"When bloodstream infections set in, fast treatment is crucial — but it can take several days to identify the bacteria responsible. A new, rapid-diagnosis sepsis test could cut down on the wait, reducing testing time from as much as a few days to about 13 hours by cutting out a lengthy blood culturing step, researchers report July 24 [2024] in Nature.
“They are pushing the limits of rapid diagnostics for bloodstream infections,” says Pak Kin Wong, a biomedical engineer at Penn State who was not involved in the research. “They are driving toward a direction that will dramatically improve the clinical management of bloodstream infections and sepsis.”
Sepsis — an immune system overreaction to an infection — is a life-threatening condition that strikes nearly 2 million people per year in the United States, killing more than 250,000 (SN: 5/18/08). The condition can also progress to septic shock, a steep drop in blood pressure that damages the kidneys, lungs, liver and other organs. It can be caused by a broad range of different bacteria, making species identification key for personalized treatment of each patient.
In conventional sepsis testing, the blood collected from the patient must first go through a daylong blood culturing step to grow more bacteria for detection. The sample then goes through a second culture for purification before undergoing testing to find the best treatment. During the two to three days required for testing, patients are placed on broad-spectrum antibiotics — a blunt tool designed to stave off a mystery infection that’s better treated by targeted antibiotics after figuring out the specific bacteria causing the infection.
Nanoengineer Tae Hyun Kim and colleagues found a way around the initial 24-hour blood culture.
The workaround starts by injecting a blood sample with nanoparticles decorated with a peptide designed to bind to a wide range of blood-borne pathogens. Magnets then pull out the nanoparticles, and the bound pathogens come with them. Those bacteria are sent directly to the pure culture. Thanks to this binding and sorting process, the bacteria can grow faster without extraneous components in the sample, like blood cells and the previously given broad-spectrum antibiotics, says Kim, of Seoul National University in South Korea.
Cutting out the initial blood culturing step also relies on a new imaging algorithm, Kim says. To test bacteria’s susceptibility to antibiotics, both are placed in the same environment, and scientists observe if and how the antibiotics stunt the bacteria’s growth or kill them. The team’s image detection algorithm can detect subtler changes than the human eye can. So it can identify the species and antibiotic susceptibility with far fewer bacteria cells than the conventional method, thereby reducing the need for long culture times to produce larger colonies.
Though the new method shows promise, Wong says, any new test carries a risk of false negatives, missing bacteria that are actually present in the bloodstream. That in turn can lead to not treating an active infection, and “undertreatment of bloodstream infection can be fatal,” he says. “While the classical blood culture technique is extremely slow, it is very effective in avoiding false negatives.”
Following their laboratory-based experiments, Kim and colleagues tested their new method clinically, running it in parallel with conventional sepsis testing on 190 hospital patients with suspected infections. The testing obtained a 100 percent match on correct bacterial species identification, the team reports. Though more clinical tests are needed, these accuracy results are encouraging so far, Kim says.
The team is continuing to refine their design in hopes of developing a fully automated sepsis blood test that can quickly produce results, even when hospital laboratories are closed overnight. “We really wanted to commercialize this and really make it happen so that we could make impacts to the patients,” Kim says."

It was never quite accurate to accuse Shaoyuan of not thinking things through before doing something. He thought things through a lot. It was why he was so good at his job. Give him a task, and he'll make precise plans, execute down them to the letter, and get the job done.
However, there were only so many tasks to give. And when Shaoyuan ran out of tasks—like, say, Aristides' request for information on Gerald Franke's finances—he found new tasks for himself.
Like, say, breaking Franke's favourite stress ball out.
The break-out was easy. Pickpocket the key from the guard, unlock the cell's gates, grab the captive, and get out. Double back to kidnap the guard as a convenient patsy, then rummage through his pockets for his car keys.
It was only as Shaoyuan was peeling away from the house with a stolen car, stolen captive, and stolen (dead) guard, did it occur to him that this was maybe a bad idea.
"Don't get caught," Aristides had said. "Try to avoid drawing attention, but if you do, make sure it isn't to us. Though I'm sure that goes without saying."
Don't get caught: check. Avoid drawing attention: check—conditionally. Shaoyuan hadn't drawn any attention for the first part of the job. But the second? Well, once they found the guard and his charge missing, he had no control over how much attention they'd give the situation. A lot, he would guess.
Which led him to the parking garage.
Humanity was great at creating sins against god, but chief among them, in Shaoyuan's honest opinion, was the parking garage. They were uncomfortably stark during off-hours, which made it near impossible to find a decent hiding place in them. Also, they smelled like exhaust fumes and gasoline, which sucked when you had enhanced senses, and were perpetually a bit wet, which was just unpleasant.
The only thing parking garages had going for them was their limited sniper sightlines and points of egress. A bodyguard's wet dream, emphasis on wet. And for all Shaoyuan's work experience, he'd never had the pleasure of trying to assassinate someone in a parking garage. He imagined he'd probably be singing a different tune if he had.
Once he parked—"parked" being a rather strong word for what he did—he sat for a moment, thinking things through.
There were a few options open to him, at this point.
One: he could kill the captive, and wash his hands clean of this mess. After a certain amount of torment, death—a clean death, especially���was a mercy. And Shaoyuan had gotten the feeling that this captive had long passed that threshold.
Two: he could call for help.
As a rule, Shaoyuan didn't call for help. He didn't need to. If he was ever in a big enough debacle that he needed to call for help, then he was probably already off to say hi to god on his way down to hell. Decades of nanoengineering advances had served to turn him into a nigh unkillable horror. Dragging someone else down with him only meant exactly that: dragging someone else down with him.
However. Someone was already here with him. Someone he'd already decided to free.
Death was a freedom, but a rather final one. If the captive opened their mouth and asked for it, right now, Shaoyuan could give it to them. But, in the very least, it would be nice to give them another option.
So, he texted Celestinus.
He didn't interact with Celestinus regularly, but he knew of him. It was an open secret that he was Aristides' partner, and a quick and dirty background check told Shaoyuan that he was once a civilian doctor, pulled into the business through underground fighting rings. It wasn't exactly clear whether he was the one fighting, or the one patching up the fighters, but Shaoyuan hadn't cared enough to dig further. Especially after he'd realized that if he was doing his job right (read: not getting shot at), he wouldn't be working with the man all that closely.
Regardless, he still had his contact information, which came in handy now.
The read receipts appeared on his messages nearly immediately, which was a real surprise. Both the read receipts, and the rapid response. It was—he checked the clock—two in the morning, which Shaoyuan had been led to believe was normal sleeping hours for reasonable people.
Then again, Celestinus was in a gang. That sort of thing usually didn't involve reasonable people.
The minutes crawled by, with no reply. Either he'd been ignored, or Celestinus was on his way here. Both options necessitated pulling the captive out of the car, because no one ever liked doing surgery in the backseat of a Nissan Altima, and no one deserved to die in the backseat of a Nissan Altima, either.
And before anyone asked, technically, Shaoyuan had killed that guard before dumping him into the trunk of the car, so it didn't count.
By the time he finished moving the captive, he heard the distant rumbling of an approaching car. It took a bit of finagling to find a decent perch along the ceiling that would hold his weight—parking garages, humanity's sins against god, etcetera—but he got into place long before the sleek black vehicle drove into sight.
He frowned. It was one of Aristides' armored SUVs, which was an interesting choice on Celestinus' part for a situation where no one would be actively shooting at him.
At least it gave him a conveniently large landing area. He dropped down onto the roof soundlessly, then knocked on the roof—after hooking a hand and a foot around the bars along the roof. That was a lesson he'd learned the hard way.
Once the car jolted to a halt—thankfully, without flinging him off this time—he peered over the roof. He hid his amusement as Celestinus startled again, then knocked against the windshield.
"Yeah, yeah," Celestinus grumbled, muffled through the glass. "Where's the fire? You look distinctly not in need of a first aid kit."
"I don't need one," he replied. "Someone else does."
"This better not be a trap."
Ah. That explained the armored vehicle. "It's not."
"Right," Celestinus said sarcastically. "Because that's something someone who's setting a trap wouldn't say."
Fair point.
To the man's credit, despite his suspicions, he still got out of the car. Which didn't exactly speak highly for Celestinus' sense of self-preservation; Shaoyuan made a note to keep an eye on him. Just in case.
Shaoyuan waited for Celestinus to haul out a ridiculously large—he assumed—first aid kit, then led him to where he'd left the captive.
It was not Shaoyuan's first time performing impromptu surgery out in the field, but having it not be on himself was a bit of a novelty. And for all his grumbling, Celestinus worked quickly—he only barely caught the man from crushing the tracker in time.
"Wait," he said, the start of another plan taking shape in his mind. And the end of the plan, for that matter. It wasn't exactly a complicated plan. There was only one step: murder.
"What?" Celestinus asked. "It's a tracker."
"And it's still broadcasting," he pointed out.
"Exactly. It needs to go, before their people find us."
"Or we let them, and I kill them."
"Is your solution to everything murder?"
The plans he had been drawing up in his head for said murder burst like a soap bubble. He blinked at Celestinus, and felt as though he was missing a bit of context to explain why the man was suddenly looking at him with such disapproval.
"I haven't killed anyone while on Aristides' payroll yet," he said slowly.
"Yeah, yet," Celestinus replied. "I have heard about your history, you know."
Ah. That.
He'd always wondered how much Celestinus knew about his history. On the rare occasions he encountered the man, he'd always gotten the vague impression that Celestinus felt somewhat negatively towards him, but he'd chalked it up to the normal distrust people held towards newcomers—especially newcomers who'd inexplicably skyrocketed through the ranks to become something like Aristides' left hand.
Now, at least, he knew where they stood.
He schooled his expression. "Right," he said. "You disapprove."
"Wh–you killed your entire family!" Celestinus sputtered. "Of course I–what kind of question is that?"
Shaoyuan let go as Celestinus yanked his ankle free. "A stupid one, I guess," he replied.
He plucked the tracker off the ground and stood, pulling his bloodied gloves off. Not that it made much of a difference; his hands would always be bloody, one way or another. Celestinus was right to mistrust the monster in their midst.
But monsters were good at doing what needed to be done.
"Take them back to base and treat them there," he said. "I'm going with the Shaoyuan Nie solution for any followers."
It was what he did best, after all.

12/11 UPDATE
Just finished up with the semester! I was able to make A+'s in all of my courses. Even after a horrible final presentation I had to give for my final exam. But the classes I took this semester were Modern Physics Laboratory, Electricity and Magnetism, Thermal Physics, and Mathematical Physics II. I had planned to apply to work in the nanofabrication lab downtown but literally that same semester the founder of the lab left to a different school and now the website just doesn't exist at all. I'm guessing this was all for a reason but still I was so bummed!
Anyways I was able to sign up for my spring classes and I'll update them later but I'm taking a lot of general ed requirements and quantum mechanics, electrodynamics, and a research lab class. I really wanted to take the microscopy elective they were offering but the schedule was just not working with my other classes.
I was wanting to minor in astronomy and astrophysics but they weren't offering most of the electives this semester so unsure if that'll even be possible at this point lol.
I was accepted into a physics conference at Tulane University in New Orleans omg. Still figuring out the travel details with my school. I can't wait to goooo and just experience my first conference. Maybe this will make me a little less scared of future endeavors. I had a bad quasi-research experience while I was getting my degree in nanoengineering and I'm a little iffy on academia as my end goal now. But we'll see, maybe it was just a bad one-off type of thing.
Also built my first PC and it works so well, I can play all the Stardew mods I want!
A lot of applications are closing in December and January so I need to start applying for some things if I want a REU with a really big entity. I'm thinking about NIST and some electrical engineering type things. I still need to look into it. My university also gets a lot of funding for the summer, so that's an option too!
I also want to talk about my summer experience at Princeton and their plasma physics lab but I think that deserves its own post.

Imagine walking into your kitchen and instantly knowing if the fish you bought yesterday is still fresh—or entering an industrial site with sensors that immediately alert you to hazardous gas leaks. This isn't science fiction—it's the promise behind our newly developed nanomechanical sensor array, a powerful tool we've created to detect and analyze complex gases in real-time. In our recent study published in Microsystems & Nanoengineering, we introduce a miniaturized array of silicon and polymer-based sensors capable of detecting various gases quickly and accurately. This array utilizes a simple yet ingenious principle: when gas molecules enter the sensor, they diffuse into specific polymers, causing them to swell slightly. This swelling generates mechanical stress detected by tiny piezoresistive sensors embedded in silicon. It's like watching a sponge expand as it absorbs water—but at a microscopic scale, with the expansion measured electrically to detect and identify gases.
