Healing a nasty wound is SO satisfying like the sense of pride and accomplishment…. had a patient who sustained a skin tear from her wheelchair and had full-thickness skin loss and a free flap of 99 year old lady skin just DANGLING off her arm… mixed up a concoction of medihoney and collagen powder, pasted her skin back on, and used steri-strips to hold it in place…. covered it up and BEGGED her not to touch it…. unwrapped in a week and it had healed! wrapped it again, and in another week you couldnt tell she had a skin tear at all. SO satisfying. or my guy with exposed spine whose wound bed is now 100% perfect pink granulation tissue. by and large most of my patients dont get better in any capacity so when i do “fix” a problem no matter how insignificant in the long run, it feels great.

How well does it heal? Have you had any issues during the process?
healing is different for everyone, but the majority of people only have what are considered “minor” complications, which are complications that do not require a hospital stay or a non-planned visit to the OR. after being in the phallo community for ~7 years & being in groups with thousands of post-op people, ive only heard of two people having catastrophic complications that resulted in complete loss of their penis, and both were successful on their second attempt. my surgeon, Dr. Jens Berli @ OHSU, has done phallo for over 100 patients and has never had complete phallus loss.
I had virtually no issues healing. I have psoriasis which causes rapid wound healing, so I healed faster than most people do. The only complication I had was a minor infection when an internal suture poked its way out of my penis, but that was cleared up with a 7 day course of oral antibiotics, antibiotic ointment (specifically MediHoney), and an in-office visit where my surgeon removed the infected suture (without anesthesia bc it was very shortly after surgery when i had no sensation in my penis, so i didnt feel anything except a slight tugging sensation)

Products I use for blemishes

Magnesium Sulphate paste: this is what I use on blind cysts and all those other painful, lumpy, under-the-skin sort of blemishes which you just want to bring to a head so that you can deal with them. It really doesn’t smell good, and it doesn’t tend to dry out easily, either, so I recommend applying with a cotton bud and then covering with a bandage or plaster of some kind, making sure that you don’t move the paste from covering the spot.
DMK Actrol Powder: this is the best thing ever for spots that are open, or nearly open, and oozing quite a lot of pus. You just dab a little bit on and leave it to parch and shrink literally everything in its wake. DMK is for professional use only, so I recommend asking a friend with an aesthetician’s licence to buy some for you.
Drying lotion: I buy mine from Boots because it’s cheaper than Mario Badescu and works exactly the same. This is useful for zits that are quite small, nearing the end of their little lives, and just need that last push to disappear completely.
Paula’s Choice BHA 9%: this is strong stuff, and I have to buy from the US because it’s not sold in the UK, but it’s so good for clearing congestion and painful blackheads which aren’t quite blemishes yet, but will be, given the chance.
Lion Pair Acne Cream W: this is from Japan and contains antibacterial and anti-inflammatory agents to prevent more acne from forming, and soothe and heal what’s already there. I use this at any point during the blemish’s life to reduce redness, keep the area clean and free of irritation, and prevent other zits from forming in the same place. It’s a cream and can be worn under makeup, unlike most of the other options on this list.
MediHoney: this medical honey is amazing for zits that really caused some grief and went down fighting. I use it to heal the sore skin after a painful zit has finally disappeared, and to prevent scarring from larger cysts and blemishes. I recommend applying with a cotton bud and then covering with a clean (preferably sterile) bandage, in much the same way as Magnesium Sulphate.
COSRX Master Patch XL: I find that different hydrocolloid patch brands work better than others, and COSRX is my favourite. Maybe it’s a placebo, but they really do seem to work quicker and more effectively, and they stick to my skin well, too. I like this XL size because it fully covers the spots and can be used to target a full cluster of several if needed.
Acropass Trouble Cure Patch: these are microdart patches with salicylic acid, and I like using them after my deep cleansing routine as well as to target specific zits that were born from congestion and need to be defeated thick and fast. They’re expensive, but they work very well in situations when plain hydrocolloid won’t cut it.

I'm in wound care clinic today. For wounds with lots of tissue that's stuck to the wound, you can apply medihoney and then xeroform dressing on top.
For wounds with lots of drainage, apply Aquacel dressings and pt should elevated the area.
Vashe is a wound cleanser that can treat pseudomonas. One of the pts I saw today had a greenish looking wound, so the doctor asked the RN to put Vashe on the wounds. There's also Dakin's solution and Irrisept solution. You put the Vash on some gauze and soak it and let it stay on the wound for 15 minutes before dressing the wound.
Vashe - Vashe Wound Solution helps to cleanse the wound and accomplish the goals of wound bed preparation in a biocompatible, safe, effective, and natural way. This solution of hypochlorous acid (HOCl) acts as a preservative that inhibits microbial contamination within the solution. Vashe Wound Solution has the highest concentration of pure HOCl.
Dakin's solution - Dakin’s solution is a mixture of sodium hypochlorite (0.4% to 0.5%) and boric acid (4%) diluted in water. The boric acid (HȝBOȝ) acts as a buffering agent to maintain a pH of between 9 and 10, as alkalinity outside this range is found to be much more irritating. Boric Acid is also a mildly effective antiseptic. Irrisept - Irrisept is jet lavage containing low concentration Chlorhexidine Gluconate (CHG) 0.05% in sterile water for irrigation. CHG acts as a preservative to help inhibit microbial growth in the solution.
The mechanical action of the Irrisept system removes particulate and debris. The unique bottle design allows users to control the delivery pressure of the solution through manual bottle compression. Grasping the bottle firmly, the user can control the direction and pressure needed to help remove particulate and debris.
OptiLock is good for wounds with lots of drainage.
OptiLock - OptiLock is a super absorbent polymer core dressing. It takes in a remarkable amount of wound fluid — even under compression — and locks it away to help prevent maceration. Even more, this wound dressing is non-adhesive, meaning minimal disruption to the wound and gentle, pain-free removal.
OptiLock can be used as a primary dressing on partial-thickness and full-thickness wounds, such as: pressure, venous or neuropathic ulcers; post-operative wounds; superficial or partial-thickness burns (first- or second-degree); lacerations; abrasions. OptiLock can also be used as a secondary dressing on full-thickness wounds.
Santyl is another product that works for stuck on tissue. It has collagenase, which breaks down tissue that is stuck to a wound. With its unique mechanism of debridement, Collagenase SANTYL◊ Ointment has been shown to help wounds progress toward closure through byproducts that contribute to the migration and proliferation of fibroblasts, keratinocytes, and endothelial cells.
SANTYL Ointment uniquely and actively debrides by cleaving necrotic tissue at seven specific sites along the denatured collagen molecule, unlike endogenous debridement that only works at one site.5,7 While this effectively removes barriers to healing, it also aids wound progression by creating polypeptide bioactive byproducts8-11 that have been shown to signal cells associated with closure.
Epifix - EPIFIX is a dehydrated human amnion/chorion membrane allograft. EPIFIX is SMR2T Technology (Selective Membrane of Reparative and Reconstructive Tissue) which provides a semi-permeable protective barrier that supports the healing cascade. The product protects the wound bed to aid in the development of granulation tissue and provides a human biocompatible extracellular matrix that retains 300+ regulatory proteins.
Saw a pt who had a wound that was on the bottom of his foot. It was like a slit and there was a lot of callus around it because the pressure on his feet when he walks would hit that area of his foot. The doctor removed the callus tissue because that will help it heal. When wounds are dry, they won't heal. You can put Hydrogel into a wound so it can be more moist and it will heal better than a wound that is dry and not healing. You can cover it with a non-adhering dressing called Adaptic. Adaptic dressing is more confortable to remove. Hydrogel may also be more comfortable for pts.

sorry if this is a weird question but when did you start smoking again after you got top surgery? my surgeon wants me to wait 8 weeks but that is looking a little unrealistic 😭
not weird! i started smoking cigs like 2 weeks after my surgery & it gave me complications. 1 of my nipples had an open wound bed and i had to use medihoney to wet heal it, it took like a month to fully heal the wound bed. looking back in my camera roll i was smoking weed a little less than a month after my surgery & cigarettes about 6 weeks out. but i was really bad!!!! and did the opposite of what i should have done lol!
i would say the longer you can do edibles or even dabs to stave off the urge to smoke the lower risk u will have of complications. if pics of my necrosis nipple will help i can post them LOL

i got colonised with MRSA ( an antibiotic variation of staph ) from a hospital stay, and have been fighting infections since. i have a lot of small wounds from being me ( clumsy ) and owning cats, so sometimes those get infected and i have to deal with them. i cant afford to go to the doctor every single time i have one do this, because that's my life. my doctor showed me what to do to help heal them so that's what i do. the wound care supplies are to help heal up those wounds since they got infected and mrsa tends to ulcerate!
i promise i'm alright, i haven't self harmed in probably 5, 6 years maybe. just sick and doing what i can ps. it's medihoney and hydrocolloid bandages, if anyone cares.

Tomorrow is my appointment with the podiatrist and I'm really scared my toe is infected because the broken skin caused by my eczema is like orange/yellow-y. I'm on antibiotics but I'm scared they won't do the nail removal until the infection is gone and I don't know how to clear that up? Also the eczema keeps spreading and it isn't gonna stop until the nail is removed and all wounds are healed.
Anyway, I had to order a bunch of new pain free gauze pads and cohesive bandages so I'm ready for wound care. I decided to get some medihoney too because its good at helping wounds heal and might come in handy. But I also ordered a Calico Critter blind bag for 6 bucks so I have a reward for surviving whatever happens tomorrow.

Week 5
Healing's been going well except for the pernicious open part at the underside of my phallus. I just bought medihoney and applied it today, hopefully that will have more of an effect?
Phallus is shrinking as swelling goes down from the Coban wrap. It's about 6.75 inches in girth and 5ish inches in length while sitting (6" while standing and it's stretched due to gravity). It's less uncomfortable to walk around without holding it, but not entirely.
Yesterday I walked to the corner store to get food which was exciting (though J had to drive me back). I've been having a higher pain day today on the donor leg because of that, unfortunately, but it was still cool to be able to do it! The donor site pain is a dull ache with very rare nerve sparks. I still need to keep it propped up horizontally most of the time as having it down at a 90° sitting angle really aches. I slather it in aquafor every day and the lattice structure is looking shallower (thus less unnerving) with each dressing change, as the skin grows underneath. It's also changing from the raw red color to a purpleish.
Graft site is healing fine, the pain and burning are gone replaced by a maddening itch. I peeled off all of the yellow scabbish tape today (which was slowly coming off naturally), some of it a bit prematurely I think, but not ridiculously early. The parts that weren't necessarily ready to be exposed are stinging more now :( oops. The skin looks splotchy pink, again like a sunburn of sorts, and has some scant flakes of dead skin, but otherwise has no difference in appearance than the rest of my normal skin.
My cane is less necessary for walking around the house, but still very needed for stairs. It's good for balance and saving energy on longer walks.
And for all the suffering I've gone through, it's nice to have something between my legs. That feels right.

Each month, debra of America’s “Ask the Nurse” series will highlight a commonly asked question received by our Nurse Educator, Emily Griffith, BSN, RN. This month’s post focuses on honey. Let’s get started!
QUESTION: I heard honey is good for wound healing. What kind of honey should I use?  
ANSWER: First and foremost, you should only use Medical grade honey on open wounds. Regular store-bought honey should NEVER be applied to the skin, especially in infants, as it may contain spores of Clostridium botulinum, which can cause infantile botulism (muscle paralysis). Medical grade honey can be prescribed by your physician and is often used for moderate to heavily draining wounds. Medihoney is a common brand used and can come in either a gel or different types of dressings. Medical grade honey is a beneficial tool to have for wound healing and infection reduction, especially when it comes to the overuse of topical antibiotics that patients can become de-sensitized to over time. Fun fact: you can store Medihoney gel or dressings in the refrigerator to provide a cooling effect for painful or itchy wounds.
