The lawful good approach to people asking you invasive medical questions is to simply politely tell them that your health is your own private business and that they shouldn't be asking people about things like that.

The chaotic neutral approach is to tell them the truth, or some part of it, but wording it in an unsettling enough way to make them regret asking. Nothing prevents you from saying "yeah I'm fine now but if I stop taking my medication for long enough I'll start bleeding out of my orifices" when you mean "if I go off testosterone my period will start again."
