NHC disability services is a community of 155 national health-related organizations that bring diverse voices together to forge consensus and drive patient-centered policy. Our members are patients, their families and caregivers, and the leading health-related organizations representing providers, researchers, businesses and payers.

NHC supports the provision that these revisions do not invalidate or limit remedies, rights and procedures under other federal laws, including the Americans with Disabilities Act (ADA), that provide equal or greater protections for individuals with disabilities. This is an important safeguard to prevent discriminatory treatment and ensure that laws work in concert to protect the rights of individuals with disabilities.

NHC supports the inclusion of a requirement for individualized assessments that consider unique aspects of individual circumstances and reduce the burden to meet evidence requirements. NHC CarePath

NHC CarePath is a service offering that allows individuals to access the services and programs they need. The program is available through multiple statewide partners, including ServiceLink Aging and Disability Resource Centers, Area Agencies on Aging and specialized Disability resource centers, and NH Family Caregiver Program. The program provides information, support and referrals to individuals of all ages, income levels and abilities in New Hampshire.

The Government is seeking a judicial declaration that the Defendant committed the alleged wrongful acts. However, the Court believes that such a declaration would not resolve any justiciable case or controversy and would be nothing more than a tool for the Government to use in its efforts to win the real case or controversy — removing NHC from Medicare and Medicaid participation. The Court declines to grant the Government’s request for such a judicial declaration.

NH ServiceLink Aging and Disability Resource Centers provide objective information and support for people of all ages, income levels and abilities. Services include person-centered options counseling, NH Family Caregiver Program, state health insurance assistance and Senior Medicare Patrol.

The NH developmental services system offers adults with disabilities a variety of supports and services, including residential options. Services are provided through a network of non-profit Area Agencies across the state, contracted by the Department of Human Services (BDS). The NHC is a member of this network and provides support coordination services.

Role of Alpha Fetoprotein in hepatocellular carcinoma by MuhammadWaqar Mazhar in Journal of Clinical and Medical Images, Case Reports

Abstract

Hepatocellular carcinoma prevelance rate is higher in Pakistan due to HCV mortality rate, consumption of Alchol, and regular smoking, higher level of AFP progression normal liver cells into fatty liver cells, after inflammation it convert into HCC.In this study, we find the correlation between AFP and hepatocellular carcinoma. AFP involve in development of liver cancer, LFT’s test elevation and HCV also cause of cancer.

Introduction

Hepatocellular carcinoma is the 4th most common malignancy in worldwide and it is leading cause of cancer like disease in liver, and it exceed more than 1 million deaths per year by 2030 [1]. Acute hepatitis and acute liver failure are the most serious medical condition that require early diagnosis by release of IL-6, TNF-α and elevated alanine amino transferases, aspartate aminotransferases, alkaline phosphatase and α -Fetoprotein that progress healthy liver in to fatty liver known as steatosis and then inflammation occur in this and leads to hepatocellular carcinoma [2]. Most cases of HCC due to the virus like HCV and HBV, Diabetic and obesity, alcohol related diseases, non- alcohol related diseases, carcinogens like aflatoxins compounds [3]. HCC is the most common cancer that have high mortality rate in cancers due to mortality of HCV and NLFD. In Pakistan HCC ratio high due to prevalence and mortality rate of HCV [4]. The major treatment of HCC are chemotherapy, radiotherapy, transplantation and surgery. Because the most cases diagnose at the late stage, surgery cannot be performed and drugs are the only treatment of HCC [5]. Most patients in HCC become more drug resistance drug resistance. Drug treatment is the best choice of patients who are not edible for surgery. HCC is usually resistance to chemotherapeutic drugs. Because it hinders liver cancer treatment. In recent years targeted drugs use as medication and immune checkpoint inhibitors are introduce for treatment [6].

In the previous research evidence indicates that alpha-fetoprotein has high false-positive rate in diagnosis of early stage of HCC. The EASL clinic practices shows that AFP as a biomarker for liver transplantation and drug indicator [7]. The AFP level increased in many patients’ ad its risk for progression of HCC. AFP, currently the only biomarker available for HCC drug treatment, function as immune suppressor and promote malignancy transformation in HCC [8]. HCC is resistant to traditional chemotherapeutic agents such as doxorubicin, tetrahydrofolate, oxaliplatin, cisplatin, and gemcitabine. currently the recommended drugs include such as targeted therapeutics and immune checkpoint inhibitors [9].

AFP is a glycoprotein that secreted by endoderm embryonic tissue. The lower level of AFP in blood due to AFP is decrease in mature hepatocytes and that AFP gene expression is blocked. It is possible that AFP involved in HCC development and progression become an important factor affecting HCC diagnosis and treatment. AFP plays an important role in promoting cancer cell proliferation and, inhibition cancer cell apoptosis.

LFT’s test performed for liver injury, alanine aminotransferases, aspartate aminotransferases and alkaline phosphatase. These enzymes are commonly elevated in liver disease patients. Alkaline phosphatase and AFP play important role in the diagnosis of cancer.

Case Study

The patient name was sikandar, age 56 patient feel pain in their abdomen and sudden loss of weight. The patient has already hepatitis C infection and their PCR results were positive with high viral load. Due to serious illness it admitted in emergency ward 12, Nishter Hospital Multan. The doctors panel referred some test and kept in observations for better health condition.

The total bilirubin level was 2.05mg/dl in their blood and their normal values 0.6 - 1.2. The serum glutamate-pyruvate transaminase level is 43U/L and normal values up to 40. Aspartate amino transferases and alkaline phosphatase level were high in blood respectively 151 U/L and 493 U/l show in (Figure 1). Its indicate liver injury and cirrhosis. The AFP test indicates correlation with Hepatocellular carcinoma. The AFP level in patient was 6101ng/ml and normal values were 0.1 – 10. Higher level of AFP indicates that HCC have positive relation with AFP to proliferate cancer. The test formed by fully automated state of the Art analyzer Beckman Coulter 700 AIJ.

After blood reports, doctor suggest ultarosund Computrised Tomography whole abdominal view. In view, spleen size becomes enlarged 6cm, calculi in gall bladder, heterogeneous patchy atrial enhancement of right lobe, and some nodules seen in both lobes of liver. The doctor findings the AFP correlation with HCC, splenomegaly, ascites, cholelithiasis and protosystematic collaterals.

The patient diagnosed with hepatocellular carcinoma at last stage, and doctor reffered to liver transplantation in india. But after 4 weeks he cannot survive.

Conclusion

Hepatitis C was the major risk of hepatocellular carcinoma in Pakistan. Smoking and alcohol have big problem to influence HCC in humans. The case study show that alpha fetoprotein has correlation with HCC. Higher Alkaline phosphatase and serum Bilirubin level enhance the liver carcinoma. AFP play role in cell proliferation, cancer cell differentiation and cell cycle arrest.

Hepatitis C virus (HCV) infection is a major leading cause of chronic hepatitis, liver cirrhosis, and hepatocellular carcinoma (HCC). The recent development of new direct-acting antivirals (DAAs) that are highly effective and well-tolerated has facilitated the achievement of sustained virological response rates in more than 90% of patients with HCV infection, regardless of the stage of liver fibrosis, and is expected to reduce the rate of progression to HCC or risk of waitlist dropout among liver transplant candidates with HCC. However, several studies have raised concerns about the risk of HCC in HCV patients treated with interferon (IFN)-free DAA-based therapy, although other studies have failed to obtain findings supporting these results. These conflicting results prompted us to search for further reliable studies to draw a conclusion about this pivotal issue. This article focuses on the current situation regarding the occurrence and recurrence of HCC in HCV patients treated with IFN-free DAA-based therapy. Overview of Concerns Regarding the Risk of Hepatocellular Carcinoma Induced by Interferon-free Direct-acting Antiviral-based Therapy Liver cancer is the fourth leading cause of cancer death and the sixth-most frequent cancer worldwide [1]. Hepatocellular carcinoma (HCC) is the most frequent type of primary liver cancer globally [1]. Hepatitis C virus (HCV) was responsible for 21% of liver cancer mortality as the global level in 2015 [2]. In Japan, HCV infection was the factor contributing most frequently to liver cancer mortality, at 69%.

So I’m getting an incomplete on my Qual and I don’t know what feels worse.

Knowing that I could’ve easily turned it in on time, but just didn’t because I basically fucking forgot to.

Or knowing that even if I had done so, I’d have gotten an incomplete because it’s a pile of shit.

Like, it wasn’t that I thought that it was like good political science, I just thought it was good. And the confirmation that I really don’t know what I’m doing, that it isn’t just imposter syndrome, despite all of the false confidence that my friends, family, and teachers have been giving me, that I need to put in obscene amounts of effort just to make something that, if I put in more effort, could be construed as passable?

That really hurts. And I’m scared.

I’m scared because the teacher that I’ve outright said I respect the most, and has a firm grasp on my abilities, told me that he didn’t think I could thesis as is. That he was concerned that if I did, I would crash and burn, and asked me if I had anything to change his mind.

And I gave him empty promises about only taking 3 classes each semester and being more willing to go to the HCC and all of that.

And that’s probably bullshit? I think I don’t have enough faith in the HCC to actually go there with regularity. More realistically, I’m going to rely heavily on friends (who are also thesising) and on whoever my advisor is (good fucking luck if it’s one of the ones I don’t know/who doesn’t care about me). So if he is right, if I’m not capable of thesising, if I’m not prepared, then I shouldn’t be at Reed next year.

And that all terrifies me.

But on the other hand, I didn’t fail the Qual, per se, and that means that I should probably be happy.

Căn cứ Luật Tổ chức Chính quyền địa phương ngày 19 tháng 6 năm 2015;

Căn cứ Nghị định số 63/2010/NĐ-CP ngày 08 Tháng Sáu 2010 của Chính phủ về kiểm soát hành chính;

Căn cứ Nghị định số 48/2013/NĐ-CP ngày 14 tháng 5 năm 2013 và Nghị định số 92/2017/NĐ-CP ngày 07 tháng 8 năm 2017 của Chính phủ sửa đổi, bổ sung một số điều của Nghị định liên quan đến sự kiểm soát của Thủ tục hành chính;

Căn cứ Thông tư số 02/2017/TT-OG 31 tháng 10 năm 2017 của Văn phòng Chính phủ để kiểm soát hướng dẫn nghiệp vụ của thủ tục hành chính;

Xét đề nghị của Vụ trưởng Vụ Kế hoạch và Đầu tư,

PHÁN QUYẾT:

Điều 1. Công bố quyết định này cùng với các thủ tục hành chính được sửa đổi, bổ sung trong lĩnh vực thành lập và hoạt động của hợp tác xã thuộc thẩm quyền của Ủy ban nhân dân cấp huyện giải quyết áp dụng trên địa bàn tỉnh Thừa Thiên Huế.

Điều 2. Sở Kế hoạch và Đầu tư có trách nhiệm cập nhật các thủ tục hành chính được công bố tại Quyết định này Hệ thống thông tin về thủ tục hành chính của tỉnh theo quy định; UBND huyện, thị xã, thành phố Huế công bố, thủ tục hành chính tại trụ sở cơ quan, trung tâm hành chính của huyện và trên trang của các đơn vị thông tin điện tử; thực hiện giải quyết các thủ tục hành chính thuộc thẩm quyền theo hướng dẫn tại phụ lục kèm theo Quyết định này.

Văn phòng Uỷ ban nhân dân cấp tỉnh có trách nhiệm cập nhật các thủ tục hành chính được công bố tại Quyết định này trên cơ sở dữ liệu quốc gia về thủ tục hành chính; thủ tục hành chính báo cáo Cục Quản lý (Văn phòng Chính phủ) cho phép việc mở công khai các thủ tục hành chính đã được sửa đổi, bổ sung trên cơ sở dữ liệu quốc gia về thủ tục hành chính; hỗ trợ, hướng dẫn các Sở Kế hoạch và Đầu tư để kịp thời cập nhật các thủ tục hành chính trong hệ thống thông tin thủ tục hành chính cấp tỉnh.

Điều 3. Quyết định này có hiệu lực kể từ ngày ký.

Điều 4. Nhân dân Văn phòng Tỉnh ủy ban, Giám đốc Sở, Thủ trưởng cơ quan chuyên môn thuộc Uỷ ban nhân dân cấp tỉnh; Chủ tịch huyện, thị xã và thành phố Huế và các tổ chức, cá nhân liên quan chịu trách nhiệm thi hành Quyết định này. /.

Quyết định số 2029/QĐ-BKH ngày 2017/12/29 của Bộ Kế hoạch và Đầu tư về thủ tục hành chính được công bố được sửa đổi, bổ sung hoặc bãi bỏ lập trường và hoạt động của hợp tác xã trong phạm vi chức năng quản lý của Bộ Kế hoạch và Đầu tư. Đầu tiên T-TTH- 286 536 -tT Đăng ký thành lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh của liên hiệp hợp tác xã.

Tổng cộng có 01 thủ tục hành chính

NỘI DUNG CỤ THỂ CÁC THỦ TỤC HÀNH CHÍNH ĐƯỢC SỬA ĐỔI VÀ BỔ SUNG TÀI SẢN CỦA ỦY BAN NHÂN DÂN HUYỆN THẨM QUYỀN

1. Đăng ký thành lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã

a) Trình tự thực hiện:

- Bước 1: Khi thành lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh, hợp tác xã gửi đến cơ quan đăng ký của hợp tác xã nơi chi nhánh, văn phòng đại diện, kinh doanh đặt đăng ký 01 bộ hồ sơ thành lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã.

- Bước 2: Khi nhận được hồ sơ, cơ quan đăng ký của hợp tác xã:

+ Hỏi người để thực hiện các thủ tục nộp bản sao hợp lệ Giấy chứng minh nhân dân hoặc hộ chiếu hợp lệ (Những quy định phải “nộp bản sao Giấy chứng minh nhân dân hoặc hộ chiếu còn hiệu lực; hoặc bản sao đăng ký giấy chứng nhận ...“tuân thủ các quy định tại Điều 6 Nghị định số 23/2015 / NĐ-CP ngày 2015/02/16 về vấn đề bản sao từ sổ gốc, chứng thực bản sao từ bản chính, chứng thực chữ ký, chứng thực hợp đồng, giao dịch).

+ Nếu uỷ quyền thì phải có các giấy tờ sau: Hợp đồng cung cấp dịch vụ giữa việc thành lập hợp tác xã, hồ sơ dịch vụ hợp tác xã và tổ chức, có được kết quả và giới thiệu của tổ chức này đối với cá nhân để áp dụng, nhận được kết quả; hoặc văn bản ủy quyền cho các cá nhân để áp dụng, nhận được kết quả theo quy định của pháp luật.

Nếu hồ sơ hợp lệ, hợp tác xã cơ quan đăng ký phải cấp biên lai cho hợp tác xã.

- Bước 3: Nếu các dòng, các nội dung hoạt động của chi nhánh, văn phòng đại diện, địa điểm kinh doanh phù hợp với dòng hoạt động của hợp tác xã và phù hợp với các quy định của pháp luật liên quan thì trong thời hạn 05 ngày làm việc kể từ ngày nhận được một ứng dụng hợp lệ , cơ quan đăng ký đăng ký chứng nhận hợp tác xã của các chi nhánh và văn phòng đại diện, địa điểm kinh doanh của hợp tác xã, và cập nhật về đăng ký hồ sơ hợp tác xã.

- Sau khi được cấp giấy chứng nhận đăng ký, chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã được khắc và quyền sử dụng con dấu của nó.

- Nếu không có giấy chứng nhận đăng ký, cơ quan đăng ký của hợp tác xã để ý bằng văn bản nêu rõ lý do cho các hợp tác xã cho biết.

- Bước 4: Trong trường hợp thành lập hợp tác xã của chi nhánh, văn phòng đại diện, địa điểm kinh doanh ở quận hoặc tỉnh, thành phố khác ngoài nơi hợp tác xã đặt trụ sở chính thì trong thời hạn 05 ngày làm việc kể từ ngày được cấp giấy chứng nhận đăng ký của chi nhánh, văn phòng đại diện, địa điểm kinh doanh, các hợp tác xã phải:

+ Thông báo bằng văn bản cho cơ quan đã cấp hợp tác chứng nhận đăng ký nơi hợp tác xã đặt trụ sở chính, để bổ sung hồ sơ đăng ký của hợp tác xã;

+ Thông báo là một bản sao của giấy chứng nhận đăng ký của chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã.

- Bước 5: Trong trường hợp thiết lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh ở nước ngoài, hợp tác xã phải tuân thủ các luật và quy định của quốc gia đó.

Trong thời hạn 15 ngày làm việc kể từ ngày cấp có thẩm quyền của một chứng chỉ nước ngoài mở chi nhánh, văn phòng đại diện, địa điểm kinh doanh, các hợp tác xã phải:

+ Thông báo bằng văn bản cho cơ quan đã cấp hợp tác chứng nhận đăng ký nơi hợp tác xã đặt trụ sở chính, để bổ sung hồ sơ đăng ký của hợp tác xã;

+ Thông báo là một bản sao của giấy chứng nhận đăng ký của chi nhánh, văn phòng đại diện, địa điểm kinh doanh có thẩm quyền của nước ngoài.

b) Làm thế nào để thực hiện:

Nộp trực tiếp tại các huyện Civic Center.

c) Thành phần hồ sơ:

- Thông báo về việc đăng ký lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh;

- Bản sao giấy chứng nhận hợp lệ đăng ký của hợp tác xã;

- Nghị quyết của các thành viên Quốc hội về việc mở chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã;

- Quyết định bằng văn bản của Hội đồng quản trị về việc bổ nhiệm các chi nhánh đại diện, văn phòng đại diện, địa điểm kinh doanh;

- Bản sao một chứng minh nhân dân còn hiệu lực, Hộ chiếu hoặc chứng thực cá nhân đại diện hợp pháp khác của chi nhánh, văn phòng đại diện, địa điểm kinh doanh;

d) Số lượng hồ sơ:

Thư mục 01.

e) Thời hạn giải quyết:

05 ngày làm việc kể từ khi nhận đủ hồ sơ hợp lệ.

e) Đối tượng thủ tục hành chính:

h) Kết quả của việc thực hiện thủ tục hành chính:

Giấy chứng nhận đăng ký của chi nhánh, văn phòng đại diện, địa điểm kinh doanh của hợp tác xã.

100.000 / lần, thanh toán tại thời điểm nộp hồ sơ (theo Quyết định số 67/2017 / QĐ-UBND ngày 22/8/2017 của UBND tỉnh Thừa Thiên Huế nhân dân cấp tỉnh).

k) Các hình thức tên, tờ khai:

Thông báo về việc thành lập chi nhánh, văn phòng đại diện, địa điểm kinh doanh (theo mẫu quy định tại Phụ lục I-5 Thông tư số 03/2014 / TT-BKH).

l) Các yêu cầu, điều kiện thực hiện thủ tục hành chính:

Lines, các nội dung hoạt động của chi nhánh, văn phòng đại diện, địa điểm kinh doanh phải phù hợp với các hoạt động của hợp tác xã.

m) Căn cứ pháp lý của thủ tục hành chính:

- Luật Hợp tác xã ngày 2012/11/20;

- Nghị định số 193/2013/NĐ-CP ngày 2013/11/21 quy định chi tiết một số điều của Luật Hợp tác xã;

- Nghị định số 107/2017/NĐ-CP ngày 2017/09/15 sửa đổi một số điều của Nghị định số 193/2013/NĐ-CP;

- Thông tư số 03/2014/TT-BKH ngày 26/05/2014 hướng dẫn việc đăng ký hợp tác xã và chế độ báo cáo trong hoạt động của hợp tác xã

- Quyết định số 67/2017/QĐ-UBND ngày 22/8/2017 của UBND tỉnh Thừa Thiên Huế quy định về số lượng và nộp lệ phí đăng ký kinh doanh trên địa bàn tỉnh Thừa Thiên Huế./.

Each fall during BC Craft Beer Month, the BC Craft Brewers Guild organizes an annual gathering for its members and their many friends in related industries around the province. The BC Craft Brewers Conference evolved into its current form in 2017, and it’s now safely established as the industry event of the year for the BC brewing and craft beer community. It’s not just for brewers; anyone who is a fan of the BC craft beer industry, or who would like to become part of it, can benefit from what they will learn by attending.

In Year 3 BCCBC has taken a giant leap forward, with a new website and a substantially expanded format. Let’s pop the cap on Beer Me BC’s breakdown of the most impressive ways the Conference has grown for 2019.

Ten signs that BCCBC is bigger than ever in 2019

1. New Location

The Conference takes place just before BC Beer Awards. For the first two years, the Conference was held at the same Croatian Cultural Centre that BCBA uses. This year, significant growth has triggered a move to a new, larger location: the Harbour Convention Centre.

Note: HCC is NOT the Vancouver Convention Centre at Canada Place. It’s the former Edgewater Casino at the Plaza of Nations, on Vancouver’s False Creek waterfront next to BC Place Stadium. Location →

2. Two Days

Up until now, the Conference was a Friday affair, nestled up next to BC Beer Awards on the Saturday. The #BCCBC2019 events schedule has now expanded into Thursday to handle it all. Schedule →

7. Thursday Dinner & Mingler
