likewise put into a situation regarding the visit of the Fatima statue to IHM that removed his visceral dread of praying to Mary and Jesus through painted or sculpted representations of them.  
None of these dominos that appeared to be buffeting my friend towards a vocation in the Blue Army made sense to me. Why was my friend such a high value target for conversion to devotion to Our Lady?  Finally, he confided in us that as a young child he had been awed by a beautiful Christmas card of Mary holding Jesus.  He subsequently, cut Mary out, pasted her up above his bed and prayed to Her nightly. By appearances Mary is not making a new claim to a convert; she is exercising a latent consecration that has lain dormant for decades.  My friend has always had an underlying Marian devotion and he is going through a process of rediscovering the childlike faith that he had once possessed - a faith of simplicity that is the sweetest oblation to Our Lord.
In my case, forever the observer, my subterranean tensions remained hidden at our lunch.  I spent the weekend pondering the lingering questions about my own vocation.  In A Grief Observed, C.S. Lewis draws a simile between being a widower and having a leg amputated.  I guess I have reached the emotional state he described as functional hobbling.  I now know that I can effectively hop through my work day – no matter how full of dysfunction.  What remains uncertain is whether I can stumble along effectively as I resume my duties as a parent of a nine-year old girl or whether I can go a step further and emotionally juggle answering God’s call to seek ordination as a deacon.  To be effective in either role, there seems to be a necessity for a prerequisite joyfulness and hope that I seem to be lacking.  Still my current inclination is to step over the gunwales and into the waves and try to walk towards Jesus in hopes that I find something in His eyes to replace my inner storm with serenity.
Finally, I had another experience with hidden issues over the weekend.  Scrolling through people’s personal errata on Facebook, I happened across a video link that promised hope for divorced people.  Because I am so emotion starved, the video, called Rejoice,  (http://www.youtube.com/watch?v=pdxLwqGsRkQ) seemed to exercise a gravitational pull at my heartstrings.  The attraction disconcerted me as the scourge of divorce is as irrelevant to my current personal situation as is a couples nights out.  I felt compelled to watch the film even to the point of ignoring the timewasting Facebook game that has obsessed me for a week.  I even rebooted when it froze and accessed it through several different links until I was able to watch the whole thing.  In the end, I was reduced to strange tears as the video left me with an impression of hopefulness for the hero and heroine.  This romantic rock submerged in my sub-conscious surprises me whenever I run afoul of it, but it used to make Pam smile whenever she could conn me into watching the Notebook or something similar.  I will have to be more careful in my social media scrolling in the future.

Why don't you ship Captain Canary?
That’s a complicated question with a complicated answer.
It’s hard because I’m compelled to be honest because I respect my anons enough to give a serious, honest answer to pretty much every ask I actually take the energy to respond to, but I feel like any person who would ask this doesn’t actually want my honest answer from it. Last time I answered a question about CC, I was accused of attacking the anon because I summarily disagreed with the points they were making and explained why. 
So, if I say this question feels a bit like an accusation, I hope you understand that it’s because it makes me nervous to go into negative detail about another ship and to know that it can ruffle feathers even with the best intentions. I’ve always been 100% disinterested in bullshit like ship wars and I don’t consider CC as in any way counter to anything else. I just… don’t ship it.
But I’ll answer my reasons for why honestly, leave it out of the ship tags, and hopefully you respect my answer.
The reason I don’t ship CC is a mix of on-screen chemistry, characterization, Sara’s seeming genuine dislike of Len at the outset and lack of attraction to him throughout, and… the shippers.
To go into detail on those. Their on-screen chemistry is amazing, and to a certain extent, I do ship them. I ship roguecanary (len x mick x sara), or Len and Sara for a fling, a dirty make-out in a dark corner sometime, a quickie in a supply closet sometime. I also ship them in the sense that they could, in theory, be a much much slower burn. I genuinely thought they might have a slow progression across two seasons or so, back when it first looked like there might be something between them, that they had some mild flirting. I think I might of liked that, and I certainly wasn’t against it. Killing off Len kind of accelerated that timeline though, if it existed.
But what I don’t ‘ship’ about their on-screen chemistry is that it feels so much more like friendship to me than romance. And that’s fine for some people. But the same way I don’t ship sn*wb*rry because I literally cannot see them as anything other than friends, and simply do not see romantic chemistry there at all, I also don’t see any genuine romantic chemistry between Len and Sara. That they become friends is sort of undeniable and I loved to see them playing cards and having insight into one another, but I didn’t feel that needed to be romantic and I thought it was better off being what it was, simply. 
Characterization-wise? Well, this isn’t anyone’s fault, but I was a pretty hard Nyssara shipper, so Sara’s prior characterization made me not want to ship her with anyone other than Nyssa for a while. I’ve finally relaxed off that after it became clear I’d never get to see them together again, but it was hard letting go of that canon ship. I also found the logical ship for Len to be with Mick, because of their amazing dynamic. I found their chemistry to veer more toward romantic than Len and Sara’s did half the time. Moving in sync, the angsty break up, literally willing to die for each other, having a little memento ring, inside words, Mick being the only one Len seemed to initiate contact with. 
So none of that helped. But none of that made CC a nOTP either? I mean, it’s not really a nOTP across the board. like i said, i do ship roguecanary or len and sara for a fling or an open thing, a casual understanding.
But Sara disliking Len at the outset did make give me an anti-romance sense. And throughout the entire season, she seems disinterested in him romantically.
Contrary to people with shipper goggles on when they watch that deleted scene from the pilot, I see her being annoyed by him and like she finds him distasteful. they get along for a drink by the end of the episode and she’s teasing him, they get closer, but she also says that she literally doesn’t like him when they’re dying of the cold. teasing? maybe, idk, but it’s not really something you say to someone you’re crushing on. it just seems so very platonic and also like the notion of being with him makes her uncomfortable. it’s hard for me to read her otherwise. he alludes to his feelings about her when he’s deflecting talking about mick and she looks surprised then impassive, completely shutting it down. he mentions thinking of a future with her when she’s already upset and doesn’t want to talk to him and she… well she basically tells him to fuck off? but as a “soft” no instead of a hard no, because maybe she doesn’t have her feelings sorted out but now is obviously not the time and she’s not interested in pursuing things with him with how things stand. 
but basically, she never demonstrates reciprocal interest in him at all. and we know that she’s not especially shy when it comes to expressing herself, her attractions, or her intentions. i know it would be more complicated with a teammate, but i don’t think she’d be dishonest about her feelings either; she values honesty far too much for that.
and this makes him a pursuant of a person who is totally uninterested in him, and that’s a place i’ve been? people uncomfortably trying to pursue their attraction to me when i’d shut them down already? it’s not a pleasant position to be in, after a while. and i trust that len’s not so much a creep that he’d push it after getting a clear answer from her, but it still wasn’t something i found… romantic? endearing? a little cute in the scene where he says “my feelings about you” when deflecting, but if he were pursuing it further after she gave him that “have to steal a kiss” line without getting a clear signal from her that she’d changed her stance, it would have actually pissed me off.
anyway… so there’s all that.
there’s also… if you look at that ask i linked above, that’s the type of thing that really pushes it into… i have the ship tag blacklisted because stuff like that annoys me. and look, it happens in the coldflash fandom too, and i ignore it then as well. i’m never a fan of reducing a character down to a ship, attributing their development to a single romantic relation, saying they’re “so in love” when they’re literally really not. and look, gifset edits for your fave ship are cute af, i get it. it’s fun to speculate how your ship’s characters are going to feel when they see each other again. 
but i get asks about ‘don’t you think len would have died for sara as well as mick?’ and my answer is… literally no. i don’t. i don’t think that’s how they saw each other, or how deep they felt about each other. and given that the producers have literally said sara has basically moved on, posts making her arc this season about len, or about how him coming back is going to affect her, just make me sigh. but i’m a canon-compliant asshole with my meta even with shipper goggles on, most of the time.
and i’ve seen a lot of those shippers try to reduce len’s relationships with lisa, mick, and other characters (including, yes, barry, who was important to his arc preceding legends) to being lesser than his friendship with sara. that’s so discomfiting to me. and it’s even more discomfiting to me how they reduce all of sara’s best emotional moments to being about len? her overcoming the order to kill martin was aided by len but not about him. her decision to talk to mick in the brig and that awesome conversation and the emotional insight she had? i’m tired of seeing people make that about her feelings about len. that was a great moment for her. she has so many great moments like that. so many great relationships i wish the fandom would explore more, rather than focusing on just a single romance.
and… look, i’m a person who prefers things when they don’t go canon half the time. my canon vs. non-canon ships are radically different. i love WA in the flash and wanted it to be canon and it is, but i love CF just as much and would never want it to be canon. with CC, the ways i would’ve wanted it to be canon never happened, and the ways it’s romanticized and populized in the fandom tend to rub me the wrong way maybe because of that. 
So… long answer there. but it’s really honest. i don’t hate CC, and i do respect the fandom and genuinely wish them all the best. these generalizations, as with any generalization, are obviously not going to apply to every shipper or every fic or every meta. but they’re trends i’ve personally encountered and opinions i’ve either seen in the tags or had to interact with and eventually, it slowly turned me off the ship as a major romantic pairing.
