A Patient-Therapist's Anti-Psych Manifesto
Okay yall, I broke out my laptop for this, so buckle up, I’m about to have opinions.
I don’t owe anyone my credentials, but because I know the first thing out of some folks’ mouths is always “what gives you the right” let’s nip that in the bud right now.
I have been in and out of psychiatric care since I was seven years old. I have severe medical trauma from the experimental treatments I was subjected to, and have spent time in outpatient, inpatient, and all manner of different kinds of care. I’m also a published anarchic anthropologist, and a fully credentialed and actively practicing private therapist. To many, these are rightfully mutually exclusive roles. To me it is survival. Let’s explore some dialectics.
Dialectic: Per Merriam-Webster, a dialectic is any systematic reasoning, exposition, or argument that juxtaposes opposed or contradictory ideas and usually seeks to resolve their conflict : a method of examining and discussing opposing ideas in order to find the truth
In this case, we’re holding a few irreconcilable realities in tension with each other and working to resolve those irreconcilabilities.
Dialectic 1
Creating a class of healthcare professionals whose job is to dispense care to the masses inherently creates a hierarchy.
Any hierarchy that exists can and will become unjust under enough stress, with enough bad actors, with enough systemic intersections, if it is made so, etc.
People still need healthcare, including mental healthcare.
Dialectic 2
Because we already have unjust hierarchies involved in our medical care and research system, the question of who gets to define what is “mental healthcare” and what isn’t is inherently skewed in favor of kyriarchical** values.
Kyriarchy: a social system or set of social systems built around domination, oppresion, and submission
Many non-hierarchical forms of mental health care are devalued in our society and therefore do not receive the resources to operate at scale despite being extremely effective tools.
There will likely always need to be some form of “service” healthcare model in our society, even if it is wildly different from what we have now, because the worst person you know deserves care and it may need to be from people who are incentivized to provide it, and in privacy or isolation from others in the community.
Dialectic 3
Indefinite and involuntary detention can never be ethically or humanely performed. Period.
Some people need episodic or long term intensive care that comes from having someone available to them 24/7, and this is extremely difficult to provide at scale to an entire society in their homes, and your answer cannot be to offload the work onto relatives.
Current inpatient and residential programs typically serve, at best a holding pattern, and at their worst are breeding grounds for abuse and we will be hard-pressed to create models that do not replicate this pattern in our current systems.
We could keep going several layers deeper, but this is already getting long, so now I want to ask the next question.
These all feel really impossible to work with, Butts, you said I was supposed to reconcile all this and that feels super intimidating. What do we do with these dialectics?

One of the things I’ve learned as an anarchic anthropologist turned therapist is that if you take what we think we know now about mental health, the nervous system, and chronic stress, and look back to this moment when Maslow and the Blackfoot community tried to communicate the resiliency of their community to the world, we can learn a lot.
A huge amount of mental health care, in my experience, boils down to learning how to regulate your nervous system, provide for your hierarchy of needs in your life, including the accommodations you need for your physical, cognitive, spiritual, and social world, and seeking, traditional or non-traditional therapy, pharmacology, and/or traditional medicine for the remainder of your needs.
What I mean by this is: mutual aid is mental health care, socializing with your friends is mental health care, taking a bubble bath is mental health care
But so are practices like MAST  (link)- a non-hierarchical therapy style that allows people to support each other through therapeutic interventions via mutual aid (a genuine therapeutic concept we discuss in our training!!)
I imagine a world where we dare to question all of our assumptions about what therapeutic intervention needs to look like. Where “mental health care” looks like creating a society that seeks to meet every level of need for as many people as possible, and offers additional, voluntary community built and operated services to meet additional needs that arise.
What if we worked to minimize the need for inpatient services by providing ADL support crews for anyone who requests it? Need to just be a lump in bed for a week in order to be okay at the end? Ask for a crew to come do dishes and make meals and tidy and field calls and check in on you. Feeling manic and need someone to be your impulse control? Request one. Like theoretically these are things we can all do for each other regardless, but what if there were trained volunteers from the community, motivated and available who could be on call whenever they were needed for anybody no matter what? What if you didn’t NEED to have a friend who was available? What if you didn’t need to wonder if they would be annoyed because everyone is there by choice and by specialty?
Imagine if you didn’t have to wait until you were in crisis to call? You could just do it because you needed or wanted the help and that was fine too. Because the goal was prevention. Make sure no one gets so overwhelmed or stressed that they reach crisis in the first place. Make sure everyone has community resources.
The task rabbit mutual aid is the one I think is the most under-served in our communities. I think a lot of us are still afraid to truly take that last step into anarchic community building. After all, time is the most precious resource we have, and giving that to others without a guarantee of others giving back feels very scary. When I’ve done task-rabbit type mutual aid though, it’s always been my favorite experience, and I truly cannot recommend it enough. It provides such an immeasurable boost to the entire community’s resiliency.
I think another really useful direction is teaching yourself a little bit about polyvagal theory. It sounds like pop science, but it’s pretty cool stuff. Things like diaphragmatic breathing, certain manual manipulation techniques, etc can help you regulate your nervous system in moments of stress or intense emotion, as well as adjusting you into a better regulated state over time if you experience chronic dysregulation, such as from PTSD, ADHD, or Autism**
**This is not me saying it will cure your ADHD or Autism, it will not, but it can tone down the intensity of emotionally/autonomically dysregulated moments, or make them a little easier to end on your own time.
In the end, mental health, like so much, is deeply personal. There will be no "one size fits all" option. But we can create a society that provides a high quality standard of living for everyone, with the majority of their needs being met as a baseline, and create services that account for needs that may be episodic, additive, or unusual, as will almost certainly always eventually occur.
So the question is, when you begin to imagine outside the confines of the four walls of the psychiatrist's office

I've been thinking a lot about something concerning Romance The Backrooms, so I wanted to write it down and share it.
Content Warning: Discussion of suicidal thoughts, self harm, mental health issues, and homelessness. There is a hopeful ending.
I got the idea for RtB in October, when I was living on a friend's couch and trying to find permanent housing. That ended up being a really difficult month for me, because I self-harmed for the first time in months and, as a result, I went to inpatient.
I was there for a while, and I got better. With all the time I had in thee, I worked on creative things, and really fleshed out RtB. I couldn't live with my friend anymore after October ended, but the social worker told me that she would find me a group home.
However, when I was transferred to a different wing of the hospital, I was given a new social worker. And this one said that I couldn't get into a group home, and he was going to discharge me into a homeless shelter asap.
That resulted in a spike of suicidal thoughts. Why had I come this far, only for this to happen to me? Should I just end it? But, no--if I did that, who would be around to make Romance The Backrooms? If my light snuffed out now, that game would never be made.
I've been suicidal since I was 10 years old, and it's usually not big, grand things that make me want to stay alive. It's the dogs I would never pet and the movies I'd never be able to watch that make me say, "One more day." And RtB became my "One more day." It became the reason why I persevered.
After I was discharged, my friends told me that I still needed inpatient help & psychiatric care. So after toughing things out for a few days, I brought myself to the ER and was admitted. A few days later, I was brought to another inpatient, and two weeks later, that inpatient sent me to a short-term residential program.
It was a wonderful program, and I met some fantastic people there. I also worked more on RtB--I figured out all of my love interests, and drew them for the first time. On the characters profiles, which you can view here, there are drawings with the description, "Concept art from when I was in treatment in December 2023." These were all done while I was at the residential program.
I was able to scrounge up some money to be able to get an Airbnb after I discharged--it was cheaper than other options, and would hopefully be temporary while I searched for permanent housing.
Unfortunately, finding housing is difficult when you haven't worked in 2 years (mainly thanks to pseudo-dementia, which I've documented extensively in this game here), and don't have the money for a security deposit. I also wasn't poor enough or disabled enough to get help from the government. Very unfortunate.
Long story short, I ran out of funds to stay at the Airbnb in 2 months. I was able to get into a short-term housing program for homeless folks with mental health issues. That was when I met Kevin.
Kevin told me he could help me get hotel & housing vouchers if I went with him after our time at the program was done. I had no other options, so I did. I spent the money I had keeping us afloat at a motel while we waited for the voucher, and went across the state to a different one when Kevin told me the one we were at wouldn't accept the voucher. When he left to retrieve it, I waited very patiently for him to return.
He never did.
I suspect the whole thing was some sort of scam all along, and even though part of me knew it could be, I went with it because it was either go with Kevin, or go to the homeless shelter.
The next day, I had to check out of the motel--I had literally no money left. I gathered my bags and sat in the lobby--well, I say lobby, but there weren't any chairs or tables, so I was basically sitting on the ground in the corner of the room, with sun from the window scorching my back--while I tried to come up with a plan.
I made a lot of phone calls to people who might be able to help me, but nothing came through. Even 211 had nothing, because the homeless shelters in the town I was in were full.
I became suicidal again at that moment. Again, the eternal question: why had I come this far, only for this to happen to me? Should I just end it?
No, goddammit. If I go now, Romance The Backrooms will never be made, I thought to myself. I want to be around to make it. I want to be around to see it through. Come on, Bez. Things looks like shit, but let's keep going anyway.
Not long after that thought came to my mind, another person entered my head, someone else I could call. I wavered, but eventually decided to try calling them, and they answered. They were able to help.
It's thanks to them that I am where I am today. I have housing for a while, and I got a job that I'm satisfied with.
Best of all, I'm working on Romance The Backrooms.
I'm so happy I stayed around to see it through.

re: psych ward alternatives (so tw for psych stuff) - what do you think about PHPs and residential (for mental illness)? it seems like people tend to have less horrible experiences but then i doubt a PHP would be appropriate for someone who is actively suicidal, and residential still sounds like… not a great experience a lot of the time. i’ve seen both used for people with eating disorders in particular, do you think that would be a better alternative to psych wards or still more of the same shit?
Hey! So I have a pretty long answer to this cause I think it’s a complex topic and I’m going to do my best to answer it with nuance. You probably already know a lot of this stuff, but I’m just going to ramble a bit for any followers who aren’t as familiar. First I do just want to define the ways I’m using the different terms cause I know that these things can mean different things in different countries . When I talk about psych wards, I am referring to inpatient units, whether part of a larger hospital or not, that have the power to hold people involuntarily and is usually intended for shorter term stays for crisis stabilization. In America, this is usually the most restrictive level of care. Residential treatment is a separate facility that is focused on long term mental health treatment for weeks, months, or years, and can be either focused on a specific diagnosis (eating disorders or substance use, for example), or be a general mental health facility. Residential treatment really varies depending on the specific facility, but usually is on a secure unit with many restrictions and carceral aspects, although there are often some freedoms that are not present in the psych ward and many residential treatment centers only take voluntary patients. When I refer to Partial hospitalization programs, I’m talking about programs where you still live at home, but go to a facility multiple days a week to get group therapy and treatment while still having more freedom than either psych wards or residential treatment.
On a societal level, I don’t think that residential treatment or PHP is an alternative to psych wards that fixes the problems with psych wards. The underlying factors and approach to treatment that creates the conditions of violence and coercion in psych wards are still just as present in almost all levels of care in the current mental health system, and although the way that these things show up is going to look different based on the unique aspects of different levels of care, residential treatment and PHP doesn’t solve the larger problem. The way the American mental healthcare system is structured is so based on this pathologization model where autonomy is deprioritized over pretty much everything else. The specific abuses of involuntary treatment in psych wards is fundamentally caused by the dynamic in the psychiatric system where violence is easily justified in the name of cure. Mental illness is treated as the worst case scenario, with little room for other ways of understanding one’s lived experience and where often time the things people actually want (like access to specific meds, trauma treatment, ability to use stigmatized coping methods) is denied. The psych system is very much set up to be a “one size fits all” approach to wellness where instead of making room to understand neurodiversity as an inherent part of human experience, all mental illness is instantly assumed to be able to be scientifically observable and treatable in a way that ends up excluding many peoples lived experience of mental illness. The psych system is really not well set up to actually provide care and support to try to improve the mental distress that is very real and that people do need and want help for. Which is why I personally dream for a restructuring of the entire psych system that moves beyond the DSM, embraces harm reduction, is actively abolitionist, and empowers people to make autonomous choices. There’s no way to find that in a system based off control, birthed from eugenics, racism, and ableism, where capitalism shapes every available treatment option.
So under those conditions, residential treatment and PHP are not really a liberatory alternative to psych wards that solve the problems from psych wards. I wouldn’t advocate for more residential treatment on a policy level and act like it would be a solution to psychiatric incarceration. Residential treatment in particular still has many of the same problems as psych wards, as it really is still institutionalization and all the violence and coercion that comes with that. There is still supervision, power imbalances and abuses of those powers, and little respect for autonomy. A lot of places that are certified as “residential treatment centers” are basically just long term psych wards. Although there are some that do allow for more freedom, it’s such a broad category that it’s hard to know what to expect. PHP is something that I do feel more positive about advocating for, as it has less total control and you are not locked up, but it still works under some of the same mentalities.
On an individual level, however, I think that residential treatment and PHP can be options that individuals use as personal alternatives to psych wards in terms of their own treatment needs. There just simply aren’t enough widespread accessible alternatives for us to use as mentally ill people, and when our distress gets above a certain level where we cannot stay safe or don’t have the support we need in our communities, we often have to engage with the psychiatric system whether we like it or not. I think it’s unrealistic to pretend like we don’t have some sort of relationship with the psych system, whether that relationship is based in total rejection of any psych treatment, whether we take meds but don’t go to therapy, whether we can cope with just outpatient treatment or whether we have higher support needs that cannot be easily met outpatient. There can absolutely be benefits to mental health treatment, even under the current system—we usually just have to put up with a ton of bullshit in order to access moments of healing. And I think that residential treatment can be a better option than psych hospitalization depending on your needs, and that absolutely PHP is a less restrictive environment that can be a good option for some people. I personally have structured my approach to mental health treatment in order to avoid psych wards at all costs, but I’m more open to residential treatment because there is more risk management I can do there. With residential treatment, I could research beforehand, compare different places, and figure out what my own personal level of tolerance and nonnegotiable needs were. In my residential treatment facility, I was able to have my phone, do art, and knew that I could never be involuntarily held or forcibly restrained and drugged. For me, that worked as an alternative to psych hospitalization, but for some people, months spent in treatment is not what they need for short term crisis stabilization. Lots of residential treatment centers are also just as fucked up as psych wards and just as restrictive, so you need to be able to do research and make sure that you’re not ending up somewhere that’s just as fucked up. I think that PHP is a helpful thing that I wish was more accessible, and generally doesn’t have as many risks as higher levels of care. Even though a lot of PHP is bullshit, having the structure and support during the day can be invaluable when you’re really in distress. It really depends on your needs and what options are available to you, but I think it’s really important for us as mad/mentally ill/ neurodivergent people to be made aware of what all our options are so that we can make informed decisions.
Overall: I don’t think residential treatment and PHP are alternatives to psych wards in a societal sense because I don’t think that they solve the underlying problems of psych wards, but I think that on an individual level when you’re navigating the current system, that they can be important levels of care to consider and make informed decisions about.
