Studying code calmly and avoiding a burnout. Day 1
If you saw a list of tasks that I said I would do until 23:59 on Sunday, you realized that was a lot for 3 days.
And guess what, he almost had a burnout of his own doing? Myself IEEJIJEIJIE
Yesterday, learning with my elders and colleagues, I understood that I wanted to do a lot of things and almost had my third burnout just to show the recruiter what I can do if I pass the process I want.
BUT it won't do me any good to have a github full of repositories, if I have a burnout and I don't want to touch the code and it gets worse if I'm in a work process because then I'm going to miss a hell of a chance because in the past I've done more than that I could.
SO today Monday I set myself small goals.
[ ] Finish the 1 module Javascript course
[ ] Take the Introduction to computer science with python (1 and 2)
[ ] Object orientation with python
[ ] Project 1 final with Html, css and bootstrap + a bootstrap course individual
I changed plans and that lightened the load. Now I think I can handle it and not freak out.
I will try to make daily updates. I want to be proud of myself in the future when I look at all this bullshit bounout, frustration, etc. And I also share it here for the sake of ego/vanity and wanting to be heard/seen. Which is fine, as long as it's not hurting anyone.
(I don't need any advice comments about ego and vanity. I already have someone to teach me about and I deal very well with my shadows, thank you.)
I wish you a great week, drink water, sunbathe if possible and don't give up.

Mastering the Balance: How an Online MSc Fuels Work, Study, and Career Growth
"Somewhere, something incredible is waiting to be known." – Carl Sagan. From an apple falling off to the accidental invention of a microwave, science has given us some of the most significant yet brilliant concepts. Science is magic, except with a piece of evidence. It is where reality and imagination come together.
From outer space to the deepest point of the Pacific Ocean, humans have made discoveries through magnificent research only because of how advanced science is. When you step into the world of science, you step into a world of possibilities that have the potential to redefine our future.
This is what makes science outstanding—its ability to innovate. This is what science programmes prepare you for. They make you question, and where there is curiosity, there is scope for something better. An MSc is one such programme.
You would be happy to know that MSc graduates earn 25-30% higher salaries on average than those with just undergraduate degrees. This implies that it is worth it as a career booster.
Now if you are wondering how to balance your work, study, and career growth with an MSc, here is how you can do it.

Online Degrees: What Makes Them a More Popular Choice
Lately, the education system has moulded itself to cater to the students' dynamically changing and evolving needs. The Western media and education system also impact the Indian way of education. To a considerable extent, we have successfully accommodated more students to be a part of the system through online degrees.
Some significant reasons have promoted online learning. One can say that the learning methodologies and course structures have changed. Online degrees are often more celebrated for the following reasons:
Higher flexibility: Online degrees have fewer time restrictions than offline or on-campus degrees. This makes it easier for students to lay their focus on academic learning without having to worry about these restrictions.
Easy accessibility: Colleges and universities that offer online degrees or courses make it a point to provide students with adequate study material. Offline, students can access libraries; however, online, it can get a bit difficult.
Therefore, students are provided with the material with easy access, making it convenient for them. Lectures that students may miss are also recorded and saved for them to go through for a thorough revision.
Affordability: Undoubtedly, online degrees are more affordable than offline degrees. Miscellaneous costs such as commute, food and accommodation are largely saved as they are entirely avoided online.

Juggling Success: Mastering Work, Study, and Growth Online
Here are the following ways you can use to balance your work, study, and career growth while pursuing an online MSc degree:
1. Schedule for effective time management
Online degree courses are highly desirable for the advantages they offer. However, daily, they can also get quite mundane, such as a decline in the students' motivation levels. As a result, they may not stay disciplined and might be unable to manage their time more efficiently. Try to fix a schedule and compartmentalise all your work to make this easier. You can practice the following:
Prioritise your work according to the amount of time it needs
Give that work or task a specific period within which you must complete it.
Keep track of time as you begin working on the task.
Don’t overwork to avoid burnout, demotivation, and passive procrastination.
Quick Tip: Apply Parkinson’s Law. If you have the whole day to complete your work, you will take the whole day, but if you have only an hour, you will complete the task within an hour. Assign a deadline to your tasks.
2. Set practical goals
Goal setting is not the same as overachieving. It simply means setting a practical and achievable number of goals that you are sure of completing daily. As a result, they must align with your schedule. Plan your goals more wisely if your academic classes are more time-consuming on a specific day. This can save you some extra time and also avoid compromising the target you set for yourself.
3. Maintain your motivation level
Motivation is a habit; it cannot be momentary. To be consistent, you must know how to draw motivation from the work that makes you happy. If you do not enjoy your work, you won’t be able to give what it takes, and ultimately, your career growth would never be stable. As a result, you must reinforce positively. This means fixing a reward for yourself at the end of every goal completion. You can reward yourself every time you complete a goal or remain disciplined. You get access to rewards only when you achieve the desired target. This principle is a simple psychology that works on positive reinforcement for building good habits.
4. Socialising
When you socialise, you interact with different opinionated people who have a different experience to share as opposed to yours. Their stories can be highly inspiring for you. Your career growth is never an individual journey. It is a compilation of your work, your contribution, and the stories of others who guide you and the network you build. Over the years, you must develop a strong connection circle. This benefits you on an individual level by helping you broaden your perspective and is also helpful for your career growth.

Conclusion: Final Words
Working with an online degree and managing other miscellaneous responsibilities can be daunting. Rising competition and limited time and opportunities can be even more overwhelming. However, you can always create more opportunities when engaging in multiple discussions and interactions online. Several colleges and universities have begun offering online webinars, seminars, meetings, and interactions between teachers and students to facilitate a more amicable environment.
Science is a subject of logic, making it a bit more challenging. What would keep you going is your interest in the subject. Remember always to keep it intact. If you read books and watch films that depict the most complex scientific concepts, you would be able to enhance your understanding and ultimately perform better. All the best!

Hello, can I have a health reading please? :) ☀️ - Flora 🌺
Hey there Flora!
You received the seven of swords! This card serves as a reminder that you can’t do everything at once, and that you may need to be strategic in your approach when considering your health. Instead of focusing on everything at once, try and break these things down into what needs the most attention - so that you can avoid overdoing it and experiencing a burnout! This card also suggests that you may be someone who wants to experience results as quickly as possible, which could mean that you may not dutifully accept the whole process. In regards to health, this could be something like using quickfire diets, or two week exercise programmes rather than maintaining a healthy routine for the long term. If this is the case, then it may be worthwhile revaluating these habits, so that you can be the healthiest version of yourself!
Thanks so much for getting involved hun, I hope you have a lovely day ahead!

Atypical/Elliptical.
There was a tweet highlighted yesterday by one of the Neuro-Divergent accounts I follow, building pace on the back of a compare/contrast photo of an autistic female, and an autistic male. If you haven’t seen it, you can guess how it went, she’s all cute and ‘sailor suit’, he’s in a cluttered room, overweight, in food-stained clothing. Lazy stereotyping at best, offensive and dangerous in reality. The dangerous tweet I reported was one from a contentious incel, stating that females don’t have autism, further down the page of “Would you like to report any other tweets?”, we have that other old favourite “Autism isn’t real.” Yes, I’m shaking my head.
I’m not going to go into in-depth analysis of incel beliefs and values, I’ll just hover over the suggestion that this particular variant was whipping up his followers that ‘Women don’t have autism’, based on his interpretation that the female whose picture he was using was conventionally attractive, and neatly presented. If you tell someone the sun’s 93 million miles away, they accept it, but if they see a sign saying ‘wet paint’, they feel compelled to put their finger in it to check, then complain that they have paint on them. (I know, I don’t touch wet paint, I lick it, it keeps life interesting.) ‘Everybody knows’ that a common feature with autism is the special interest, that we will fixate obsessively on a certain topic, or subject, and woe betide any mere mortal who can’t escape before we get into full flow, what with us not always picking up on non-verbal cues, like snoring. It’s entirely possible that the ‘girl’ had a special interest of dressing and presenting herself in a certain way, even ‘normal’ people do that, hanging their entire identity on presenting a certain way, designer clothes, certain styles of dress, Angry Bird eyebrows. Step back, and absorb that, the girl wasn’t ‘properly’ autistic because she didn’t have food in her hair, wasn’t wearing a Star Trek uniform, looked ‘normal’. Specifically, she looked the kind of ‘normal’ that incels have experience of being rejected by, because they expect to have nice-things handed to them on a plate, and then blame everyone else when they’re denied. There’s a certain example of a petulant, pouty individual, who sulks when they don’t get their own way floating to mind.
Using the newfangled terms neuro-divergent, and neuro-typical, and pausing just for a second to point out that no, we’re not ‘all a bit autistic’ any more than we’re a ‘bit vegetarian’ or a ‘bit left-handed’, neuro-typical people are assumed to be the norm, anything else is deviant. I’ll hold my hands up to that, I don’t iron my laundry, or peel my vegetables, you can stop clutching your pearls, I’m not going to steal them, what would I want pearls for? People with neurodevelopmental disorders are atypical, outsiders, outliers, ‘other’, and it’s more than a little annoying that ‘everyone knows’ that, specifically autistic people, have a tendency to see themselves as different from others. (You started it, telling us we were wrong and weird for our plethora of sensory aversions, and routines, just because they don’t make sense to you.) We’re atypical, whether that’s because we’re genuinely distressed if our ‘usual’ brand of socks, or cereal, or soap is discontinued, or because we won’t cross the road if the light isn’t green, even if there’s nothing coming. Other examples are available.
I’ve spent vast chunks of my life being bounced between “Why are you doing it like that?” and “HOW do you do that?”, I don’t have any savant-skills, but I’m on an elliptical axis, I do some things differently. (The axis isn’t just elliptical, it’s occasionally highly irregular, I have multiple other medical issues, autistics are often blessed like that, to the untrained eye, it might appear I’m neurotic, or hypochondriac, or do my shopping on NHS direct. I’m an unfortunate combination of chromosomes and chronology.) You neuro-typical types bimble along happily enough on your spherical orbits. Yes, you have spikes, too, I know, but it seems that they’re the exception rather than the rule, your orbits appear far more regular than mine. I’m deviating from all-autistics, to ‘me’, there are common factors, but we’re not a one-size-fits-all contingent, I don’t get upset if different types of food touch on my plate, but I can’t use oven-gloves, and I’ll go all day without a drink of water rather than share a drinking vessel, we’re all different.
I’m sometimes envious of the spherical orbit, the regularity of being able to remember to prepare and eat three meals a day, not being afraid of bridges, being able to choose a direction and travel in it without sensory overload, it might as well be necromancy or Olympic level athleticism, it just isn’t ‘there’ for me. When my orbit is within ‘yours’, I’m highly efficient, that’s the “HOW do you do that?” phase. I just do. There isn’t really much of an alternative, but it’s not very healthy, I have all of your weird scripts and rules tumbling around my head, like that stage where you’re learning a new language, everything has to be double-processed, and checked, it’s clunky, not fluent. I’m 43, and I still don’t dream in your language, I can concentrate for periods, but remembering all of the verb endings tends to kick the tenses out of the window, we’re no longer congruent, and I don’t make sense to you.
When I’m within your orbit, I take short-cuts, as verbose as I am here, I omit the unnecessary, because I don’t have the cognitive or physical energy for all of it. I’m a flat-pack item of furniture, I don’t need ‘all’ those screws and fixings to be functional, do I? I unintentionally infuriate and antagonise, because I don’t want to stop for a cup of tea, or chat about TV programmes, I want to complete the task set, before I run out of energy. (I know, but the externally imposed sanctions for non-completion generally have a ripple-out impact on others. My intense bursts of activity alienate other people, because they want to slow down, and chat, but that’s not the task in hand, and I know that my brain and body are temperamental, I *need* to finish within time, and properly, in case I’m less-functional the next day, I always stacked/banked work to make sure I was ahead of myself, to avoid letting other people down if I was ill.)
When our orbits converge, it’s phenomenal, on a ‘work’ level, a life-admin level, or, that holiest of Grail, an interpersonal level, those brief instances are stellar, apart from me freaking people out by my intensity sometimes, I’m an acquired taste. I’m really good at some things, a large proportion of which have yet to demonstrate a particularly useful potential, but there’s time yet. I’m steering very firmly away from the lazy stereotypes of ‘special talents’, I’m resilient and resourceful because I have to be, I often view things from an alternative perspective, and connect-the-dots that others don’t. I still can’t use oven-gloves.
When my orbit swings outside yours, it’s difficult, sometimes impossible for aims to be reconciled, That’s the kick in the teeth on a regular basis, last week, or last month, or yesterday, or earlier today, I might have been functional, or even brilliant, then, all at once, I’m not. “You were fine yesterday!”, yes, I know, I was there.
Chromosomal and chronological factors sometimes spin me out of orbit. I might have been able to walk to Tesco one day last week (Coincidentally, I wasn’t, but that’s not the point.), that doesn’t mean I can do it every day, it’s a cross-over complexity with my telephone directory of other ailments, as well as the autism. When I’m out of orbit, whether it’s sensory overload, burnout, or just my day-to-day ‘wrongness’, I process differently. A ‘normal’ action, like parking a car (I don’t know why I use driving analogies, I’ve never taken my test.) becomes a pantomime of a driving test, where the instructor speaks a foreign language, it’s an unfamiliar car, on unfamiliar roads, and the car’s on fire, and full of wasps, with an angry pig in the back seat. I don’t have muscle memory, or subconscious competence for a lot of functions people take for granted, not just oven gloves, sometimes events conspire to throw me out of spherical orbit, and everything becomes far more complicated than it needs to be. The elliptical orbit makes ‘just’ my ultimate four-letter word, and I know plenty of others. Some instances of being out-of-orbit are predictable, sensory overloads, other illnesses, compounded difficulties around other life-events, my toe having poked through my sock, and being strangled in my boot, it can feel like being an adult-sized toddler, and the temptation to throw down and scream on the supermarket floor because I’m tired is an unwelcome, but regular occurrence.
“Oh, we all get like that sometimes! Can’t you just...?” If I could have ‘just’, I would already have ‘just’, wouldn’t I? 43 years of having been chastised for being difficult, or ruining everyone else’s picnic feed very firmly into the ‘masking’ phenomenon. Charlatans and snake-oil sellers, and Gwyneth Paltrow, as well as even more insidious practitioners are always trying to promote some thing or another that will make us fitter, healthier, more productive, then, to continue the Radiohead theme, many medical types throw back “You do it to yourself.”.
Autism is a lifelong developmental disorder. I can’t consistently ‘try to be less like that’ any more than I can try to be less right-handed, or biologically female. (Yes, I *could* attempt to alter both of those, but to what end?) I’ve had a lot of medical interventions since the brain aneurysm ruptured, and 99% of them have tried to un-autistic me. That’s normal, because autism is abnormal. It’s also normal because autistic females broadly present differently to males. Broadly, I have observational experience from working in education, the ‘old’ perspective was that boys were more frequently autistic than girls, and, more-autistic. Slight tangent on the common misconception of the autistic spectrum, if I may? “We’re all a bit autistic, haha!”, no, no, we’re not, any more than we’re all a bit epileptic. The autistic spectrum isn’t a continuum-spectrum, from 0-100% autistic, while it is clear that some people are severely autistic, and others are not, it isn’t actually a point-scoring exercise, unless you’re UK benefits agencies.
Males and females are conditioned and socialised differently, after millennia of girls-do-this-boys-do-that, humanity is cautiously asking why. I’ll leave my wonky femininist soapbox under the desk, apart from the fact that females are ‘supposed to’ be quiet, and kind, and compliant, and all the gubbins that the incels say. I’m 43, I was raised pink-for-girls-blue-for-boys, there were a lot of things Girls Didn’t Do, it’s OK, I’ve done most of them now, don’t tell my Dad. Much like left-handed children in days gone by were forced to write with their right hand, there has been, and still is, to some extent, pressure on males and females to behave differently, as if keeping our reproductive paraphernalia in a more-or-difficult-to-kick location is an absolute-for-everything. I don’t think it is, but we’ve already established I’m atypical. Not all 40-something-year-old people, with, or without autism had the same childhood experiences I did. There’s no place for detail here, some of the embedded lessons weren’t kindly taught. That Pavlovian response system stuck, be quiet, be pleasant, be demure and train that flinch into a smile. (Various parties ought to apply for funding for having ‘tamed’ this particular shrew. I’m not tamed, I’m barely even domesticated, but I have a shed-load of coping mechanisms.)
Females shouldn’t feel the need to be less-than, to defer to males, but, in a disturbing number of arenas, that’s the norm. I spent the largest part of my life being afraid of men, because of what some men had done, and hating myself for holding a belief that was anathema to the absolute core of my being. (Chapter whatever, fundamentally knowing that males were not ‘better’ than females, but feeling obliged to concede, to avoid disturbing the peace.) The #MeToo disclosures and discourse picked that metaphorical scab, I’ll never go back to that half-life.
I’m atypical because, after decades of excruciating path-of-least-resistance masking, I’ve managed to mask proficiently to a point where I can ‘act normal’ for short stretches. I shouldn’t have to. I’m not suggesting I should be allowed to climb on top of the curtain poles, and throw things, but I don’t see why not-acting-feminine should be seen as disturbing or threatening. It hurts, not just the bras, and the stupid shoes, and the sitting-all-cramped-up, but the emotional and physical toll of carrying oneself ‘female’. When I had the full spectrum cognitive functioning assessment after the brain injuries had settled, the neuro-psych pointed out that a consideration was always ‘At what cost?’. The popular analogy for physical or cognitive energy is a ‘battery’ (A cell, doofus, a ‘battery’ is a number of cells together- behold, I’m reaching my cranky-pedantic cut-off stage.) In order to do anything at all, you need enough ‘charge’ to complete the task. Yes, given, BUT, with autistic masking, there isn’t just the ‘charge’ for the task, there’s the additional charge involved in keeping everything else running, without breaking down, or burning out, the energy overdraft. I’m virtually constantly in my ‘overdraft’, and it’s a bitch to pay back.
I’m elliptical because I frequently swing inside, or outside a typical orbit, I can be ‘miles ahead’ at some points, but ‘miles behind’, and struggling to keep up at others, it’s not a reliable pattern, I can’t predict all of it, and I am SICK of well-meaning “Oh, don’t be so hard on yourself!”. I’m rarely being ‘hard’, I’m usually being practical, if I do x and y on one day, I won’t be able to do z as well. (”Don’t call yourself disabled!” can be a blog for another day.)
This has been an attempt for me to shake myself out of a fog of not-writing. Autism is opaque and oblique, it can be brilliant at times, when things ‘click’, but it’s almost-always difficult to articulate in a way that’s palatable, let alone digestible, I know, it sticks in my own throat enough. The ‘experts’ trot out their theories, sometimes without consultation, and the organisations that set out to ‘cure’ us are pedaling the myth that autism is a disease. It’s not, it’s a divergence. Take this as ‘A Portrait of This Autist’, I can’t speak for anyone else, but I do think it’s important to speak.

Thank you for this enlightening post! Being autistic myself, I have some thoughts on this as well as I see many hallmarks of autism in Viktor due to which I regard this a valid interpretation (not canon, but more on this at the end of this post). I am careful when voicing this kind of analysis as some fans get easily upset when their favourite character is interpreted in such a way. So apologies in advance in case I step on someone's toes with my findings.
Please view my post as additional data input on the subject.
First (and this is mostly directed at the NTs reading this), when talking about empathy, please note that there are several types of empathy. In the following, I’ll talk about emotional empathy and cognitive empathy. Emotional empathy works well for autistics unless we struggle with alexithymia, whereas cognitive empathy (i.e. putting oneself in someone else’s shoes) tends to be super hard. When talking about empathy regarding Viktor, he shows good emotional empathy and is good at picking up other people’s moods, i.e. when he first spots Yuuri at the banquet, when he worries about Yuuri and sensing when he’s upset, but when trying to help, he messes up because he very obviously cannot put himself in Yuuri’s shoes. However, throughout the show, he slowly becomes better at understanding Yuuri.
However, it’s also obvious that he is blissfully unaware of these issues. For if he knew about them, he would be more considerate from the start and might even communicate his differences to avoid misunderstandings. And this is where I believe that Yakov is mistaken. His harsh reaction when seen in this context, is the reaction of someone who got used to Viktor’s idiosyncrasies over the years but interprets them in a hurtful way because he doesn’t know why Viktor is the way he is. And because Viktor doesn’t know either, this is a constant source of misunderstandings and, hence, conflict. Been there, done that. It doesn’t matter how close you are. This phenomenon is called double-empathy problem and it’s a root cause of misunderstandings in autistic/allistic interactions and matches my experience pre-discovery. (I personally prefer this explanation for Yakov’s reaction, but I think both can coexist within the scope of the show).
Some more reasons why Viktor being autistic is a valid interpretation:
To me, the most prominent hallmark is actually his figure skating talent/genius and the perfectionism he exhibits in this department. (Note that this only applies to a subset of autistics, but the intensity with which we pursue our passions can lead to ingenious results.) Viktor is a once-in-a-lifetime figure skating legend. To bring his vision to the ice, he commissions music and costumes according to his parameters and choreographs his own high-difficulty programmes, which are masterpieces full of transition elements and attention to detail. It is super rare for figure skaters to do all this completely on their own, not even Yuzuru Hanyu does all this himself and I consider him a one-in-a-million talent. In addition, Viktor’s skating is super precise. All these things demand dedication. It’s hinted in the show that the major part of Viktor’s life consisted of skating and it’s not a stretch that due to this high level of dedication, eventually, he burned out and lost his inspiration over it.
Viktor is blunt to the point of being outright rude. His bluntness is also reflected in his flirting. No wonder, Yuuri runs for the hills.
Suffering from loneliness due to a lack of meaningful connection is common in autistics as we often form bonds through shared passions, which often isn’t possible with allistics to the extent we crave this. Viktor is canonically lonely (and I doubt training is the only reason for this as many top skaters practise a lot but still maintain friendships) and forms a bond with Yuuri over their shared passion. Loneliness and burnout can also cause depression (especially in autistics) and there are hints that Viktor is depressed at the beginning of the show.
Masking: Viktor masks throughout the first four episodes of the show and then starts taking off the mask because of Yuuri (the beach scene). He takes on carefully crafted personas when being around people, and doesn’t know how to be himself.
His canonical airheadedness could be well explained in this context as hyperfixating a lot on his passion or on other things crossing his mind, and basically being in his own space-time.
Making sudden life-changing decisions due to the onset of a new passion: Autistics struggle with change and there are a few hints that Viktor can’t handle it very well, too, but this trait can be overthrown by a new passion absorbing our entire existence. I see this reflected in Viktor’s decision to move to Japan and become Yuuri’s coach, which the video of Yuuri skating his free programme incites. At this point, Viktor’s long-time passion for skating has withered away, but he hasn’t yet a new passion to focus on. This kind of limbo can cause mental health problems for autistics and they can end all of a sudden when we fall in love with a new passion. Been there, done that. It’s canon that the video struck Viktor like a bolt of lightning. His passion for skating isn’t dead, but changes to the desire of being Yuuri’s coach (romantic feelings certainly play a role here as well) which ultimately leads to the reignition of his passion, or rather, a reinvention as he becomes Yuuri’s coach AND rival.
Given all this, Viktor being autistic is a valid interpretation. I’m not gonna say he IS autistic as this would mean it’s canon but it’s neither mentioned in the show nor stated by the creators. Please note that many autistic-coded characters accidentally come across as autistic because they’re either based on common autistic stereotypes with manifest as e.g. a socially awkward nerd or because their creators are undiagnosed autistics.
However, I regard this interpretation as way likelier and better backed up by the source material than interpretations of Viktor being evil/selfish/having ulterior motives towards Yuuri which I still see circulating in the fandom.
PS: Yuuri, too, exhibits several autistic traits, and some of them overlap with Viktor’s while others manifest differently. It’s a spectrum after all. And if I were to interpret both characters this way, it’s hard to unsee why they fit together so well.
Bonus funfact: Queer people are likelier to be autistic than straight people.
Victor nikiforov and empathy:
Following my fifth (?) rewatch I found yet another reason to be completely in love with Yuri on Ice, and that is Victor being a wonderfully accurate and non-demonized example of someone with low empathy. Seriously as someone who’s autistic and can’t relate to the influx of “actually we’re all super in tune to other’s feelings and have the MOST empathy!”, watch these scenes because this is what it’s like.

This line at first seems to be Yakov thinking that Victor is arrogant, but Yakov knows him extremely well. He knows that Victor has a hard time viscerally placing himself in another’s shoes, and that’s a major obstacle as a coach.
Victor has trouble understanding what to do in situations that are highly emotional for Yuuri. Instead of being able to directly consider things through Yuuri’s perspective, we see him rely pretty often on environment cues, behavioral cause and affect, or straightforward commands.

He gets his habit of scolding yuri after a routine from Yakov because that’s what he’s familiar with. It’s not something Yuuri responds to particularly well, but it’s only when Victor pays attention to how others react when Yuuri finishes skating that he changes tactic. Victor sees how the crowd is going wild at the (Japan nationals) and it’s only then it occurs to him that Yuuri’s confidence may be jeopardized if he chooses that moment to criticize his performance. This is something he likely would’ve realized much sooner if he could share Yuuri’s stress easier instead of merely observing that it’s happening.
Then later on in episode 7, we get to the scene that actually inspired this post.

At first Victor approached the situation almost like an experiment, trying out cause and effect to disastrous results. When you have a much lower threshold for being able to experience another’s feelings second hand, it’s difficult not to think of other’s negative emotions simply as problems that need to be solved. Problem: Yuuri won’t perform well because he’s too anxious, he will only be MORE upset if he loses, and Victor has already tried to reduce his anxiety to no avail. Idea for solution: raise the stakes and see if increased pressure can replace his nerves with resolve, causing a good performance. Afterward, Yuuri will be proud of himself and happy again. Execute plan, observe results, adjust accordingly. When comfort doesn’t come naturally, this tends to be kind of the default. From experience it comes from a genuine place of caring even when it backfires (and it actually doesn’t at times), basically trying to actually fix the thing upsetting your loved one instead of play acting feelings you don’t get.
… however Yuuri was COMPLETELY within his rights yelling at Victor because regardless of intent he picked the worst thing on planet earth to say in that exact moment. I mean I’ve had my fair share of “oh crap someone is upset and it’s my responsibility” fails but my fuck-

He’s shown to be pretty shit at comforting Yuri/ detached emotionally from the situation, but he is neither coldly unbothered nor strategically hiding it. It may appear callous to bluntly say “I don’t know how to help you” to a loved one who’s crying, but as someone who’s been there, that’s an insanely vulnerable thing to admit. “I don’t feel your pain, I don’t get it. But I love you, and just because I’m not feeling it myself doesn’t mean I don’t care that you’re hurting.”

When Yuri finally just straight up tells Victor what to do, he is relieved and simply does it. Things are different between them after this episode because Yuuri finds the power to communicate his needs clearly and effectively. Victor doesn’t speak “subtlety” well. Yakov and Yurio both speak their mind to him directly regardless of if it’s polite, Which is quite telling because they’re the two characters he has the longest history with. It shows us the kind of communication style Victor is accustomed to and needs to navigate interpersonal relationships.
I’m not making a statement that Victor is autistic or even disordered per say. I feel his unusual circumstances provide enough explanation, but I really do see that he consistently struggles with empathy in the same way I do. He’s probably the only character I’ve been able to see that aspect of myself in with scenes that aren’t played for jokes.

Top Tips On Preventing Employee Burnout While Using Podiatry EMR Services?
Unnecessary administrative burdens and activities contribute to physician burnout, which can have a negative impact on primary care physicians. In hospitals and healthcare systems, physician burnout is a persistent problem faced by podiatry emr service providers. Fortunately, hospitals may address this issue in a number ways, including adjusting staff shifts and implementing programmes that attempt to relax clinical staff members.
5 Proactive Strategies to Reduce Physician Burnout As a Result of EHR Software
1. When a new EHR software acquisition is made, train new staff employees — When it comes to implementing new software technology, medical practises must ensure that everyone is on the same page. Each member of staff has a different learning curve, so practises should begin training physicians as soon as the podiatry software is purchased. The training will assist practises in avoiding any inconveniences or disappointments that may arise as a result of the new system. The transition to the new software system will be lot easier, and work processes will become more productive as a result.
2. Improve the login and password process for EMR software – Typing a password to launch a new window in the EHR software system can be frustrating and time-consuming for the physician. A proximity password device can be used by practises to eliminate the need to type a password entirely. It is also recommended that staff members not discuss their own EMR software passwords in order to protect patient privacy and confidentiality.
3. Use voice recognition software or hire a medical scribe – When doctors spend more time on their computers than with their patients, the patient-physician relationship suffers. Medical offices can fight this problem by hiring a medical scribe to record talks and notes from each patient visit. This simplifies data entry, allowing clinicians to devote more time to their patients and reducing the risk of burnout. Patient experiences may also improve as patient wait times are significantly decreased. Hiring a medical scribe can be expensive, so if your practise can't afford one, you can use an integrated speech recognition electronic health record software that allows you to dictate clinical documentation and have it turned into digital notes.
4. When possible, use a virtual assistant – Physician burnout is caused by repetitive EHR software procedures that impede productivity. Virtual assistant systems can assist with all administrative activities linked to EMRs, such as contacting patients and sending prescription orders to pharmacies. This frees up more time for the doctor to deliver the best possible treatment to his or her patients.
5. Choose an EHR software system with a specialty-specific user interface – Complex EMR software interfaces are not user-friendly and, because they are not tailored to your speciality, can lead to fatigue and stress. Providers who utilise a speciality-specific interface don't have to use tools and features that aren't relevant to their specialisation, can access the information they need quickly, and so give better treatment to patients.
Moving forward, you can apply the solutions listed above to reverse the burnout produced by software systems. Your practise efficiency can improve and your stress levels can be reduced by properly utilising podiatry emr software solutions. For more info, connect with us at EMR-EHRS!

Problem 1: a belief that technology has changed, and is changing the world.
Problem 2: a revolutionary view of history
Problem 3: an élite view of history——It is about the owners of the factories rather than the labourers, it is about innovative geniuses rather than the peasants labouring to produce food for them, and it is about the wise politicians who see the potential of these technologies to transform the world in their own image.
Problem 4: male heroes of the revolution
Problem 5: the Fourth Industrial Revolution as a self-fulfilling prophecy
The Fourth Industrial Revolution: what it means, how to respond
14 Jan 2016. Klaus Schwab
https://www.weforum.org/agenda/2016/01/the-fourth-industrial-revolution-what-it-means-and-how-to-respond/
Meaning
A Fourth Industrial Revolution is building on the Third, the digital revolution that has been occurring since the middle of the last century. It is characterized by a fusion of technologies that is blurring the lines between the physical, digital, and biological spheres.
The impact on people
It will affect our identity and all the issues associated with it: our sense of privacy, our notions of ownership, our consumption patterns, the time we devote to work and leisure, and how we develop our careers, cultivate our skills, meet people, and nurture relationships.
The revolutions occurring in biotechnology and AI, which are redefining what it means to be human by pushing back the current thresholds of life span, health, cognition, and capabilities, will compel us to redefine our moral and ethical boundaries.
Shaping the future
To shape the Fourth Industrial Revolution, we must develop a comprehensive and globally shared view of how technology is affecting our lives and reshaping our economic, social, cultural, and human environments.
In the end, it all comes down to people and values. [It's more about how to shape the future that works for us. Think about people first in the long run.
5 ways the Fourth Industrial Revolution transformed 2017 (and 5 ways it did not)
18 Jan 2018 Nicholas Davis
https://www.weforum.org/agenda/2018/01/5-ways-the-fourth-industrial-revolution-transformed-2017-and-5-ways-it-did-not/
Five ways transformed
Ethics: addressing biases and assumptions in technologies
Crypto frenzy: bitcoin went mainstream, blockchain goes hyper
The new geography of innovation: shifting power from West to East one algorithm at the time
Platform economies: from euphoria to backlash
Cyber security: warfare in a world of zeros and ones
Five ways it did not
Narrowing of the gender pay gap
A concerted push around technology for development
More agile governments
Policy adoption of universal basic income
Widespread technological unemployment
Value in Media

How to Build a Sustainable Career in Photography
by Jackson Couse12 Sep 2016
https://photography.tutsplus.com/tutorials/how-to-build-a-sustainable-career-in-photography--cms-27051
Three keys to a sustainable photography practice:
How to reduce costs
How to build resources
How to build revenue.
Photography is a Vocation, Not a Profession
It’s important to look forward and to focus on what is real.
Think about photography as a portable skill, it is something you do, not something you are.
Think about what photography means to you, what it’s all for and whether this is a real possibility before starting it. Think about whether you are ready to work as a photographer.
How to keep your photography practice going
Reduce Your Expenses: Live Within Your Means
Adjust Your Expectations
Don't get too addicted to buying and comparing cameras.
Make decisions that allow you to sustain your photography practice, that let you grow as an artist, that keep your business healthy, and focus on what you care about.
My thoughts
Photography is not what we usually expect. Many things will change when start to take it as a career. However, the most important thing is how to keep being interested in it sustainably. There are lots of things to consider before we decide to do it as a career. We need to know exactly what we think about photography and what we are really interested in doing in this big field first. Aim and set targets for the long term, we can always change it as we go but always plan ahead.
===============================================
Resource two: Sustainable Photographic Career
Notes
Everyone starts from an amateur.(love)
Start a business and do what you love. (but the money involved)
Problems come out (insecurity, ignorance and inexperience lead to doing a lot of work with little return.)
Educate the market as to what is should expect from a professional photographer.
Burned, most burnout or go bankrupt. (pressure)
The repetitive process happens to new photographers. (endless circle)
Hobbyist: Image maker (working for joy)
Professional: Business owner (working to make a living)
==========================================================================
Resource three: Lightroom Vs ACR
Notes
1.Lightroom
Database. Images in a central ‘Catalog’, which is indexed (you don’t need to open a file to see what is in it, you are just shown a preview).
Great for catalogue: easy to use and to find files. The downside: the Lightroom catalogue is usually stored on your computer, not your hard drive. The changes made on a particular photo are stored on the computer, so it's complicated when changing computers all the time.
Changes are non-destructive, more changes are possible that ACR.
Only processes photographs.
The best idea to use Lightroom: Locate your ‘Catalog’ on an external hard drive.
2.Bridge/ACR
Bridge: A file browser, its sole function is to easily look at and arrange your files. It shows all file formations, and it will launch the programme when opening the file.
ACR: Changes are also non-destructive. Changes are set when a file gets opened into Photoshop.
Bridge is pretty handy if the workflow includes other Adobe programmes.
Storing digital files
1. What filing system do you use?
I usually use Bridge to browser my files as it's quite handy to see all files with different formations at the same time.
2. What might be good practice for storing files?
Giving each file or folder a clear name, so it's clear to know what the files or folders about.
Create a folder in a logical hierarchy, and put the folders of the same category together.
3. What backup system do you have in place?
I'm usually saving my files on my computer as the computer hard drive is more reliable than the external hard drive. I would still back up some of my files on the cloud drive just in case.
Different industries have different ideas, but this short article from the National Library gives a good overview:
Tips for preserving digital photos:
Use lossless formats.
Back up your digital photos.
Keep more than one copy of photos.
Label discs and drives clearly and systematically.
Check files, drives, and discs regularly.
Keep original copies of scanned photographs.
File naming tips:
Avoid using capitals and characters such as #, $, % /, &.
Use underscores instead of spaces in filenames.
If using a date in the file name, record it in the format YYYY-MM-DD. For example 20120909 (9 September 2012)
Try to keep the original digital image file order.
=======================================================================
221.257 Assignment One reflection
1. Which test was my most successful?
The third test work.
2. Why
From the perspective of colour, test work 3 is bright and vibrant. These saturated colours bring out the inner beauty of the fruit. At the same time, the way of using beautiful colours to contrast with the contents of the photos, portraying the idea of beauty and grotesque between food and the remaining. From the perspective of angle, test work 3 shows more angels than the other two works, allowing the audiences to engage the theme from different perspectives. I just think the test 3 looks pretty and it conveys my theme better than the other two.
3. Was one of my tests that I thought wasn’t successful well received by the rest of the class or the lecturers?
I think they are all well-received, but I personally don't like the second test work. I think it might have something to do with the simplicity of the content, and the difference in styles when putting it with my other two works.
My idea of test 2 was to try to make some different changes on the background, in that case, I was using two bright colours to form a contrast, but I don't think it is ideal.
4. What did they see in it that I didn’t?
From the comments that I have received, I have found myself a bit short at looking at my own chosen artists. I have been using mostly the artists that other people suggested. I think I do have some of them, however, I just need to include more, especially the photographers from my country.
Also for my last body of test work where I was using some new-purchased fruits for my subjects, I was suggested to use some more rotted fruits instead, portraying the idea of beauty and grotesque better.
5. At this stage, how do I think I will continue this project?
I think I have got two choices to take my work forward, one is to keep on doing still life photography, specifically food photography. The other way is to experiment documentary photography, but this is the type that I am very new at. I think I would definitely try it, but I would prefer doing food photography a bit more.
I could consider using the idea of using food and natural resources in the photoshoots.
What research do you need to move forward?
I need to look at more practitioners who are doing commercial food photography, like the ways they use for photographing the food and the presentation they would do in the end to present their photos.
Technical information would be necessary to look at especially in this current condition, for example looking at information about how to set up my little 'studio' at home, how to arrange the props for a good composition and importantly how to control the lighting.

6 Tips To Work From Home Successfully During Lock Down
Well, many organisations and schools are having to implement work from home infrastructure at speed without much warning or preparation and with the potential addition of a quarantined situation. As a result the reality of working from home has been intensified, to which effect I wanted to provide you with some of the tips to help you succeed whilst working from home;
1) Act As If You Are Going Into The Office
Working from home once in a while is a luxury and most people are excited by the prospect, however here we are presented with the reality of working from home being imposed upon us as a full time obligation.
That doesn't mean you need to break your usual routine, yes you might have some more time in bed as a result of a reduction in your commute. But it is important to get up, get ready for work and set up a space that you can work from comfortably and hopefully ergonomically. That means, not setting up shop in your bed or from your sofa, but actually sitting at a table to ensure you have good posture.
(So yes that does mean getting dressed, even if it is just your top half for video calls)
2) Have a Healthy To Do List
Structure your day, when you are working from home you have to be your own manager and therefore you have to manage your productivity (and potentially those in your team if you have to manage others).
Segment what you will do and when you will do it over the course of the day ; provide yourself with breaks so that you are able to keep focused and avoid burnout.
Without office based distractions you may actually be more productive!
3) Think Like An Entrepreneur
As the Corona Virus pandemic affects the global economy, I believe it is on each of us to look at how we add value to our organisations, teams or suppliers.
Think - what can you do to define the problems and seek out the solutions to help your company to stay above water. It is on all of us to go over and above, to rally together to make sure the business survives.
Thinking 'intrapreneurially’ is essentially having a entrepreneurial mindset within your career and therefore looking at your programme, project as if it is a your own business. What would you do to help, how could you add value - if your job was on the line how would you approach each problem. This current pandemic calls for more than 'thinking outside of the box', it calls on us to throw away the box!
4) Stay In Contact
In an age of technology, working remotely or working from home does not have to mean working in isolation. If you would typically have a weekly team meeting, you can still have the team meeting via lots of different technology (eg. Zoom / Microsoft Teams etc.). If you don't usually have a team meeting, well it is time to start having one.
Usually when working from home, I would have said that changes to your environment are important which might have meant working from a local coffee shop or hot-desking office but if we are in lock down that might not be allowed.
So, why not keep the team spirit going by scheduling calls and meetings as normal, checking in your colleagues to see if they are coping OK or if they need your assistance. Go on that online course that you have been pushing back but that would make you stronger or add to your value when things are back to normal.
You might even find that you are able to increase your productivity at home and have more meetings without the office distractions.. oh but then we need to consider the at home distractions...
5) Accept Distractions
You are at home so there will be a myriad of distractions that you are faced with :
The Doorbell / Deliveries
The Washing
The Kids
The TV
Social Media
The Partner
Lets accept that some of these distractions can be prevented and some cannot, and the distractions are a reality - so long as they are not taking over the whole working day. I saw an organisation saying online that if children are coming into the room you are working from whilst you are on a video call, let them say hi! Now this might not be acceptable in the average industry but in this situation, if the schools are closed you have a good reason for your child being at home. Try to adapt and explain to to your children as much as possible what your working time means - but children will be children. If your company understands then even better.
The others, try not to order up a storm just because you are home but a good work schedule with clearly defined breaks will make some allowances for some of the above.
With that said try not to get absorbed into the TV or social media, it isn't productive and really you want to prove to your organisation that post Corona Virus you can be trusted to continue to work from home on occasion.
The final point on this;
6) Get Some Exercise
...and fresh air (if you can find it), if you are used to a daily commute you may also be used to a little walking, cycling etc. so where possible try to feed a little of this into your daily routine. Even if it is a walk around the block, in your garden or to your local shops.

The Dawn (Diabetes: Attitudes, Wishes and Needs) of Type 2 Diabetes Mellitus in Trinidad, A Small Middle Income Developing Country

Authored by Kameel Mungrue
Abstract
Objective: To investigate how people with T2DM perceive diabetes care and to establish a person-centred model that emphasizes the individual needs of the patient, in the context of current chronic care, education and psychological support and self-management.
Design and Methods: This was a cross-sectional observational study conducted at the Arima and Sangre Grande Health Facilities. Questionnaires, which were constructed using internationally validated questionnaires, were administered by interview among adult patients with diabetes.
Results: Of the301 participants entered into the study the age at diagnosis gradually decreased as well as the highest proportion of cases shifted from 69 years to the 49-58 and 59-68 age groups. 169 people ranked between 0-3 on the PAID-5 scale, indicating that 56.1% of the sample experienced no real emotional distress or depression as a result of their diabetic condition. At one site a total of 46.9% of people felt that their care was well organized most of the time to all of the time while at the other site, 63.5% of people felt that their health care was well organized most of the time to all of the time. Furthermore, patient adherence to diet and exercise interventions were poor, compared with the use of medications. Of the patients interviewed, 58.5% had never taken part in any diabetes educational programme.
Conclusion: Psychosocial problems are prevalent in our setting. Issues with patient satisfaction, compliance and education need to be addressed to holistically meet the needs of the patient.

Introduction
According to the World Health Organisation (WHO), the number of adults living with type 2 diabetes (T2DM) has almost quadrupled since 1980 to 422 million in 2016, accounting for 1.5 million deaths in 2012 [1]. The new report calls upon governments to ensure that people are able to make healthy choices and that health systems are able to diagnose, treat and care for people with diabetes. Rapid economic development accompanied by environmental, social and behavioural change occurred in many low and middle-income countries (LMICs) since the 1980s. In fact the epidemiological transition characterized by falling infectious disease rates with increasing rates in non- communicable diseases occurred in Trinidad and Tobago (TTO) as far back as 1947 [2]. The prevalence of T2DM in TTO range from 78-112 per 1000 population, The Ministry of Health estimates the prevalence of T2DM at 111 per 1000 population or 1 in 8 adults [3]. The economic burden -for the supply of medical treatments and therapeutic interventions, as well as delivering optimal healthcare services- impacts adversely on an already overburdened health system. Barcelo and collegues placed TTO in group 1 countries with an expenditure for T2DM per capita gross national product (GNP) >6000 (USD) and an average cost per person of 577(USD) [4].
Despite the provision of dedicated services such as chronic disease clinics most people with T2DM do not achieve target blood glucose levels by international guidelines [5,6]. This implies that optimal management of T2DM go beyond clinical domains. In 2001, the global Diabetes, Attitudes, Wishes and Needs (DAWN) study provided insights into the psychosocial challenges faced by people with diabetes [7]. The study showed that psychosocial problems can be barriers to achieving adequate glycaemic control [8]. Exploiting the increasing popularity of social media and the opportunities they provide for social support, in 2011, international experts and organisations, including the International Diabetes Federation, the International Alliance of Patients' Organizations (IAPO) and the Steno Diabetes Centre, in collaboration with Novo Nordisk implemented the DAWN (2) study [9]. The DAWN 2 study builds on the experience gained in the behavioral and psychological sciences since the first DAWN (1) study in 2001 [10]. It seeks to bridge gaps highlighted by the DAWN(1) study and to bring about improvements in all strata of health care including the individual, health care organizations, communities and government policy makers [11]. A review of the literature found no published data for the Caribbean. Thus, this is the first study of its kind to explore the psychosocial challenges among patients with T2DM, in Trinidad. Specifically, the study will assess the ability of people with diabetes (PWD) to self-manage their condition and to determine levels of psychosocial distress.
The aim of this study is to replicate DAWN(2) among people with T2DM with particular emphasis on the perception of diabetes care and to establish a person-centered model that emphasizes the needs of the individual in the context of current chronic care, self-management, education and psychological support. In addition the study also aims to identify the shortfalls of the current health system in Trinidad and Tobago in regard to care delivered to patients with T2DM. Further to identify causes of patient non-compliance, avenues facilitate dialogue and collaboration to strengthen patient involvement and improve self-management and psychosocial support in diabetes care and to establish a validated survey system for assessing and benchmarking psychosocial and educational aspects of diabetes care delivery across the Caribbean.
Methods
We used a prevalence study design. The population consisted of all adult patients (>18 years) with T2DM seeking care at primary health care facilities (PHCF). We used a multistage sampling process. Trinidad is divided into four region health authorities (RHA) for the purposes of delivering health care. In the first stage we randomly selected two of the four RHA, i.e. RHA-1 and RHA-2. RHA-1 has 15 PHCF and the RHA-2 has 12 PHCF. In the second stage we randomly selected one PHCF from each RHA. We considered these two PHCF as clusters therefore all clients attending these PHCF were eligible for entry into the study. We choose one facility to avoid varying clinical practices and to control simultaneously for the possible confounding effects of the different variables.
All patients with a physician diagnosis of T2DM or a fasting blood sugar of >126mg/dL, or a random blood sugar of >200mg/dL or an HbAlc 7% were eligible for entry into the study. All patients with gestational diabetes or type 1 diabetes were excluded from the study. All data were collected using a specifically designed data collection instrument, which include demographic data, i.e. age, gender, ethnicity, body mass index (BMI), education level and treatment together with items from the DAWN (2) questionnaire. In addition, we also used the Problem Areas in Diabetes Questionnaire, PAID (5). The PAID measure of diabetes related emotional distress correlates with measures of related concepts such as depression, social support, health beliefs, and coping style, as well as predicted future blood glucose control of the patient. The diagnostic accuracy of the scale is acceptable, achieving a sensitivity 95% and a specificity rate of 89%. A major strength of the PAID-5 is that it takes less thanl min to complete; yet it has comparable 'diagnostic' performance of the four-item Diabetes Distress Scale [12]. Each question has five possible answers with a value from 0 to 4, with 0 representing "no problem" and 4 "a serious problem". The scores are added up and multiplied by 1.25, generating a total score between 0-100. Patients scoring 40 or higher may be at the level of "emotional burnout" and warrant special attention.
The Summary of Diabetes Self-Care Activities (SDSCA) measure is a brief self-report instrument for measuring levels of self-management across different components of the diabetes regimen [12]. The SDSCA assesses levels of self-care and not adherence or compliance to a prescribed regimen because of the difficulties associated with identifying, for a given patient, a specific unchanging standard against which behavior should be compared [13-15]. Diabetes self-care includes a range of activities (e.g., self-monitoring of blood glucose, eating a low- saturated-fat diet, and checking one's feet) and it is now well established that these different components do not correlate highly [14,16,17]. Because self-care is multidimensional, it is necessary to assess each component separately rather than to combine scores across components [13]. Hence we focused on the general diet only exploring the following 5 items (followed your eating plan, fruits and vegetables, high fat foods, carbohydrates, followed a healthy eating plan). Participants could respond: not at all during the past 7 days, or to each of 1-7 days. Scores ranged from 1 (no weekly participation in diabetes self-care activity) to 8 (participation in diabetes self-care activities every day during the past week). Item scores were averaged resulting in an overall score for each item of the self-care activity. A sample size of 300 was calculated if participants completed the entire the questionnaire the maximum attainable average is 448, i.e. if all 300 participants performed the activity every day i.e. the desired outcome.
The final component of the study examined perceptions of diabetic care. This was assessed using the Patient Assessment of Chronic Illness Care (PACIC) which consisted of twenty (20) items. Each item was scored 1-5 with the exception of items 1 and 9, which were scored 0 and 1, in our setting. Hence our mean score varied between 0.9-4.8 in comparison to PACIC scores of 1-5. All questionnaires were pretested and subsequently adapted to a Trinidadian population. All data were stored, retrieved and analyzed using SPSS version 22. A value p<0.05 was considered significant. Ethical approval for this study was obtained from the Ethics Committee of the University West Indies St.
Results
There were 320 eligible participants who met the criteria for entry at the two primary care clusters selected. The sample consisted of 194 (64.5%) participants from one cluster and 107(35.5%) from the other, thus 301 participants entered the study and 19 refused, giving a non-response rate of 5.9%. All 301 participants fulfilled the entry criteria were entered into the study and were available for analysis. The mean age was 60.78 years (SD11.5) with an interquartile range of 51-70 years, table1. There are two major diaspora in Trinidad, Africans and South East Asians (SEA) both representing approximately 35% of the population respectively. However T2DM is higher among SEA compared to Africans [3]. Notwithstanding, 42% of our sample consisted of participants of Africans and 33.2% were SEA. There were more females (179, 59.5%) than males 122 (40.5%), f:m 1.5:1.
Participants were asked about their living situation. The majority reported that they lived with a spouse/partner or children (151, 34.6%), and only 53(12.2%) participants lived alone. Of those who lived with children, 96(22%) lived with a child >18years old while only 52(1.9%) lived with a child <18 years. The majority of participants in the study were retired (132, 42%), only 82 (26.1%) were currently employed full time and 19(6.1%) reported that they were unable to work due either to their T2DM directly or to complications arising. In Trinidad family support for patients with T2DM is high, as 195(64.8%) participants said their family was 'very supportive'. Further 98(32.6%) participants reported that their health care team was 'somewhat supportive' and 176(58.5%) reported 'very supportive'.

Further 98(32.6%) participants reported that their healthcare team was 'somewhat supportive' and 176(58.5%) reported 'very supportive'.
Most participants (285, 94.7%) did not experience discrimination and stigma even along ethnic lines. Of the small number of participants 16(5.3%) who felt they were discriminated because of their T2DM, identified co-worker harassment and judgmental behavior or failure to be hired as main sources of discrimination.Prospective and cross-sectional studies consistently point to the fact that diabetic patients are more likely to develop micro- as well as macro-vascular complications [18-20] which reduce the quality of life of patients, incur heavy burdens to the health care system, and increase diabetic mortality [21-23]. About 50% of the subjects of UKPDS had substantial macro- or micro-vascular abnormalities at the time of T2DM diagnosis [24]. We therefore inquired about the common macrovascular (coronary artery disease, stroke and peripheral vascular disease) and micromascular (retinopathy, nephropathy and neuropathy) complications patients were currently experiencing. More than half the number of participants in the study, 167 (55.5%) had either a mico-or macrovascular complication. The most common complication reported was retinopathy (79, 26.3%); other complications included neuropathy (42, 14%) and coronary heart disease (39, 13%). Further sleeping problems (54, 18%) were more common than sexual dysfunction (5, 1.7%). Hypertension was the most common comorbidity (46, 15.3%).
Since retinopathy, was the most common complication, a stepwise logistic regression was performed to determine the risk factors most likely associated retinopathy. Two significant (p<0. 05), risk factors were identified age and duration of diabetes. Older participants were twice as likely to develop retinopathy (OR 2.23, 95%CI 1.05-4.96, p=0.038), while the longer the duration of T2DM, the greater the risk of developing retinopathy (OR 2.89, 95%CI 1.68,4.86, p=0.001). Current guidelines from the American Diabetes Association/European Association for the Study of Diabetes (ADA/EASD) and the American Association of Clinical Endocrinologists/American College of Endocrinology (AACE/ACE) recommend early initiation of metformin as a first- line drug for monotherapy and combination therapy for patients with T2DM [25,26]. Oral antidiabetic agents (OAA) were the most commonly prescribed and used treatment (242, 80.4%) of which (198, 65.8%) was prescribed Metformin. Metformins first-line position was strengthened by the United Kingdom Prospective Diabetes Study (UKPDS) observation that the metformin-treated group had risk reductions of 32% (p=0.002) for any diabetes-related endpoint, 42% for diabetes-related death (p=0.017), and 36% for all-cause mortality (p=0.011) compared with the control group [27]. Insulin therapy was administered to 126(42%) participants. Most participants (190, 63.1%) reported that they received all their medication free, but were required to wait for long periods before receiving them. Of the 111(36.9%) who made out of pocket payments for some of their medications, (87, 78.4%) indicated its negative impact on their financial resources.
Living with T2DM requires that patients develop a range of competencies that allow them to take greater control over the treatment of their disease. This requires educational programs with the aim of enhancing active involvement of patients so that they become partners in their health care process that promotes health and achieves better outcomes. Although programs are available 176(58.5%) participants never participated. Of the 126(41.%) who did participate, 18(6.0%) found it not helpful, 41(13.6%) reported it was somewhat helpful and 80(26.6%) found it very helpful. On the other hand 114(21.7%) participants relied on doctors/nurses to fulfill this need, and in our setting family and friends (101, 19.2%) were also an important source of information.
Participants were asked about the impact of diabetes on physical health; more participants reported that it negatively affected their physical health than those who said it did not (112, 37.2%). Also, 122 people stated that diabetes did not affect their financial situation adversely. Other areas explored were the relationships with family and friends in which 65 participants reported that the relationship improved, (171) reported that there was no change, while . 138(50%) reported that leisure activities was adversely affected.
Only 105(34.9%) participants reported that they followed a healthy eating plan daily for the last week, while 42(14.0%) participants had not followed a healthy eating plan at all. Furthermore, 95(31.6%) participants did not engage in physical activity for 30 minutes or more in any of the previous 7 days. however, a high proportion of participants (212, 70.4%) reported that they complied with the doctor's instruction and took their diabetes medication exactly as prescribed. More than half of the sample also checked their feet daily (166, 55.1%).
The WHO-Five Well-being Index (WHO-5) was used to evaluate quality of life. The majority of participants (153,50.8%, score >71) felt contented with their quality of life, despite having T2DM, 5(1.7%) participants scored <28, which indicates likely depression while 45(15%) participants scored between 29-49 indicating that they are at risk for depression.
When the PAID questionnaire was administered two participants failed to answer all the questions and were therefore unable to be ranked. We found 169 people ranked between 0-30 on the PAID5 scale, indicating that 56.1% of the sample experienced no real emotional distress or depression as a result of their diabetic condition. There were 44(14.6%) people ranked between 40 and 70. There were 86 participants ranked between 80 and100, indicating the possibility of diabetes related distress, which warrants further assessment.
Using (SDSCA 6) we found that overall performance on each item as well as an aggregate of all items was poor. Although 105(34.9%) participants followed a healthy eating plan daily for sevens only 42(14%) did not reduce their fat intake, table 2. The optimal performance for this category is 478 the attained performance was only 72.8(15.2%). Fruit and vegetable (F & V) consumption was also consistently poor 22 participants reported that they did not consume F & V over the past 7 days.
In regard to examination of the feet, 188 people (62.5%) did not have their feet examined by a healthcare professional. In addition 215(71.4%) participants mentioned that no one enquired if they had felt worried or sad. Lastly, 193(64.1%) of people said that no healthcare professional ever asked about the types of food they ate. In the previous 12 months, 192(63.8%) participants reported that their HbA1c was not measured while 91(30.2%) indicated that were not sure if it had been measured.
We report a mean score of 1.5 for the PACIC arm of the study. There was a significant difference (p<0.05) between the two centers studied, more patients (63%) in RHA-1 were satisfied that their health care needs i.e. "well taken care of ", "most of the time", compared to RHA-2 (47%). The main complaint was the long waiting times experienced prior to their consultation. Other complaints included no written list of instructions or treatment plan were ever given (223,74%) to follow, and 231(76%) also indicated that there was no discussions on their current treatment plan, nor were they asked about goals or targets. The majority of participants (272,90%) reported that they were not encouraged to attend specific educational interventions. Further 249(83%) of participants reported that their values, beliefs and traditions were never solicited in regards to their treatment. Few participants 45(15%) were referred to a dietician and 186(61.8%) said that they were never asked about problems they were experiencing with their medication. Apart from the contact in the clinic there was no other contact between the participant and their doctor/nurse.
Discussion
An important finding of the study was a steady increase in the number of persons diagnosed and treated with T2DM between the 1990's and the first decade of the 21st century and the vulnerability of adolescents and young adults. At the two PHCF studied, for the first time they admitted patients in the age group 19-28, a disease once thought to be a metabolic disorder exclusively of adulthood [28]. This trend in the occurrence of T2DM is not unique to our setting. For example, in Japan 80% of all new cases of diabetes in children and adolescents were diagnosed as T2DM [29]. Similar patterns have been reported in Taiwan [30], and to a less extent in Europe, U.K. and the U.S.A. [31,32]. T2DM is progressive, and one main factor responsible for this is a continued decline in p-cell function [33]. In addition overt T2DM does not occur until the p-cell fails to compensate appropriately to the peripheral insulin resistance state. The ability of the p-cell to secrete sufficient insulin to adequately respond to the peripheral insulin resistance state depends on multiple factors, including p-cell mass [34] and secretory capacity [35],which are influenced by genetic and environmental factors [36].While little can be done in regard to genetic factor, much can be achieved by tackling environmental factors. Improving the physical environment such asincreasing availability of healthy food choices and opportunities for physical activity together with a structured lifestyle-change program is vital to reducing T2DM among people at high risk. Both randomized clinical trials and real-world implementation studies have proven that structured lifestyle-change programs can help prevent or delay T2DM by 60% in people with prediabetes.
High quality chronic illness care can be defined as having a PACIC score between 4 and 5 [15], we report for the first time a score of 1.5. PACIC consist of five (5) subscales; items 1-3 are Patient Activation which assess actions that solicit patient input and involvement in decision making; items 4-6 are Delivery System Design/Decision Support which evaluate actions that organize care and provide information to patients to enhance their understanding of care; items 7-11 are Goal Setting, these items evaluate the acquisition of information for and the setting of specific, collaborative goals; items 12-15 are Problem solving/ Contextual counseling which consider potential barriers and the individual's social and cultural environment in making treatment plans; and items 16-20 are Follow Up/Coordination which evaluates the arrangement of care that extends and reinforces office based treatment and making proactive contact with patients to assess progress and coordinate care. Houle et al in 2012 reported a score of 2.8 while scores of 3.1 and 3.2 have been reported from studies done in the USA [37]. In the developed world most people report a PACIC score ranging between 3.1-3.2 [38-46]. This finding therefore suggests a lower standard of care compared with the developed world. Some contributing factors included the long waiting times, being unable to see the same doctor at return visits, lack of empathy from members of the healthcare team and the consultation was impersonal and was mainly used to repeat prescriptions. Further there was a lack of diabetes educational programs and support groups. Emotional distress due to diabetes was assessed using Problem Areas in Diabetes Scale (PAID)-5, which has been validated as a reliable tool to assess diabetes emotional distress [47]. The majority of participants viewed T2DM as a condition, which they were able to cope with, and showed little to no signs of worry or anxiety; this finding is consistent with other [48].
The quality of life for diabetic patients was assessed using the WHO5 Well Being Index. We found 83% of participants scored greater than 50 indicating that these individuals were generally in good-high mood. Similar findings have been reported in the literature [49]. Further 17% of participants had scores below 50 indicative of a low mood but only 2% had scores below 28 which represents a high risk group for clinical depression [50]. Although the study was not intended to measure the prevalence of depression, the finding was markedly different from the 17% reported by Frederick [51]. Depression is common among people with diabetes [52], especially among those with diabetic complications [53]. Depression has been associated with poor adherence to medication [54], poor glycemic control [55], as well as with the development of diabetic complications [56] and increased mortality [57]. Several studies focusing on the prevalence of depression in people with diabetes have been done, showing different depression rates [58,59].
Although participants reported that they were compliant with prescribed medications and checking their feet as recommended, they were less compliant with following a healthy diet, monitoring their blood sugar and getting the recommended 30 minutes or more of physical activity. Nicolucci et al. [11] in a previous DAWN 2 study found that persons were more likely to follow self-care advice about medication and diet and less likely to follow blood glucose testing, physical activity and foot examination guidelines. In Trinidad and Tobago, there is a free drug distribution program called the Chronic Disease Assistance Program (CDAP), through which medications such as insulin and oral antiglycimic agents are available to all citizens. (5) This may in part contribute to the high compliance in our setting, since Nicolucci et al. [11] reported that 25% of respondents in the initial DAWN2 study had difficulty paying for their diabetes medication, which was contributing to noncompliance. On the other hand blood glucose monitoring was poor because participants are required to purchase test strips. Similarly only 50% of participants reported that their HbA1c was measured over the past year.
Diabetic retinopathy was the main complication among participants. Using binary logistic regression two significant risk factors were identified i.e. duration of diabetes (having been diagnosed with diabetes for more than 10 years) and age over 50. The main implication of this finding is the demand for ophthalmologists of which there are few employed in the public health sector. Retinopathy patients require other types of medication, and this adds to the burden that diabetes places on the health sector.
The sample of 301 persons was restricted to only two PHCF and therefore suffers from generalizability. The study cannot account for undiagnosed T2DM and therefore underestimates disease burden, mainly due to the study design. Nevertheless, it suggests a potentially meaningful burden of disease. Thus, we recommend a nationally representative sample of adolescents and young adults to address the emerging challenge. The questionnaire length affected cooperation rates and participants had to be encouraged to participate. However when this information is combined through a mixed method approach, robust results can be obtained, which are both comprehensive and quantifiable, adding an invaluable perspective to the analysis of the situation.
Conclusion
In conclusion the study provides evidence of the unmet needs of the patient with T2DM. In a changing world and more so in the developing world provision of service has to go beyond the provision of pharmaceutical agents if we are to favorable impact on the outcomes of chronic noncommunicable diseases such as T2DM.
