Treating a patient with a lot of audience** is iritating. It feels like I always have a practical exam during treatment time inside the wards. But then this conversation happened after treating the patient: Relative: He's already improving,right? Me: Yes, let's just pray for his faster recovery.=) Relative: Who will be his PT tomorrow? Me: It would not be me...Its my day off tomorrow ma'am Relative: I wish it would be you. *smiles* me: *smiles* Even though there are tons of stresses that's getting to me lately, this conversation warmed my heart and reminded me why I chose this profession. We Physical Therapist do not just treat patients. We do TOUCH their lives, and how we interact with them can encourage or discourage them in getting back to their tracks. **Relatives/Visitors- It is actually almost required that patients in out country have atleast one relative with him/her in the hospital at all times. Visitors have limited hours though

I have to start by saying that all of your work is as inspiration! Princess Murders The Hero is fantastic serial fiction & has spurred me to finally start outlining & worldbuilding for fictional series I’ve had in my head for a long time. My only regret is that you’re one of many terrific fiction & RPG creators whose Patreon I’ve yet to subscribe to due to my personal wonky income & awful budgeting which I’m hoping to change this year.

My question actually was: Did you have any plans to self-publish a collected ebook version of Book One of PMTH similar to your other novel Stars Without Brightness on your Itch.io page?

PMTH Morning Report: NK Cells

Good morning staff, zombies, and minions of Princeton Medbloro Teaching Hospital!

I am a baby medblr here, taking a stab at my first morning report. Today's topic is...wait for it...my favorite cells...natural killer cells! Those bad ass little buggers whom you can thank your lucky stars for playing a role in keeping you a healthy, and relatively happy, humanoid. 

Give them credit because they truly are:

These lymphocytes don't get as much credit as their ever so popular B and T cell counterparts, but don't let the absence of genetic rearrangement deter your from what these gems do. They are a part of the innate immune system, and they are ready for action! Watch out infected cells, watch out neoplasms, NK cells are hunting you down!

How do you identify one of these super duper cells you ask? Well, lucky for us they are waving a giant flag. Thanks CD56! Of course, there are other markers but I like this one the best. Anyway, some NK cells let their CD56 shine more than others. What do I mean by that? You can see it with the use of our friend,flow cytometry!

But don't think CD56 is all lonely on the surface...oh no. There's a party going on. We have NKG2D getting all flirty with danger, and revving up the engines. All the while NKG2A is flirting' with the HLA's (Human Leukocyte Antigen) on cells passing by, and calming shit down. 

However, our party would not be complete without the KIRs (Killer Cell Immunoglobulin-Like Receptors) making everything more interesting. These aren't generalists like NKG2D or NKG2A. Oh no, these dudes are picky. KIR2DL1 is looking for HLA-C2, KIR2DL2/3 is looking for HLA-C1, KIR3DL1 is looking for HLA-Bw4. But shhhh there's a little secret that people don't like to talk about...these KIRs can be frisky sometimes. I just listed the inhibitory KIRs, but substitute an S for the L (ex. KIR2DS1), and you've got yourself an activating KIR!

Yes, Yes, I know there is a lot of random information here. It's important though because whether or not an NK cell kills, and how activated it is, depends on the BALANCE of those activating and inhibiting signals. 

Why do we give a shit about this? There are lot's and lot's of reasons, but I'll list one. Tumor immunology! Turns out information about somebody's HLA and KIR genotypes may be able to predict a patient's response to immunotherapies, and maybe even outcomes in cancer patients. Pretty neat huh?

I shall now implement some Leslie Knope-level presentation skills to explain why I should have money to procure Jaegers. With the acronym:

J.A.E.G.E.R

J-Joy. Because who ISN’T going to be happy piloting a jaeger? 

A-Adaptable. Do you need hospital protection in the psych ward? Perhaps someone to guide helos with trauma patients? Maybe a hand to grab a thoracentesis kit from the highest shelf? A Jaeger can do ALL of that!

E-Energy Efficient. Nuclear energy! Super safe nuclear energy I SWEAR

G-Game Changer. We will definitely be sticking it to every teaching hospital. EVER.

E-Eradication. A jaeger will be more thorough than permethrin, more deadly than ivermectin. I’m sure I can find some studies proving me correct.

R-Resident satisfaction. Hell, not just the residents. We will ALL be satisfied!

However, if someone were to hypothetically have a heart attack after eating doped brownies and was dying in the parking lot, they'd probably go to CCU. But after a few days that person might also have been discharged home and would be going to cardiac rehab 3 days a week...

A work of 2 years in the making, it's not very long! Just over 100k words. Book 2, which I'm outlining right now, is bound to be a bit more sprawling than this.

A lot of Book 1 is setup, but I think I've done a satisfactory job of getting all the characters situated and determined: the three major characters: the WARRIOR, the PRINCESS, and the HERO, representing Violence, Royalty, and Compassion, the three spokes of Glory. There's actually two more spokes to Glory: Wisdom, represented by the WITCH, and Community, represented by the WARLORD. you'll get to see them in Book 2!
