Although much has been written about health disparities in the context of social justice, relatively little attention is given to frameworks for addressing discrimination across the medical education and faculty landscape. Structural discrimination in medicine systematically disadvantages affected persons (eg, because of race, ethnicity, sex, gender, ability, LGBTQ+ [lesbian, gay, bisexual, transgender, queer or questioning, or another diverse gender identity] status). Initiatives aimed at tackling the unmet needs in health care must address inclusive excellence by focusing on at least 3 levels of intervention at personal (eg, interpersonal communication), institutional (eg, implementation of equitable policies/practices), and cultural (eg, establishment of values and expectations) levels. As institutions embark on the challenging yet transformative journey toward racial equity, they may experience 6 phases as described by dRworks (Dismantling Racism Works), including familiar dysfunction; explicit commitment to diversity, equity, inclusion, and belonging (DEIB); culture shift/not knowing; relational trust; DEIB goals clarified; and DEIB practice.1 We adapted this framework for the context of medical education to serve as anticipatory guidance for DEIB efforts supported by examples from undergraduate medical education and cardiology.
