Role of Occupational Therapy in Maternal and Child Health at a Community Level.
Mothers are known to be the backbone of many families; now, what happens when the backbone dislocates and cannot function properly. "Maternal health refers to the health of women during pregnancy, childbirth, and the postnatal period. Each stage should be a positive experience, ensuring women and their babies reach their full potential for health and well-being" World Health Organization. In our community block at Inanda, one of our aims is to provide beneficial information and intervention to the mothers and assist them in their motherhood journey.
Milgrom & Beatrice (2003) states that taking care of an infant is more difficult if women restrain their own needs; when experiencing difficulties with their infant, they tend not to express their feelings but pay attention to physical discomfort rather than the difficult emotional situation. In Occupational Therapy, the mother's mental health is an important factor. There are emotional responsibilities linked to motherhood, and in performing all these responsibilities, mothers tend to neglect themselves. Early intervention during pregnancy decreases the risk for postnatal depression. According to Grote & Bledsoe (2007), results showed that optimism during pregnancy renders women less vulnerable to the development of clinically depressive symptoms at six months and 12 months postpartum. Therefore, OTs can focus on improving optimism in pregnant women and mothers by centering the intervention on preparing the pregnant women for motherhood, mood and emotional well-being, adapting and accepting your body after birth, physical recovery after birth, introducing new leisure pursuits and stress management. At the Inanda community, we have a Blaumeier project where we use projective media, including dance, storytelling, art, and movement. This project allows for support groups, leisure groups, and movement as a primary means of providing therapy targeting mothers and it is  conducted at the park (we are still looking for clients to participate in this project). The health promotion talks at the clinic address occupations during perinatal care, promoting mother-infant bonding, promoting positive self-efficacy, self-confidence, and self-acceptance after giving birth, facilitating successful transitions to reduce maternal role strain. Education on stress management and informing the mothers about the Blaumeier project.

The prevalence of mental health disorders among mothers and mothers-to-be impacts children's health and development. Lawn et al. (2006) stated that separation of maternal and child health is unrealistic as mothers, new-born babies, and children all benefit from a functioning health system providing intervention throughout the extension of care from pre-pregnancy pregnancy, childbirth and the postnatal period into infancy and childhood.  In OT, child health is concerned with all aspects of children's growth and development, and we believe that every child has the unique opportunity to achieve their full potential.; at Inanda, we work with children at the local creche and primary schools to bring them to the park and stimulate play through nature; with the children at the creche, we are concerned with teaching basic concepts through play stimulation and school readiness skills and physical development. At the primary schools, we provide intervention for children with learning difficulties. At the local clinic, we screen children for development when they have an immunization appointment. It is also important to work closely with the mother when working with children to ensure that we equip them with the knowledge they will use to promote the optimal function of the child.
I come from a rural area (Eshowe), and mothers in my community are unexposed to maternal health, which indicates the unavailability of maternal health in low socio-economical communities. Silal et al. (2012) identify that services can be unaffordable, unavailable and unacceptable for many women, creating barriers that affect how much and when women can access them. The inequalities in the distribution of these access barriers, with differences between socio-economic groups and geographic areas being the most important. Inanda is a low socio-economic community, and the conditions that women are living under (GBV, unequal gender roles) can lead to maternal depression and declining health. We must provide low-cost intervention for the mothers and children and we  are using nature (the park) which is low cost and encouraging the community to use the park. At the park, we implement different projects for mothers and children and do health promotional talks at the clinic to give mothers a chance to ask questions and gain information. You can rest assured that everything I am learning from this block, I am taking it back home to my community as all this knowledge is foreign to us.
Good health contributes to society's economic productivity and development by decreasing the costs of ill-health and ensuring a good healthy nation; therefore, maternal and child health at a PHC level should be provided at all costs!!
