In my D&D setting, the reason orcs don’t “shave” their tusks (because let’s face it, they can be impractical) is because if do, their jaw will drift back, causing a lot of pain in their temples, and will eventually kill them.

Their jaw will start drifting during puberty, so it’s common for pre-pubescent orcs to shave their tusks.

Orcs who lose their teeth are typically given replacements.

If an orc is in the church (cult) of Scrofek, they will not be given replacements for broken tusks, because they’re seen as a disgrace for not dying in battle. Sometimes when an orc loses their tusks, (usually because of old age) they will leave the cult.
