My evil twin is my alibi
I am still on hiatus…kind of…I just wanted to post this draft before heading back to tumblr post hiatus
Bridgette Cheng is happy to visit her cousin in Paris.
Really, with just a pair of blue contacts, the two could be twins.
But while Marinette is naturally sweet and a fashionista, Bridgette is a wolf in sheep’s clothing who will not hesitate to bite you if you provoke her. She is actually quite popular at her school, and a mischievous prankster with a clean record of never getting caught (she is an expert at manipulating security footage). She is a lot like Megan Parker. Sweet to everyone else but an evil genius to her brothers.
When Marinette admits she is being framed by Lila and her friends don’t know who to believe, Bridgette suggests that the middle ground won’t work. Perhaps a schedule will do. Keep Lila and Marinette away from each other.
Of course Lila won’t settle for this.
Meanwhile Marinette obligés Bridgette’s request not to be mentioned. Apparently Bridgette isn’t keen on being questioned about her life in China or something like that.
When Lila knows Marinette is busy sewing or baking, she scratches herself and claims Marinette did it.
Only for the class to be confused because Marinette (Bridgette) had been with them the whole time.
Then while Marinette is hanging with her friends, Bridgette actually bullies Lila by tripping her into dog poop or pouring water on her outfit, claiming it was an accident. Not that there is anyone around to witness this, still a girl can’t be too careful.
But when Lila angrily accused Marinette, the class are still her alibi.
Lila should have been akumatized, but Bridgette happily caught an akuma and is planning to keep it as a souvenir.
Bridgette pranks escalate. Hidden joy buzzers, dead animals found in Lila’s locker, sleeping pills added to her water bottle.
Pretty soon, Ms Bustier believes Lila’s lying disease is making her act against Marinette. And her new attitude of sleeping in class or showing up to class in a mess doesn’t suit Ms Bustier. She suggests that Lila be transferred to a different class or even a new school. Dupont doesn’t need a student with a horrible record.
Lila agrees wholeheartedly. By now she is terrified of sweet Marinette. (Bridgette really scares her with that fake smile and cruel voice). No one believes her now. She is like the girl who cried wolf.
And when Lila is sent to a hospital because Marinette (Bridgette) claims she worries the school clinic won’t be able to cope with Lila’s many illnesses, the doctors give her a clean bill of health and refute her many disabilities, calling her a compulsive liar. Liar is expelled and forced to attend therapy.
Bridgette leaves Paris satisfied. She got to hang with her cousin and prank someone.
Only…someone stole her akuma souvenir!
(It was a Sentimonster)
But too late. Prisons and therapy centres are heavily guarded against akumas.

How to get HRT in the UK?
so basically it starts by talking with your GP. Swap your GP if you have a hostile one. They should refer you to a specialist regarding gender dysphoria. From there, you’ll get hormones. The wait time for a GIC is over 2 years, everytime a GP needs to ask a GIC specialist. There are occasionally reasons to get bumped up, but a lot of people have died waiting.
these are articles about wait times:
https://www.theguardian.com/society/2019/feb/26/trans-patients-in-england-face-soul-destroying-wait-for-treatment
https://www.gaystarnews.com/article/mike-webberley-suspended-trans-kids/#gs.v6ce8d (I do not know how this affects the availability of private practitioners, but this decision stopped the healthcare 1600 patients were getting.)
https://www.bbc.com/news/uk-wales-48093698
this is a resource i’m using for this. it’s from february 2017. the articles on the wait times are from 2019.
https://www.mermaidsuk.org.uk/assets/media/17-15-02-A-Guide-For-Young-People.pdf
Either way, you will need to bring information & plan what you are going to say. therefore, I’m going to answer this ask by giving a lot of links & also some transgender history so you can catch your practitioners up on understanding who you are as a transgender person. i will probably be giving a more “tucute” explanation since “truscum” explanations seem to be mainstream in the UK & so if your needs do not conform to “truscum” explanations, you’re more likely to get pushback & hence need more help.
It appears that “dysphoria is a requirement” is relatively mainstream in the UK. Personally, I got  dysphoria just reading how the beginning of this document by Mermaids conflates body modification as a requirement for validation for trans women & men & lack of as androgyneous/genderqueer. body modification is valid, but yikes! Then again, Mermaids might just be describing what you’re about to wade through, I do not know.
From the Mermaids Guide For Young People (PDF page 19-20, printed page numbers 17-18)
How to get treatment
Getting medical treatment as a trans person means that you will have to see quite a few doctors and gender specialists. ‘Treatment’ may be defined as receiving counselling, hormones, or even surgery. It’s up to you to decide if you want any of these things. Your GP is the first person you will need to visit to get the ball rolling. If you are going to do this, it may be helpful to plan a strategy. Work out what you want to say beforehand. • Bring general information with you that will explain your situation clearly. • Have a clear idea of what you want from the appointment and be direct when asking for this. (For instance, a referral to a gender clinic.) • Be realistic in what you expect to achieve from your appointment. • If your GP isn’t supportive or helpful don’t be afraid to swap and remember you do not need to give a reason for doing this. PALS can help you and can be contacted through your local Primary Care Trust or GP Surgery.
NHS vs Private
Some trans people opt for the private route instead of NHS. There are advantages and disadvantages to each of these routes, and just because you start on one it doesn’t necessarily mean you cannot change part way through. Although it is considered unethical (see BMA.org.uk) you may experience some difficulty going from private back into NHS routes.
Pros & Cons
NHS
+ It’s free.
- There are long waiting lists for NHS gender clinics
- You will have less control over timescales/treatment.
- You may not have much choice over surgeons/procedures.
Private
+ Will not have to wait long for treatment.
- It can be quite expensive.
- You will have to pay for consultations with specialists consultations with surgeons and surgery itself. You may have to also pay for hormone prescriptions.
+ You have a choice of surgeon if you choose to have surgery.
+ You will be treated with respect.
Hormones
Sex hormones are responsible for the changes in human bodies that produce male and female characteristics. The male hormone testosterone is produced in the testes and the female hormone, oestrogen is produced in the ovaries. For some trans people hormone therapy can be an important part of their transition, but may not be for everyone. In the case of MTFs, oestrogen is often prescribed to feminise the body and hormone blockers are given to halt or regress masculinisation. In the case of FTMs, testosterone will bring on many of the male secondary sex characteristics.
It is important to take responsibility for yourself as to whether hormone treatment is right for you. Changing the makeup of your body is one of the most serious decisions you may ever make and much consideration must take place before reaching a decision. In order to aid with this most doctors would recommend between 3 months and 2 years’ “real life experience” (living in your chosen gender) in order to make sure you want to live full time as the man or woman you feel yourself to be. Without hormone therapy, however, passing can be difficult for some. As with any prescribed lifetime medication it is advisable to maintain a healthy lifestyle, exercise and a balanced diet, as well as not drinking excessively, taking drugs or smoking. Hormones might not be prescribed if you are not in good health. You will need to have regular blood tests and will be referred by your GP to an endocrinologist.
because one of the things the mermaids guide recommends is collecting information, i would suggest giving this article from 2003 that discusses the insanity of the lived life experience requirement: http://www.trans-health.com/2003/real-life-test/ it basically says that when it originated,
there was no research before it became a thing. it was merely the experience a wealthy donor to trans research (who was FTM) had with his doctor. (so already, the data was likely restricted to trans men & the effects of testosterone. for trans men, a lot of transitioning can be handled with HRT, while undoing a testerone puberty is nowhere near that easy.)
hormone replacement therapy during it was considered valid & that the test was for genital surgery not hormone replacement therapy.
on top of that the real life experience test was developed circa 1966-1969 when women had significantly fewer civil rights (in the USA where this test was developed women were banned from opening credit accounts, serving on juries, could be blocked from universities on the basis of sex/gender, be fired for gender non-conformity, etc. basically being a housewife was the default expectation.https://www.cnn.com/2014/08/07/living/sixties-women-5-things/index.html so basically if a woman didn’t like living under sexist patriarchy she was told to either conform or pretend to be a man.
also in that era, there was a perception that “transsexual” people were basically so gay they wanted to be straight (& that assumption didn’t get stopped until Lou Sullivan died of AIDS like a lot of gay men in his era.)  but likewise if a heterosexual transsexual woman was merely into men/androphilic they could figure out they were a gay man instead of a straight woman. (bisexuals & asexuals, omg where they disregarded.)
also consider (to refuting lived life experience before HRT) that we use our ID’s a ton in life. so having this lived life experience when we don’t have corrected ID documents is backwards.
i gather you can change your legal name rather easily, (with a deed poll), but not your gender marker. granted, availability of ID gender marker correction (such as self-determination) is not exactly within your control, but it should stress to your doctors how this is literally the way you need to transition under current UK law. https://www.ngalaw.co.uk/knowledge-centre/changing-your-legal-gender-in-the-uk  (page 17-18 of the PDF, page 15-16 of the printed document under the header Names.) for more information again, read The Mermaids Guide.
also note that only binary gender identities are considered valid under current UK law & thereby there are no nonbinary marker option.
Good Luck, Peace & Love,
Eve

Week 2 : Still going through a little less than alOT
Green Monday’s beat Monday blue’s anyway! It’s only the second week of fieldwork and I’m pretty sure my stress levels are almost at capacity. A lecturer told my class, that when we do anything new, stress always comes with it. I’m really holding onto this right now, hoping that it will get easier as we go along!
Wow! I find it difficult to believe that there’s only 7 hours that we spend at prac on a Monday because of how much happens during the day. I did a personal hygiene and grooming session with my client, this being something that was of importance to her especially because her partner visits her often, I found that I was able to meet my secondary aims quite easily because it was linked to my principles in my activity session write up. With Muscle Tone, I’m currently at frenemy and we started at enemy, I’ve found that with the NDT practical’s and the tut that we did with my supervisor, I’m becoming more comfortable with it, far from friends but better than where we started. During the session, I was able to incorporate the clients affected hand and use weight bearing to improve tone. We completed all the activities that we set out to do and the way in which I presented my activity, by means of the client and I writing a list, assisted the client in remembering what and when we were doing each activity. Nail Painting was used as an incentive during assessments and as part of the treatment session. This particular activity solely helped me to build my therapeutic relationship with my client and provided weight bearing to an extent, besides that it was a mess but thoroughly enjoyed by my client! I should have incorporated a mirror in my treatment session and structured my activity more realistically. Due to my clients impaired attention, it’s more suitable for me to do one activity at a time than various small activities and after a particular period of time, divert her attention to the activity at hand rather than dwelling in the distraction. I’m glad that my supervisor was able to observe my session and give me feedback, due to this I am able to improve my performance, handling and hopefully pick up therapeutic opportunities more easily. It’s not Throwback Thursday or Flashback Friday, but last week’s blog topic was integrating theory into practice, and we’ve just practiced supine to sitting on Friday during the NDT practical and it’s one of my favourite movements (rereading this I don’t even know why I have a favourite movement) and even with hypertonia in the affected lower limb and impaired trunk control, bed mobility wasn’t as easy as doing it with my classmates but it was manageable and I felt a slight sense of achievement in knowing that I knew what I was doing as well as that I was in complete control and my client was safe.
The client centred approached, developed my Carl Rogers, focuses on the client having an active role in their treatment whilst the therapist has a non-directive and supportive role. As an OT student, client centred means aiding my client in Areas of Occupation that are important and significant to the individual and improving their client factors through the areas of Occupation that are important to them and thus creating a balance than enables both, the therapist and the client to achieve their desired goals. For one of my clients, being able to bath his lower limbs is insignificant to him, when I asked him if he’d like to learn this technique, he refuted saying that it would delay his breakfast that would in turn delay him going for his group session and I therefore had to structure my intervention accordingly.
At the facility, I met an incredibly friendly women who is my clients ward neighbour, she’s been at the facility for more than 2 decades, she therefore knows everything and everybody at the facility, and because of the impact that therapy has had on her life, she tries “to do therapy with others”. For instance, she’s always prompting my client due to my clients impaired memory and motivating her to help herself. She gave a number of ladies colouring to do over the weekend and told me how difficult it was because for one of the ward members that she had to hold her hand and colour in. I’m happy that therapy can have such an impact on peoples live and that I get to be part of this profession, but its not that that I find heartening, I find it heartening that this woman, I’m unaware of her psychosocial or environmental factors but she has managed to make a life for herself at the facility and in that helps others too. What I am happy about is that through this profession I get to see magical thing and amazing people. This lady made my day and has been at the back of my mind since and for some reason maybe when Dr Seuss said “Oh the places you’ll go, you’ll be on your way up! You’ll be seeing great sights! You’ll join the high fliers who soar at high heights.” Maybe I don’t have to look so far because there’s enough great sights and high fliers all around me.
Until next week, Z!

The fear triplet
Tumblr media
The fear of going nuts, the worry of letting go in sex-related climax as well as the fear of dying-- in these three worries humanity has actually lived for thousands of years. They are not personal, they are cumulative. They come from the cumulative unconscious.
Fear of going crazy
The anxiety of going bananas is in every person. For the straightforward factor that the knowledge has actually not been permitted to establish. Knowledge threatens to the vested rate of interest. So for countless years, they have been cutting the very roots of intelligence.
Man lives nearly uprooted. He needs to be made rooted out, to make sure that he can end up being depending on the society, on the culture, on the faith, on the state, on the moms and dads, on everybody. He needs to depend. He himself has no origins. The minute he realises that he has no roots, he feels he is going crazy, he is going outrageous. He is shedding every assistance, he is falling right into a dark ditch ... due to the fact that the understanding is borrowed, it is not his very own. The respectability is borrowed. He himself has no respect for his very own being. His entire character is borrowed from some source-- the college, the church, the state. He himself has absolutely nothing of his own.
The parents state if you don't follow us, if you disobey, you will certainly be condemned.
So everyone takes place holding on to the group, continuing to be part of a faith, a church, a celebration, a country, a race. He hesitates to be alone, which's exactly what you [therapists] are doing when you bring him to his own depths. All that crowd, all those links, disappear. He's left alone and there is no one else on whom he has constantly depended.
He has not grown his very own knowledge-- that is the problem. Unless he begins growing his very own intelligence, he will always remain scared of being crazy.
To therapists
In deep treatment the concern grips unexpectedly, since the individual is losing all the props, sustains, the group is vanishing farther as well as further away, he is being left alone. And also suddenly there is darkness as well as there is concern. He has never ever been trained, disciplined for being alone which is the function of meditation. No therapy is total without reflection, since just reflection could give him his lost roots, his strength of being an individual. There is absolutely nothing to fear. The conditioning is that you have to be afraid each moment, at each step.
So aid the individual to comprehend that this is nothing to be worried, there is absolutely nothing to be worried. It is a produced anxiety. Every child is born brave. He can have fun with the serpents without fear. He has no concept of concern or fatality or anything. Reflection brings the individual back to his youth. He is reborn.
Fear of sexual orgasm
Sex is man's whole energy, his life power, and religious prophets and messiahs, carriers of God, they all are doing the very same job in various words, different languages, but their work coincides ... to earn man an adversary of himself.
And the standard strategy is-- since sex is the most powerful energy in you-- sex should be condemned, guilt needs to be developed. Develops a problem for the individual.
His nature is sensuous, sex-related, and his mind is full of rubbish against it. He remains in a split. He can't go down the mind, due to the fact that going down the mind means dropping the culture, the religion, God, everything. He's not qualified to do that unless he has ended up being a private and is able to be alone with no fear.
So male hesitates of sex as far as his mind is worried, however his biology has nothing to do with the mind. The biology has its own means of operating, so the biology will attract him to sex as well as his mind will be standing there continuously condemning him. So he makes love, but quickly. That hurry has a very emotional factor. The hurry is he is doing something incorrect. He is doing something against God, against faith. He is really feeling guilty as well as he could not take care of not to do it, so the only concession is: do it, but be fast. That prevents the orgasm.
Absence of orgasm, males and women
A guy who has unknowned orgasm feels unfinished, disappointed, upset, because he had never been in a state, which nature gives freely, where he can have relaxed totally and turn into one with the existence, at the very least for a couple of moments.
Because of his hurry, he can not handle the orgasm. Sex has actually come to be comparable to climaxing. That is not real regarding nature is worried. Climaxing is only a part, which you could handle without climax. You can replicate youngsters, so biology is not bothered with your orgasm. Your biology is satisfied if you duplicate kids, and they could be replicated just by ejaculation, there is no demand for orgasm.
Orgasm is a significant present of nature. Male is deprived as well as since he is so quick in making love that the lady is deprived. The lady requires time to warm up. Her whole body is sensual, as well as unless her whole body is throbbing with delight, she will not have the ability to experience orgasm. For that there is no time.
So for numerous years women have actually been totally refuted their bequest. That's why they have actually ended up being so bitchy, so constantly unpleasant, always all set to deal with. She's being deprived of her whole opportunity of blissfulness. Then she ends up being negative.
Orgasm and meditation
Orgasm is going to give you your origins, which have actually been removed from you. Don't be stressed. As well as blended will be the fear in orgasm to go bananas. If in climax one goes bananas, aid him to go bananas. Only after that he will certainly have the ability to have it in its completeness. The climax kicks back every fiber of your mind, your heart, your body.
Tumblr media
It is tremendously vital for reflection that an individual has the experience of climax. Then you can make him understand what reflection is. It is an orgasmic experience with the entire presence. If the climax can be so lovely therefore helpful, so healthy and balanced with a single person, reflection is getting involved in unity with the whole that borders you, from the smallest blade of yard to the largest celebrity, numerous years away.
Fear of death
The initial [concern] is of being alone. Much of the concern of death will be ruined by the very first experience of being alone as well as having no worry. The remaining much of the concern of fatality will be instantly damaged by the experience of orgasm, due to the fact that in an orgasm the individual goes away. The vanity is no extra. There is an experiencing, but the experiencer is no more.
These first 2 steps will aid to address the third step extremely easily. As well as with each step you need to go on growing his meditativeness. Any type of treatment without meditation can not aid much. It is just shallow, touching occasionally, as well as quickly the male will be the same once more. A real transformation has never ever happened without meditation, and these are stunning circumstances as for reflection is concerned.
So [specialists should] make use of the initial to make him alone. Utilize the second to provide him courage and also inform him to drop all thoughts, just go crazily orgasmic. Do not trouble exactly what takes place. We are here to care for you. With these two actions the third will certainly be very simple. That is the simplest. It looks the biggest concern of guy. It is not true.
You aren't sure fatality, exactly how can you hesitate of it? You have always seen other individuals passing away. You have never seen on your own dying. Who knows, you are possibly the exception, since there is no evidence that you are going to die. Those that have passed away have actually offered proof that they were mortals.
Death can be joyous
Experiencing fatality-- people dying in disgust, in misery, in suffering, in all type of discomfort, seniority-- that offers you the concern of fatality. Because nobody has actually recognized the death of an informed male, exactly how perfectly he passes away, just how joyously he dies. The moment of his death is of significant luminance, silence, as if delight is emitting from every pore of his being. Those that are near him, those who have been fortunate to be near him, will be simply amazed that fatality is much more marvelous than life has ever before been.
But this type of fatality takes place just to individuals that have actually lived absolutely, without fear, who have actually lived orgasmically, without bothering regarding boneheads and just what they are stating. They [these boneheads] know nothing concerning it, as well as they take place stating about it.
The worry of fatality will certainly be the most basic out of the 3. You have to solve the very first two, and afterwards you tell the person that death is not the end of life. If you practice meditation deeply as well as reach to your innermost centre, you will instantly locate an everlasting life current.
The bodies ... there have actually been numerous. There have actually been lots of kinds to your being, however you are all the same. But it needs to be not simply a belief-- it has actually to be made their experience.
Meditation, the supreme solution
So remember one thing: your therapy groups ought to not be normal therapy-- just somehow whitewashing and providing a man a feeling that he has actually found out something, he has actually experienced something as well as after a week or more he coincides. There is not a bachelor in the whole world, who is absolutely psychoanalysed. And also there are hundreds of psychoanalysts doing psychoanalysis, and not a single instance they have had the ability to complete yet, for the basic factor since they have absolutely nothing to do with meditation. And without reflection you could go on paint externally, however the inner reality remains the same.

MenoPAUSE – The Beginning Of A New Period
Tumblr media
The word Menopause is the completion of your periods and is a crucial postpone that every woman must bring to back off and plan for a very surprising period in womanhood. In this manner, welcome this new time and all the time your body spends to prepare for it.
The Menopause indicates the completion of the conceptive cycle. This is undeniably a critical change and adjusting to it may not become alright for certain women. The most broadly perceived reactions of menopause are mental scenes and vaginal dryness. Weight get and moderate absorption are also two or three diverse symptoms. Discernment and care are two key qualities that make menopause a smooth ride. We should explore what Menopause is about.
Menopause is the enduring discontinuance of the period considered to happen when 12 menstrual cycles are missed. Menopausal change (peri-menopause) is the period between the start of sporadic menstrual cycles and the last menstrual period. There are checked fluctuations in hormones, estrogen level will when all is said in done fall, while FSH (Follicle Stimulating Hormone) and LH (Luteinizing Hormone) increase.
What's the enormous change?
The best change your body encounters during menopause isn't the stopping of your periods yet the modification in Hormone levels. This is really why women experience such a noteworthy number of reactions. The drop in estrogen and progesterone, by and large, affects a woman's body some of it.
Effect on Weight and mental health: Women all things considered store fat in the thighs and hips. Nevertheless, the drop in estrogen levels may incite weight gain in the stomach or waist giving them an alteration in their appearance, driving low certainty. A couple of women, can't adjust to this and become less social.
Fragile bones: Just more than 55 or 60 is the age most women make osteoporosis. Lower estrogen levels furthermore realize a loss of bone mass or bone thickness, making them unprotected against joint torment and weak bones. This extends the peril of women to osteoporosis or bone split.
Urinary Tract Infections: Decreased estrogen levels in like manner lead to the reducing of the urethra. As estrogen can trigger the making of antimicrobial proteins for the bladder, lower estrogen levels leave the bladder defenseless against pollutions and a couple of women may experience repeated occasions during menopause.
Pity: Easier said. Your mother, relative, sister or mate who is in their 60's has each benefit to yell in case they don't find the house by and large or if they have to move toward you for something twice. The normal mentality issue during menopause is Depression. Estrogen is said to assemble serotonin, the invention in the mind that lifts and coordinates your mentality. Lower levels of estrogen play ruin and from now on mental scenes and instability.
About 20% persevere through the gloom, more during peri-menopause (The advancement stage to menopause and starts a long time before menopause, typically toward the beginning of the forty's, for specific women it starts in the late thirties) than late in post-menopause. Perspective issue rising in association with menopause is managed by basic thought providers, gynecologist and a specialist working in an organized exertion.
Rest issue: About 40-half suffer rest disrupting impact, by and large on account of hot flashes achieved by estrogen deficiency. Before starting treatment various purposes behind mien issues, for instance, hypothyroidism should not be blocked.
How to treat signs of Menopause?
Menopause is a trademark wonder of a woman's body encounters. Changes that go with menopause are the result of this trademark change. Mental scenes and melancholy, in any case, stay a test for certain women and they are moreover set to altogether influence the rest of the family. A huge bit of these movements can be overpowered with some obstruction and tirelessness from the people around a woman who is encountering menopause. Regardless, the clinical bearing is indispensable in extraordinary cases.
Treating Depression:
In case major must be treated with antidepressants, for instance, SSRI. In case smooth and associated with hot flashes during peri-menopause or after cautious menopause, hormone treatment has an occupation.
Hormone Replacement Therapy (HRT) for melancholy is so far asking to be refuted.

Healthcare Big Data Analytic - Brief product and services showcase
UnitedHealthcare: Detect Fraud, Treat Diseases, Making Customers Happy
UnitedHealthcare has found several ways to use analytics. The firm studies social networks to detect potential cases of medical fraud and identity theft. It looks at speech to text call centre data to find likely attrition candidates – who sound different from happy customers and propose remedies. It attempts to forecast the probability of success in certain disease management programs as patients react differently to therapy schedules. Finally, from these analytical procedures, UnitedHealthcare and OptumInsights are researching prospective sources of income.
Cigna: Analysis in ‘Omni-Channel Environment’
Like UnitedHealthcare, insurer Cigna is also analyzing call centre speech to text data to improve customer service. Beyond that though, Cigna conducts analysis in what is called as the “omni-channel environment” by looking at the Web data and information from other external sources. The insurer also tests and then analyzes procedures that happen over the telephone to manage the disease.
McKinsey and BeyondCore: Identifying the ‘next 5%’
Approximately 20 percent of physicians are liable for all U.S. healthcare expenses. These patients often have various chronic circumstances, which makes it difficult to cut costs. Although the' next 5 percent' may pose important possibilities for saving. A joint assessment of McKinsey's 30 million business complaints and analytics provider BeyondCore recognized countless microsegments–heart problem patients and broken legs, or patients with arthritis or septicimia–that could cost a hospital unit as much as $100,000 per patient. A hospital with 500 beds can see only 19 such patients a year. A single full-time care management that arranges house visits and helps patients maintain appointments and bring medicines can save a lot.
Express Scripts: Help patients manage Meds
Patients filed more than 1.5 billion prescriptions annually with Express Scripts, a pharmacy benefit managing organization. The company is using data about these transactions to drive both behavioural changes and process improvement. Patients may receive customized messaging about less expensive ways to refill prescriptions, for an example. Meanwhile, Express Scripts’ predictive analytics can identify patients at the greater risk of skipping or otherwise missing doses and then proactively intervene.
Partners Healthcare: “Intelligence System for the EHR”
The Boston based Partners HealthCare system – which brings together Brigham and Women’s Hospitals, Massachusetts General Hospitals and Harvard Medical School, among others – is connecting its financial, operational and clinical analytical systems. Partners have developed the Queriable Patient Inference Dossier, or QPID, to support this effort, which it describes as an "intelligent system for the EHR" that allows for real-time queries, analytics and reporting at the point of care.
Intermountain Healthcare: Evaluate Health Outcomes
Intermountain Healthcare, a Utah-based system of 22 hospitals, 185 health groups and an affiliated insurer, has partnered with Deloitte on two tools that use a provider’s vast collection of Electronic Health Records (EHRs) – more than 90 million in Intermountain’s case – to perform health outcome analysis. OutcomesMiner, announced in July, helps organizations better understand how certain factors contribute to patient’s outcome, these researchers see how empirical supports or refutes the hypothesis. PopultationMiner, meanwhile helps users study the relationship between treatment and outcomes, with the aim of developing new medications or improving existing ones.
Farsite and Wexner Medical Centre: Reducing Readmission after Heart Attack
Medicare reimbursement for acute cardiology favours continuous care over episodic, rush to the hospital care. But less than half of patients hospitalized following a cardiac episode complete semi-weekly rehabilitation for 18 weeks. Behavioural economics suggest that simply telling patients to participate won’t work. To improve patient engagement care OSU (Ohio State University Wexner Medical Centre) and analytic firm farsite studied what makes patients skip appointments, factors include comorbidities, transportation and seasonality such as dark winter afternoons demotivates patients. In a subsequent trial model, patient’s families would send encouraging text messages over the 18-week rehabilitation period.
Camden Coalition of Healthcare Providers: All-payer claims database
The all-payer claims database unites public health data (namely birth and death records) with service dates, procedural codes, specialist seen and charges, all gleaned from claim data. The challenge: acquiring information from various organizations as more than one healthcare provider is visited by patients. Starting in 2002, the healthcare providers' association of Camden (N.J.) gathered information from the three main hospitals in the city, encrypted and standardized it, combined it with GIS information, and lastly recognized large users of healthcare facilities in Camden. Critically, linking data sets helped researchers identify 961 high utilizers across the three hospitals – nearly 33% of the total number of patients – who would have been otherwise missed if each hospital’s data remained siloed.
Children’s Hospital of Philadelphia: Rules Engine Atop EHR
At the recent Strata Rx conference, the Children’s Hospital of Philadelphia demonstrates its Centre for Biomedical Informatics (CBMi) care assistant. Built using the JBoss Drools Rules Engine, the care assistant is a clinical decision support framework that uses 1500 rules to search 200 variables to return recommendations to the hospital’s EHR users. In an 8-month trial involving more than 1500 premature new-borns, clinicians use an advanced care assistant version more likely to use the correct growth chart and development milestones, administer an RSV vaccine and recommend a blood pressure screening before the baby’s first birthday.
Public Health Data: Valuable, but proceed with caution
The doctor’s appointment is a good time to collect patient data. But most patients go months between appointments, patient self-reporting is largely unreliable and both EHRs and insurance claims are often incomplete. Data scientist aiming to examine population health, then, should look to public health sources such as FDA Adverse Event Report System. This is also incomplete ; it also includes duplicate occurrences (recorded by various organizations), and it requires standardization and scrubbing.

The SOPs mandate that the federal government’s grassroots health-delivery employees—Auxiliary Nurse-Midwife (ANM), Accredited Social Well being Activists (ASHA) and anganwadi workers—go to households to test if any little one has a excessive fever and/or low blood sugar. Each panchayat is meant to have a well being centre with tools to test sugar stage and Oral Rehydration Answer (ORS) packets to detect signs and start fast therapy. Kids had been wailing whilst new sufferers had been being introduced in. Nicely-equipped? Dr Bhagat contended that the well being division was able to deal with the illness however as youngsters began falling sick, their household panicked and introduced them to the federal government hospital within the city, as a substitute of taking them to their native Main Healthcare Centre (PHC). He mentioned the frenzy on the hospital induced pointless delay in therapy, which aggravated the kids’s situation and contributed to their loss of life. A affected person on the SKMCH, Muzaffarpur. Picture credit score: Pushyamitra Ram refuted Dr Bhagat’s declare about PHCs’ preparedness. He mentioned the rationale he needed to carry his little one to SKMCH is that their PHC is missing in amenities. The federal government has additionally failed to supply the free ambulance service that has been instituted beneath the SOP for AES therapy. Dr Bhagat mentioned well being employees have been ordered to ship sufferers to the hospital by ambulance; if they’ve to rent a personal automobile, they need to be reimbursed instantly. Nevertheless, Ramkishore Mahato of Kodaria village narrated an occasion of how this provision too will not be being adopted. He shared how he needed to cough up Rs 700, an quantity he can ill-afford, to carry his daughter to SKMCH from his village 45 kilometres away. When requested if the state of affairs may worsen like what occurred in Uttar Pradesh’s Gorakhpur in 2017, the place greater than 60 infants had died reportedly owing to scarcity of oxygen, an official at SKMCH mentioned they’ve made fool-proof preparations. He mentioned the hospital has sufficient oxygen cylinders and medicines. Additional, he mentioned docs and nurses have been placed on 24X7 obligation. Whereas the native administration is claiming it’s totally ready, Union Minister of State for Well being Ashwini Choubey, who’s an MP from Bihar, conceded earlier this week that well being consciousness programmes took a backseat this summer time as authorities officers had been engaged in election obligation. PUMPING IRONY: Too Good to Be True?
I love hearing about life-changing medical breakthroughs as much as the next geezer, but more for their entertainment value than their prescriptive qualities. Anyone who’s been paying attention in recent years can’t help but find some amusement in the oscillating nature of scientific inquiry: “PSA Tests Save Lives!” “PSA Tests Lead to Unnecessary Treatment!” “Stents Prevent Heart Attacks!” “Drug Therapy More Effective Than Stents!” “Fish Oil Boosts Brain Health!” “Fish Oil Offers No Protection Against Dementia!”
The most recent example of researchers toppling a long-held treatment approach? Last week, headlines announced the results of a major study suggesting that some 70 percent of breast-cancer patients who would otherwise be candidates for chemotherapy don’t really need the toxic treatment. “This is very powerful,” study coauthor Ingrid A. Mayer, PhD, told the New York Times. “It really changes the standard of care.”
At least until new research refutes its findings.
That may sound predictably cynical, but it’s a plausible conclusion to reach after hearing about the work of renowned Greek researcher John P. A. Ioannidis, PhD. Since the mid-1990s, Ioannidis has been scrutinizing clinical studies from all over the world to determine their usefulness. His findings reveal why so many of these medical breakthroughs turn out to be less conclusive than the headlines would suggest.
“Overall, not only are most research findings false, but furthermore, most of the true findings are not useful,” he notes in a 2016 PLoS Medicine report.
It might be easy to dismiss Ioannidis as some anti-science crank if you choose to ignore his résumé — which includes stints at Harvard, Johns Hopkins, the National Institutes of Health, and Stanford — as well as the grudgingly positive response to his critique from his peers in the research community. His initial salvo, a 2005 paper in PLoS Medicine, featured a mathematical model that predicted with surprising accuracy the rates at which the results of published clinical studies were refuted. And the numbers were jaw-dropping: 80 percent of nonrandomized studies, 25 percent of standard randomized studies, and 10 percent of large randomized studies.
“You can question some of the details of John’s calculations, but it’s hard to argue that the essential ideas aren’t absolutely correct,” Doug Altman, director of Oxford’s Centre for Statistics in Medicine, told the Atlantic in a 2010 Ioannidis profile.
Those ideas go to the heart of what’s wrong with medical research and why the elderly, especially, should think twice before embracing any new healthy-living protocol that comes down the pike.
The basic problem, Ioannidis argues, is that scientists bring their biases to bear on every aspect of their research: what questions they ask, who they choose to participate, what measurements they use, how they analyze the data, and how they present their results. Too often, he notes, researchers set out to prove a theory and design the study to reach that result, rather than focusing on an issue and accepting whatever results emerge.
“At every step in the process, there is room to distort results, a way to make a stronger claim or to select what is going to be concluded,” Ioannidis told the Atlantic. “There is an intellectual conflict of interest that pressures researchers to find whatever it is that is most likely to get them funded.”
To earn financial support and career advancement, scientists need to publish their research in well-regarded journals. More eye-catching results are more likely to be printed in such publications, or to make the front page of your newspaper or headline the evening news — only to be refuted at a later date.
Even the most venerable peer-reviewed journals are not immune to the more-than-occasional scientific faux pas. In a 2005 report published in the Journal of the American Medical Association, Ioannidis and his team reviewed 49 of the most widely cited research findings between 1990 and 2003 — including popular studies on hormone-replacement therapy, coronary stents, aspirin to control blood pressure, and vitamin E to lower the risk of heart disease — and found that among the 34 retested studies, 14 (41 percent) had published inaccurate or exaggerated results.
The real problem, Ioannidis argues, lies in the unreasonable expectations we place on the scientific community. As long as healthcare consumers expect that every finding issued from research institutions is correct, scientists will continue to deliver questionable “breakthroughs.” If we lower those expectations and begin to recognize scientific inquiry as the process it is, we may begin seeing more responsible — albeit less headline-grabbing — thinking about health-supporting strategies.
“Science is a noble endeavor, but it’s also a low-yield endeavor,” Ioannidis notes. “I’m not sure that more than a very small percentage of medical research is ever likely to lead to major improvements in clinical outcomes and quality of life. We should be very comfortable with that fact.”

Hairloss
Hairloss Thinning hair My True story receding hairline Just like many of you, I actually am the victim of hair loss..... balding Progressively more Singaporean men are losing their particular 'crowns of glory' which trend may be having before and earlier. About the pavements in these recent years, we may location a relatively number of young men which are bald or balding. Examine this phenomenon to declare decade ago when this kind of happening is not typically seen. This particular balding trend among Singaporean men will be sparking a rising proliferation associated with hair remedy centres. Hair treatment companies have pretty much sprung way up all through the island with the actual wants of Beijing information, Yunnan, Svenson, Bossin, and so forth. Factors behind hair loss can easily be hereditary, or maybe caused by stress, drug prescription drugs, poor diets, oily top of the head etc and these brings about reveal into the numerous forms or perhaps types associated with hair loss such since male pattern baldness, telogen effluvium, alopecia areata, and so forth. An ordinary human loses regarding 40 to 100 fur every day and this decline is not mind boggling. However if more than this particular quantity is lost each day, you should seek some sort of doctor before it is as well late. Hair grows from follicles, and if a single balds and takes no activities, the follicles will finally die and generally there will be no longer any expansion. It will be as well late for any action. Notice that when I claim bald, normally a human will not head out totally bald, the follicles in the sides of the actual human being scalp, the pieces on the years are set for life, and hair expands forever at these particular areas. Baldness follows the pattern referred to as the Norwood chart. You will discover three development of growth of hir: anagen (the growing phase), catagen (the intermediate phase) and telogen (the shedding phase). Frizzy hair loss occurs when the actual tresses grows and garden sheds quickly. The average lifetime cycle of the hair is actually 2 to 3 years and each hair growth to be able to tumble can last 25 cycles, hair loss occurs whenever these process are faster and finished so quick that the rounds end, the follicles passes away away from and the hair cannot grow back again. I understand on this page, there tend to be many jargons and technical terms that are used. The particular internet supplies a rich array of resources for which one can research in level along with understand more about hair loss. In this article, My spouse and i will just roughly wrap up what I understand as well as expertise about hair reduction from my very own studies along with very own personal experience. In the past, We have very heavy volumes of prints of hair to the point this I complained about needing to go to the herrefris?r often as my very own frizzy hair grows so rapidly. Taking a shower, there would be not any frizzy hair lost as my very own hair is so solid. Following a medical treatment several two years ago originating from a supposedly reputable clinic in addition to consuming the drug nimigen, I notice hair damage, my tresses shed within the dozens around the sleep and at the tub sinks daily so considerably so that I stopped getting this drug nimigen and also stop the laser treatments. Through the consumption of nimigen, my tone becomes too dry currently supposed for you to prevent facial oiliness nevertheless it overdoes. Till nowadays, I still do not have more expertise in the real reason for my hair thinning though We believe it has for you to perform significantly with typically the laser treatment plus the ingestion of nimigen. On hindsight, I shall not get completed with the treatment method and the eating typically the medicine and this medical doctor nonetheless asked his nurse practitioners in order to force me back again to continue with typically the remaining treatments and also, refuting all my allegations squarely. I spent close to help $3K only to lose even more. In any case this is the past currently certainly nothing can be accomplished to turn back the clock. What I want to help share with viewers here are my experiences and exactly I have gone through to fights impotence hair loss and hopefully it could possibly supply some useful glimpses or even lessons to those who are currently suffering from frizzy hair loss. Okay for the actual start, I initially ended all the causes that I believe have contributed to help my hair loss along with that is in order to cease with the medical remedies as well as the nimigen consumption. However despite having that, two months down the road, the hair loss does not diminish, in order that it was time to help take many real actions. From the internet, I came to learnt associated with a couple drugs called Propecia, an oral medication, being regarding $95 dollars with that time regarding 36 pills, to be obtained just one pill a morning; and a topical locks spray called minoxidil (called Regaine, Rogaine, Growell, and many others in the market) furthermore at a $95 for one package that could last a month. I actually went to a health practitioner and has been prescribed Propecia, taking just one pill any day for three months, after which I discontinued. The reason why do I cease? Firstly, there were no advancement and second, I commenced to experience that which was talked about as the side consequences with this drug, i. at the. My partner and i begin to truly feel less of a man, though it was a minor side effect that the drug reported and therefore it affect simply a tiny percentage of users. With the two mentioned here factors, I actually stopped by using this drug and the less- than- manly feeling, which was temporary finally subsided. I performed not utilize Minoxidil, since I also learned associated with its side effects far too. I want to collection down the possible edge effects of these a couple drugs here: Propecia: Lack of 'manliness'Minoxidil: Increase scalp level of sensitivity, itchiness, dandruff, increase awareness to the sun. Along with the most significant drawback of these two scientific proven drugs is that once you quit using these kind of two drugs, anything brand new hair that you include raise will fall again! Thus these two drug treatments are to be employed for life! Imagine the expense engaged to maintain these newly formed curly hair: 100 dollar (for each of these kinds of therapies per month), this kind of amount translates to $1200 and more each year, and so on...... So after ending the use connected with the Propecia, I started to explore natural ways associated with get rid of. One method that will I thought of was using mild shampoo similar to Johnson Baby Shampoo, even so its mild concentration wasn't strong enough to crystal clear typically the oilness of my very own head. My hair reduction may be due to oiliness and dandruff ?nternet site does notice these symptoms such as dandruff on my cushion. Therefore i decided to discover the help of specialists and there was any vast range of choices coming from the dermatogists along with locks treatment centres. My partner and i proceeded to go to one such locks treatment method centre for a new free discussion and all these 'professionals' scanned my hair, only to reveal several patches of oil and these 'professionals' introduced me any offer to clear my very own scalp greasiness. The deal cost a new bomb and I did not such as the pushiness of these specialized inside introducing these packages in my opinion. Also I have heard several stories connected with people having used far more than $10, 000 with these supposedly good remedies only to lose perhaps more tresses or having no advancement seen, apart from wasting their very own hard-earned cash! Thus, frizzy hair treatment centers scared us off with that point and I actually decided to consult a top hair health-care professional in Singapore instead. It absolutely was not low-priced. First consultation by yourself price $80. The skin specialist scrutinized my hair and also mentioned there was nothing inappropriate. She prescribed me along with two shampoo, Nizoral ($25) and a Gentle Wash ($21) to be revolving interchangeably everyday and any sebum regulator for being applied on the scalp through the night after washing ($18). Nevertheless , there was still zero enhancement. In fact, the actual residue from sebum regulator produced very small sticky bright residue on my hair when my hair dried to be able to the extent in which my colleagues thought my very own hair had dandruff. In any case the hair was still incredibly oily and coated a number of dandruff too, despite making use of the supposedly strong and also beneficial shampoos that the actual leading dermatologist prescribed. I actually proven this fact while I jumped into one particular of the hair therapy centre at a store shopping center for curiosity and also does a free consultation with the hub. The actual 'scanner' that the heart operators used magnified this scalp glands many periods over, revealing the slimy spots, it really look disgusting with this sort of hundred moments over addition of typically the sebaceous scalp boucles. Typically the operators said my scalp glands were clogged having oil, and this might cause hair loss; but My partner and i did not wish to take any kind of chance with this heart and off I remaining. I went for any second discussion with the actual dermatologist I saw in the past. Again, the dermatologist explained there was nothing inappropriate having my hair or having my scalp as well as the final results from the particular 'hair-pull' test this lady transported out on me discovered no extraordinary hair loss. Right after hearing the tale of the therapy I actually did and the drug nimigen I consumed, this lady thought that it could be telogen effluvium triggered by the medication, a problem which could only get away inside times in order to come; though the oily scalp may be a contributing factor too. The girl told me that she could possibly not do anything except in order to help me identify often the cause of my locks loss through a scalp biopsy ($589) as well as a blood test ($169); all price explained without GST. Hearing the particular prices, I hesitated however in an urge to find out the real cause, We decided for taking the jump and carried out the actual scalp biopsy and the actual blood test. I have the blood test very first and the pain has been nothing compared to the scalp biopsy I underwent later on. It had been zero joke, having a part of your head scalp getting removed surgically and the particular pain was awful in spite of the application of anaesthetics prior to the treatment. After equally the tests, the health care worker rubbed a Bactroban ointment on the part regarding my very own scalp which had been operated through to kill often the bacteria; after sewing this specific part of the remaining hair. There was still little ache that subsisted just about every time My partner and i shampoo typically the sewn section of the scalp. I was told to send back a couple weeks later to take out the actual stitch and to help obtain the connection between the diagnosis. I was provided the remaining of often the Bactroban product to employ daily on the affected scalp to ease the pain in addition to kill the actual bacteria. A couple weeks later, We returned to the same medical clinic to remove typically the stitch and the dermatologist presented with me the result; and also the effect indicated this it was telogen effluvium. The dermatologist said it absolutely was good news as that condition which is often due to help the medication I took would finally subside later. I would be longing for this to come soon. But sadly, days pass and then months, it was futile. Experiencing tons and tons connected with hair around the bathroom drain and getting up everyday to witness the hundreds of hair strewn throughout this bed and on this bedroom floor did nothing to assuage me that this illness would go apart. Often the visits to typically the dermatologist have also drained my family financially and a determined move, I decided to attempt out some off-the-counter natural hair regrowth available at pharmacies. We tried out the Himalyan Frizzy hair Loss Cream (Herbal) to get about one month but it was useless so My partner and i attended see the skin doctor again for your fourth and also fifth time (that seemed to be in April and May previous year), but again the particular dermatologist assessed and stated there was nothing completely wrong having my hair, no men pattern loss or no matter what. But indeed, our frizzy hair was getting thinner as well as thinner by often the day. She approved us with stronger shampoos along with sebum regulator that we employed without seeing any kind of improvements. It was at that time i finally decided not to see the health-care professional again as presently there was simply no use in all! Hair persisted to fall and tumble. From then on (Jun 07), I use 'more or a lesser amount of recognized my fate' in addition to established my hair reduction fact. We tried Kaminomoto Hair Growth Gearbox as well as a good shampoo identified as Melaleucca Shampoo recommended through my friend but this was really not any employ at all. The telogen effluvium which I was diagnosed with did not seem to be to be the frizzy hair illness that struck me personally because it simply did not stop in just a short period. It was over 12 months and my hair continue to continued to fall like rain. Over the total of this hair loss tribulation, I executed numerous studies on hair loss over often the internet, which further set up me with all information concerning hair loss. My partner and i tried most I might, from observing things i used (less oil), observing my very own diet, exercising frequently along with even cutting down on some espresso and green tea. But it did not support in the loss. We have also shared our findings together with others upon an internet discussion discussion board posting and the bond saw some 55, 000 odd readers before it absolutely was finally removed by the actual boss. With my information discovered from the web and my tribulation, We shared with some connected with these forumers; some involving them even emailed us to ask questions. For hairloss, there is definitely no one fixed columbia crash panacea: what works intended for one might not exactly work with regard to others. The hair damage did affect my lifetime somewhat: waking up inside the morning and immediately after shampooing, I counted the actual number of strands connected with hair that I misplaced and any fewer inside count would make myself a little bit happier. I applied to enjoy the breeze but now I prevent currently being 'confronted' with the gust associated with wind for fear that the wind would certainly blow away some involving my hair. Your assist that Singapore can get breezy at times! And absolutely, this self-esteem would suffer a bit of loss and this did not assist in which I am still without having a girlfriend at my age! Hair thinning would certainly dealt out me a serious problem in this 'image-conscious' community; though it may not be usually true: I actually still do my very own reasonable share of admirers while some girls are certainly not specific over looks. Going into the actual MRT daily, I actually always seen young male at different periods associated with balding; sometimes I could see good friends who I have not really seen for some years totally bald!

Stem cell therapy in Mumbai
There has been a lot of debate on the benefits of stem cell technology. There is a section of scientists who believe that stem cell technology can change the whole way we treat diseases. Instead of external chemical medications, stem cell technology will pave the way to treat a disease from within the body. Again, there is a section of scientists who believe that the cost of establishment of stem cell research infrastructure will surely surpass the benefits the technology promises to deliver. However, no scientist has ever refuted the speculation that stem cell technology has lots of potential. This is exactly the reason why the governments of a number of developed countries have started investing in the development of the required infrastructure for stem cell research. In India too, the medical administration has started focussing on the development of stem cell research centres in selected universities and medical colleges.
The most important thing at this juncture of time is the establishment of stem cell banks in the major cities of the country. We need high deposits of stem cells to facilitate quality research and stem cell therapy in Delhi, Mumbai and Bengaluru. In India, stem cell researchers heavily rely on the supply of embryonic stem cells. Embryonic stem cells are easily available in the umbilical cord which can be retained after every child birth, albeit with the permission of the mother and the family. On the other hand, it is very costly as well as difficult to extract stem cells from adult bone marrows. This is exactly the reason why umbilical cord banking is so much encouraged in our country.
From the perspective of the depositors too, banking of the umbilical cord too proves to be profitable. When you donate your umbilical cord for research, you also create a safe deposit of stem cell tissues, which can be demanded back at the time of need. Stem cells extracted from the umbilical cord of a person can be used treat several body disorders of the same person, such as baldness, infertility, diabetes etc. Even if any person does not face any such problem and subsequent need of stem cell therapy in his lifetime, there is no harm in depositing the umbilical cord, as it is discarded as bio waste at the end of its life cycle.
The good thing is that today, there are a number of centres of stem cell therapy in mumbai who also provide the facility of stem cell banking. If you want to contribute to this cause, you can turn up at one of those centres and register yourself as a donor. They would provide you with a donor card, which you will need to avail any benefit in the future.

Far removed from reality - Tanu Weds Manu Returns plays mental health stereotypes at a deafening high pitch
Four years into it, Tanu and Manu’s marriage has hit rough weather, bad enough for them to seek professional help. A series of events leads them to get separated reaching to the point of divorce. Meanwhile, Manu meets Datto, who is a Tanu lookalike and ends up falling in love with her and they decide to get married. Tanu returns to his life, initially refuting his new relationship, but later accepting it. Datto realizes that deep inside, Manu ardently loves Tanu and so, walks away from between them, uniting them again.
The movie opens with the couple visiting St. Benedict Mental Rehabilitation and Recovery Centre in London (also called St. Benedict “Mental Asylum” by the female lead) for marital counseling. The institute looks weird and isolated, unlike a regular hospital or clinic. The team attending them consists of 3 professionals (probably the lead interviewer being a psychiatrist, and of Indian origin). The clinical encounter progresses in the most disorderly and unprofessional manner, with the husband and wife relentlessly attacking each other verbally, while all that the professionals do is make notes, respond inappropriately or offer tissues to the sobbing lady.
The consultation abruptly culminates with the lead professional ordering for Manu’s detention in the “asylum”, simply for losing calm and getting agitated (apparently for the right reasons with clear absence of any gross abnormality) and he is forcefully taken away by 3 or 4 security guards.
Informing Manu’s friend about his detention, Tanu says that he is in a pagalkhana, adding that pagalon ke doctor ke saamne pagalon wali harkat karega to use pagal hi samjhange na, rakh liya usko unhone vahin par. She also imagines Manu being given electroconvulsive therapy (ECT), while he wears a bewildered look as the current passes through his head (see below).
When Manu’s friend goes to get him released, he jokes that he was expecting Manu to be wearing striped clothes (referring to how inmates in an asylum are supposedly dressed). As they walk out of the place through the lobby (that looks like that of a prison more than a hospital), his friend cracks a joke again – wo dekho do pagal chess khel rahe hain.
The intention of the moviemakers clearly is to bring about a comical effect, and invite laughter from the audiences, and they do this by playing several mental health stereotypes aloud -
o   that of the mental asylum
o   that of the puerile psychiatrists who completely lack clinical judgment
o   that of the mentally ill persons with funny, inane mannerisms or who get violent to the point that they need to be held
o   that of the ECT being a crude and barbaric treatment modality, and it being given to anyone who gets agitated
o   that of the use of archaic terms - pagal, pagalkhana, pagalon ka doctor
It’s high time moviemakers use a more sensible and sensitive approach in depicting psychiatrists, psychiatric illnesses, psychiatric patients, and psychiatric treatment facilities. If not now, when? We’ve almost reached the end of the 2nd decade of the 21st century....
