Internship 101: How to conquer your internship year?

What is internship year?

Internship is a year in between. A year of transition from being a medical student to becoming a physician. You will rotate in all the major departments, even the ones you dislike. So take a deep breath and get started, soon it will be done.

How much will I “learn” in my internship?

To be very honest, very minimal in terms of new knowledge. You will learn the practical and paperwork side stuff that comes with the job mostly. I hate to break it to you, but the last place you were actually taught in was medical school and it is over. From internship onwards, it’s about how much YOU work on yourself, how much you study and what exams you take. No one, and I mean no one will come and offer you material to study or register you in exams to take or any of that. It’s all up to you now.

You can literally get through every day as an intern doing minimal to nothing, at the same time you can get through internship having taught yourself how to act professionally, how to deal with patients (away from simulated scenarios) and having studied for whatever exam of interest. So it’s all up to you.

Is it okay if I am an intern and still cannot decide what speciality I want to major in?

It is very okay, Relax. Do not rush yourself into deciding what speciality you want. This is something you will have to do for the rest of your life, so take your time to decide.

Some people know right away what speciality they want, but that’s a minority.

Internship will give you some but not all perspective that you need. You might have liked a speciality on paper, but when you do it as a physician you realize the bigger picture. So don’t limit yourself and keep an open mind.

So how can I organize my internship year and take my exams, what exams can I take and how many can I take in this year?

Considering it is now september, it’s a good time to start. Usually the first couple on months right after graduation you want to do nothing, you are so happy with the fact that the ability to go to work and come back home not having to open a book actually exists. It’s okay to enjoy that for a couple of months.

The exams you can/ prepare for this year: BMLE (Bahraini) , SLE (Saudi) , EMSTREX (UAE), USMLE (especially step 2 CK), IFOM, PLABS, MCCEE (Canadian).

If you are still undecided about residency and would like to take your time, then the 2 exams for you this year are the SLE and BMLE. The BMLE enables you to work in Bahrain after internship until you get into residency, and the SLE is easiest to take during internship and will get you into residency once you decide.

The exams I can talk about:

1-BMLE - UPDATED 2019-: (Mandatory for all Bahrainis): This used to have fixed dates but starting this year it is booked for through the prometric system https://www.prometric.com/en-us/clients/NHRA/Pages/landing.aspx, you can only book it once you are done with your internship. For this reason, what you can do during the year is prepare for it. Pass or fail exam, once you pass you get your licence as a general physician and can work in any hospital in Bahrain.

Usual dates: Open dates, booked through Prometric. You need an eligibility ID from NHRA in order to book, can only be booked once done with internship.

Time needed to study: 1-2 months if free, 3 months if while working/ doing on calls.

Material used: Master the boards for step 2 CK by kaplan (not the only book out there, any book that gives you a review of all the major specialities will do, I used this and that’s why it’s mentioned here) + questions for American Board of Family Physicians is recommended.

2 -SLE: You can book it at any time online, you are allowed to take it once you start internship. You can repeat it I think 3 or 4 times per year and the highest mark is counted.

Usual dates: open dates, taken in Bahrain at a prometric centre in GOSI but if you are interested in applying for residency while in internship, take it by January max.

Time needed to study: 1-2 month (before the booked date, if you have built up you base -I will explain that below-)

Material used: There are certain Qbanks that are available online, use them. This is one option: http://bit.ly/2aVFYGj

3 -EMSTREX: UAE exam, taken in Al Ain University in UAE (1 hour drive from Dubai) on fixed dates.

Usual dates: twice a month on a Saturday starting from December – February.

Time needed to study: 1 month (before the booked date, if you have built up you base -I will explain that below-)

Material used: online sample questions

4-USMLE:

Step 2 CK is the perfect exam to take during internship, because it is essentially the same material covered that you will need for all the previous licensure exams. And your knowledge is fresh from med school.

If you want my advice, even in you do not want to take it, study from its materials first. It will give you a strong base for all the previous 3 exams. This is what I meant when I mentioned the time needed above, if you have a good base to start with. Otherwise you might need longer time.

When I say that you will only need a month to study before each of the previous exams, this is based upon you having a strong base to build up on. Material directed towards The USMLE step 2 CK would give you that base.

Usual dates: open dates. booked through prometric.

Time needed to study: about 3 months

Material used: Uworld Qbank + Master the boards book by Kaplan

-I have no experience with the MCCEE or the PLABs but a lot of people did them during the internship, again it depend on where you want to do your training, it might be that you should only do the MCCEE and disregard the others.

This might be a lot for you to take, so let me put it in a suggested simplified plan for you.

-October, November: Use Master the boards for step 2 CK by Kaplan. Very good book to review and study. Well written and easy to understand.

-December: revise the questions specific for the SLE from any resource you would like. And aim to take the exam by end of December.

-January: revise the questions specific for the EMSTREX and aim to take it by end of January, early February.

-February + March: free for you to take a rest, or repeat the SLE if you wish, or even take it for the first time here. You can also study from Uworld Qbank here if you want to sit the USMLE step 2 CK.

-April: Study for the BMLE and take it once you are done with the internship.

Choose the exams you would like to take and not all of them; it’s up to you. For example, if you know you don’t want to apply for Dubai, then no need to take the EMSTREX and so on.

Hey Snarklings!

Are you surprised that I’m still alive? I know I am (lols am I even relevant anymore?) Before I get into my whole MCAT prep strategy, I just wanted to say that I’m sorry for leaving you guys in the dust. I have been very stressed lately and I just felt like I was letting you guys down which was why I made a really hard (but spontaneous) decision on my academic career. Of course I cannot comment right now on what I did/planning to do, but an upcoming blog post would be up in the next couple of days to explain the whole grey area that I was stuck in. Okay, it’s time to put away the tissue boxes and dive into every pre-meds worse nightmare: The Medical College Admissions Test (MCAT)

Disclaimer: This is my method and it may or may not work for you, so please take it with a grain of salt because, the way that I learn may be different from you, but it is a good idea to see how others approach this scary test.

What is the MCAT?

In short, the MCAT is a standardized test taken on a computer program to measure a prospective medical student’s potential to succeed medical school. Think of it as foreshadow of one’s ability to do well on painstakingly long exams that are cut up into blocks: Since most of medical schools in the US and Canada require doctors in training to take many board exams that can be 8-9 hours long.

Another reason why the MCAT is administered is to see whether the applicant can use background knowledge of the subject (biochemistry, biology, chemistry, psychology/sociology, physics, critical reasoning skills, etc) and integrate with other sciences to synthesize and draw an objective answer to a novel/unknown situation. This makes the MCAT different from others standardized tests such as SAT or ACT since these tests are more on memory recall and regurgitation, while the MCAT is more on integration and application to unknown situations. Think of it like this, when a doctor walks into a patient’s room, they know some things (the knowledge they’ve been taught in med school) and there’s probably things that they don’t know about a patient’s health, but when a doctor integrates what he/she knows and connect it to the unknown they can solve problems.

What is the MCAT tested on?

Since, one of the reasons why the MCAT is administered is to test your endurance on future medical school board exams such as: USMLE step 1, step 2 CK, step 2 CS, Step 3 if you are in the United States or the MCCEE, NAC, MCCQE part 1, MCCQE part 2 in (Canadian boards), and Comlex Level 1, Level 2CE, Level 2PE, Level 3 (Osteopathic med schools/DO). They do this by testing you on undergraduate courses and are broken down into 4 sections/blocks

Chemical and Physical Foundations of Biological Systems

Critical Analysis and Reasoning Skills (CARS)

Biological and Biochemical Foundations of Living Systems

Psychological, Social and Biological Foundations of Behaviour

Prior to Taking the MCAT

I highly recommend taking the required courses before taking the MCAT. The AAMC recommends to take the following pre-med classes:

Introductory (first year) Biology (2 semesters/1 year)

Introductory/General Chemistry (2 semesters/1 year)

Organic chemistry (2 semesters/1 year)

Biochemistry (1 semester)

Introductory Physics (2 semesters/1 year)

Psychology (1 semester)

Sociology (1 semester)

Now I never took a sociology class and I am planning on taking physics in my upcoming (and last) school year of uni. But that doesn’t really matter since you can just self study for it.

So MCAT prep cannot be MCAT prep without a good set of MCAT books for you to review on. Now, there are many MCAT companies out there and a lot of people are confused on which one they would get to the point that everyone keeps on asking the same question: What books are YOU using? Hey, are those books good? Listen, your success on the MCAT does not depend on what company that you choose to get your books from. Why? Because it is you and how well you reviewed and practice those questions on a DAILY BASIS. I know people who got into med school by using Kaplan, I know people who got rejected who used Kaplan, and the same goes for other prep companies such as The Princeton Review, Examkrackers, The Berkely Review, etc. It doesn’t matter. Just pick one and stick with one. Do not. I repeat. DO NOT BUY TWO BOOKS OF THE SAME SUBJECT BECAUSE YOU THINK THAT ONE COMPANY DOES A BETTER “JOB.’ Again, It is you who will determine your own success, not a book.

Now, for me, I was about to buy the Kaplan MCAT series but, my mother surpised me on my 21st birthday with the NextStep MCAT Content Review and Practice Passages. At first, I was bummed out but as I went along, I realized that the books are doing its job: Helping me review what I learned in undergrad (wipes tears away; literally every time I turn a page I get flashbacks). And the best part is: It was free since it was my birthday gift so I did not have to spend a dime.

When I found something that I did not fully understand or the book did not give me a detailed answer I usually used Khan Academy videos (There’s a whole section just for the MCAT with practice problems). Other videos/channels worth mentioning:

AK Lectures for biochemistry and general chemistry (his explanations for thermodynamics is so straight to the point, they’re super clutch)

The Organic Chemistry Tutor: MCAT Test Prep General Chemistry Review Study Guide Part 1 and MCAT General Chemistry Lectures Review Prep Part 2 – Equations & Practice Questions

Leah4SciMCAT for doing MCAT Math Without a Calculator and Amino Acids for MCAT/Biochem students. I also looked at Fischer Projections for MCAT Orgo and Biochemistry which helped so much since she sues fun mnemonics and straight to the point concepts that just makes the learning fun and easy.

Bozeman science for their anatomy and physiology sections for every organ system mentioned in the biology MCAT book. I also looked at a video or two from the AP chemistry, AP biology, and AP physics (which I remembered fondly back in high school and so I’m familiar with them thus, I didn’t look at every video but just ones that I had trouble with, ex: transcription vs translation.)

My method for studying: I tend to study in rotations or two chapters of the same subject a day until completion. The image below is how that looks like in my mini day planner:

Since I do not have a job nor am I in school during the summer I was able to study for 8 hours a day and found myself going through rotations quicker than I expected. Before I go to bed I reviewed what I learned and anything that I do not finished I would have to finish in the morning before starting a new rotation.

Active Learning

So when I start a new chapter I use my metacognitive abilities: What do I know vs what do I don’t know.

Let’s take the endocrine system as an example. I know that the endocrine is invovled in hormone secretion and regulation of the metabolic oathways that maintains homeostasis. I know an example of this would be aldosterone (a steroid hormone) and vasopressin (a peptide) hormone helps regulate homeostasis by repsonding to low fluid levels and by increasing fluid retention. But what I do not know is how exactly they achieve this: thus when I’m studying that’s my goal or my purpose. I am learning to apply and find answers to my questions.

Every time a paragraph says something about a hormone I would turn it into a question and write the answer in my own words. Here’s an example on tropic versus non tropic hormones:

In orange I wrote the section of the chapter: Regulation

I wrote a capital Q for question and A for answer.

And sometimes I write a little flow chart for easy summary of what I learned

The reason why I used this system was for 3 reasons: (1) By writing out the section in a different colour, it acts as a study guide (you know with all the topics/questions pertaining to one part of the chapter for a test). (2) Turning the info in a paragraph into a question and answers makes it interactive and makes you think about what you are learning; sort of like flashcards. I would cover the answer portion with another piece of paper and just talk/aggressively whisper out the answer (the answer doesn’t have to be word by word but just similar to what it is). (3) When it is time for revision: Your notes are in a Q/A format or like test which is better than writing a bunch of notes and just passively looking over.

Here’s another one on the hypothalamus-anterior pituitary-adrenal cortex axis (HPA)

As you can see I love flow charts for sequential pathways and showing the cause and effects of each step (what increases or decreases, which hormone is involved, etc)

I will write a more detailed post on what I put into my MCAT notebooks and what I look for when reading or extracting information from the review books. So just hold on and be patient.

Practice, Practice, Practice

Knowledge is power but practice makes perfect. Remember that the MCAT will not really test your ability to recall facts straight from the book; this is not a trivia show. It is your ability to apply known information (your MCAT content review) and integrate them with other information in an unknown situation (the MCAT passages). That’s why I like that my MCAT books have practice passages at the end of each chapter. I also bought extra practice passages from NextStep because they really do help me understand the content and the timing. If you do not have practice passages right now, go to the Khan academy MCAT section and there’s loads of them (but they are really easy though).

Also, I really enjoyed watching videos on YouTube on how to approach passages and seeing what other people’s strategies and methods are. I tend to integrate what others do with my own method, but everyone is different so just take it with a grain of salt. After going through a few passages or so, review your mistakes! Then again look up your Q/A notes and see where you went wrong. When you do this, you’re allocating your attention areas that need it. This really helped me retain information whilst, practicing applying concepts with other concepts. Another reason why, is that the questions on the MCAT are really different from your college classes. Again, you’re not regurgitating facts out, you’re applying what you know with what you don’t know by looking at superficial similarities and coming up with the best answer. Since this is a new skill, the only way to learn it is through practice.

Spaced Repetition Is Key

Then the next day before starting a new chapter, I quickly look at all the previous chapters by talking aloud (or aggressively whispering if you’re around people, sorry to the girl at Starbucks who has to put up with my) and just make connections to it. Over time, I looked at all the previous chapters so much that I can recall many concepts.

USMLE STEP 1 EXPERIENCE Amjad Elmashala (Step 1 score 269) So many of you have asked for my experience, I’m sorry for the delay as I just had the step 2 CS exam two days ago. I am a recent graduate of Al-Quds University in Palestine, andnow an intern. I was confused at the beginning whether to take step 2 CK (like all graduates from my university) or step 1. My friend „Kinana Abu-Rayyan‟ talked to me for hours about the benefits of doing step 1 before step 2, and now I know she is right. She‟s always right hahaha! So, I started my preparation on the 15th of July, 2016, but the real kick in was not until July 30th, when another friend of mine Mohammad Adawi decided to begin preparing for the step 1 as well. I read a lot of experiences to decide how I want to proceed with this, and I saw so many students scoring in the 250s and 260s who have only done UWorld “UW”, First Aid “FA” and Pathoma (UFAP). My target was to get at least 250 in the beginning of my preparation. I started with pathology, since it was the least terrifying to me and I did not want to get disappointed or bored from the very beginning. I watched one chapter of Pathoma videos each day, then I went on to read the same chapter from the book. At the same time, I was doing the UW pathology questions for that particular chapter at night. My friend and I usually then talked at 10 or 11 pm when we were done to discuss some of the questions that were either confusing or that we got wrong, and share our ways of thinking on these particular questions. It was really beneficial to see how he was thinking. This took us about 25 days and anywhere between 5 and 8 hours per day. We were also skimming through FA especially towards the end of the 22 days, but I wouldn’t say that we really studied it. Then we decided to move on to biochemistry, we watched Kaplan 2014 biochemistry videos (44 hours) with Dr. Turco and went through FA. Genetics was explained in the first half of the videos, so we took a couple of days to answer UW questions in Genetics, then we went back to finish the metabolic part and answer all the biochemistry questions from UW. This process took us two weeks. Then we went on to study physiology, we read the BRS and FA at the same time, and answered the UW physiology questions. We needed 10 days to get that done. After that, we started watching Pharmacology videos. I watched Kaplan 2014 videos and my friend watched 2010 videos. The 2010 videos have much more details and I think they are worth doing more than the 2014 videos. We did the pharmacokinetics; pharmacodynamics and autonomic nervous system drugs from Kaplan. We read FA at the same time and answered UW pharmacology questions. We left the antimicrobials until we study microbiology. This took us another 10 days. Then, we studied microbiology and immunology from FA we watched videos only for the antimicrobials from Kaplan 2014, and we were going through the UW questions at the end of each. These two took us two weeks, but we knew that we will have to come back to them many times to master them – so many details. After that, we watched the Kaplan 2014 videos for anatomy and read FA, then went through UW anatomy questions. This is one of the topics that made us suffer all the way during our studying. I still don‟t know what to recommend for you. Anatomy took us 10 days of preparation. Finally, we watched the Kaplan 2014 videos for behavioral science, psychiatry, and biostatistics for the last couple of weeks of our first preparation. They were okay but definitely not enough, especially for international medical students who barely study these topics. We went through UW questions, and they were pretty tough, we didn’t have the right framework for answering the questions correctly. That‟s why in our second time of studying, we read the 100 cases in medical ethics by the amazing Conrad Fischer. It took us one day to finish, which was the day of the Classico. It‟s a very well written book and very easy to understand. So these were the first 3 months, give or take a few days. At this time, I took NBME 13. I always liked to challenge myself and see how I‟m progressing. My friend didn’t like to do that, so it really depends on what you prefer. I got 17 questions incorrect, but some of the questions were missing. Do not multiply the number of correct questions by 1.3 or 1.4, this really doesn’t help you predict your score. Just know how many questions you got incorrect and learn from your mistakes. The only way to find out is to take an online NBME or to find someone who took the same NBME online and got the same amount mistakes as you did. From around the 10th of November until the 7th of December, it was time to go through FA from cover to cover one time. Two pieces of advice that I would like to give you here because I noticed them after we were done with the FA. The first one is NEVER EVER leave UW questions or any other questions for a long period. You really lose your sense of reading the questions fast and answering unknown questions to you correctly. The second piece of advice is to do Biochemistry then, for example, Cardiology and Endocrinology, then to go back to Microbiology and then do another 2 chapters from the second half of the book, and so on and so forth. This way you will not be bored by the dry material presented in biochemistry,microbiology, and immunology. During the same period, I watched some of the USMLERx videos, which go through the FA book page by page. The new videos are excellent, and I highly recommend them. They were equivalent to Kaplan videos. Unfortunately, I didn’t have the time to watch all of them. After that, I took NBME 15 and got 250. I was relieved at that moment because I have already reached my target. On December 9th, we registered for the online UW for a couple of months along with the two self-assessments (which I highly recommend). UW took us 25 days to complete for the second time. During this period, we started taking one NBME each week, so that by the 5th of January,we have done 3 more NBMEs. For the following 2 weeks, my friend Kinana Abu-Rayyan convinced me to start doing NBMEs. Before that, I was planning only on taking 3 or 4 of them. But she has a great spirit. So we started doing 50-100 questions per night, and we would talk for two or even three hours to discuss every single question. At this point, NBME became a learningand not an assessment tool. NBME helps a lot with exposing you to difficult questions that you might have never heard of. It also makes you better at guessing the right answer, even when you have no clue how to get to it. We also did UWorld Self-Assessment (UWSA) 1 and 2 at three weeks and two weeks before the exam respectively. My scores were 274 and 269, and I was on the 96th and 92 nd percentile in each UWSA. I did not care much about the number, but the percentile gave me some reassurance. My friend and I traveled to Jordan to take the test in Amman on the 30th of January. Our scheduled test was supposed to be on the 1 st of February. On the night before the exam, I didn’t sleep well – I was too nervous. I woke up and got ready for the test. I arrived at the center 45 minutes earlier. This is where I had my bad experience. The people at the center tried to log me in at around 8 am, but the system got stuck, so I had to wait for a whole hour to fix the problem. Then, they tried to log me in again at 9 am, and it didn’t work. All examinees were already in except for me. At 10.20, they came to me and asked me if I want to reschedule my test for another day. I told them “No”, I mean I wanted to be free at the end of the day and just relax. So they tried to log me in another couple of times and didn’t work for reasons that I still don’t know. At 11.40, they told me that, if I want, they can get me another test in 15 minutes. This time I refused, I was so tired of waiting, I had tachycardia for most of the time outside and I vomited once. So, I decided not to take the test, go back home and reschedule for the next week. I was depressed for the following three days. I was sleeping for 12 hours at times! My friendstayed with me for that period. I reviewed a little bit of biochemistry and microbiology two days before my rescheduled test. I made sure that I would not get disturbed at that night. I went to bed at 9 and had a wonderful night sleep. I woke up in the morning refreshed. I packed my snacks and kept on my pajamas, and I headed to the center and hoped not to experience what I experienced 5 days ago. I was not stressed for the second time as the first time. As soon as I began the test, I felt I was back home answering UW questions. The questions were more like UWSA than NBME, and they were long. I didn’t have any problem with time management. I always had at least 15 minutes left to go over questions I flagged. I was able to revise two complete blocks during the test. I decided to take a break after the first two blocks, then the second two blocks, and then after each block till the end of the exam. When I was on break, I was staring outside the windows and smiling :) because first I know that I’m going home today with that burden off my back, and second, the questions I got weren’t extraterrestrial. I got 7-8 questions per block which were difficult and 2-3 which were really challenging. Materials used during preparation Pathology Pathoma once, UW and FA … and no, I didn’t read Goljan or Robbins Biochemistry Kaplan videos 2014, UW and FA Pharmacology Kaplan videos 2014, first 3 chapters of Kaplan book, UW, and FA Physiology BRS physiology, UW, and FA Microbiology & immunology UW and FA Anatomy Kaplan videos 2014, UW and FA Behavioral science and biostatistics Kaplan videos 2014, 100 cases of ethics, UW and FA National Board Medical Examinations (NBMEs) NBME 1 → 14 incorrect questions NBME 4 → 10 incorrect questions NBME 5 → 12 incorrect questions NBME 6 → 10 incorrect questions NBME 7 → 17 incorrect questions (9 mistakes in the last block, I was very tired at that moment) NBME 11 → 15 incorrect questions NBME 12 → 16 incorrect questions NBME 13 → 6 incorrect questions NBME 15 → 15 incorrect questions NBME 16 → 12 incorrect questions NBME 17 → 13 incorrect questions NBME 18 → 15 incorrect questions, which corresponds to 261 … it was the closest to the real exam and I recommend that you take it last and online.
