Renal insufficiency with palpable purpura, elevated rheumatoid factor, and hypocomplementemia = mixed cryoglobulinemia syndrome. The diagnosis can be confirmed by measuring serum cryoglobulin levels.
Mixed cryoglobulinemia syndrome is a vasculitis due to the deposition of immune complexes (polyclonal IgG and IgM rheumatoid factor) within the vascular wall of small- and medium-size vessels. Most cases arise in those with chronic hepatitis C virus (as is likely in this patient with a history of intravenous drug use), but patients with lymphoproliferative disorders, autoimmune disease, and other infections are also at risk.
Patients with mixed cryoglobulinemia syndrome most commonly develop a triad of manifestations:
Palpable purpura
Weakness Arthralgias
Other nonspecific symptoms (eg, nausea) and peripheral neuropathy also frequently occur. Laboratory examination typically reveals elevated rheumatoid factor and hypocomplementemia. Approximately 20% of patients develop glomerulonephritis (red cells, red cell casts, proteinuria) with or without renal insufficiency.
Mixed cryoglobulinemia syndrome is typically associated with chronic hepatitis C virus infection and usually manifests with the triad of palpable purpura, fatigue, and arthralgias.  Peripheral neuropathy, systemic symptoms, and glomerulonephritis with renal insufficiency are also common.  Diagnosis is typically made by measuring serum cryoglobulin levels.
Most cases of mixed cryoglobulinemia syndrome are secondary to an underlying inflammatory condition, which generates the immune complexes that drives the vasculitis. As such, the treatment of mixed cryoglobulinemia syndrome is typically divided into 2 stages:
Initial immunosuppressive therapy – stabilizes end-organ damage (eg, glomerulonephritis) using rituximab plus prednisone
Treatment of underlying disease – targeted therapy against the condition that triggered the cryoglobulinemia

god i really wanna start a 100 days of productivity challenge but this feels like the worst timing for me. i know there’s no perfect time, u just have to get up and do it, but i’m currently pretty ill, i had 2 UTIs + an obstruction that caused bladder hypertrophy and that whole thing probably led to acute glomerulonephritis cause i got a pretty annoying case of nephritic syndrome. to me it manifested through like some generic uti symptoms + some mild hypertension + A LOT OF EDEMA and obviously fatigue. i wanna study but my back hurts a lot right where my kidneys are yk, and i just got this persistent general discomfort that’s really hard to ignore. and i still wanna force myself to try and study a bit but to do that i’d first have to clean up my desk and i’m definitely not feeling up to that, since it’s more physical labor.
it’s 2:30 am and i currently feel just really bad. my back hurts so much and the edema persists even though i’m on diuretics, and i’m tired, and i just wanna miss class tmr so badly. if i go, i’d wake up at 5:30 and spend the whole day on my feet; from 6 am til 4 or 5 just walking around the hospital seeing patient after patient, nonstop, making my back pain worse and definitely making my ankles swell up even more. and after that i’d have to go straight to work, where i can at least sit down a lot and maybe even put my feet up for a while, but still, i’d only be back home at like 11 pm.
do i suck it up and do it. i think i should. i have to, actually. but i wouldn’t be very productive, i’d literally just be dragging myself through it and giving it my worst, i wouldn’t be learning anything, the only good thing to come out of it is that i wouldn’t piss my professors off more than i already have
