Isn't the new vaccine (Shingrix) far more effective and for ages 50+? I've had luck with many patients having insurance help with the cost, so it's not as expensive. Honest question. - an inquisitive NO

@tehallicatz0rs Shingrix is supposed to be much more effective than the live vaccine. Like 96% with shingrix vs maybe 60% at best with the old (and way lower efficacy in the very old)

That's exactly what I thought, but isnt the age requirement also lowered because of improved efficacy? I was told 50+ for Shingrix. I'm not disputing the "needs zoster vaccine for herpes that not zoster" statement.

Sadly Shingrix is not registered in the state where I am, it is registered within the EU, and according to my pharmacy gal it can be imported as aĀ ā€œunregistered medication for special purposesā€. Not sure how much is it gonna cost tho.Ā

Also obesity covers some screening things - screening lipids and A1Cs usually. And @irazuexperiment I do my own PAs too (though my partners don’t). I don’t have as many as they do though because you learn what will get covered and what won’t and you learn what will get a PA approved. It’s a good exercise, though in practice it does take up valuable time I don’t really have.
Yep learned that (and used it today when ordering labs).
My big gripe was that I was doing prior auths and hunting down acceptable ICD-10 codes for other residents’ patients, because that’s how our program does it (and I had twice as many to do because the resident assigned to the task the day before just...didn’t do any of them), and it was very frustrating since all of these things could’ve been avoided in the first place.
But hey, I’m learning. And telling the interns what I’ve learned so they’ll stop creating this work for me to do haha.

Post-SOAP I found a TRI/TY in New York
Never been luckier to be a DO pre-merger, although the program is only now applying for ACGME accreditation so I’m not sure how this will affect me looking for PGY-2 positions or doing aways

Treating relatives
I am curious how other medical professionals have had to deal with treating or taking care of relatives/loved ones. This could go from anything to the sniffles to terminal diseases. I have fortunately not had to do this yet, but will probably feel obliged to at some point.Ā

(Scene: Urgent Care exam room #3. Cranquis is performing vital medical activities requiring years of training and experience typing rapidly in the electronic medical record, while a teenaged patient and her mother wait to be discharged.)
Patient: Hey ma, can I have some money to buy bacon on the way home?
Mother: Ha! No.
Cranquis: I’ll be right back with your discharge papers.
Cranquis:Ā Ok, here’s your note for school, here’s your instruction sheet, and here’s a small ā€œWill Work for Baconā€ sign I printed out for you, just in case.
Patient: What?!

Hi WayfaringMD! I'm not asking a question so I guess you can file this under random encouragement. :-) First, I love your blog- I just discovered it recently. I'm a 30 yr old non-trad current med school applicant who wants to go into Family Med and is also a Christian. So I've had a lot of fun reading through your blog. It just makes me all that more excited to get started! Also, I love your sense of humor and I'm pretty sure if we ever crossed paths we'd be friends. :-)

Please help me Medblrs! Tell me your opinions about oto/ophthalmoscopes
I need to purchase an otoscope and an ophthalmoscope for medical school (want to buy one base with exchangeable heads) but there are so many options and my professor said anything is fine when I asked for recommendations. I’ve heard the panoptic ophthalmoscope from Welch Allyn is great but that’s over-budget.Ā

Hi, i hope you're doing well. I am a 4th year med student and i am having issues regarding research. Could you or any fellow medblrs guide me about it. The place i am from, its extremely hard for me to find good supervisors. The lectures we have had on research are very theoretical. I tried to look around for courses but most of them are super expensive. All in all, it has been really difficult for me to start a good research project and i would really appreciate some words of guidance. Thanks!
Hi anon! Hope you're well too!
I'm sorry to hear about your research struggles. But I'll be completely honest up front, I'm not at all the best person to ask about this. I'm currently in my fourth and final year of med school, and here we have a different system of going about higher degrees in medicine which is extremely exams / entrance oriented. That aside, I've personally not looked into research options till now, so I'm pretty much not equipped to give you any directions, sorry.
I'm calling upon my fellow medbr/gradblr people @docresa @cancerbiophd @loreleys-road-to-md @wayfaringmd @drdesertfox @dxmedstudent @quilavastudy help out (and of course, anyone else who has any info on medical research to share, please do help out anon!)
If you could also mention which country you are from, I'm hoping someone else from the same place /with similar experiences can also help out!
A few general ideas I can think of : have you tried asking your professors / seniors / peer circles? Even contacting your college education guidance office / similar people can help, maybe? Most of the times they'll have recommendations to ongoing research programs in your country/elsewhere you can enroll into.

Hey human docs, question for you. One of my clients was told by her physician to give a particular supplement to her dog for her separation anxiety.
What are the rules about this? I can't make recommendations for humans so I don't understand why a physician would be ok making med recs for pets. But my boss thinks because pets are legally property, the same rules may not apply.
Thoughts? I mean given that dogs and cats metabolize meds and supplements differently than people (like Tylenol can kill a cat) this seems irresponsible at the least.
