Study sheds new light on severe COVID's long-term brain impacts - Published Sept 24, 2024

More than a year after COVID-19 hospitalization, many patients have worse cognitive function than those who weren't hospitalized, a symptom that comes with reduced brain volume and brain injury markers on blood tests, according to a new study, the largest of its kind in the United Kingdom.
The multicenter study from the COVID-CNS Consortium included 351 patients who were hospitalized for COVID and 2,927 matched controls. The researchers, led by a team at the University of Liverpool and King's College London, published their findings yesterday in Nature Medicine.
Looking for brain-fog markers Study goals included understanding biological causes and long-term outcomes of neurologic and neuropsychiatric complications following COVID hospitalization. Researchers tested participants' cognitive skills and examined findings from brain scans and blood tests 12 to 18 months after hospitalization.
Greta Wood, MBBS, first author of the study and an academic clinical fellow in infectious diseases at the University of Liverpool, said in university press release that many patients hospitalized with COVID report persistent symptoms, often called "brain fog."
"However, it has been unclear as to whether there is objective evidence of cognitive impairment and, if so, is there any biological evidence of brain injury; and most importantly if patients recover over time," she said.
Cognitive deficits resembled 2 decades of aging Of the patients hospitalized for COVID, some did and some didn't have new neurologic complications. The researchers found that both groups had worse cognition than expected for their age, sex, and level of education.
One of the most striking findings was that post-COVID deficits in hospitalized patients look similar to 20 years of normal aging. The team also found that people who had been hospitalized with COVID had reduced brain volume in key areas and abnormally high levels of brain injury proteins in their blood.
The team saw the greatest deficits in people who had the most severe infections, had post-acute psychiatric symptoms, and had a history of encephalopathy.
In a promising finding, longer-term follow-up of 106 patients pointed to a trend toward recovery.
Could other severe infections cause similar problems? Benedict Michael, MBChB, PhD, the study's corresponding author and professor of neuroscience at the University of Liverpool, said COVID isn't just a lung condition, and some of the most severely affected patients are those who have brain complications.
That the cognitive impairments occurred alongside brain-cell injury markers and reduced brain volume on magnetic resonance imaging suggest there may be measurable biomechanisms, he said. "Now our group is working to understand whether the mechanisms that we have identified in COVID-19 may also be responsible for similar findings in other severe infections, such as influenza."
Gerome Breen, PhD, a study author and psychiatric geneticist with King's College London, said the work might be helpful for guiding similar studies of patients with long COVID who had milder respiratory symptoms who also report brain fog and for the development of treatment strategies.

A new study presented in the paper “Top Surgery and Chest Dysphoria Among Transmasculine and Nonbinary Adolescents and Young Adults” concludes that such surgery is of great help to trans people assigned female at birth.
The researchers, Mona Ascha,  Daniel C. Sasson and Rachita Sood, write:
This nonrandomized, multicenter, prospective, control-matched study showed that top surgery was associated with statistically significant improvement in chest dysphoria, gender congruence, and body image at 3 months post-surgery. Surgical complications were minimal.

Hi Everyone, 
It has been difficult to think of something that would excite me and my classmates. I do have a passion for health and welling being, so I wanted to pick a topic in that realm. 
I find it very interesting that plants can be used for medicinal purposes. More specifically, cannabis plants can be used to treat multiple medical conditions. I have recently done research for a NEUR*4000 course that looked at the effects of cannabis on schizophrenia. Schizophrenia is a very interesting and complex disorder. Research has proven that antipsychotics that are prescribed to individuals with schizophrenia are ineffective. Cannabidiol, also known as CBD, is a naturally occurring and primary component in cannabis plants (Koleza et al., 2020). CBD differs from THC as it does not elicit psychomimetic effects, making it the non-psychoactive component in Cannabis Sativa (Boggs et al., 2018). In individuals with schizophrenia, the effects of THC can trigger amplified psychotic episodes, provoking relapse (Koleza et al., 2020). However, studies have been conducted to determine the possible therapeutic effects of CBD due to its antipsychotic profile (Peres et al., 2018). Multiple studies have proven that CBD reduces psychotic symptoms in schizophrenia (McGuire et al., 2018). 
Another way that nature is used to alleviate symptoms of schizophrenia is through interactions with animals. Currently, I am working in Caledon at a Ranch that houses individuals with schizophrenia. The ranch is not only home to the clients but some farm animals as well. There are 2 miniature ponies, 3 alpacas, 2 sheep, 5 goats, 16 chickens, and 3 peacocks. The ranch sits on 23 acres of land. The animals provide comfort and responsibility to the client. There is also a community garden on the property. The clients on the ranch and in the community learn to care for the animals and community gardens. The program aims to teach life skills to the clients. 
What other medical conditions could be treated or benefit from CBD? Research has shown that CBD can be used to ease chronic or neuropathic pain (Rapin et al., 2021). Furthermore, mental health conditions like anxiety and depression can be treated with CBD (Rapin et al., 2021). Findings have shown that CBD is effective when individuals experience moderate to severe symptoms (Rapin et al., 2021). 
I find it so amazing how nature can be used to help individuals with mental health issues and improve well-being. It is not a very nature-isc view but nature is involved to some level.

Medical Masks Versus N95 Respirators for Preventing COVID-19 Among Health Care Workers
Abstract
Background: It is uncertain if medical masks offer similar protection against COVID-19 compared with N95 respirators.
Objective: To determine whether medical masks are noninferior to N95 respirators to prevent COVID-19 in health care workers providing routine care.
Design: Multicenter, randomized, noninferiority trial. (ClinicalTrials.gov: NCT04296643).
Setting: 29 health care facilities in Canada, Israel, Pakistan, and Egypt from 4 May 2020 to 29 March 2022.
Participants: 1009 health care workers who provided direct care to patients with suspected or confirmed COVID-19.
Intervention: Use of medical masks versus fit-tested N95 respirators for 10 weeks, plus universal masking, which was the policy implemented at each site.
Measurements: The primary outcome was confirmed COVID-19 on reverse transcriptase polymerase chain reaction (RT-PCR) test.
Results: In the intention-to-treat analysis, RT-PCR–confirmed COVID-19 occurred in 52 of 497 (10.46%) participants in the medical mask group versus 47 of 507 (9.27%) in the N95 respirator group (hazard ratio [HR], 1.14 [95% CI, 0.77 to 1.69]). An unplanned subgroup analysis by country found that in the medical mask group versus the N95 respirator group RT-PCR–confirmed COVID-19 occurred in 8 of 131 (6.11%) versus 3 of 135 (2.22%) in Canada (HR, 2.83 [CI, 0.75 to 10.72]), 6 of 17 (35.29%) versus 4 of 17 (23.53%) in Israel (HR, 1.54 [CI, 0.43 to 5.49]), 3 of 92 (3.26%) versus 2 of 94 (2.13%) in Pakistan (HR, 1.50 [CI, 0.25 to 8.98]), and 35 of 257 (13.62%) versus 38 of 261 (14.56%) in Egypt (HR, 0.95 [CI, 0.60 to 1.50]). There were 47 (10.8%) adverse events related to the intervention reported in the medical mask group and 59 (13.6%) in the N95 respirator group.

Limitation: Potential acquisition of SARS-CoV-2 through household and community exposure, heterogeneity between countries, uncertainty in the estimates of effect, differences in self-reported adherence, differences in baseline antibodies, and between-country differences in circulating variants and vaccination.
Conclusion: Among health care workers who provided routine care to patients with COVID-19, the overall estimates rule out a doubling in hazard of RT-PCR–confirmed COVID-19 for medical masks when compared with HRs of RT-PCR–confirmed COVID-19 for N95 respirators. The subgroup results varied by country, and the overall estimates may not be applicable to individual countries because of treatment effect heterogeneity.

The Delay of Puberty (DP) implies the absence of secondary sexual signs and relevant physical changes in boys and girls aged 14 years. The delay in sexual development in the boys implies the lack of an increase in the testicular volume (<4 ml) and if the length of the testicles do not reach 2.5 cm (<4 ml) at the age of 15 years. The main signs of the delay of sexual development in girls is the lack of Telarche and Adrenarche by the 13 years and the absence of Menarche by 15 years [1-3]. In accordance with the ICD-10, the delay of sexual development is highlighted as an independent endocrine disease. DP is one of the actual problems of endocrinology. Even though the frequency of the recovery of the DP among both sexes is the same, but boys are more often treated with complaints: 9: 1 [4,5]. At the same time, in modern literature there is not enough data in this area, there are no work and fully dedicated to the epidemiology of various forms of DP in boys and girls. There are many publications on separate only issues. There are no multicenter studies of DP in boys and girls in any foreign literature, nor in the near abroad over the past few decade.

Longterm Mortality among Chronic Complex Patients with polypharmacy: Results of Community- Based Prospective Study
Introduction: The polypharmacy is common in ambulatory care, hospital and nursing home patients. Over the last 20-30 years, problems related to aging, multi morbidity, and polypharmacy have become a prominent issue in global healthcare. In the developed countries around 3-4% of the people could be identified as chronic complex patient and they are increasingly at risk of polypharmacy. The main objective of this study we as to evaluate the association of polypharmacy and mortality risk. Materials and Methods: We carried out a multicenter and prospective cohort study of mortality incidence from 01.01.2013 to 30.09.2016 among 825 adult patients registered in the electronic health record of Primary Care as Chronic Complex Outpatient. To predict hazard ratios, mean survival time, and survival probabilities used a multivariate Cox regression.

Conventional landscape photography tends to overwhelm place with image. It is usually presented in fragments rather than in grounded sequences. Once wrenched from its context, the image, no matter how well intentioned or well researched, floats off into artland. In exhibition of unrelated or loosely related landscapes with vague or lyrical titles identifying the subject or perhaps the site, a false image of unity is lifted from a fragmented world. No matter how aesthetically pleasing the results may be, places are boiled down into commodities. The photographer, having "been there", feels she's captured the place, but communicating it is another matter altogether. Cryptic titles and captions, or none at all, further distance the viewer from the subject by transforming it into a non-referential object. Juxtaposition of two or more images sometimes proves a compelling strategy for making points; so do collage, aerial views, inventive installations, handwritten or autobiographical captions, names, dates, times—bits of information that bring the viewer closer to the images, traces of the awe and delight (or horror and discomfort) of being there. In order to illuminate place not only for the artist but for others, photography must be more than a vignette or a series of vignettes. It must offer a sufficiently thorough and multilayered view to function as a visit or revisit to the place itself. In a series (whether in a book or not), one image can inform another and images and texts can inform one another, extending the sense of presence beyond the individually framed view. But without generous caption information, even series can become simply handsome glimpses of a place that disappears from consciousness as fast as it disappears from sight. [. . .] Then there is the dilemma of meaning. Estelle Jussim and Elisabeth Lindquist-Cock state uncategorically that "landscape construed as the phenomenological world does not exist; landscape can only be symbolic." Many documentarians would disagree, yet the symbolic can also be seen as the thread that ties the local to the outside viewer. Photographer Mark Klett says his goal is to "lessen the distance one often feels when looking at landscape photographs.... The longer I work, the more important it is to me to make photographs that tell my story as a participant, and not just an observer of the land." Given the lack of public skills in reading photographs, given that photographic content is sometimes buried in beauty, contemporary landscape photographers are often condemned to making pictures as pretty as those of their predecessors. Dramatic clouds and sifting light can overwhelm more mundane information. Yet who can resist beautiful landscape pictures of one kind or another? Those photographers who reject strong composition and moving "effects" to make consciously "flat" images can produce works striking in their simplicity but eventually difficult to distinguish from others like them, stalling the memory factor that is of utmost importance in picturing places. And what viewer is moved to identify with a place or spends much time with neutral, factual images unless there is a hook—heightened beauty, familiarity, unfamiliarity, repulsion, fear, or attraction? The role of aestheticization is the most difficult issue within the issue of communicating not only how the landscape looks, or seems, but how it is.
