As the smartphone falls in price while its capabilities improve, it is becoming a valuable tool in the diagnosis of a growing number of diseases and ailments around the world.
When Yonatan Adiri’s mother fell down a bank and briefly lost consciousness when travelling in China, an initial diagnosis suggested she had a few broken ribs, but nothing more serious. Doctors were keen to fly her to Hong Kong for treatment.
But Yonatan’s father was worried and took photos of the CT [computerised tomography] scans of the injuries, emailing them to his son. Yonatan showed the images to a trauma doctor, who instantly diagnosed a punctured lung. The flight to Hong Kong might have killed her.
“Who knows what would’ve happened if he hadn’t taken photos?” Yonatan wonders.
The experience inspired the Israeli entrepreneur – formerly Israel’s chief technology officer under the late President Shimon Peres – to explore how the smartphone could be developed into a medical grade diagnostic tool.

The result was Healthy.io, a start-up pioneering “medical selfies”, as he calls them. The first product is a urine test kit that screens for signs of urinary tract infection, diabetes, kidney disease and cancer.
The standard urine test involves a special dipstick featuring 10 tiny pads that change colour if they detect various substances, such as blood, sugars or proteins, in the urine sample. Normally a trained clinician analyses the colour changes by eye, but Healthy.io’s smartphone app can do it equally well using its computer vision algorithm.
A smartphone chatbot called Emily takes people through the process step-by-step using voice, text and video.
The patient slots the dipstick into a colour-coded cardboard frame then scans the whole thing with the phone. The image is sent to the cloud for analysis and the results go to the patient’s doctor.
“It’s not a wellness device, it’s a medical device,” says Mr Adiri, emphasising that the product has been approved by the US Food and Drug Administration and by European Union regulators.

As the test can be done very easily at home, he believes it has the potential to save health systems “hundreds of millions of pounds” in clinician time and earlier diagnosis of diseases that would be far more expensive to treat later on.
The app and kit, which costs £9.99, has been used more than 100,000 times around the world, the company says. Pharmacy chain Boots is currently trialling the service in the UK, and the National Health Service is using it to monitor kidney transplant and diabetes patients.
The main advantage of a smartphone is that it has “huge computational power, a high-resolution screen, excellent cameras and is, importantly, connected and available worldwide,” says Dr Andrew Bastawrous, founder and chief executive of Peek Vision, a tech company pioneering simple eye tests for developing countries.
About 36 million people in the world are blind, many from easily treatable diseases, he says. Earlier diagnosis could save the sight of the majority of these people.

But in remote, poorer areas of the world, bulky and expensive medical equipment is hard to come by.
Peek Vision’s eye test app, Peek Acuity, displays the letter E on the phone screen and this changes in size and orientation during the test.
The patient points in the direction he or she thinks the letter is pointing and the tester swipes the screen in that same direction. The app works out if the answer was right or wrong. The test results are stored in the cloud and sent to the nearest trained clinician.
“Our app is designed so that almost anybody can use it,” says Dr Bastawrous, an academic and eye surgeon who works at the International Centre for Eye Health at the London School of Hygiene and Tropical Medicine.
“Training takes just a few minutes, so it can be used by non-specialists, including teachers, community leaders and general health workers.”

The phone’s camera can also simulate what the world looks like to the person tested, helping parents clearly understand why their child might need treatment, he says.
So far, more than 250,000 people have been screened in Kenya, Botswana and India, with the costs being picked up by partner charities and governments. The app has won clinical approval in Europe, and Dr Bastawrous says it is “at least as accurate as a conventional eye test”.
“The ability to record data at the point of care, take images that could be analysed using artificial intelligence or by experts who cannot be everywhere at once, increases the possibility of universal healthcare being realised across the spectrum of healthcare specialities,” he concludes.
There are plenty of other companies and researchers exploring the potential of the smartphone as a medical diagnostic tool, often in conjunction with add-on pieces of kit.

Cellscope, for example, has developed an attachment for the smartphone camera that enables parents to probe a child’s ear and take a video of the inside, which is then viewed by a doctor remotely.

Meanwhile, research teams are developing plug-in sensors that can detect a range of diseases, from HIV to Ebola, from a small sample of blood. And attachments can turn the phone’s camera into a microscope capable of examining red blood cells for signs of malaria.

“There have been some exciting developments in the evolution of the smartphone as a diagnostic tool,” says Niamh McKenna, health lead at consultancy Accenture. “Clinical diagnosis on the move has great potential for application in remote areas.
“However, we need to remind ourselves that ultimately smartphones are being developed as consumer technology and not as medical devices,” she adds.
“Trying to repurpose them in this way can lead to confusing consumer propositions and potentially come up against regulatory issues – like data protection.”
But if cheap, portable and accurate diagnosis of treatable conditions saves lives as promised, the smartphone could become the most important invention of the last 20 years.

It is never too late to turn your life around or start being “tough”, believes Benny Yeoh.
Yeoh, 38, says that years ago, an unhealthy lifestyle resulted in him being overweight.
“Being in event management then, I had to work at midnight and would often have my meals at mamak stalls in the wee hours of the morning. So, I started to gain weight.
“Then, when I was hospitalised for kidney stones, it was a wake up call. The doctor advised me to reduce my weight and start exercising,” he shares.

He started with cardio exercises and then proceeded to do weight training. From 93kg, Yeoh, who is 178cm tall, says that his weight went down to 74kg.
Mondays to Fridays are his workout days while weekends are his “cheat days”, he says good-humouredly.
Where food is concerned, Yeoh doesn’t stick to any special dietary rules except to “eat clean”.
“I tell myself to stay healthy, and that includes eating healthy.”
He reveals that for breakfast, it’s raw oats with a protein shake; lunch time, it’s brown rice with chicken breast and salad; and for dinner, it’s mainly protein such as chicken breast, eggs or beef.
Yeoh, who is from Kuala Lumpur, admits that even his family members were surprised at his journey into fitness.
“To them, it was impossible for a person to lose weight and achieve peak fitness after the age of 30. Career wise, it is also difficult for a person to move from the corporate sector into a totally different field,” he shares.
In 2016, Yeoh decided to take part in the R U Tough Enough (Malaysia) competition, and won.
“When I joined, I told myself I just wanted to challenge myself to be better and complete all the obstacles and quests. That year, all the challengers were very fit and tough, and very young too, from 20 to 30 years old. I was one of only two contestants over 30. So, I never expected to win,” he recalls.
Yeoh believes that the formula for success in such competitions involves brains over brawn.
“In R U Tough Enough, there are many who are physically strong, but speaking as a judge, what defines the winner is a strong mindset. You need mental strength and emotional resilience. That always comes first, followed by physical strength,” explains
Yeoh, who is ambassador and head judge for R U Tough Enough 2019.
Currently, Yeoh is a fitness activation specialist for a chain of fitness clubs, handling mass workout activities. He also heads a team of judges and marshals for the clubs’ fitness activities.
Overall, he shares that his journey with R U Tough Enough has made him more confident in life.
“If there are obstacles in my life now, I just think back that if I could win R U Tough Enough 2016, then I can surely overcome this obstacle,” he says.
Never give up
No matter what background you come from and what difficulties you’ve been through, it is still possible to be successful in life, asserts the winner of R U Tough Enough South-East Asia 2018, Mohd Saddam Mohd Pittli.
When the 28-year-old Mohd Saddam was just 11, his mother passed away and he had to learn to be independent to help support his nine siblings.
The Teluk Intan boy found comfort in running and eventually decided to join the army after completing his secondary school.Mohd Saddam, who has been a soldier for almost 10 years, believes that enlisting in the army has made him stronger.
He then became a fan of R U Tough Enough in 2015, but only took part in it in 2018, after his brother and a friend – who was participating in the event – encouraged him to do so.
Coming in first in the Spartan Race in 2016 and 2017 also motivated him to sign up.
However, he admitted that he did not expect to become a finalist representing Malaysia, let alone win the regional Toughest in South-East Asia 2018 title.
“I was not feeling well during the closed-door auditions but I did my best to complete all the tasks,” he recalls.
Mohd Saddam encourages people to push beyond their limits and move out of their comfort zone.
“It was worth it because, over a week later, I received a call telling me I had been selected as one of the finalists to represent Malaysia at the final showdown.”
Mohd Saddam trained two and a half hours daily in the mornings and afternoons to prepare for the competition.
“My fitness regime included running to strengthen my legs and improve my endurance, as well as weightlifting and pull-ups to increase my upper body strength,” he shares.
But, he admits he doesn’t really have any special dietary plan.
“At the end of a long day of training, I enjoy good food, but I just make sure that the food intake is beneficial. I also drink plenty of water every day to stay hydrated,” he reveals.
To him, both physical and mental resilience are important in such events.
“The competition is unique because the obstacles, which are different every year, are designed to put your mental as well as physical strength to the test,” he says.
He adds that participants need to stay focused, have the right mindset, and plan their strategy to complete the game without penalty. They also need to have the physical strength for challenges like climbing walls or carrying big tyres.
Mohd Saddam encourages people to push beyond their limits and move out of their comfort zone. His advice is to “always believe in yourself and never give up because you don’t know what you can achieve until you try”.
To Mohd Saddam, winning the competition is just the beginning.
“My long-term goal is to run boot camps for Malaysians so that I can help those who want to stay fit and live healthier lifestyles,” he says.
R U Tough Enough is a tournament where the winner will be crowned Toughest in South-East Asia and take home US$15,000 (RM60,000) in cash.
