i think for a long time dean genuinely doesn't know if cas even likes him. not likes likes him but enjoys his company, wants to spend time with him, etc. cas cares about him but he's not sure in what way. and based on the information available to dean and his specific combination of issues it's a reasonable question, and an active one at least through cas refusing to kill him in s9.
