Abstract Background: Extramedullary plasmacytoma accounts for less than 5% of plasma cell neoplasm. Primary breast plasmacytoma is extremely rare condition encountered in clinical practice without any bone marrow involvement or as secondary from multiple myeloma. It can mimic various benign and malignant lesions in the breast hence it should be kept in differential diagnosis as management would completely differ. Meticulous approach is needed in evaluation like CT scan of upper airway, Histopathological examination, Bone marrow biopsy, emphasizing the importance of each in arriving at a diagnosis. The incidence of breast plasmacytoma is very rare accounting to less than 5% in which only 15-16 cases of solitary breast plasmacytoma have been reported over the last 80 years and hence there is a lack of standard clinical or radiological criteria criteria in the diagnosis and management of breast plasmacytoma. In this study we report solitary extramedullary plasmacytoma of left breast in 70-year-old women with meticulous stepwise approach.

Abstract The aim of this manuscript is to bring our experience from Central African Republic (CAR), an endemic malarial area, during the COVID-19 pandemic. We present two cases treated at MINUSCA level 2+ hospital in Bangui, central African republic, during December 2021 and January 2022. COVID-19 and malaria infection were confirmed by appropriate tests. First case is man, 36 years old, who came to hospital, complaining about muscle pain, headache and fiver. Second case is a man, 41 years old, who came to hospital due to fiver, caught and malaise. Symptoms started to appear a day ago. We can conclude that in malaria endemic areas, in light of existing COVID-19 pandemia, based on similar symptoms characteristic of both diseases, it is essential to suspect both diseases and keep in mind the possibility of coinfection. Therefore a patient with uncharacteristic infectious symptoms should be tested for malaria, so as well for COVID-19 infection.

Abstract The growing use of automated devices in neuro-recovery treatments necessitates the availability of lightweight, simple-to-use, smart, and adaptable frameworks. RobHand was designed with these goals in mind. It is a hand exoskeleton that is especially suitable for individuals who have finger spasticity since it is simple to insert and allows both bending and expansion of the fingers from an underactuated position. The primary attributes, mechanical plan, and turn of events, as well as approval of the kinematic model of the device, are introduced in this work, which has been completed in accordance with the proposals of the new IEC 80601-2-78 norm, which formalises the concept of RACA (Recovery, Evaluation, Remuneration, Easing) robot and addresses parts of productivity and wellbeing.

Abstract Background: Photobiomodulation (PBM) has been used to prevent and manage Oral Mucositis (OM) due to its bio stimulating properties. We evaluated the impact of PBM for OM on the Quality of Life (QOL) of Head and Neck (HNC) patients undergoing radio-chemotherapy. Methods: Patients were allocated randomly to the Laser (LO) and Control Group (LS). PBM was used three times a week. QoL assessment were undertaken at the 1st, 6th, 12th, 18th, and 24th radiotherapy sessions. Results: The overall QOL scores decreased in both groups. A comparative analysis between the LO and LS groups demonstrated a worsening over time in the saliva, taste, swallowing, and recreation domains (p<0,05). The LS group demonstrated a greater impairment of QOL in chewing at the 1st (p= 0.011), 18th (p= 0.023) and 24th sessions (p= 0.012). Conclusion: PBM prevented the manifestation of more severe OM degrees, improving the QOL at oral related domains.

Analytical and Clinical Validation of a Novel Blood- Based Biomarker of Liver Transplant Rejection

Abstract Background: We have discovered and validated a microarray-based test that analyzes blood gene expression profiles (GEP) as an indicator of immune status in liver transplant recipients with stable liver function. Methods: Analytical performance studies to characterize stability of RNA in blood during collection and shipment, analytical sensitivity (input RNA concentration), analytical specificity (interfering substances) and assay performance (clinical validity, and intra-assay, inter-assay, inter- laboratory reproducibility). Results: Total RNA extracted from whole blood specimens collected in PAXgene Blood RNA tubes was stable up to 3 days at room temperature (stable RNA yield). Under routine ambient shipping conditions, storage and shipping temperatures did not affect results. However, specimen shipments exposed to temperatures >400°C or to ambient temperatures for >3 days were unacceptable for processing. Analytical sensitivity studies demonstrated tolerance to variation in RNA input (50 to 400 ng per 3’ IVT (in vitro transcript] labeling reaction). Specificity studies using genomic Jurkat DNA spiked into 3 ’IVT reactions at 10-20% demonstrated negligible assay interference. The test was reproducible across operators, runs, reagent lots, and laboratories. External validation demonstrated that the TruGraf Liver blood test accurately classified patients in 84% of 155 samples. Conclusions: The previously published biomarker is the first non-invasive test to be demonstrated to have clinical utility in assessing immune status of LT recipients with stable liver function and shows promise as a reasonable and necessary tool supporting personalizing immunosuppressive therapy.

Intraparenchymal Hemorrhage Secondary to Rhino Orbitalcerebral Mucormycosis: A Case Report and Literature Review

Mucormycosis is a rare invasive opportunistic fungal infection with high morbidity and mortality, with Intraparenchymal Hemorrhage (ICH) being one of the less common complications cited in available literature. In this case report, we present a case of a 65 year old Filipino diabetic male with non-small cell lung cancer and recent history of steroid use who presented with headache. This patient developed proptosis, chemosis, and purulent discharge from the right eye, and eventually had sudden onset decrease in sensorium on the 6th day of admission. CT scan showed findings of ICH on the right frontal and patient underwent craniotomy and evacuation of hematoma. Histopathology and culture findings confirmed the diagnosis of Rhino-orbital-cerebral mucormycosis. The patient was treated with Amphotericin B initially then shifted to voriconazole based on culture and sensitivity testing. The patient was managed in-patient for 3 months and eventually survived 18 months from the diagnosis.
To date, no case reports of ICH secondary to Rhino-orbital-cerebral mucormycosis are available in our locale. Additionally, patients from all previous published case reports all succumbed to the infection. This case demonstrates that a high index of suspicion and early workup is warranted in patients presenting with headache and having risk factors for mucormycosis. Early diagnosis and prompt treatment are keys towards achieving a good outcome in these patients.

Abstract
Introduction: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal system. Histopathological and immunochemistry examination takes an important part in confirming the types of GISTs, to choose appropriate therapeutics for patients. Patients demographics, anatomic sites, histopathology and immunochemistry characteristics of GISTs are unknown in St Francis Hospital Nsambya (SFHN) and Uganda Cancer Institute (UCI).
Objectives: This study aimed to explore the demographics, anatomic sites, histopathological and immunochemical characteristics of GISTs in St Francis Nsambya Hospital and Uganda Cancer institute.
Methodology: A cross sectional study was conducted. The study population included patients with a histological diagnosis of GISTs who presented between 2018 to 2021 in both facilities. Clinical, pathological and immunochemistry data was collected and analyzed.
Results: A total of 146 records/blocks of patients were reviewed and out of these only Histology and immunochemistry was done on 81 blocks/ patients. Majority of the patients were aged 40 to 59years, (42%) and had a median age of 57 years with IQR (45-65). There was a male predominance of 51.9%. The commonest tumor site was the stomach, (81.5%). The commonest histological type was spindle cell (79%) followed by mixed (11.1%) then epithelioid 8 (9.9%). Most of cases in our study were benign (80.1%) and mitotic count ≤ 5/50 HPF in 71.6% cases. All cases with low mitotic rate were mostly benign with a p value of <0.0001.
Conclusion: We found GISTs were more common in male gender, stomach being the commonest site and 19% of GISTs were below 40 years. Spindle cell was the commonest histologic type and most of them were CKIT positive. Malignancy was correlated with non-spindle cell type and high mitotic rate.

Assessment of Knowledge, Perception and Practices about Malaria in Some Rural Communities of South West Region, Cameroon
Understanding local knowledge about malaria can help in designing sustainable community-based malaria control programs. Thus, the purpose of this study was to generate information on knowledge, perceptions and practices and the preventive measures as regards to malaria in the rural farming community. A cross-sectional survey was conducted September to October 2013 among 851 households from 9 villages in Ekondo Titi Subdivision using structured questionnaire. Questions assessed knowledge, perceptions and practices about malaria among tribal villagers. The data were analyzed using SPSS 20.0 statistical software program. Most of the respondents 58.2% attributed malaria to mosquito bites. However, some of the respondents (28.8%) mentioned drinking dirty water, midges and standing in the sun as the causes of malaria transmission. Avoiding stagnant water (60.9%) and Clearing of bushes (71.5%) were the most frequently mentioned malaria preventive measures perceived and practiced by the respondents. Only 11.05% of 851 participants owned at least one long lasting insecticide treated net (LLIN). Thirty-nine point nine (39.9%) of respondents indicated that they experienced the last malaria episode more than three months ago. Malaria chemoprophylactic treatment was related to the educational status of the participants (p=0.001). Despite this fair knowledge and good attitudes, practices towards malaria prevention and control were poor. A considerable proportion had misconceptions about the cause and transmission of malaria suggesting the necessity of health education. To close the gap between knowledge about transmission and ownership and use of bed nets as a preventive measure, there is the need to re-energize the CHWs activities and implement the concept of night watch in the rural communities.

A framework for bedside help that coordinates an automated bed and a portable controller
The proposed approach can certainly learn the input-output data and discover the non-linear relationships which are inherent in the training data. Additionally, the proposed approach can provide solution of the forward kinematic problem with reasonable errors at and in the vicinity of kinematic singularities. It is well known that, the main drawback of parallel manipulators is the existence of singularities within its workspace, an Artificial Neural Network (ANN) based solution is proposed in this paper. Additionally, the proposed approach can provide solution of the forward kinematic problem with reasonable errors at and in the vicinity of kinematic singularities. The approach is implemented for the 3-RPR, 3-PRR, and 3-RRR planar parallel manipulators.
In this paper, we proposed to use neural networks for forward kinematic solution of three different architectures of planar parallel manipulators, which can be elaborated to generate the best estimation of forward kinematics of the manipulators under study. Even though the manipulators passing through the kinemaic singularities, the proposed approach can provide solution for the problem with reasonable errors. The results of this paper can be used to find the forward kinematics solutions at critical points (singularity points) which can be then avoided, as long as we specify them, in dynamic control stage.

Gasification of coal and biomass: a net carbon-negative power source for environment-friendly electricity generation in China
Realizing the goal of the Paris Agreement to limit global warming to 2 °C by the end of this century will require most likely deployment of carbon-negative technologies. It is particularly important that China, as the world’s top carbon emitter, avoids being locked into carbonintensive coal-fired power generation technologies and undertakes a smooth transition from high- to negative- carbon electricity production. We focus here on deploying a combination of coal and biomass energy to produce electricity in China using an integrated gasification combined cycle system with carbon capture and storage (CBECCS). Such a system will also reduce air pollutant emissions, thus contributing to China’s near-term goal of improving air quality. We evaluate the bus-bar electricity-generation prices for CBECCS with mixing ratios of crop residues varying from 0% to 100%, as well as associated costs for carbon mitigation and co-benefits for air quality. We find that CBECCS systems employing a crop residue ratio of 35% could produce electricity with net-zero life-cycle emissions of greenhouse gases (GHGs), with a levelized cost of electricity (LCOE) of no more than 9.2 US cents per kWh. A carbon price of approximately $52.0/ton would make CBECCS costcompetitive with pulverized coal power plants. Therefore, our results provide critical insights for designing CBECCS strategy in China to harness near-term air quality co-benefits while laying the foundation for achieving negative carbon emissions in the long run.

Pharmacoeconomics deals with systematic studies on cost and outcomes of pharmaceutical products and services. The Journal disseminates scholarly knowledge on a wide array of topics within the purview of health economics including cost minimization and cost-benefit analysis. The Journal focuses on promulgating systematically generated knowledge from pharmacoeconomic evaluation methods having substantial implication on patient care and health-associated quality of life.
